O objetivo deste estudo foi verificar, por meio de questionário, a percepção dos ortodontistas quanto a época ideal para o tratamento ortodôntico da má oclusão de Classe II de Angle, avaliar a eficiência deste tipo de tratamento em uma ou duas fases e identificar os diversos métodos de tratamento utilizados. A amostra constituiu de 163 ortodontistas brasileiros que responderam ao questionário via e-mail. O questionário continha perguntas do tipo aberta, semi aberta e fechada, e, para sua validação, foi realizado um teste piloto entre 15 ortodontistas, participantes ou ex-alunos do Programa de Pós-graduação em Odontologia da Universidade Metodista de São Paulo, que não foram inclusos neste trabalho. Os dados foram tabulados com a utilização de frequência absoluta (n) e frequência relativa (%). Para verificar a correlação entre as variáveis ordinais foi utilizado o Coeficiente de Correlação de Spearman. Para verificar a associação entre variáveis qualitativas nominais foi utilizado o teste do qui-quadrado. Em todos os testes foi adotado nível de significância de 5% (p<0,05). Todos os procedimentos estatísticos foram executados no programa Statistica v.5.1 (StatSoft Inc., Tulsa, USA). Observou-se que os ortodontistas brasileiros preferem tratar os pacientes portadores da má oclusão de Classe II durante a fase da dentadura mista tardia, onde a procura por tratamentos nos consultórios e/ou clínicas tem maior ocorrência entre 10 a 13 anos e no sexo feminino. Dentre as modalidades de tratamento, verificou-se que 25 a 50% dos profissionais tratam seus pacientes em duas fases, seja na utilização do Aparelho Extra Bucal (49,1%), Bionator (33,1%) ou propulsores mandibulares, como APM (17,8%), e, Elásticos Classe II (8,6%).

The Chronic Venous insufficiency is characterized as a set of physical changes including how most serious complication of venous ulcers, characterized by irregular and progressive loss of continuity of the skin. The occurrence of venous ulcers in people with chronic venous insufficiency generates dependence on them with health services, with long-term treatments that cause limitations and high-impact changes, affecting their quality of life, affecting the physical, psychological, social, cultural and spiritual as an important public health problem. This study aimed to describe the experience of having a venous ulcer, in the scenario of primary health care services to Health, which includes Primary Care Units and Family Health Strategy in the city of Natal / RN, based on the life histories of users. This is a qualitative study, exploratory and descriptive, with the Oral History of Life as a methodological framework. From the ponto zero was the recruitment of participants who formed the network, totaling six employees, of both sexes and aged between 57 and 79 years. After approval by the Research Ethics Committee - UFRN under the Protocol 653 788/2014 and CAAE 30408014.0.0000.5537 was held data collection, between the months of July and August, through interviews, using identification and characterization of the instrument employees and open questions. Interviews were recorded, transcribed, transcriadas and returned to employees for a conference. The narratives were subjected to Content thematic analysis technique, according to Bardin, allowing the construction of three themes that encompass categories, namely: Axis I - Perspectives on the changes: the impact wound in social relations (changes with ulcer venous, venous ulcer and social and family relationships); Axis II - Brands in body and soul: the story of being hurt (conceptions of the body injured; therapeutic itinerary in primary care services); and Axis III - Reconstruction of being hurt: coping mechanisms (redefinition of the wounded body, resilience to chronic wound). The impact of having a chronic venous ulcer generates impact of physical, psychological and social order. As aspects related to changes after the appearance of venous ulcers, survey participants reported the presence of pain, physical limitations, psychological distress, social and emotional isolation, incapacity, aesthetic discomfort and dependency on health services; the family was the aspect thatshowed no significant change after the occurrence of wound for most participants, an ally in the therapeutic process as a support network. The redefinition of the body and the wound are the main coping mechanism of chronic condition. The services in the Primary Care Network play a fundamental role in the rehabilitation of patients with venous ulcers, although there are difficulties in accessing appropriate treatment and need for expanded services, with permanent professional training of health teams and providing the resources managers to strengthen the comprehensive care of people with venous ulcers in Health Primary Care.
Trajetória de pessoas com AIDS em situação de vulnerabilidade social: à luz da história oral de vida

The epidemic caused by HIV presents a global, dynamic and unstable phenomenon, which depends on the individual and collective human behavior. Efforts to deconstruct the stigmatized image caused by infection of AIDS are still often associated with adoption of socially unacceptable behavior to be a circumscribed the susceptibilities of vulnerable individuals and communities to infection, illness and death by HIV. This study aimed to: narrate the trajectory of life of people with AIDS more vulnerable enrolled in the Municipal Social Assistance Parnamirim / RN. It is a study of qualitative, exploratory and descriptive approach, taking oral history of life as technical and methodological framework. The colony consisted of 186 people with AIDS. The network was comprised of 13 employees of both sexes, aged between 19 and 62 years old with positive diagnosis and agreed to voluntarily participate. After approval by the Ethics Committee of the Federal University of Rio Grande do Norte (CEP / UFRN), in the opinion No. 719,926 CAAE: 30408114.5.0000.5537 on 6 June 2014 data were collected from August to September 2014. The employees signed the Informed Consent and Informed and letter of assignment. Held transcribing the interviews and later returned to respondents to retest, ie so that they confer what allowed us to carry out transcreation after consecutive readings. The reports were analyzed through Bardin content analysis. Guiding the analysis of the accounts of employees, we find three themes: Prejudice and discrimination in living with AIDS; Reacting to the diagnosis and the accession process to antiretroviral treatment; and religious coping in people with AIDS. It can be concluded in this study, that employees have shown great emotional impact after positive diagnosis for HIV / AIDS, especially with regard to social life, the family ties, work and above all to the prejudice of society. Treatment with antiretroviral drugs was seen as a motivation to regain dreams and plans for a future once uncertain, and even if it is not a cure therapy, provided the employees improved quality of life.

Squamous cell carcinoma (SCC ) is the most common malignancy of the oral cavity (OSCC), with a high mortality rate. Due to this, the discovery of biomarkers that facilitate the understanding of the biological behavior of the tumor and improve treatment is necessary. Urokinase type plasminogen activator (uPA) and its receptor, uPAR, are responsible for the proteolysis of structures of the basement membrana and extracellular matrix, facilitating tumor invasion. This study aims to assess the immuno expression of these proteins in 46 cases of squamous cell carcinoma of the oral tongue (OTSCC). These results were related to the presence of metastasis, clinical TNM staging, locoregional recurrence, outcome of the lesion and histological grading. Immunostaining of each case was evaluated semiquantitatively, in the front of invasion and center of the tumor, in which scores were assigned: 0 (0% of positive cells), 1 (1-10% of positive cells), 2 (11 -50% positive cells) and 3 (more than 50% positive cells). The expression of uPA was observed in 93.5% (n=43) of the cases in the front of invasion, with predominance of score 2 (n=16; 34.8%) and in 67.9% (n=31) of the cases in the center of the tumor, with predominance of score 1 (n=15; 32.6%). Overall, the immunoexpression of uPA was not associated with clinical parameters. Regarding the malignant histological grading, a higher expression of uPA was observed in cases of high-grade malignancy comp ared to low-grade malignancy (p=0.05). Regarding the morphological parameters, increased expression of uPA was observed in the worst mode of invasion (p=0.03 ). The expression of uPAR was observed in 73.9% of cases in the front of invasion, with a predominance of score 1 (n=21; 45.6 %), and in 47.5% (n=21) of the cases in the center of the tumor, with a predominance of score 0 (n=25; 54.4%). Although no statistical differences were observed in relation to lymph node metastasis, clinical TNM staging, outcome, and histological grading, there was a higher expression of uPAR in cases with locoregional recurrence (p=0.04). Regarding the tumor intra -localization, it was observed an increased expression of uPA and uPAR at the front of invasion in relation to the center of the tumor (p<0.001). Regarding the correlation between uPA and uPAR, there was no statistical sign ificance. Based on these results, it is suggested that uPA and uPAR are involved in the progression of CELO, mainly in the deeper region of the tumor.

There is a bidirectional association between periodontal disease (PD) and diabetes mellitus, in which diabetes favors the development of PD and PD, if left untreated, can worsen the metabolic control of diabetes. Thus, periodontal disease should be treated to restore periodontal health and reduce the complications of diabetes. Therefore, the objective is assess the effect of full mouth periodontal therapy decontamination (Full Mouth Desinfection - FMD) in diabetic type II patients with chronic periodontitis during 12 months. Thirty-one patients in group one (G1) and 12 in group two (G2) were followed at baseline, 03, 06 09 and 12 months. There following clinical parameters were accessed: probing on bleeding (BOP), visible plaque index (PI), probing depth (PD), clinical attachment level (CAL) and gingival recession (GR). For diabetic patients, there were also made laboratory tests to evaluate blood parameters: fasting glucose and glycated hemoglobin. The results had been analyzed in two ways: all sites in the mouth and another with diseased sites. The Mann-Whitney, Friedman and Wilcoxon tests were used with 5% significance. Intergroup analysis of all sites it is clear that there was no significant difference over time concerning PD, BOP, PI, CAL and RG. However, when evaluating the diseased sites, we observed significant difference for CAL and PD, with higher values in G1. The intragroup analysis for all sites showed a statistically significant reduction at PD, PI and BOP in both groups. Intragroup analysis of periodontal affected sites showed a statistically significant reduction in PD, BOP and CAL in both groups. There was also a statistically significant increase in RG values. There was no significant change concerning glycated hemoglobin and fasting glucose in the G1. Therefore, it can be concluded that there were improvements in periodontal parameters over the 12 months of research, but without changes in glycemic levels of diabetic patients. Thus, periodontal therapy proved effective in maintaining oral health.

Faults in the genes responsible for repairs to the DNA can influence the onset of cancer or affect the response to treatment. This research evaluated the frequency of three single nucleotide polymorphisms (SNPs) in two repair genes DNA RAD51 172g> T (rs1801321), RAD51 135G> C (rs1801320) and XRCC3 T241M (rs861539) in individuals without cancer (n = 130) and patients with oral squamous cell carcinoma (OSC) and carcinoma oropharyngeal squamous (ORSC) (n = 126) and investigated possible relationships of these findings with clinical and pathological data and clinical outcomes: tumor response to radiotherapy and chemotherapy, disease-free survival, and overall survival. It was found that the allele and genotype frequencies were in equilibrium Hard-Weinberg equilibrium. The presence of at least one polymorphic allele in XRCC3 (rs861539) gene is associated with histological grade (WHO) higher (p = 0.007). We observed a higher recurrence rate trend (p = 0.08) and more advanced stage (p = 0.08) in the group that had at least one polymorphic allele of RAD51 gene (rs1801321). The presence of the analyzed SNPs not proved to be a risk factor for the development of CEO or CEOR; however, when combined with smoking or drinking, increased the risk of developing cancer from three to one hundred and fifty times. The tumor response to radiotherapy and chemotherapy was similar in patients with and without SNPs. No polymorphism showed statistical significance in relation to recurrence-free survival or overall survival. We conclude that the presence of at least one polymorphic allele of the SNPs rs861539 in XRCC3 gene, rs1801320 and rs1801321 in the RAD51 gene increase the risk of development of OSC and ORSC, when associated with the habit of drinking or smoking. Polymorphisms studied in XRCC3 and RAD51 genes are not associated with response to radiation therapy, relapse-free survival or overall survival.

T. gondii can infect the gut mucosa by direct invasion of epithelial cells in the small intestine and these cells may respond directly to infection promoting a local immune response. C57BL/6 mice orally infected with a high parasitic load of T.gondii are highly susceptible, presenting a lethal ileitis. Recently, it was demonstrated that pretreatment with STAg protects C57BL/6 mice against intestinal pathology in oral T. gondii infection. To investigate the mechanisms induced by STAg in the small intestine in oral T.gondii infection, BALB/c and C57BL/6 mice were treated with STAg 48 hours before oral infection with 30 ME-49 cysts and sacrificed at 8 days of infection. Previous treatment with STAg were able of decrease parasitism and pathology in peripheral organs of BALB/c and C57BL/6 mice and induced a increase in amounts of goblet cells, IgA positive cells, Paneth cells and expression of cryptidin in the small intestine of both lineages of mice, moreover BALB/c mice presented higher amount of these cells comparing with C57BL/6 mice. The results suggests that STAg is able of promoting protective mechanisms in both lineages of mice, although these protection is more evidenced in BALB/c mice, and these mechanisms could be in part mediated by increase in goblet, Paneth and local secretion of IgA in the small intestine of mice orally infected with T.gondii.

The coexistence of gingival recession (GR) with root coverage indication and non-carious cervical lesions (LCNC) generates the need for a protocol that respects and promotes health of dental and periodontal tissues and allows treatment predictability. The main objectives of this theses were: (1) verify, through clinical evaluations, the connective tissue graft for root coverage on direct and indirect restorations made of ceramic resin; (2) analyze the influence of the battery level of the LED curing unit in the composite resin characteristics; (3) assess the influence of restorative materials, composite resin and ceramics, on the viability of gingival fibroblasts from primary culture. Nine patients with good oral hygiene and occlusal stability diagnosed with LCNCs the anterior teeth including premolars associated with gingival recession (class I and II of Miller) and only gingival recession were selected. After initial clinical examination, occlusal adjustment was performed and the patients had their teeth randomized allocated on direct composite resin restoration of LCNC, polishing and GR treatment with connective tissue graft and advanced coronally flap CR group (n = 15); and indirect ceramic restoration of the LCNC's and GR treatment (CTG+CAF) Group C (n = 15). The GR presented teeth with no clinically formed LCNCs cavity were treated using (CTG+CAF) being the control group (n = 15). Sorption and solubility tests, analysis of the degree of conversion and diametral tensile strength were performed in composite resin samples (n = 10) photoactivated by 100, 50 and 10% battery charge LED unit. The viability of fibroblasts on composite resin, ceramics and dentin disks (n = 3) was examined. Clinical follow-up was performed for three months. The data obtained at different stages were tabulated and subjected to analysis for detection of normal distribution and homogeneity. The results showed that: the LED unit with 10% battery affects the characteristics of the composite resin; restorative materials present biocompatibility with gingival fibroblasts; and the association of surgical and restorative treatment of teeth affected by NCCL and GR presents successful results at 3-month follow-up.

Ao Médico Dentista cabe o diagnóstico e o tratamento de todas as perturbações que afetem a cavidade oral e estruturas anexas e, embora a cavidade oral seja uma zona de fácil visualização e acesso, as alterações que vão acontecendo passam muitas vezes despercebidas, ou não lhes é dado o devido valor fazendo com que, as patologias malignas sejam na maior parte das vezes, reconhecidas tardiamente não permitindo outro tipo de atuação senão paliativa. Uma das desordens que acontecem com recorrência é o Líquen Plano (LP). Tratando-se de uma patologia crónica, mucocutânea e autoimune, relativamente comum, que afeta maioritariamente as mulheres numa proporção de 3:2 para os homens, em idades compreendidas entre os 50 e 70 anos de vida. É uma dermatose caracterizada por ter potencial de recorrência, não sendo, contudo, contagiosa. O Líquen Plano poderá atingir a pele, couro cabeludo, unhas, mucosa genital e a cavidade oral. Na cavidade oral, designa-se como Líquen Plano Oral (LPO) e afeta a mucosa oral em 70% dos casos, sendo descrita pela Organização Mundial da Saúde (OMS), como sendo uma lesão prémaligna. Tendo em conta estes fatores, o objetivo deste trabalho é a análise da potencialidade de malignidade do LPO, uma vez que, existem distintas vertentes de entendimento e caracterização do mesmo. Para tal foi utilizada uma metodologia qualitativa de natureza bibliográfica com a aplicação de um conjunto de dados de pesquisa em websites, artigos de revistas e obras de índole cientifica, permitindo assim a realização de uma revisão bibliográfica.

Introdução: Os indivíduos portadores da má oclusão de Classe II Divisão 1 possuem diversas etiologias e podem ser encontrados em todas as etnias. As suas características clínicas e radiográficas se agravam com o crescimento e uma vez não tratada, continuam com esta má oclusão por toda sua vida. Para o tratamento ortodôntico na dentição mista recorre-se a dispositivos como aparelhos extra-orais, aparelhos funcionais fixos e removíveis. Objetivo: O objetivo deste trabalho é fazer uma revisão sobre o diagnóstico e tratamento da Classe II Divisão 1 na dentição mista, salientando a importância do tratamento nesta fase. Materiais e Métodos: Para a concretização do presente trabalho foi realizada uma revisão bibliográfica no presente ano, recorrendo-se ao livro “Ortodontia Contemporânea” do autor William Proffit, diversos motores de busca online, nomeadamente, PubMed, Medline, Elsevier e Scholar Google, utilizando como palavras-chave: “Class II Division 1”, “Mixed Dentition”, “Functional Appliance”, “Extra-Oral Appliance”, e revistas de Ortodontia, nomeadamente, American Journal of Orthodontics, Seminars in Orthodontics, The Angle Orthodontist e Dental Press de Ortodontia e Ortopedia Facial. A pesquisa foi realizada sem qualquer tipo de limites temporais, sendo dada uma maior importância a artigos mais recentes. Os artigos foram selecionados segundo o seu rigor científico e interesse para o tema. Numa fase mais avançada de revisão bibliográfica foram usados artigos citados na bibliografia dos artigos selecionados na primeira pesquisa efetuada. Conclusão: A dentição mista é considerada a fase ideal para iniciar o diagnóstico, prevenção, interceção e possível correção dos problemas dentários e/ou esqueléticos associados a uma Classe II Divisão 1. A Ortopedia Funcional dos Maxilares, removível ou fixa, e o uso de aparelhos extra-orais constituem recursos terapêuticos disponíveis para o tratamento desta má oclusão.

Introdução: A peri-implantite, um processo inflamatório que destroi e afeta tanto os tecidos moles como o tecido osseo em redor de um implante dentário Esta patologia vai ter grande influência a quando da escolha de uma solução terapêutica, sendo as principais opções terapêuticas a cirurgia regenerativa e ressetiva. Objetivo: Efetuar uma revisão bibliográfica no que diz respeito á peri-implantite e aos seus tratamentos cirurgicos convencionais, abordando tambem questões epidemiologicas da patologia e fatores de risco. Metodologia: A presente revisão bibliográfica foi baseada em informação ciêntifica publicada. Recorreu-se ás seguintes bases de dados: “MEDLINE/Pubmed”, “SciELO”, “EBSCO HOST” e “Science Direct”, através da introdução das seguintes palavras-chave: “Peri-implantitis”, “Mucositis”, “Peri-implant infections”, “Diagnosis”, “Treatment”, “Risk factors”, “Surgical treatment”. Conclusão: Para cada situação clínica de peri-implantite, é importante recorrer a diversos parâmetros de diagnóstico, como por exemplo o índice de placa, profundidade e hemorragia à sondagem, supuração e a evidência de perda óssea radiográfica. Aquando de reabilitação oral com implantes, o paciente deverá obedecer a um programa de manutenção eficaz, com o objetivo de manter a integridade dos tecidos peri-implantares. Na literatura científica são descritas diversas possibilidades de tratamento, existe enumero protocolos terapêuticos a efetuar. O médico dentista deve adoptar com base no diagnostico rigoroso o melhor tratamento a efetuar. Nesta linha de pensamentos existem várias aborgens terapêuticas que podemos efetuar, sendo a cirurgia ressetiva e a cirurgia regenerativa soluções têrapeuticas de elevada importância no que diz respeito ao sucesso no tratamento da peri-implantite.
