OBJECTIVES: : To evaluate the outcome after Hartmann's procedure (HP) versus primary anastomosis (PA) with diverting ileostomy for perforated left-sided diverticulitis. BACKGROUND: : The surgical management of left-sided colonic perforation with purulent or fecal peritonitis remains controversial. PA with ileostomy seems to be superior to HP; however, results in the literature are affected by a significant selection bias. No randomized clinical trial has yet compared the 2 procedures. METHODS: : Sixty-two patients with acute left-sided colonic perforation (Hinchey III and IV) from 4 centers were randomized to HP (n = 30) and to PA (with diverting ileostomy, n = 32), with a planned stoma reversal operation after 3 months in both groups. Data were analyzed on an intention-to-treat basis. The primary end point was the overall complication rate. The study was discontinued following an interim analysis that found significant differences of relevant secondary end points as well as a decreasing accrual rate (NCT01233713). RESULTS: : Patient demographics were equally distributed in both groups (Hinchey III: 76% vs 75% and Hinchey IV: 24% vs 25%, for HP vs PA, respectively). The overall complication rate for both resection and stoma reversal operations was comparable (80% vs 84%, P = 0.813). Although the outcome after the initial colon resection did not show any significant differences (mortality 13% vs 9% and morbidity 67% vs 75% in HP vs PA), the stoma reversal rate after PA with diverting ileostomy was higher (90% vs 57%, P = 0.005) and serious complications (Grades IIIb-IV: 0% vs 20%, P = 0.046), operating time (73 minutes vs 183 minutes, P < 0.001), hospital stay (6 days vs 9 days, P = 0.016), and lower in-hospital costs (US $16,717 vs US $24,014) were significantly reduced in the PA group. CONCLUSIONS: : This is the first randomized clinical trial favoring PA with diverting ileostomy over HP in patients with perforated diverticulitis.

Metalworking fluid-associated hypersensitivity pneumonitis (MWF-HP) is a pulmonary disease caused by inhaling microorganisms present in the metalworking fluids used in the industrial sector. Mycobacterium immunogenum is the main etiological agent. Among the clinical, radiological and biological tools used for diagnosis, serological tests are important. The aim of this study was to identify immunogenic proteins in M. immunogenum and to use recombinant antigens for serological diagnosis of MWF-HP. Immunogenic proteins were detected by two-dimensional Western blot and candidate proteins were identified by mass spectrometry. Recombinant antigens were expressed in Escherichia coli and tested by enzyme-linked immunosorbent assay (ELISA) with the sera of 14 subjects with MWF-HP and 12 asymptomatic controls exposed to M. immunogenum. From the 350 spots visualized by two-dimensional gel electrophoresis with M. immunogenum extract, 6 immunogenic proteins were selected to be expressed as recombinant antigens. Acyl-CoA dehydrogenase antigen allowed for the best discrimination of MWF-HP cases against controls with an area under the receiver operating characteristics (ROC) curve of 0.930 (95% CI=0.820-1), a sensitivity of 100% and a specificity of 83% for the optimum threshold. Other recombinant antigens correspond to acyl-CoA dehydrogenase FadE, cytosol aminopeptidase, dihydrolipoyl dehydrogenase, serine hydroxymethyltransferase and superoxide dismutase. This is the first time that recombinant antigens have been used for the serodiagnosis of hypersensitivity pneumonitis. The availability of recombinant antigens makes it possible to develop standardized serological tests which in turn could simplify diagnosis, thus making it less invasive.

As computer chips implementation technologies evolve to obtain more performance, those computer chips are using smaller components, with bigger density of transistors and working with lower power voltages. All these factors turn the computer chips less robust and increase the probability of a transient fault. Transient faults may occur once and never more happen the same way in a computer system lifetime. There are distinct consequences when a transient fault occurs: the operating system might abort the execution if the change produced by the fault is detected by bad behavior of the application, but the biggest risk is that the fault produces an undetected data corruption that modifies the application final result without warnings (for example a bit flip in some crucial data). With the objective of researching transient faults in computer system’s processor registers and memory we have developed an extension of HP’s and AMD joint full system simulation environment, named COTSon. This extension allows the injection of faults that change a single bit in processor registers and memory of the simulated computer. The developed fault injection system makes it possible to: evaluate the effects of single bit flip transient faults in an application, analyze an application robustness against single bit flip transient faults and validate fault detection mechanism and strategies.

The significance of the Brianconnais domain in the Alpine orogen is reviewed in the light of data concerning its collision with the active Adriatic margin and the passive Helvetic margin. The Brianconnais which formerly belonged to the Iberian plate, was located on the northern margin of the Alpine Tethys (Liguro-Piemont ocean) since its opening in the early-Middle Jurassic. Together with the Iberian plate the Brianconnais terrane was separated from the European plate in the Late Jurassic-Early Cretaceous, following the northern Atlantic, Bay of Biscay, Valais ocean opening. This was accompanied by the onset of subduction along the northern margin of Adria and the closure of the Alpine Tethys. Stratigraphic and metamorphic data regarding this subduction and the geohistory of the Brianconnais allows the scenario of subduction-obduction processes during the Late Cretaceous-early Tertiary in the eastern and western Alps to be specified. HP-LT metamorphism record a long-lasting history of oceanic subduction-accretion, followed in the Middle Eocene by the incorporation of the Brianconnais as an exotic terrane into the accretionary prism. Middle to Late Eocene cooling ages of the Brianconnais basement and the presence of pelagic, anorogenic sedimentation lasting until the Middle Eocene on the Brianconnais preclude any sort of collision before that time between this domain and the active Adria margin or the Helvetic margin. This is confirmed by plate reconstructions constrained by magnetic anomalies in the Atlantic domain. Only a small percentage of the former Brianconnais domain was obducted, most of the crust and lithospheric roots were subducted. This applies also to domains formerly belonging to the southern Alpine Tethys margin (Austroalpine-inner Carpathian domain). It is proposed that there was a single Palaeogene subduction zone responsible for the Alpine orogen formation (from northern Spain to the East Carpathians), with the exception of a short-lived Late Cretaceous partial closure of the Valais ocean. Subduction in the western Tethyan domain originated during the closure of the Meliata ocean during the Jurassic incorporating the Austroalpine-Carpathian domain as terranes during the Cretaceous. The subduction zone propagated into the northern margin of Adria and then to the northern margin of the Iberian plate, where it gave birth to the Pyrenean-Provencal orogenic belt. This implies the absence of a separated Cretaceous subduction zone within the Austro-Carpathian Penninic ocean. Collision of Iberia with Europe forced the subduction to jump to the SE margin of Iberia in the Eocene, creating the Apenninic orogenic wedge and inverting the vergence of subduction from south- to north-directed. There are several experimental models describing in vivo eosinophil (EO) migration, including ip injection of a large volume of saline (SAL) or Sephadex beads (SEP). The aim of this study was to investigate the mechanisms involved in the EO migration in these two models. Two consecutive injections of SAL given 48 hr apart, induced a selective recruitment of EO into peritoneal cavity of rats, which peaked 48 hr after the last injection. SEP, when injected ip, promoted EO accumulation in rats. The phenomenom was dose-related and peaked 48 hr after SEP injection. To investigate the mediators involved in this process we showed that BW A4C, MK 886 and dexamethasone (DXA) inhibited the EO migration induced by SAL and SEP. To investigate the source of the EO chemotactic factor we showed that mast cells, macrophages (MO), but not lymphocytes, incubated in vitro in presence of SAL released a factor which induced EO migration. With SEP, only mast cells release a factor that induced EO migration, which was inhibited by BW A4C, MK 886 and DXA. Furthermore, the chemotactic activity of SAL-stimulated mast cells was inhibited by antisera against IL-5 and IL-8 (interleukin). SAL-stimulated MO were only inhibited by anti-IL-8 antibodies as well SEP-stimulated mast cells. These results suggest that the EO migration induced by SAL may be dependent on resident mast cells and MO and mediated by LTB4, IL-5 and IL-8. SEP-induced EO migration was dependent on mast cells and may be mediated by LTB4 and IL-8. Furthermore, IL-5 and IL-8 induced EO migration, which was also dependent on resident cells and mediated by LTB4 . In conclusion, EO migration induced by SAL is dependent on mast cells and MO, whereas that induced by SEP is dependent on mast cells alone. Stimulated mast cells release LTB4, IL-5 and IL-8 while MO release LTB4 and IL-8. The IL-5 and IL-8 release by the SAL or SEP-stimulated resident cells may act in an autocrine fashion, thus potentiating LTB4 release.

There are several experimental evidences that nitric oxide (NO) is involved in the microbicidal activity of macrophages against a number of intracellular pathogens including Leishmania major, Trypanozoma cruzi, Toxoplasma gondii. It is also well known that eosinophils (EO) have microbicidal activity against many parasites such as Schistosoma mansoni, Trichinella spiralis, T. cruzi and L. amazonensis. The purpose of this study was to investigate if NO is involved in the microbicidal activity of EO against L. major. Eosinophils harvested from peritoneal cavity of rats released spontaneously after 24 and 48 hr a small amount of nitrite. This release was enhanced by the treatment of cells with IFN-gamma (200 IU/ml). This release was blocked by addition of the NO synthase inhibitor, L-NIO (100 mu M) into the culture. To determinate the leishmanicidal activity of eosinophils the parasites were incubated with activated eosinophils with IFN-gamma and the ability of surviving parasites to incorporate [³H]thymidine was evaluated. IFN-gamma-activated eosinophils were able to kill L. major and to release high levels of nitrite. The ability to destroy L. major and the release of NO were completely blocked by L-NIO. These results indicate that activated eosinophils release NO which is involved in the microbicidal activity of these cells against L. major.

Introduction: Isocyanates are sensitizing chemicals used in various industries such as polyurethane foam production or paint-related purposes. Acting as haptens recognized by T-lymphocytes, they can cause allergic asthma and rarely hypersensitivity pneumonitis (HP). We aim to present a case report of acute HP due to hexamethylene diisocyanate (HDI) in a paint quality controller, a profession not generally considered at a high risk for work-related Isocyanates exposure. Case report: A 30-yr-old otherwise healthy female, light smoker working as a paint quality controller developed shortness of breath, malaise, sweating and chills at workplace six hours after handling a HDI-based hardener. Upon admission to emergency department, symptoms had progressed to severe respiratory failure. HR computer tomography (HRCT) showed bilateral ground-glass attenuation without pleural effusion. Rapid clinical and radiological improvement occurred under facial oxygen supply and systemic steroid therapy. Occupational medicine investigations revealed regular handling of HDI using latex gloves without respiratory protection. Assessment at workplace showed insufficient air renewal (1.5 times per hour), inadequate local aspiration and HDI exposure at levels of 1-4.25 ppb/m3 (Swiss Occupation Exposure Limit 5 ppb/m3). Biological monitoring after identical work procedure executed by a co-worker showed HDI exposure (5.1 micrograms hexamethylene diamine/g creatinine). Resumption of work was disadvised because of the life-threatening event. Discussion: The diagnosis of occupational HP is highly supported by classical findings on imagery and typical symptoms occurring within approved latency interval, associated with rapid clinical improvement. Although neither broncho-alveolar lavage nor specific IgG diagnosis (en route) were performed during the acute episode, various blood tests managed to rule out evidence of an infection or autoimmune disease. Other causes of HP seem unlikely as the patient did not have any recurrence of symptoms since absence from work. Workplace evaluation provided significant information on HDI exposure and allowed substantial recommendations to diminish Isocyanate exposure for the 20 still healthy laboratory co-workers. Although the entryways (air or skin) and precise mechanism of toxicity remain unclear, the present case clearly shows that Isocyanates may trigger acute HP in susceptible workers in a profession not generally considered at a high risk.

The Zermatt-Saas Fee Zone (ZSZ) in the Western Alps consists of multiple slices of ultramafic, mafic and metasedimentary rocks. They represent the remnants of the Mesozoic Piemonte-Ligurian oceanic basin which was subducted to eclogite facies conditions with peak pressures and temperatures of up to 20-28 kbar and 550-630 °C, followed by a greenschist overprint during exhumation. Previous studies, emphasizing on isotopie geochronology and modeling of REE-behavior in garnets from mafic eclogites, suggest that the ZSZ is buildup of tectonic slices which underwent a protracted diachronous subduction followed by a rapid synchronous exhumation. In this study Rb/Sr geochronology is applied to phengite included in garnets from metasediments of two different slices of the ZSZ to date garnet growth. Inclusion ages for 2 metapelitic samples from the same locality from the first slice are 44.25 ± 0.48 Ma and 43.19 ± 0.32 Ma. Those are about 4 Ma older than the corresponding matrix mica ages of respectively 40.02 ± 0.13 Ma and 39.55 ± 0.25 Ma. The inclusion age for a third calcschist sample, collected from a second slice, is 40.58 ± 0.24 Ma and the matrix age is 39.8 ± 1.5 Ma. The results show that garnet effectively functioned as a shield, preventing a reset of the Rb/Sr isotopie clock in the included phengites to temperatures well above the closure of Sr in mica. The results are consistent with the results of former studies on the ZSZ using both Lu/Hf and Sm/Nd geochronology on mafic eclogites. They confirm that at least parts of the ZSZ underwent close to peak metamorphic HP conditions younger than 43 m.y. ago before being rapidly exhumed about 40 m.y. ago. Fluid infiltration in rocks of the second slice occurred likely close to the peak metamorphic conditions, resulting in rapid growth of garnets. Similar calcschists from the same slice contain two distinct types of porphyroblast garnets with indications of multiple growth pulses and resorption indicated by truncated chemical zoning patterns. In-situ oxygen isotope Sensitive High Resolution Ion Microprobe (SHRIMP) analyses along profiles on central sections of the garnets reveal variations of up to 5 %o in individual garnets. The complex compositional zoning and graphite inclusion patterns as well as the variations in oxygen isotopes correspond to growing under changing fluid composition conditions caused by external infiltrated fluids. The ultramafic and mafic rocks, which were subducted along with the sediments and form the volumetrically most important part of the ZSZ, are the likely source of those mainly aqueous fluids. - La Zone de Zermatt-Saas Fee (ZZS) est constituée de multiples écailles de roches ultramafiques, mafiques et méta-sédimentaires. Cette zone, qui affleure dans les Alpes occidentales, représente les restes du basin océanique Piémontais-Ligurien d'âge mésozoïque. Lors de la subduction de ce basin océanique à l'Eocène, les différentes roches composant le planché océanique ont atteint les conditions du faciès éclogitique avec des pressions et des températures maximales estimées entre 20 - 28 kbar et 550 - 630 °C respectivement, avant de subir une rétrogression au faciès schiste vert pendant l'exhumation. Différentes études antérieures combinant la géochronologie isotopique et la modélisation des mécanismes gouvernant l'incorporation des terres rares dans les grenats des éclogites mafiques, suggèrent que la ZZS ne correspond pas à une seule unité, mais est constituée de différentes écailles tectoniques qui ont subi une subduction prolongée et diachrone suivie d'une exhumation rapide et synchrone. Afin de tester cette hypothèse, j'ai daté, dans cette étude, des phengites incluses dans les grenats des méta-sédiments de deux différentes écailles tectoniques de la ZZS, afin de dater la croissance relative de ces grenats. Pour cela j'ai utilisé la méthode géochronologique basée sur la décroissance du Rb87 en Sr87. J'ai daté trois échantillons de deux différentes écailles. Les premiers deux échantillons proviennent de Triftji, au nord du Breithorn, d'une première écaille dont les méta-sédiments sont caractérisés par des bandes méta-pélitiques à grenat et des calcschistes. Le troisième échantillon a été collectionné au Riffelberg, dans une écaille dont les méta-sédiments sont essentiellement des calcschistes qui sont mélangés avec des roches mafiques et des serpentinites. Ce mélange se trouve au-dessus de la grande masse de serpentinites qui forment le Riffelhorn, le Trockenersteg et le Breithorn, et qui est connu sous le nom de la Zone de mélange de Riffelberg (Bearth, 1953). Les inclusions dans les grenats de deux échantillons méta-pélitiques de la première écaille sont datées à 44.25 ± 0.48 Ma et à 43.19 ± 0.32 Ma. Ces âges sont à peu près 4 Ma plus vieux que les âges obtenus sur les phengites provenant de la matrice de ces mêmes échantillons qui donnent des âges de 40.02 ± 0.13 Ma et 39.55 ± 0.25 Ma respectivement. Les inclusions de phengite dans les grenats appartenant à un calcschiste de la deuxième écaille ont un âge de 40.58 ± 0.24 Ma alors que les phengites de la matrice ont un âge de 39.8 ± 1.5 Ma. Pour expliquer ces différences d'âge entre les phengites incluses dans le grenat et les phengites provenant de la matrice, nous suggérons que la cristallisation de grenat ait permis d'isoler ces phengites et de les préserver de tous rééquilibrage lors de la suite du chemin métamorphique prograde, puis rétrograde. Ceci est particulièrement important pour expliquer l'absence de rééquilibrage des phengites dans des conditions de températures supérieures à la température de fermeture du système Rb/Sr pour les phengites. Les phengites en inclusions n'ayant pas pu être datées individuellement, nous interprétons l'âge de 44 Ma pour les inclusions de phengite comme un âge moyen pour l'incorporation de ces phengites dans le grenat. Ces résultats sont cohérents avec les résultats des études antérieures de la ZZS utilisant les systèmes isotopiques de Sm/Nd et Lu/Hf sur des eclogites mafiques. ils confirment qu'aux moins une partie de la ZZS a subi des conditions de pression et de température maximale il y a moins de 44 à 42 Ma avant d'être rapidement exhumée à des conditions métamorphiques du faciès schiste vert supérieur autour de 40 Ma. Cette étude détaillée des grenats a permis, également, de mettre en évidence le rôle des fluides durant le métamorphisme prograde. En effet, si tous les grenats montrent des puises de croissance et de résorption, on peut distinguer, dans différents calcschists provenant de la deuxième écaille, deux types distincts de porphyroblast de grenat en fonction de la présence ou non d'inclusions de graphite. Nous lions ces puises de croissances/résorptions ainsi que la présence ou l'absence de graphite en inclusion dans les grenats à l'infiltration de fluides dans le système, et ceci durant tous le chemin prograde mais plus particulièrement proche et éventuellement peu après du pic du métamorphisme comme le suggère l'âge de 40 Ma mesuré dans les inclusions de phengites de l'échantillon du Riffelberg. Des analyses in-situ d'isotopes d'oxygène réalisé à l'aide de la SHRIMP (Sensitive High Resolution Ion Microprobe) dans des coupes centrales des grenats indiquent des variations jusqu'à 5 %o au sein même d'un grenat. Les motifs de zonations chimiques et d'inclusions de graphite complexes, ainsi que les variations du δ180 correspondent à une croissance de grenat sous des conditions de fluides changeantes dues aux infiltrations de fluides externes. Nous lions l'origine de ces fluides aqueux aux unités ultramafiques et mafiques qui ont été subductés avec les méta-sédiments ; unités ultramafiques et mafiques qui forment la partie volumétrique la plus importante de la ZZS.

Anopheles darlingi is the most important Brazilian malaria vector, with a widespread distribution in the Amazon forest. Effective strategies for vector control could be better developed through knowledge of its genetic structure and gene flow among populations, to assess the vector diversity and competence in transmitting Plasmodium. The aim of this study was to assess the genetic diversity of An. darlingi collected at four locations in Porto Velho, by sequencing a fragment of the ND4 mitochondrial gene. From 218 individual mosquitoes, we obtained 20 different haplotypes with a diversity index of 0.756, equivalent to that found in other neotropical anophelines. The analysis did not demonstrate significant population structure. However, haplotype diversity within some populations seems to be over-represented, suggesting the presence of sub-populations, but the presence of highly represented haplotypes complicates this analysis. There was no clear correlation among genetic and geographical distance and there were differences in relation to seasonality, which is important for malarial epidemiology.

Based on the results of comparative analyses of 1,039 specimens of several progenies of Anopheles nuneztovarifrom three localities in Colombia, eight costal wing spot patterns were observed. Patterns I and III were the most frequent: 77.96% and 11.36%, respectively. Using the diagnostic characters ratio of the length of the basal dark area of hind tarsomere II/length of hind tarsomere II, ratio of the length of the humeral pale spot/length of the pre-humeral dark spot, and the ratio of the length of the subcostal pale spot/length of the distal sector dark spot (DS-III2/Ta-III2, HP/PHD, SCP/DSD) approximately 5% of the adult females were misidentified as a species of Nyssorhynchus, different from An. nuneztovari. Approximately 5% of the specimens showed DS-III2/Ta-III2 ratio less than 0.25 (range 0.21 - 0.24), and among them 3.34% shared a HP/PHD ratio less than 1.50. Consequently, 1.52% of An. nuneztovari individuals can be misidentified as Anopheles oswaldoi. In those specimens with the DS-III2/Ta-III2 ratios higher than 0.25, 34.45% displayed SCP/DSD values greater than 0.50 and of these, 3.65% displayed HP/PHD values greater than 1.8. This combination of characters could lead one to misidentify samples of An. nuneztovari as Anopheles rangeli. Similarly, 2.43% of the females could be identified erroneously as either Anopheles aquasalis or Anopheles benarrochi. Individuals with a HP/PHD ratio greater than 2.0, could be misidentified as Anopheles trinkae, Anopheles strodei or Anopheles evansae. A distinct combination of diagnostic characters for An. nuneztovari from Colombia is proposed.

This study investigated the sandfly fauna of two quartzite caves in the Espinhaço Mountain Range, located in the municipality of Diamantina, state Minas Gerais. From August 2010-July 2011, entomological sampling was performed in the caves of Salitre and Monte Cristo with two HP light traps exposed in the photic and aphotic zones of each cave. The sandfly fauna consisted of 17 species, among which Lutzomyia cipoensis was predominant (54.76%). The male/female ratio in the total captures was 1:2.5. The aphotic zone showed the highest frequency of specimens captured (65%). A greater density of sandflies occurred during the summer (January-February), coinciding with the period of higher temperatures, humidity and rainfall. The presence of Lutzomyia longipalpis, Lutzomyia whitmani and Lutzomyia pessoai, proven or suspected vectors of leishmaniasis, is of concern because the area is visited by many tourists.

The diagnosis of single-lesion paucibacillary leprosy remains a challenge. Reviews by expert dermatopathologists and quantitative polymerase chain reaction (qPCR) results obtained from 66 single-plaque biopsy samples were compared. Histological findings were graded as high (HP), medium (MP) or low (LP) probability of leprosy or other dermatopathy (OD). Mycobacterium leprae-specific genes were detected using qPCR. The biopsies of 47 out of 57 clinically diagnosed patients who received multidrug therapy were classified as HP/MP, eight of which were qPCR negative. In the LP/OD (n = 19), two out of eight untreated patients showed positive qPCR results. In the absence of typical histopathological features, qPCR may be utilised to aid in final patient diagnosis, thus reducing overtreatment and delay in diagnosis.

Hypersensitivity pneumonitis (HP) is an immunologically mediated lung disease due to the repetitive inhalation of antigens. Most new cases arise from residential exposures, notably to birds, and are thus more difficult to recognise. The present authors report a 59-yr-old male who complained of dyspnoea and cough while being treated with amiodarone. Pulmonary function tests revealed restriction and obstruction with low diffusing lung capacity for carbon monoxide and partial pressure of oxygen. A high-resolution computed tomography chest scan and bronchoalveolar lavage showed diffuse bilateral ground-glass attenuation and lymphocytic alveolitis, respectively. Initial diagnosis was amiodarone pulmonary toxicity, but because of a rapidly favourable evolution, this diagnosis was questioned. A careful environmental history revealed a close contact with lovebirds shortly before the onset of symptoms. Precipitins were strongly positive against lovebird droppings, but were negative against other avian antigens. The patient was diagnosed with hypersensitivity pneumonitis to lovebirds. Avoidance of lovebirds and steroid treatment led to rapid improvement. The present observation identifies a new causative agent for hypersensitivity pneumonitis and highlights the importance of a thorough environmental history and of searching for precipitins against antigens directly extracted from the patient's environment. These two procedures should allow a more precise classification of some cases of pneumonitis, and thus might avoid progression of active undiagnosed hypersensitivity pneumonitis to irreversible fibrosis or emphysema.
