Objetivo: Se comparó in vitro la acción antimicrobiana de diversas medicaciones intraconducto frente a Enterococcus faecalis y Actinomyces israelii. Material y métodos: para evaluar las zonas de inhibición microbiana se utilizó el test de difusión en agar frente a diversas pastas, incluyendo una pasta con base de metronidazol (Grinazole ®), una pasta con base de dexametasona, tiretrocina, polimixana y neomicina (Septomixine forte ®) y otra de hidróxido de calciio (Calcipulpe ®) y paroclorofenol alcanforado (KRI-3 ®), frente a E. faecalis y A. israelii. Resultados: La acción inhibitoria frente a E. faecalis fue significativamente mayor (p< 0,05) con Septomixine forte ® (30,7 mm) seguido de KRI- 3 ® (22,5 mm), Calcipulpe ® (16,6 mm) y Grinazole ® (11,4 mm). La inhibición microbiana frente a E. israelii fue significativamente mejor con Septomixine forte ® (p< 0,05) (40,1 mm) seguido de KRI-3 ® (36,6 mm), Grinazole ® (7,6 mm) y Calcipulpe ® (6 mm). Conclusión: Septomixine forte ® fue la pasta antibiótica más efectiva frente a E. faecalis y A. israelii comunmente hallados en las infecciones endodóncicas.

En qualsevol cas, cada cop que s'utilitza una nova estratègia terapèutica o es comercialitza un nou fàrmac hi ha la possibilitat que es produeixin efectes secundaris inesperats [...].

Purpose: The aim of the study was to evaluate the opinions and concerns of hospital doctors about adverse drug reactions (ADRs) and pharmacovigilance. Methods: A qualitative study was undertaken using focus groups in sessions on pharmacovigilance activities conducted in thirteen clinical services of a tertiary university hospital. A total of 296 physicians participated in these sessions by giving their opinions or expressing their doubts about ADR and pharmacovigilance activities which were recorded by different observers and subsequently analysed. Results: Doctors remarked on: a) the importance, concern, frequency and specific types of ADRs that were observed in clinical practice; b) problems of clinical decision making related to the suspected ADRs; c) methods for improving detection and reporting ADRs; d) monitoring of specific ADRs or ADRs caused by specific drugs; e) and measures to prevent and minimize the risk of ADRs. Physicians expressed doubts related to: a) the basic concepts of ADRs; b) the methods of ADR identification and evaluation; c) the objectives and procedures of pharmacovigilance programmes; d) and the impact of pharmacovigilance activities. Conclusions: Hospital doctors believe that ADRs are a matter for concern in their daily clinical practice, and monitoring ADRs as well as measures for preventing the risk of ADRs are needed. Nevertheless, doctors have doubts about what an ADR is, the accuracy of diagnostic methods, the development of pharmacovigilance activities and their impact on clinical practice. Pharmacovigilance should be better explained through a continuous feedback and close relationship with hospital doctors.

Background: Scientific evidence on treatments of chronic diseases in patients 85 years old or older is very limited, as is available information on inappropriate prescription (IP) and its associated factors. The study aimed to describe medicine prescription, potentially inappropriate medicines (PIM) and potentially prescribing omissions (PPO) and their associated factors on this population. Methods: In the context of an observational, prospective and multicentric study carried out in elderly patients admitted to seven Spanish hospitals for a year, a sub-analysis of those aged 85 years and over was performed. To assess PIMs, the Beers and STOPP criteria were used, and to assess PPOs, the START and the ACOVE-3 criteria were used. To assess factors associated with IP, a multivariate logistic regression analysis was performed. Patients were selected randomly every week on consecutive days from the hospitalization lists. Results: A total of 336 patients were included in the sub-analysis with a median (Q1-Q3) age of 88 (8690) years. The median medicines taken during the month prior to admission was 10 (713). Forty-seven point two per cent of patients had at least one Beers-listed PIM, 63.3% at least one STOPP-listed PIM, 53.6% at least one START-listed PPO, and 59.4% at least one ACOVE-3-listed PPO. Use of benzodiazepines in patients who are prone to falls (18.3%) and omission of calcium and vitamin D supplements in patients with osteoporosis (13.3%) were the most common PIM and PPO, respectively. The main factor associated with the Beers-listed and the STOPP-listed PIM was consumption of 10 or more medicines (OR = 5.7, 95% CI 1.8-17.9 and OR = 13.4, 95% CI 4.0-44.0, respectively). The main factors associated with the START-listed PPO was a non-community dwelling origin (OR 2.3, 95% CI 1.0-5.0), and multimorbidity (OR1.8, 95% CI 1.0-3.1). Conclusions: Prescribed medicines and PIM and PPO prevalence were high among patients 85 years and over. Benzodiazepine use in those who are prone to falls and omission of calcium and vitamin D in those with osteoporosis were the most frequent PIM and PPO, respectively. Factors associated with PIM and PPO differed with polypharmacy being the most important factor associated with PIM.

A major goal in antimicrobial drug discovery is the generation of new chemicals capable of killing pathogens with high selectivity, particularly multi-drug-resistant ones. Here we report the design, preparation and activity of new compounds based on a tunable, chemically accessible and upscalable lipopeptide scaffold amenable to suitable hit-to-lead development. Such compounds could become therapeutic candidates and future antibiotics available on the market. The compounds are cyclic, contain two D-amino acids for in vivo stability and their structures are reminiscent of other cyclic disulfide-containing peptides available on the market. The optimized compounds prove to be highly active against clinically relevant Gram-negative and Gram-positive bacteria. In vitro and in vivo tests show the low toxicity of the compounds. Their antimicrobial activity against resistant and multidrug-resistant bacteria is at the membrane level, although other targets may also be involved depending on the bacterial strain.

Il y a des problemes qui semblent impossible a resoudre sans l'utilisation d'un tiers parti honnete. Comment est-ce que deux millionnaires peuvent savoir qui est le plus riche sans dire a l'autre la valeur de ses biens ? Que peut-on faire pour prevenir les collisions de satellites quand les trajectoires sont secretes ? Comment est-ce que les chercheurs peuvent apprendre les liens entre des medicaments et des maladies sans compromettre les droits prives du patient ? Comment est-ce qu'une organisation peut ecmpecher le gouvernement d'abuser de l'information dont il dispose en sachant que l'organisation doit n'avoir aucun acces a cette information ? Le Calcul multiparti, une branche de la cryptographie, etudie comment creer des protocoles pour realiser de telles taches sans l'utilisation d'un tiers parti honnete. Les protocoles doivent etre prives, corrects, efficaces et robustes. Un protocole est prive si un adversaire n'apprend rien de plus que ce que lui donnerait un tiers parti honnete. Un protocole est correct si un joueur honnete recoit ce que lui donnerait un tiers parti honnete. Un protocole devrait bien sur etre efficace. Etre robuste correspond au fait qu'un protocole marche meme si un petit ensemble des joueurs triche. On demontre que sous l'hypothese d'un canal de diusion simultane on peut echanger la robustesse pour la validite et le fait d'etre prive contre certains ensembles d'adversaires. Le calcul multiparti a quatre outils de base : le transfert inconscient, la mise en gage, le partage de secret et le brouillage de circuit. Les protocoles du calcul multiparti peuvent etre construits avec uniquements ces outils. On peut aussi construire les protocoles a partir d'hypoth eses calculatoires. Les protocoles construits a partir de ces outils sont souples et peuvent resister aux changements technologiques et a des ameliorations algorithmiques. Nous nous demandons si l'efficacite necessite des hypotheses de calcul. Nous demontrons que ce n'est pas le cas en construisant des protocoles efficaces a partir de ces outils de base. Cette these est constitue de quatre articles rediges en collaboration avec d'autres chercheurs. Ceci constitue la partie mature de ma recherche et sont mes contributions principales au cours de cette periode de temps. Dans le premier ouvrage presente dans cette these, nous etudions la capacite de mise en gage des canaux bruites. Nous demontrons tout d'abord une limite inferieure stricte qui implique que contrairement au transfert inconscient, il n'existe aucun protocole de taux constant pour les mises en gage de bit. Nous demontrons ensuite que, en limitant la facon dont les engagements peuvent etre ouverts, nous pouvons faire mieux et meme un taux constant dans certains cas. Ceci est fait en exploitant la notion de cover-free families . Dans le second article, nous demontrons que pour certains problemes, il existe un echange entre robustesse, la validite et le prive. Il s'effectue en utilisant le partage de secret veriable, une preuve a divulgation nulle, le concept de fantomes et une technique que nous appelons les balles et les bacs. Dans notre troisieme contribution, nous demontrons qu'un grand nombre de protocoles dans la litterature basee sur des hypotheses de calcul peuvent etre instancies a partir d'une primitive appelee Transfert Inconscient Veriable, via le concept de Transfert Inconscient Generalise. Le protocole utilise le partage de secret comme outils de base. Dans la derniere publication, nous counstruisons un protocole efficace avec un nombre constant de rondes pour le calcul a deux parties. L'efficacite du protocole derive du fait qu'on remplace le coeur d'un protocole standard par une primitive qui fonctionne plus ou moins bien mais qui est tres peu couteux. On protege le protocole contre les defauts en utilisant le concept de privacy amplication .

Arrière-plan: les cellules tumorales circulantes (CTC) sont détectables dans de nombreux cancers et peuvent être utiles cliniquement pour le pronostic de la maladie, pour mesurer la récidive et pour prédire la sensibilité aux medicaments chimiothérapeutiques. Au cours des dernières années, l’études des CTC dans de nombreux cancers tels que le cancer du sein, du poumon, du côlon et de la prostate a grandement évolué. Alternativement, il y peu d'études à ce sujet concernant le cancer du col de l’utérus (CCU). Objectifs: Notre objectif est d’optimiser le processus d'enrichissement des CTC dans le CCU et la détection moléculaire des biomarqueurs E6 et E7. Matériel et Méthodes: Dans l’optique de mimer la présence de CTC dans le sang, nous avons dilué des cellules cancéreuses CaSki VPH16-positif provenant d’un CCU dans du sang humain prélevé sur des volontaires sains. Les CaSki ont été collectées suite à une centrifugation par densité avec le Ficoll, la lyse des globules rouges (RBC) et la lyse des RBC combinée avec un enrichissement positif et négatif à l’aide de marqueurs de surface cellulaire. Les CTC ont été détectées par la mesure d’expression des oncogènes E6 et E7 du virus du papillome humain (VPH), de la cytokératine 19 (CK19) et de la cycline p16INK4 en utilisant la technique quantitative en temps réel de Reverse Transcriptase-Polymerase Chain Reaction (qRT-PCR). Pour valider notre méthode de détection des CTC in vivo, nous avons recruté dix patientes atteintes d’un CCU VPH16 positif et six contrôles sains. Résultats: Dans le modèle de dilutions de cellules CaSki, la lyse des RBC seule ou combinée avec l'enrichissement négatif ou positif suggèrent des limites de détection de 1 CTC par mL de sang pour tous les biomarqueurs moléculaires utilisés. La sensibilité de détection est accrue lors de l'utilisation de l’enrichissement positif et négatif en réduisant le bruit de fond causé par les monocytes sanguins. Contrairement aux oncogènes E6 et E7, les marqueurs CK19 et p16INK4A ont été détectés chez des individus sains, les niveaux d'expression de base appropriés doivent donc être déterminés avec précision par rapport aux patientes CCU. Le gradient de densité par Ficoll a une limite de détection de seulement environ 1000 cellules par mL de sang. Enfin, les CTC ont été détectées dans 2/10 patientes en utilisant le marqueur CK19. Cependant, ces patientes étaient négatives pour les oncogènes E6/E7. Le marqueur p16INK4A était exprimé au même niveau dans tous les échantillons (CCU et normaux). Conclusion: Notre étude suggère que les oncogènes E6 et E7 du VPH16 sont les marqueurs biologiques les plus sensibles et spécifiques en qRT-PCR pour détecter les CTC dans le modèle de dilution de cellules de CCU dans le sang. Chez les patientes atteintes d’un CCU de stade précoce, seulement CK19 a révélé la présence potentielle de CTC, ce qui suggère que ces cellules sont rares à ce stade de la maladie. La reutilización de las aguas residuales es una parte esencial del uso sostenible del agua. Sin embargo, las actuales plantas de tratamiento no están preparadas para tratar determinados compuestos como los llamados “contaminantes emergentes”. Los hábitos de consumo actuales estan generando una serie de residuos o microcontaminantes que hace tan solo unos años no existian. Entre esas nuevas sustancias aparecen los PPCP’s (Pharmaceuticals & Personal Care Products) que son un amplio grupo de compuestos químicos utilizados en veterinaria, prácticas agrícolas, salut humana y cosmetología. El mayor problema que presentan estas substancias en la actualidad es el parcial desconocimiento sobre sus efectos, la ausencia de reglamentaciones que determinen las concentraciones máximas admisibles en los cauces de vertido, la reducción en procesos de depuración convencionales, así como los tratamientos específicos aplicables a su eliminación. Los objetivos del presente proyecto són: estudiar las tipologías de PPCP’s y sus influencias para la salud humana y el medio ambiente, estudiar los diferentes sistemas de tratamiento de aguas existentes en plantas depuradoras y evaluar su eficiencia en la depuración de PPCP

En aquesta tesi s'han estudiat les propietats antitumorals d'una variant de la ribonucleasa pancreàtica humana anomenada PE5 que incorpora un senyal de localització nuclear. Aquest estudi mostra que PE5 indueix l'apoptosi de les cèl·lules tractades i que aquesta mort és independent de l'activitat de p53. A més, l'efecte citotòxic no es veu afectat per un fenotip de resistència a múltiples drogues. Les dades també mostren que l'activitat citotòxica de PE5 és selectiva per a cèl·lules tumorals in vitro i que la capacitat citotòxica de les dues ribonucleases és semblant. S'ha estudiat l'efecte d'aquestes dues ribonucleases sobre el cicle cel·lular, l'activació de diferents caspases i l'expressió de proteïnes relacionades amb l'apoptosi i el cicle cel·lular. Els resultats indiquen que PE5 i l'onconasa maten les cèl·lules a través de mecanismes diferents. A més, PE5 però no l'onconasa, redueix l'acumulació de glicoproteïna-P en dues línies cel·lulars resistents a múltiples drogues.

Introduction: This study investigated the inhibition of the antimicrobial activity of sodium hypochlorite (NaOCl) by bovine serum albumin (BSA). The killing of Enterococcus faecalis, Candida albicans, Staphylococcus epidermidis, and Escherichia coil by NaOCl in concentrations from 2% to 0.03% was measured in the presence of BSA in concentrations between 6.7% and 0.1%. Methods: NaOCl, BSA, and microorganism suspensions were mixed, and, after 30 seconds, 6 minutes, and 30 minutes, samples were taken and NaOCl was inactivated by 5% sodium thiosulphate. The microbes were incubated in tryptic soy broth broth for up to 7 days for the detection of growth. Results: All microorganisms were killed within 30 seconds by 0.03% NaOCl when BSA was not present. High concentrations of BSA significantly reduced the antimicrobial activity of NaOCl against the four species. Conclusions: The inhibition of sodium hypochlorite by BSA was directly dependent on their quantitative relationships. The result partly explains the poorer performance in vivo of NaOCl as compared to in vitro experiments. (J Endod 2010;36:268-271)

Essential oils are good candidates for the substitution of conventional medicinal treatments. Many articles and patents for their use have been published in recent years. The most attractive aspects of using essential oils as medicaments are their natural source and rapid permeability. Besides permeability, the solubility behavior of a drug is a key determinant of its oral bioavailability. Based on these characteristics, the aim of this study was to synthesize an essential oil derivative compound, using the raw oil extracted from Syzygium aromaticum L., without previous purification. The Eugenol molecular modification may diminish the problems of water solubility and bioavailability. The Eugenyl acetate molecule was characterized and its molecular modification investigated, including its structural properties and stereochemistry. This study was performed applying techniques, such as carbon-13 nuclear magnetic resonance spectroscopy (C-13 NMR), X-ray crystallographic analysis (XRD), powder X-ray diffraction (PXRD) and microscopic recording.
