Seafood carries several contaminants, among which mercury and polycyclic aromatic hydrocarbons are those that cause major concern. Evidence exists that human populations are exposed to these environmental chemicals since ancient times, which may have driven the positive selection of specific genetic polymorphisms related to chemicals toxicokinetic. Both mercury and polycyclic aromatic hydrocarbons are able to cause DNA methylation changes in humans. Some Mediterranean populations may be particularly exposed to these contaminants, being the Mediterranean Sea at a high-risk for contamination by toxic compounds, and because of their traditionally high consumption of locally caught seafood. Starting from these premises the present thesis aims to contribute to the understanding of the molecular impact of seafood consumption on the biology of the Mediterranean population. To this end the work has been divided into four main parts: (1) the development and meta-analysis of a georeferenced database on polycyclic aromatic hydrocarbons in Mediterranean seafood aimed at identifying geographical patterns of contamination and trends that could be related to the biology of the marine organisms, to the physico-chemical properties of each hydrocarbon and to the oceanographic characteristic of the Mediterranean; (2) the development and validation of a food frequency questionnaire to estimate the intake of mercury through seafood consumption among a population living in a geographic area that is usually considered a contamination hotspot; (3) the creation of a biobank made of biological samples from members of several Italian communities together with information on their dietary habits, lifestyle and general health; (4) a review of the literature on the genetic component of individual susceptibility to methylmercury and polycyclic aromatic hydrocarbons exposure in humans, to the effects that these pollutants have on human DNA methylation, and to the evidence that Mediterranean coastal communities represent an informative case study to investigate the potential molecular impact of these chemicals.

OBJETIVO: Analisar o consumo de frutas, verduras e legumes em mulheres, segundo fatores sócio-demográficos, econômicos e comportamentais. MÉTODOS: A amostra foi constituída de 311 mulheres de três áreas de estudo, do município de Cotia, na área metropolitana de São Paulo, selecionadas por amostragem por conglomerado em dois estágios. O consumo de frutas, verduras e legumes foi avaliado por questionário de freqüência alimentar. Os diferenciais de consumo foram estudados por análise multivariada de regressão logística. RESULTADOS: A chance de baixo consumo de frutas foi maior nas mulheres do bairro pobre, com baixa escolaridade, donas de casa e desempregadas, com baixa renda familiar e tabagistas. Os diferenciais de consumo de verduras foram associados mais à cultura alimentar do que à pobreza: as mais jovens apresentaram chances sensivelmente maiores de baixo consumo de verduras. O tabagismo e o sedentarismo associaram-se ao baixo consumo. Os legumes foram associados tanto ao nível socioeconômico, quanto à cultura alimentar. Foram pouco consumidos pelas mulheres mais jovens e, de um modo geral, por aquelas de pouca escolaridade e baixa renda familiar. Também, o etilismo e o sedentarismo aumentaram as chances de baixo consumo desses alimentos. CONCLUSÃO: O consumo de frutas, verduras e legumes apresentou diferenciais relacionados ao nível socioeconômico, à cultura alimentar e aos hábitos comportamentais.

INTRODUÇÃO: A despeito do aumento expressivo da população idosa nos últimos anos, são escassos os estudos brasileiros relacionados ao consumo alimentar desses indivíduos. OBJETIVOS: Propor uma lista de alimentos mais consumidos por idosos residentes na Zona Leste de São Paulo e analisar os alimentos que contribuem para o consumo de nutrientes relevantes ao estado nutricional e, consequentemente, à saúde dos idosos. MÉTODOS: Foram avaliados 100 indivíduos acima de 60 anos, frequentadores de um centro de referência. Para caracterização do estado nutricional foi calculado o Índice de Massa Corporal (IMC). Para elaboração das listas de alimentos foram aplicados dois recordatórios alimentares de 24 horas (RA24h) em duas estações diferentes do ano, que foram analisados quanto à frequência de consumo de cada alimento e quanto à contribuição percentual de energia, macronutrientes, fibras, cálcio e vitamina D. RESULTADOS: Com relação ao estado nutricional, 52% apresentaram o IMC < 28 kg/m²; 15% entre 28 e 30 kg/m²; 26% entre 30 e 35 kg/m² e 7% com IMC > 35 kg/m². O aspecto positivo da dieta foi a preservação de hábitos saudáveis como o consumo de arroz e feijão, e também de vegetais verde-escuros. Como aspecto negativo observou-se que a dieta dos idosos é monótona, pois poucos alimentos contribuem para o consumo de vários nutrientes. Além disso, houve um elevado consumo de carboidratos refinados em detrimento do consumo de alimentos integrais. CONCLUSÕES: As listas de alimentos obtidas, além de permitirem a reflexão sobre intervenções educativas, permitem o desenvolvimento subsequente de um Questionário de Frequência Alimentar específico para esse grupo.

O maior entrave para o estudo da síndrome metabólica na adolescência consiste na carência de pontos de corte consolidados para tal grupo etário. O presente estudo se propôs a caracterizar os marcadores de risco para a síndrome metabólica em adolescentes do sexo feminino e verificar as possíveis correlações existentes entre variáveis analisadas. Tal estudo foi realizado com 60 adolescentes, do sexo feminino, entre 14 e 18 anos de idade. Foram analisados o índice de massa corporal e seus derivados, as circunferências da cintura (C) e quadril (Q), o percentual de gordura corporal (%G) e as dosagens plasmáticas de colesterol total (CT), frações (LDL e HDL) e glicemia de jejum. Apesar da predominância da eutrofi a (90%), encontraram-se alterações importantes: elevado percentual de gordura corporal (78,3%), níveis alterados de lipídios sangüíneos, 23,3% (CT), 15% (LDL) e 5% (HDL), glicemia de jejum alterada em 6,7%, sendo que em uma das adolescentes foi diagnosticado o diabetes Mellitus. Observaram-se correlações signifi cantes entre HDL e relação C/Q (r = -0,276; p=0,032) e LDL e %G (r = 0,296; p=0,021) e entre o número de alterações apresentadas pelas adolescentes e os valores %G (r=0,300, p=0,020); CT (r=0,536, p<0,001) e LDL (r=0,506, p<0,001). A importância de tais alterações neste grupo etário se justifi ca em função do seu signifi cado clínico, uma vez que irão predispor ao aumento das taxas de morbi-mortalidade por doenças crônicas não transmissíveis na vida adulta

Selenium (Se) intake is generally from food, whose Se content depends on soil Se and plant accumulation. For humans, adequate Se intake is essential for several selenoenzymes. In the Lower Tapajos region of the Brazilian Amazon, Se status is elevated with large inter-community variability. Se intake in this region, where Hg exposure is among the highest in the world, may be important to counteract mercury (Hg) toxicity. The present study was conducted in 2006 with 155 persons from four communities of the Lower Tapajos. The objectives were: i) to evaluate Se content in their typical diet and drinking water; ii) to compare food Se concentrations with respect to geographic location; and iii) to examine the contribution of consumption of different food items to blood Se. More than 400 local foods and 40 drinking water samples were collected. Participants responded to an interview-administered food frequency questionnaire and provided blood samples. Food, water and blood Se levels were assessed by ICP-MS. Since Brazil nuts may also contain significant levels of barium (Ba) and strontium (Sr), these elements were likewise analyzed in nuts. The highest Se concentrations were found in Brazil nuts, but concentrations were highly variable (median: 13.9 mu g/g; range: 0.4-158.4 mu g/g). Chicken, game meat, eggs and beef also contained considerable levels of Se, with median concentrations from 0.3 to 1.4 mu g/g. There was no particular geographic distribution of food Se. Se concentration in drinking water was very low (<1.4 mu g/L). Blood Se covered a (103-1500 mu g/L), and was positively related to regular consumption of Brazil nuts, domestic chicken and game meat. Brazil nuts were found to contain highly variable and often very high concentrations of Ba (88.0 mu g/g, 1.9-1437 mu g/g) and Sr (38.7 mu g/g, 3.3-173 mu g/g). Further studies should address multiple nutrient/toxic interactions in the diet and related effects on health. (c) 2010 Elsevier B.V. All rights reserved.

Collapsed skin folds after bariatric weight loss are often managed by plastic procedures, but changes in dermal composition and architecture have rarely been documented. Given the potential consequences on surgical outcome, a prospective histochemical study was designed. The hypothesis was that a deranged dermal fiber pattern would accompany major changes in adipose tissue. Female surgical candidates undergoing postbariatric abdominoplasty (n = 40) and never obese women submitted to control procedures (n = 40) were submitted to double abdominal biopsy, respectively in the epigastrium and hypogastrium. Histomorphometric assessment of collagen and elastic fibers was executed by the Image Analyzer System (Kontron Electronic 300, Zeiss, Germany). Depletion of collagen, but not of elastic fibers, in cases with massive weight loss was confirmed. Changes were somewhat more severe in epigastrium (P = 0.001) than hypogastrium (P = 0.007). Correlation with age did not occur. (1) Patients displayed lax, soft skin lacking sufficient collagen fiber network. (2) Elastic fiber content was not damaged, and was even moderately increased in epigastrium; (3) Preoperative obesity negatively correlated with hypogastric collagen concentration; (4) Future studies should pinpoint the roles of obesity, and especially of massive weight loss, on dermal architecture and response to surgery.

Objetivo: Verificar se a eletrolipólise pode ser um método coadjuvante na Reabilitação Cardíaca (RC) de modo a diminuir a gordura abdominal em indivíduos com Doença Cardiovascular (DCV). Métodos: Seis indivíduos, de ambos os sexos, da classe de RC do Centro Clinico de Gaia (CCG), foram distribuídos aleatoriamente em dois grupos, grupo controlo (GC) (n=3) e grupo experimental (GE) (n=3). O GE realizou 8 sessões (duas vezes por semana) de eletrolipólise por microcorrentes 30 minutos antes da classe de RC. Utilizaram-se como meios de avaliação a perimetria, adipometria, bioimpedância e ultrassonografia, análise sanguíneas, bem como a aplicação da Hospital Anxiety and Depression Scale (HADS), Questionário Internacional de Actividade Física (IPAQ) e Questionário Semi-Quantitativo de Frequência Alimentar (QSQFA). Resultados: Através do cálculo da mudança mínima detetável (MMD) verificou-se diminuição do perímetro ao nível do umbigo (3,7cm e 1,5cm) e barriga (4,2cm e 2,83cm) em dois indivíduos do GE e diminuição nas avaliações da ultrassonografia (de 1,3mm a 8,4mm) no GE. Conclusão: Do presente estudo foi possível concluir que nesta amostra existiu uma tendência para a diminuição da gordura abdominal localizada através dos dados obtidos pela perimetria e ultrassonografia.

Objective - To evaluate the effect of prepregnancy body mass index (BMI), energy and macronutrient intakes during pregnancy, and gestational weight gain (GWG) on the body composition of full-term appropriate-for-gestational age neonates. Study Design - This is a cross-sectional study of a systematically recruited convenience sample of mother-infant pairs. Food intake during pregnancy was assessed by food frequency questionnaire and its nutritional value by the Food Processor Plus (ESHA Research Inc, Salem, OR). Neonatal body composition was assessed both by anthropometry and air displacement plethysmography. Explanatory models for neonatal body composition were tested by multiple linear regression analysis. Results - A total of 100 mother-infant pairs were included. Prepregnancy overweight was positively associated with offspring weight, weight/length, BMI, and fat-free mass in the whole sample; in males, it was also positively associated with midarm circumference, ponderal index, and fat mass. Higher energy intake from carbohydrate was positively associated with midarm circumference and weight/length in the whole sample. Higher GWG was positively associated with weight, length, and midarm circumference in females. Conclusion - Positive adjusted associations were found between both prepregnancy BMI and energy intake from carbohydrate and offspring body size in the whole sample. Positive adjusted associations were also found between prepregnancy overweight and adiposity in males, and between GWG and body size in females.

Background: The effect of the intake of polynsaturated long chain fatty acids (LCPUFAs) during pregnancy on fetal body composition has been assessed by studies using mostly neonatal anthropometry. Their results have been inconsistent, probably because neonatal anthropometry has several validity limitations. Air displacement plethismography (ADP) is a recently validated non-invasive method for assessing body composition in neonates. Objective: To determine the effect of the intake of LCPUFAs during pregnancy on the body composition of term neonates, measured by ADP. Methods: Cross-sectional study of a convenience sample of healthy full-term neonates and their mothers. The diet during pregnancy was assessed using a validated semi-quantitative food frequency questionnaire; Food Processor Plus® was used to convert food intake into nutritional values. Body composition was estimated by anthropometry and measured by ADP using Pea Pod™ Life Measurements Inc (fat mass - FM, fat-free mass and %FM) within the first 72h after birth. Univariate and multivariate analysis (linear regression model) were performed. Results: 54 mother-neonate pairs were included. Multivariate analysis adjusted to the maternal body mass index shows positive association between LCPUFAs intake and neonatal mid-arm circumference (= 0,610, p = 0,019) and negative association between n-6:n-3 ratio intake and neonatal %FM (= -2,744, p=0,066). Conclusion: To the best of our knowledge, this is the first study on this subject using ADP and showing a negative association between LCPUFAs n-6:n-3 ratio intake in pregnancy and neonatal %FM. This preliminary finding requires confirmation increasing the study power with a greater sample and performing interventional studies.

Result - Positive associations between maternal docosahexaenoic acid intake and ponderal index in male offspring (β=0.165; 95% confidence interval (CI): 0.031–0.299; P=0.017), and between n-6:n-3 LCPUFA ratio intake and fat mass (β=0.021; 95% CI: 0.002–0.041; P=0.034) and percentage of fat mass (β=0.636; 95% CI: 0.125–1.147; P=0.016) in female offspring were found. Conclusion - Using a reliable validated method to assess body composition, adjusted positive associations between maternal docosahexaenoic acid intake and birth size in male offspring and between n-6:n-3 LCPUFA ratio intake and adiposity in female offspring were found, suggesting that maternal LCPUFA intake strongly influences fetal body composition.

Background: A growing body of research suggests that vitamin D might play an important role in overall health. No data exist on vitamin D intake for the Azorean adolescent population. The purpose of this study was to assess vitamin D intake and investigate a possible association between vitamin D intake and cardiometabolic risk factors in Azorean adolescents. Methods: A cross-sectional school-based study was conducted on 496 adolescents (288 girls) aged 15–18 years from the Azorean Islands, Portugal. Anthropometric measurements (waist circumference and height), blood pressure (systolic), and plasma biomarkers [fasting glucose, insulin, total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), and triglycerides (TGs)] were measured to assess metabolic risk. Homeostasis model assessment (HOMA), TC-to-HDL-C ratio, and waist-to-height ratio were calculated. For each of these variables, a Z-score was computed by age and sex. A metabolic risk score was constructed by summing the Zscores of all individual risk factors. High risk was considered when the individual had ‡ 1 standard deviation(SD) of this score. Vitamin D intake was assessed with a semiquantitative food frequency questionnaire. Participants were classified into quartiles of vitamin D intake. Logistic regression was used to determine odds ratios for high cardiometabolic risk scores after adjusting for total energy intake, pubertal stage, fat mass percentage, and cardiorespiratory fitness. Results: Mean (SD) vitamin D intake was 5.8 (6.5) mg/day, and 9.1% of Azorean adolescents achieved the estimated average requirement of vitamin D (10 mg/day or 400 IU). Logistic regression showed that the odds ratio for a high cardiometabolic risk score was 3.35 [95% confidence interval (CI) 1.28–8.75] for adolescents in the lowest vitamin D intake quartile in comparison with those in the highest vitamin D intake quartile, even after adjustment for confounders. Conclusion: A lower level of vitamin D intake was associated with worse metabolic profile among Azorean adolescents.

Epidemiologic studies have reported an inverse association between dairy product consumption and cardiometabolic risk factors in adults, but this relation is relatively unexplored in adolescents. We hypothesized that a higher dairy product intake is associated with lower cardiometabolic risk factor clustering in adolescents. To test this hypothesis, a cross-sectional study was conducted with 494 adolescents aged 15 to 18 years from the Azorean Archipelago, Portugal. We measured fasting glucose, insulin, total cholesterol, high-density lipoprotein cholesterol, triglycerides, systolic blood pressure, body fat, and cardiorespiratory fitness. We also calculated homeostatic model assessment and total cholesterol/high-density lipoprotein cholesterol ratio. For each one of these variables, a z score was computed using age and sex. A cardiometabolic risk score (CMRS) was constructed by summing up the z scores of all individual risk factors. High risk was considered to exist when an individual had at least 1 SD from this score. Diet was evaluated using a food frequency questionnaire, and the intake of total dairy (included milk, yogurt, and cheese), milk, yogurt, and cheese was categorized as low (equal to or below the median of the total sample) or “appropriate” (above the median of the total sample).The association between dairy product intake and CMRS was evaluated using separate logistic regression, and the results were adjusted for confounders. Adolescents with high milk intake had lower CMRS, compared with those with low intake (10.6% vs 18.1%, P = .018). Adolescents with appropriate milk intake were less likely to have high CMRS than those with low milk intake (odds ratio, 0.531; 95% confidence interval, 0.302-0.931). No association was found between CMRS and total dairy, yogurt, and cheese intake. Only milk intake seems to be inversely related to CMRS in adolescents.

Introduction The association of the Mediterranean diet and exercise appears to have a protective role, reducing cardiovascular risk. This study investigated the effects of education sessions on the Mediterranean diet and an exercise program in modifying eating behaviors, body composition and abdominal fat. Methods An experimental study was performed on 20 subjects with known coronary heart disease randomly assigned to experimental (n=10) and control (n=10) groups. Both groups received education sessions on the Mediterranean diet, but the experimental group also followed an eight-week program of specific exercises. A semiquantitative food frequency questionnaire was administered to analyze food intake, bioimpedance was used to measure weight, fat mass and lean mass, and waist circumference was measured to calculate waist-to-height ratio. Results After eight weeks, protein (p<0.05) and cholesterol (p<0.05) intake in the experimental group had decreased significantly compared with the control group. Between the beginning and end of the study, there were significant decreases in the control group in carbohydrate (p<0.05) and saturated fat intake (p<0.05). In both groups the percentage of total fat (p<0.05) and fat mass (p<0.05) was significantly decreased. In the experimental group the waist-to-height ratio was significantly reduced (p<0.05). Conclusion The Mediterranean diet reduced carbohydrate and saturated fat intake, reflected in reduced fat mass. The association of the exercise program showed additional benefits in reduction of protein and cholesterol intake and abdominal fat.

Dietary habits of the Irish population: results from SLÃÂÅN Annual Report 2003 The National Nutritional Surveillance Centre was established in 1992, in the Department of Health Promotion, National University of Ireland,Galway. In 2003 the Centre moved to the Department of Public Health Medicine and Epidemiology, University College Dublin. Its main functions are to provide nutrition-related information to relevant organizations in an accessible form and to monitor trends in health status in relation to food supply, availability and consumption.
