Our objective is to define differences in circulating lipoprotein subclasses between intensive vs. conventional management of Type 1 diabetes during the randomization phase of the Diabetes Control and Complications Trial (DCCT). Nuclear magnetic resonance-determined lipoprotein subclass profiles (NMR-LSP), which estimate molar subclass concentrations and mean particle diameters, were determined in 1,294 DCCT subjects after a median of five (interquartile range: four, six) years following randomization to intensive or conventional diabetes management. In cross-sectional analyses, we compared standard lipids and NMR-LSP between treatment groups. Standard total-, LDL- and HDL-cholesterol levels were similar between randomization groups, while triglyceride levels were lower in the intensively treated group. NMR-LSP showed that intensive therapy was associated with larger LDL diameter (20.7 vs. 20.6 nm, p=0.01) and lower levels of small LDL (median: 465 vs. 552 nmol/l, p=0.007), total IDL/LDL (mean: 1000 vs. 1053 nmol/l, p=0.01), and small HDL (mean: 17.3 vs. 18.6 μmol/l, p<0.0001), the latter accounting for reduced total HDL (mean: 33.8 vs. 34.8 μmol/l, p=0.01). In conclusion, intensive diabetes therapy was associated with potentially favorable changes in LDL and HDL subclasses in sera. Further research will determine whether these changes contribute to the beneficial effects of intensive diabetes management on vascular complications.

OBJECTIVE:

To study the associations between near work, outdoor activity, and myopia among children attending secondary school in rural China.

METHODS:

Among a random cluster sample of 1892 children in Xichang, China, subjects with an uncorrected acuity of 6/12 or less in either eye (n = 984) and a 25% sample of children with normal vision (n = 248) underwent measurement of refractive error. Subjects were administered a questionnaire on parental education, time spent outdoors, and weekly time spent engaged in and preferred working distance for a variety of near-work activities.

RESULTS:

Among 1232 children with refraction data, 998 (81.0%) completed the near-work survey. Their mean age was 14.6 years (SD, 0.8 years), 55.6% were girls, and 83.1% had myopia of -0.5 diopters or less (more myopia) in both eyes. Time and diopter-hours spent on near activities did not differ between children with and without myopia. In regression models, time spent on near activities and time outdoors were unassociated with myopia, adjusting for age, sex, and parental education.

CONCLUSIONS:

These and other recent results raise some doubts about the association between near work and myopia. Additional efforts to identify other environmental factors associated with myopia risk and that may be amenable to intervention are warranted.

OBJECTIVE/BACKGROUND: Many associations between abdominal aortic aneurysm (AAA) and genetic polymorphisms have been reported. It is unclear which are genuine and which may be caused by type 1 errors, biases, and flexible study design. The objectives of the study were to identify associations supported by current evidence and to investigate the effect of study design on reporting associations.

METHODS: Data sources were MEDLINE, Embase, and Web of Science. Reports were dual-reviewed for relevance and inclusion against predefined criteria (studies of genetic polymorphisms and AAA risk). Study characteristics and data were extracted using an agreed tool and reports assessed for quality. Heterogeneity was assessed using I(2) and fixed- and random-effects meta-analyses were conducted for variants that were reported at least twice, if any had reported an association. Strength of evidence was assessed using a standard guideline.

RESULTS: Searches identified 467 unique articles, of which 97 were included. Of 97 studies, 63 reported at least one association. Of 92 studies that conducted multiple tests, only 27% corrected their analyses. In total, 263 genes were investigated, and associations were reported in polymorphisms in 87 genes. Associations in CDKN2BAS, SORT1, LRP1, IL6R, MMP3, AGTR1, ACE, and APOA1 were supported by meta-analyses.

CONCLUSION: Uncorrected multiple testing and flexible study design (particularly testing many inheritance models and subgroups, and failure to check for Hardy-Weinberg equilibrium) contributed to apparently false associations being reported. Heterogeneity, possibly due to the case mix, geographical, temporal, and environmental variation between different studies, was evident. Polymorphisms in nine genes had strong or moderate support on the basis of the literature at this time. Suggestions are made for improving AAA genetics study design and conduct.

Background
Childhood deprivation is a major risk to public health. Poor health in the early years accumulates and is expressed in adult health inequalities. The importance of social mobility - moves into and out of poverty or, indeed, change in relative affluence - for child wellbeing is less well understood. Home ownership and house value may serve as a useful measure of relative affluence and deprivation.
Method
Analysis of the Northern Ireland Longitudinal Study dataset focused on cohort members aged 18 and under at the 2001 census and their families. Using housing tenure and house value reported in 2001 and 2011, moves along the “housing ladder” over ten years were identified. Outcome measures were physical disability and mental health status as reported in 2011. Logistic regression models tested if health outcomes varied by upward and downward changes in house value.
Results
After controlling for variations in age, sex, general health and social class, mental health is worse among those who moved to a lower value house. Compared to ‘no change’, those moving from the upper quintile of house value into social renting accommodation were almost six times more likely to report poor mental health (OR 5.90 95% CI 4.52, 7.70). Conversely, those experiencing the greatest upward movement were half as likely to report poor mental health (OR 0.46 95% CI 0.31, 0.68). There were smaller associations between physical health and downward (OR 2.66 95% CI 2.16, 3.27), and upward (OR 0.75 95% CI 0.61, 0.92) moves.
Conclusion
Poor mental health is more strongly associated with declines in living standards than with improvements. The gradient appears at multiple points along this proxy affluence-deprivation spectrum, not only at the extremes. Further research should explore whether circumstances surrounding moves, or change in social position explains the differential association between the health correlates of upward versus downward mobility.

The knee adduction moment (KAM) during gait has been proposed as an indirect measure of dynamic knee joint loading and has been reported to be higher in obese children [1, 2]. The KAM is primarily calculated from the resultant ground reaction force (GRF) and the lever arm length, both of which can be manipulated through weight-loss or medical interventions [1]. However, there is little data on the relationships between the mechanical, anthropometric and gait contributors to the KAM during paediatric gait. The objectives of the study were to examine the associations with the first (1st) and second (2nd) peak KAM (pKAM) and: (1) centre of pressure (CoP), KAM lever arm length, vertical and mediolateral ground reaction forces (GRF) and, (2) fat mass, height, step width, foot rotation, knee rotation and walking velocity.

Cette étude qualitative a pour objectif de comprendre comment les interventions verbales des apprenantes et apprenants contribuent à la résolution effective des tâches linguistiques orales dans une classe d'anglais langue seconde selon la théorie Vygotskienne. La recherche a montré que les apprenantes et apprenants passent un temps considérable à discuter les énoncés ainsi que de la meilleure façon de résoudre les tâches linguistiques orales. Or, la recension des écrits a révélé que peu d'études ont tenté de comprendre ce que font les apprenantes et apprenants quand ils sont appelés à résoudre des tâches orales. Dans notre étude nous avons donc essayé de jeter la lumière sur ce phénomène. Les interactions verbales de 10 apprenantes et apprenants ont été enregistrées pendant la résolution de tâches linguistiques orales dans une classe d'anglais langue seconde de l'université de Sherbrooke. Pour analyser les données, nous avons utilisé des éléments de trois méthodes d'analyse, notamment la méthode microgénétique de Vygotsky, l'analyse interactionnelle et l'analyse des conversations. Les résultats ont révélé que les apprenantes et apprenants utilisent un nombre important de stratégies afin de mieux comprendre et résoudre les tâches linguistiques orales de façon efficace. Ils recourent notamment à leur langue maternelle, la répétition et la co-construction de phrases. La discussion de ces résultats a montré que la meilleure compréhension des stratégies utilisées par les apprenantes et apprenants ainsi que comment celle-ci [i.e. celles-ci] sont utilisées pourrait contribuer positivement à l'amélioration des techniques d'enseignement par tâches des langues secondes.

La question du pluralisme religieux est au Québec, l’objet de désaccords et de variations dans son mode de régulation et ses instruments d’action publique. La consultation publique sur le projet loi n° 94, Loi établissant les balises encadrant les demandes d’accommodement dans l’Administration gouvernementale et dans certains établissements, est au cœur de ceux-ci. En se basant sur l’analyse des séances d’auditions publiques en commission parlementaire qui ont eu lieu au Québec entre mai 2010 et janvier 2011 sur le projet de loi n° 94, cette thèse vise à interroger les enjeux liés à la publicisation des prises de positions et de l’échange d’arguments entre différents acteurs. À partir d’une méthodologie par théorisation enracinée et d’un cadre conceptuel qui se rattache à la communication publique, cette thèse cherche à mettre en évidence quelques-unes des propriétés des interactions verbales et non verbales qui composent et incarnent cette activité délibérative. Elle approche ces interactions du point de vue de leur publicisation en s’appuyant sur deux principes : la participation publique en tant qu’un instant de la construction du problème public et l’audition publique en commission parlementaire comme maillon d’un réseau dialogique qui participe à la publicisation du désaccord sur les accommodements raisonnables. Mettant l’accent sur l’usage du langage (verbal, non verbal et para verbal), l’objectif de cette thèse est de mieux comprendre comment des groupes minoritaires et majoritaires, engagés dans une arène publique où les points de vue par rapport aux accommodements raisonnables sont confrontés et mis en visibilité, gèrent leur situation de parole publique. La démarche de recherche a combiné deux stratégies d’analyse : la première stratégie d’inspiration conversationnelle, qui observe chaque séquence comme objet indépendant, a permis de saisir le déroulement des séances d’audition en respectant le caractère séquentiel des tours de parole La deuxième stratégie reviens sur les principaux résultats de l’analyse des séances d’auditions pour valider les résultats et parvenir à la saturation théorique pour élaborer une modélisation. L’exploitation des données selon cette approche qualitative a abouti au repérage de trois dynamiques. La première fait état des contraintes discursives. La seconde met en évidence le rôle des dimensions motivationnelles et socioculturelles dans la construction des positionnements et dans l’adoption d’un registre polémique. La troisième souligne la portée de la parole publique en termes d’actualisation des rapports de pouvoir et de confirmation de son caractère polémique. La modélisation proposée par cette thèse représente le registre polémique comme un élément constitutif de l’engagement argumentatif des acteurs sociaux mais qui est considérablement enchâssé dans d’autres éléments contextuels et motivationnels qui vont orienter sa portée. En tant qu’elle est exprimée dans un site dialogique, la parole publique en situation d’audition publique en commission parlementaire est en mesure de créer de nouvelles intrigues et d’une possibilité de coexister dans le dissensus. Le principal apport de cette thèse est qu’elle propose une articulation, concrète et originale entre une approche de la parole publique en tant que révélatrice d’autre chose que d’elle-même (nécessaire à tout éclaircissement des points de vue dans cette controverse) et une approche de la parole publique en tant que performance conduisant à la transformation du monde social. Previous studies have reported associations between conspiracist ideation and domain-level facets of schizotypy, but less is known about associations with lower-order facets. In the present study, 447 adults completed measures of conspiracist ideation and the Schizotypal Personality Questionnaire (SPQ), consisting of nine subscales grouped into four domains. Results of a multiple regression showed that two domains of the SPQ significantly predicted conspiracist ideation, but multicollinearity was a limiting factor. In a second regression, we found that the subscales of Odd Beliefs or Magical Thinking and Ideas of Reference significantly predicted conspiracist ideation, without any multicollinearity constraints. We interpret these results as implicating two specific lower-order facets of schizotypy in belief in conspiracy theories. We further contrast the present results with previous studies indicating associations between conspiracist ideation and paranormal beliefs.

Climate change has been recognised as one of the greatest challenges of the 21st Century. Its impacts, and they way that we choose to deal with them will profoundly affect how business and society operates. This report focuses on European Union (EU) climate policy – the governance structures, rules and regulations that have been put in place at the EU level to attempt to mitigate and adapt to the impacts of climate change. Specifically, it focuses on how trade associations representing industrial sectors or broader business interest have lobbied on EU climate policy, and the impact that they have had on the policymaking process. The report then goes on to discuss whether the impacts of this lobbying align with the stated policies of the companies that are members of these trade associations.

Conspiracy theories can be treated as both rational narratives of the world as well as outcomes of underlying maladaptive traits. Here, we examined associations between belief in conspiracy theories and individual differences in personality disorders. An Internet-based sample (N=259) completed measures of belief in conspiracy theories and the 25 facets of the Personality Inventory for DSM-5 (PID-5). Preliminary analyses showed no significant differences in belief in conspiracy theories across participant sex, ethnicity, and education. Regression analyses showed that the PID-5 facets of Unusual Beliefs and Experiences and, to a lesser extent, Suspiciousness, significantly predicted belief in conspiracy theories. These findings highlight a role for maladaptive personality traits in understanding belief in conspiracy theories, but require further investigation.

This research examines the relationships between Human Resources Practices (HRP) and affective commitment, exhaustion, and cynicism of temporary workers. Furthermore, voluntariness and tenure with the client organization are assumed in the analysis as moderators. A sample of Temporary Agency workers was surveyed. It was assumed that HRP positively relates with affective commitment, while negatively with exhaustion and cynicism. Results confirmed the proposed relationships. Regarding the moderators, it has been proved that voluntariness partially weakens the relationships between HRP and temporary workers’ outcomes. However, contrary to the hypothesis, this effect cannot be affirmed also for the tenure with the client organization. Previous literature as well as implications of the findings are discussed.

BACKGROUND: Highway maintenance workers are constantly and simultaneously exposed to traffic-related particle and noise emissions, and both have been linked to increased cardiovascular morbidity and mortality in population-based epidemiology studies. OBJECTIVES: We aimed to investigate short-term health effects related to particle and noise exposure. METHODS: We monitored 18 maintenance workers, during as many as five 24-hour periods from a total of 50 observation days. We measured their exposure to fine particulate matter (PM2.5), ultrafine particles, noise, and the cardiopulmonary health endpoints: blood pressure, pro-inflammatory and pro-thrombotic markers in the blood, lung function and fractional exhaled nitric oxide (FeNO) measured approximately 15 hours post-work. Heart rate variability was assessed during a sleep period approximately 10 hours post-work. RESULTS: PM2.5 exposure was significantly associated with C-reactive protein and serum amyloid A, and negatively associated with tumor necrosis factor α. None of the particle metrics were significantly associated with von Willebrand factor or tissue factor expression. PM2.5 and work noise were associated with markers of increased heart rate variability, and with increased HF and LF power. Systolic and diastolic blood pressure on the following morning were significantly associated with noise exposure after work, and non-significantly associated with PM2.5. We observed no significant associations between any of the exposures and lung function or FeNO. CONCLUSIONS: Our findings suggest that exposure to particles and noise during highway maintenance work might pose a cardiovascular health risk. Actions to reduce these exposures could lead to better health for this population of workers.
