In the US, one in every eight deaths is due to an obesity-related chronic health condition (ORCHC). More than half of African American women (AAW) 20 years old or older are obese or morbidly obese, as are 63% of menopausal AAW. Many have ORCHC that increase their morbidity and mortality and increase health care costs. In 2013, 42.6 percent of AAs living in South Carolina (SC) were obese. The purpose of this cross-sectional study was to identify the cognitive, behavioral, biological, and demographic factors that influence health outcomes (BMI, and ORCHC) of AAW living in rural SC. A sample of 200 AAW (50 in each of the 4 groups of rurality by menopausal status), 18-64 years, completed the: Menopausal Rating Scale (symptoms); Body Image Assessment for Obesity (self-perception of body); Mental Health Inventory; Block Food Frequency Questionnaire; Eating Behaviors and Chronic Conditions, Traditional Food Habits, and Food Preparation Technique questionnaires — and measures for Body Mass Index. Most rural, and premenopausal AAW were single and not living with a partner. Premenopausal women had significantly higher educational levels. Sixty percent of AAW had between 1 and 5 ORCHC. Most AAW used salt based seasonings, ate deep fried foods 1 to 3 times a week, and ate outside the home 1 to 3 times a month. Few AAW knew the correct daily serving for grains and dairy, and most consumed less than the recommended daily serving of fruits, vegetables and dairy. Morbidly obese AAW used more traditional food preparation techniques than obese and normal-weight AAW. Rural, and menopausal AAW had significantly higher morbid obesity levels, consumed larger portions of meats and vegetables, and reported more body image dissatisfaction than very rural AAW, and premenopausal AAW, respectively. Controlling for socioeconomic factors the relationships between perceptions of body images, psychological distress, and psychological wellbeing remained significant for numbers of ORCHC^

The diagnosis of nutritional status is extremely re levant in clinical practice and population assessment, due to the association betwe en body fat and metabolic alterations. The aim of this study is to analyze th e prevalence of metabolic syndrome (MS) and its components in the pubertal stages of f emale students in Rio Grande do Norte state, Brazil, in accordance with Internation al Diabetes Federation criteria. This is a cross-sectional study with 449 students aged betw een 8 and 19 years, stratified into pubertal stages systematized by Marshal and Tanner (1969), as follows: 27.6% prepubertal, 44.3% pubertal and 28.1% postpubertal, with mean ages of 9.4±1.27, 12.4±2.23 and 15.1±1.88 years, respectively. Preval ences were analyzed using distribution of frequencies and their respective 95 % confidence intervals, while the chi- square test and odds ratio were applied to analyze the associations between variables. The general prevalence of MS was 3.3% (CI: 2% - 5%) , without occurrences in the prepubertal stage, observing that it emerges from t he pubertal stage onwards with a prevalence of 2.5% (CI 95% 0.1% - 5%), 1% (CI 95% 0.4% - 2.3%) of cases with overweight and 1.5% (CI 95% -0.1% - 3.2%) with obes e individuals, while in the postpubertal stage the prevalence is 7.9% (CI 95% 3 .2% - 12.6%), 0.8% (CI 95% -0.8% - 2.3%) normal weight cases, 4% (CI 95% 0.6% - 7.4% ) overweight and 3.1% (CI 0.1% - 6.2%) obese individuals. There was an association (p<0.02) between pubertal stages and MS ( x 2 =5.2), with an OR of 3.3 (CI: 1.2 - 5), showing tha t postpubertal adolescents are more prone to SM than pubertals, while the OR i n obese individuals was 2.1 (CI: 2– 2.2) compared to the overweight. Body mass index (B MI) ( x 2 = 29.4; p<0.001) and age range ( x 2 = 13.1; p<0.001) showed a significant linear assoc iation with MS. Of the adolescents with MS, those aged ten years or younge r exhibited higher %G. The most prevalent components in all the stages were altered waist circumference (27.2% [CI 23% - 31%]) and low HDL cholesterol (39.6% [CI 35% – 44%]), which, coupled with hypertension, displayed significant differences in the postpubertal stage in relation to the other stages. The results show that MS emerges from the pubertal stage onwards in proportion to excess childhood body fat, a fact tha t calls for prevention strategies using an educational approach, reducing the large demand on the National Health System. The polycystic ovary syndrome (PCOS) is considered the most common endocrine disorder in reproductive age women, with a prevalence ranging from 15 to 20%. In addition to hormonal and reproductive changes, it is common in PCOS the presence of risk factors for developing cardiovascular disease (CVD) and diabetes mellitus, insulin resistance (IR), visceral obesity, chronic low-grade inflammation and dyslipidemia. Due to the high frequency of obesity associated with PCOS, weight loss is considered as the first-line treatment for the syndrome by improving metabolic and normalizes serum androgens, restoring reproductive function of these patients. Objectives: To evaluate the inflammatory markers and IR in women with PCOS and healthy ovulatory with different nutritional status and how these parameters are displayed after weight loss through caloric restriction in with Down syndrome. Methods: Tumor necrosis factor-alpha (TNF-α), interleukin-6 (IL-6) and C-reactive protein (CRP) were assessed in serum samples from 40 women of childbearing age. The volunteers were divided into four groups: Group I (not eutrophic with PCOS, n = 12); Group II (not eutrophic without PCOS, n = 10), Group III (eutrophic with PCOS, n = 08) and Group IV (eutrophic without PCOS, n = 10). The categorization of groups was performed by body mass index (BMI), according to the World Health Organization (WHO) does not eutrophic, overweight and obesity (BMI> 25 kg / m²) and normal weight (BMI <24.9 kg / m²). IR was determined by HOMA-IR index. In the second phase of the study a controlled dietary intervention was performed and inflammatory parameters were evaluated in 21 overweight and obese women with PCOS, before and after weight loss. All patients received a low-calorie diet with reduction of 500 kcal / day of regular consumption with standard concentrations of macronutrients. Results: Phase 1: PCOS patients showed increased levels of CRP (p <0.01) and HOMAIR (p <0.01). When divided by BMI, both not eutrophic group with PCOS (I) as eutrophic with PCOS (III) showed increased levels of CRP (I = 2.35 ± 0,55mg / L and 2.63 ± III = 0,65mg / L; p <0.01) and HOMA-IR (I = 2.16 ± 2.54 and III = 1.07 ± 0.55; p <0.01). There were no differences in TNF-α and IL-6 between groups. Step 2: After the weight loss of 5% of the initial weight was reduced in all of the components of serum assessed inflammatory profile, PCR (154.75 ± 19:33) vs (78.06 ± 8.9) TNF α (10.89 ± 5.09) vs (6:39 ± 1:41) and IL6 (154.75 ± 19:33) vs (78.06 ± 08.09) (p <0:00) in association with improvement some hormonal parameters evaluated. Conclusion: PCOS contributed to the development of chronic inflammation and changes in glucose metabolism by increasing CRP, insulin and HOMA-IR, independent of nutritional status. The weight loss, caloric restriction has improved the inflammatory condition and hormonal status of the evaluated patients.

O estudo apresentado avaliou a adequabilidade da ingesta de nutrientes e energia de estudantes do Instituto Politécnico de Santarém (IPS), através do preenchimento de um relatório alimentar de 24 horas, preenchido on line. Foram obtidas 103 respostas válidas para avaliação, maioritariamente do género feminino (77%), com uma idade média de 21 anos e valores de IMC normoponderais (78%). Cerca de metade dos participantes apresentou um estilo de vida sedentário. No geral, a alimentação caracterizou-se por elevados consumos de arroz, massa, batatas ou cereais de pequeno-almoço, de carne ou ovos, e consumos reduzidos de peixe, leguminosas e hortícolas crus ou cozinhados. O consumo de frutos e vegetais foi maioritariamente inferior ao recomendado, assim como a ingesta de energia e fibra. Ao contrário, a ingesta de lípidos totais e lípidos saturados foi superior ao recomendado na maioria dos participantes. A prevalência de inadequação de micronutrientes como cálcio, vitamina A, vitamina C e folatos foi próxima dos 50% ou superior. Perante os dados obtidos sugere-se uma melhoria dos hábitos alimentares e de estilo de vida, nomeadamente, o aumento do número de refeições realizadas, o aumento do consumo de frutos, hortícolas e leguminosas e o aumento da prática de atividade física.

Ce mémoire présente mon projet de maîtrise dont l’objectif était d’étudier les associations entre la récompense alimentaire à l’égard des aliments riches en gras et en sucres, le statut pondéral et les comportements alimentaires. Cent cinquante femmes de poids normal ou obèses ont été invitées à compléter deux questionnaires : un sur la récompense alimentaire (plaisir explicite, motivation explicite et motivation implicite) et l’autre sur les comportements alimentaires [restriction cognitive (totale, flexible et rigide), désinhibition (totale, habituelle, émotionnelle et situationnelle) et susceptibilité à la faim (totale, interne et externe)]. Aucune association n’a été observée entre la récompense alimentaire et le statut pondéral. Toutefois, un plaisir et une motivation explicites plus importants pour les aliments en général ont été notés chez les femmes avec un score plus élevé de restriction flexible. De plus, une motivation implicite plus importante pour les aliments riches en gras tendait à être associée avec un score plus élevé de susceptibilité émotionnelle à la désinhibition et a été associée significativement avec un score plus élevé de faim externe. Ces résultats suggèrent que les comportements alimentaires ont une plus grande influence sur la récompense alimentaire que le statut pondéral.

Introduction: Body mass index (BMI) has been one of the methods most frequently used for diagnose obesity, but it isn't consider body composition. Objective: This study intends to apply one new adiposity index, the BMI adjusted for fat mass (BMIfat) developed by Mialich, et al. (2011), in a adult Brazilian sample. Methods: A cross-sectional study with 501 individuals of both genders (366 women, 135 men) aged 17 to 38 years and mean age was 20.4 ± 2.8 years, mean weight 63.0 ± 13.5 kg, mean height 166.9 ± 9.0 cm, and BMI 22.4 ± 3.4 kg/m². Results and discussion: High and satisfactory R2 values were obtained, i.e., 91.1%, 91.9% and 88.8% for the sample as a whole and for men and women, respectively. Considering this BMIfat were developed new ranges, as follows: 1.35 to 1.65 (nutritional risk for malnutrition), > 1.65 and ≤ 2.0 (normal weight) and > 2.0 (obesity). The BMIfat had a more accurate capacity of detecting obese individuals (0.980. 0.993, 0.974) considering the sample as a whole and women and men, respectively, compared to the traditional BMI (0.932, 0.956, 0.95). Were also defined new cut-off points for the traditional BMI for the classification of obesity, i.e.: 25.24 kg/m² and 28.38 kg/m² for men and women, respectively. Conclusion: The BMIfat was applied for the present population and can be adopted in clinical practice. Further studies are needed to determine its application to different ethnic groups and to compare this index to others previously described in the scientific literature.

Case-control studies evaluating the factors associated with childhood obesity are scarce in Brazil. We aimed to analyze the factors associated with obesity in Brazilian schoolchildren enrolled in the School Health Program.A case-control study was conducted on 80 schoolchildren aged 7 to 9 years, 40 of them obese and 40 of normal weight according to the cut-off points established by the World Health Organization (2007). Weight, height and waist circumference were obtained. Socioeconomic, demographic, health, eating behavior and lifestyle data were collected by applying a questionnaire to the person responsible and by determining his/her nutritional status. A binary unconditional logistic regression model (univariate and multivariate) was used for data analysis. The prevalence of obesity was 7.21%. The final model showed that duration of breast-feeding ≥6 months of age (OR 5.3; 95% CI: 1.3-22.1), excess weight of the person responsible (OR 7.1; 95% CI: 1.2-40.2), a sedentary level of physical activity (OR 4.1; 95% CI: 1.115.5), and fast chewing (OR 7.4; 95% CI: 2.1-26.9) were significantly associated with childhood obesity. The factors associated with obesity in schoolchildren were duration of breast-feeding ≥6 months, persons responsible with excess weight, and sedentary children who chew fast. The present study contributes information to be used for the health actions planned by the School Health Program.

Purpose: To study the prevalence of diabetic retinopathy in known diabetic patients attending the diabetes outpatient department (OPD) of Sind Government Hospital (SGH), New Karachi Township (NKT), Pakistan. Methods: A cross-sectional observational study was carried out at the diabetic OPD of SGH, NKT over the period of 17 months from March 2013 to August 2014. The selected patients were interviewed based on a questionnaire; laboratory investigations were performed and examination of the eye was conducted by a specialist ophthalmologist. One hundred and fifty four (154) subjects out of 305 patients contacted fully completed the study. Stratification of the data on gender basis was done, after which one-way ANOVA, χ2 test of correlation, binary logistic regression and relative risk analyses were carried out using SPSS-20. Results: It was found that 66 % men of normal weight (χ2 = 4.667, p < 0.05) and 60.7 % overweight women (χ2 = 5.143, p < 0.05) were more likely to present with diabetic retinopathy (DR). Prevalence of DR in this target population was 42.86 % (N = 66). Background DR (56 %) and maculopathy (23 %) were more prevalent than advanced conditions of the disease. There was no gender-based preponderance for the presentation of DR (χ2 = 0.663; p > 0.05), nor was this seen in different ethnic groups. Conclusion: DR is prevalent in the target population and, therefore, emphasis should be on the education of the local population of New Karachi Township on how to attain euglycemic state with regular medication, diet and exercise to avoid development and progress of DR.

ANTECEDENTES: Los dos primeros años de vida constituyen un período crítico para la nutrición de los niños. Según la Encuesta Nacional de Salud y Nutrición 2011-2013 cerca de la mitad de niños de 6 a 11 meses no acceden a una variedad adecuada de alimentos. OBJETIVO GENERAL: Establecer la diversidad alimentaria en los niños y niñas de 6 a 23 meses de edad por un lapso de 5 meses en el año 2015 ingresados en el área de Clínica y Cirugía Pediátrica del Hospital Vicente Corral Moscoso. METODOLOGÍA: Estudio descriptivo, en el levantamiento de datos se utilizó entrevistas, revisión de historias clínicas. La tabulación y análisis de los datos se efectuó en SPSS 19 empleando medidas de tendencia central y de distribución en variables cuantitativas; frecuencias y porcentajes en variables cualitativas. La muestra fue de 171 niños de 6 a 23 meses hospitalizados en el área de cirugía y clínica pediátrica del Hospital Vicente Corral Moscoso. RESULTADOS: Se demostró que menos del 23% de los infantes tienen su peso normal, predominando los emaciados y emaciados severos, más marcado en las niñas. La Diversidad Alimentaria, Frecuencia Mínima de Comidas y Dieta Mínima Alimentaria en relación al sexo no arrojaron relación estadística significativa, pero se evidenció un aumento estadísticamente significativo con el incremento de la edad; siendo todos los alimentos consumidos en más del 80% de los infantes a excepción de las legumbres y nueces. Las frutas y verduras ricas en Vitamina A, lácteos y carnes son los más empleados

Introducción: La anestesia epidural como la anestesia raquídea son técnicas anestésicas similares que disminuye la percepción sensitiva y dolorosa. La anestesia raquídea y epidural aunque siendo ambas anestesia regional, presentan distinto comportamiento y por tanto distinto manejo, eficacia y riesgos. Objetivo General: Determinar la prevalencia de anestesia epidural y raquídea en pacientes que se sometieron a cesárea en el hospital José Carrasco Arteaga. Cuenca – Ecuador. Julio a diciembre 2014 Metodología: Se realizó un estudio descriptivo analítico de corte transversal, utilizando la base de datos elaborada de 450 mujeres sometidas a cesárea en el año 2014 en el Hospital José Carrasco Arteaga. La información se recolectó en formularios, los mismos que fueron elaborados por las autoras, se elaboró una base de datos, para lo cual se utilizó Excel 2010, se creó tablas, analizándolas mediante valor de p, frecuencia y porcentaje(%).La información se analizó por medio del software SPSS versión 22. Resultados: De las 441 pacientes encontramos que las mujeres entre los 20-39 años fueron la población de mayor prevalencia sometida a anestesia raquídea y epidural con un 92,1% (164) y un 85,9% (226) correspondientemente, la anestesia epidural se uso más en las pacientes con sobrepeso 38.8% (102) y obesidad 35.0% (92), las mujeres con peso normal en el 37,6% (67) se utilizó la anestesia raquídea mayormente y de las mujeres con sobrepeso en el 36,5 (65) se utilizó la anestesia raquídea principalmente, la mayor prevalencia fue en la población casada bajo anestesia epidural con el 60% (153), la anestesia epidural se utilizó más en las pacientes de la zona urbana un 58.8% (204) y en zona rural un 62.8% (59), la escolaridad superior con el tipo de anestesia raquídea con un 47,2% (84) y con anestesia raquídea con un 35,0% (92) fue el nivel de escolaridad más prevalente, la anestesia epidural se utilizó mas en las pacientes procedentes de la zona urbana 58.8% (204) como de la zona rural 62.8% (59)

El concepto de actividad física es concebido de diferentes formas. Mostrando que existen varios factores que afectan de manera directa e indirecta la percepción que los sujetos construyen entorno a él, generando así una aproximación a diferentes definiciones de la actividad física desde varias perspectivas y dimensiones, donde predomina una noción netamente biológica. Este estudio pretende analizar, como desde las clases sociales se concibe la actividad física en sus conceptos y prácticas considerando los modelos de determinantes y determinación social para la salud. Con fin de comprender como los autores de la literatura científica conciben la actividad física y la relación con las clases sociales, desde una perspectiva teórica de los determinantes sociales de la salud y la teoría de la determinación social, se realizó una revisión documental y análisis de contenido de los conceptos y prácticas de la actividad física que se han considerado en los últimos 10 años. Para ello se seleccionaron las bases de datos PubMed y BVS (Biblioteca Virtual de Salud) por sus énfasis en publicaciones de salud mundialmente. Mostrando que la actividad física es concebida dominantemente desde una perspectiva biológica que ejerce una mirada reduccionista. Las relaciones entre actividad física y las clases sociales están claramente establecidas, sin embargo, estas relaciones pueden discrepar teniendo en cuenta el concepto de clase social, el contexto y la orientación de los autores y las poblaciones objetos de estudio. Obteniendo como resultado que los estudios documentados, revisados y analizados muestran una clara tendencia al modelo de determinantes; no obstante, algunos estudios en sus análisis se orientan hacia el modelo de determinación social. En cuanto al concepto de clases sociales los autores consideran una combinación de factores culturales y económicos sin atreverse a adoptar un concepto específico.

A prevalência da obesidade está a aumentar nos países desenvolvidos e Portugal não é excepção. O presente trabalho teve como principal objectivo caracterizar a situação relativamente aos parâmetros antropométricos e hábitos alimentares de várias categorias profissionais do sector da saúde de uma instituição hospitalar do distrito de Lisboa. Com efeito, verificou-se que existem diferenças ao nível do Índice de Massa Corporal (IMC), da massa gorda e muscular e do perímetro de cintura entre as profissões estudadas (assistentes operacionais, enfermeiros, médicos e técnicos de diagnóstico e terapêutica). De acordo com a classificação do IMC, o grupo estudado situou-se no cut-off superior do ‘peso normal’ (IMC de 24,9 kg/m2). A obesidade esteve presente em 29% dos assistentes operacionais, 9% nos médicos e 5% nos enfermeiros e técnicos de diagnóstico e terapêutica. Também se observaram diferenças entre os géneros feminino e masculino, já que neste último o IMC foi de 26,5 kg/m2 e no primeiro foi de 24,6 kg/m2. Quanto aos hábitos alimentares, através do questionário de frequência alimentar (QFA) e após comparar com as recomendações da pirâmide dos alimentos mediterrânica e com a roda dos alimentos, foi notório o desvio do padrão alimentar face ao que é preconizado por estas representações. Assim, a maioria dos indivíduos não ingere produtos lácteos, pão, cereais e derivados com a frequência que seria adequada. Também se observaram diferenças no reporte de ingestão dos mesmos alimentos entre as categorias profissionais, salientando que todas estas apresentam padrões de ingestão desviantes das recomendações defendidas pela pirâmide dos alimentos mediterrânica e pela roda dos alimentos. Na sociedade actual, o local de trabalho é cada vez mais obesogénico devido quer à mudança na natureza do trabalho, quer às condições em que este decorre. Face aos resultados obtidos foi elaborado um plano de intervenção sócio- organizacional que visa promover hábitos alimentares e estilos de vida saudáveis. /ABSTRACT: Obesity prevalence is increasing all over developed countries, and Portugal is no exception. The present study had as major objective to describe the situation relative to anthropometric measures and dietary habits of several job strains operating in the health sector of a Lisbon district’s central hospital. Indeed, it has been verified that there are differences as concern to Body Mass Index (BMI), fat and muscle mass and waist circumference among the job strains studied (operational assistants, nurses, physicians and diagnostic and therapeutic technicians). According to BMI classification, the sample showed to be lined up with the superior BMI cut-off value for “normal weight” (BMI of 24,9 kg/m2). Obesity was present in 29% of operational assistants, 9% in physicians, 5% in nurses and diagnostic and therapeutic technicians. There were differences in both feminine and masculine genders, as in the latter the BMI was 26,5 kg/m2 and in the first, 24,6 kg/m2. As concern to dietary habits, through the application of the food frequency questionnaire (FFQ), after comparing with mediterranean food pyramid and wheel of foods it was notorious the deviation of dietary habits from these representations.
