El recanvi de vàlvula aòrtica transcateter (TAVI) és un tècnica quirúrgica mínimanent invasiva que permet tractar l´estenosis aòrtica severa. El present estudi tracta de descriure de forma retrospectiva les característiques perioperatories dels pacients sotmesos a TAVI, així com els resultats obtinguts i complicacions. S'ha realitzat un estudi comparatiu entre abordatge transapical i transfemoral, i entre pacients amb Euroscore&30% i &30%. Amb això pretenem descriure i valorar l'experiència i resultats d'aquest tipus d'intervencions en el nostre centre, i comparar-la amb els resultats obtinguts per altres grups de treball

La rigidez arterial es un parámetro establecido dentro de la valoración del riesgo cardio-vascular. Actualmente disponemos de múltiples métodos para determinar la velocidad de onda de pulso (VOP) y entre ellos los más utilizados son los métodos mecánicos, como el Complior o el Sphigmocor, que precisan de dispositivos específicos y una inversión importante en aprendizaje y tiempo de exploración. La utilización del doppler guiado por ecografía 2D es una buena alternativa a estos métodos. Realizamos un estudio comparativo para evaluar la fiabilidad del eco-doppler en la medida de la VOP carótida-femoral respecto al método Complior. Se estudió la VOP carótida-femoral utilizando los dos sistemas en 40 pacientes, mostrando ambos métodos muy buena concordancia (R=0’91). En el análisis de reproducibilidad de la VOP por doppler no se hallaron diferencias intra e interobservador. En base a los resultados, el estudio demuestra que la ecografía doppler puede ser usada para medir la VOP aórtica de manera fiable y reproducible, similar otros métodos más establecidos.

RESULTADOS: Se intervino un 60,7% de los pacientes de SV aórtica, un 25,7% de SV mitral , un 13,6% de doble SV. Además, en el 22,5% se realizó revascularización coronaria. Las mujeres presentaban más edad, SV mitral, reintervención, fibrilación auricular, anemia y peor clase funcional. Los varones presentaron más tabaquismo, SV aórtica, enfermedad coronaria, cirugía de revascularización coronaria, función sistólica deprimida y tiempo de clampaje aórtico. En el análisis bivariado relacionado con mortalidad hospitalaria (8,35%) fueron significativos la edad, el sexo, la clase funcional, la posición de la prótesis, la superficie corporal, la anemia, el número de concentrados de hematíes y el tiempo de clampaje aórtico. Se constató como factores de riesgo independientes el sexo (OR 2,92; 95% intervalo confianza 1,05-8,14) y la anemia (OR 4,23; 95% intervalo confianza 1,66-10,8) tras ajustarlos con los factores de riesgo, factores confusores y el año de intervención. CONCLUSIONES: El sexo femenino es un factor de riesgo independiente para la mortalidad hospitalaria en la cirugía de sustitución valvular.

PURPOSE. To evaluate potential risk factors for the development of multiple sclerosis in Brazilian patients. METHOD. A case control study was carried out in 81 patients enrolled at the Department of Neurology of the Hospital da Lagoa in Rio de Janeiro, and 81 paired controls. A standardized questionnaire on demographic, social and cultural variables, and medical and family history was used. Statistical analysis was performed using descriptive statistics and conditional logistic regression models with the SPSS for Windows software program. RESULTS. Having standard vaccinations (vaccinations specified by the Brazilian government) (OR=16.2; 95% CI=2.3-115.2), smoking (OR=7.6; 95% CI=2.1-28.2), being single (OR=4.7; 95% CI=1.4-15.6) and eating animal brain (OR=3.4; 95% CI=1.2-9.8) increased the risk of developing MS. CONCLUSIONS. RESULTS of this study may contribute towards better awareness of the epidemiological characteristics of Brazilian patients with multiple sclerosis.

Transcatheter aortic valve implantation is an expanding procedure thus far restricted to a target population of old and high-comorbidity patients with symptomatic aortic stenosis. The need for bulky devices (up to 24F) combined with the high prevalence of peripheral vascular disease in these patients explains the increased risk of vascular complications in transfemoral Edwards Sapien (Edwards Lifesciences, Irvine, Calif) transcatheter aortic valve implantation procedures, with a rate of 20% for the transfemoral arm of either the Placement of AoRTic traNscathetER valves in the European Union (PARTNER EU) trial or the SOURCE Registry.1,2

BACKGROUND Transcatheter aortic valve-in-valve implantation is an emerging therapeutic alternative for patients with a failed surgical bioprosthesis and may obviate the need for reoperation. We evaluated the clinical results of this technique using a large, worldwide registry. METHODS AND RESULTS The Global Valve-in-Valve Registry included 202 patients with degenerated bioprosthetic valves (aged 77.7±10.4 years; 52.5% men) from 38 cardiac centers. Bioprosthesis mode of failure was stenosis (n=85; 42%), regurgitation (n=68; 34%), or combined stenosis and regurgitation (n=49; 24%). Implanted devices included CoreValve (n=124) and Edwards SAPIEN (n=78). Procedural success was achieved in 93.1% of cases. Adverse procedural outcomes included initial device malposition in 15.3% of cases and ostial coronary obstruction in 3.5%. After the procedure, valve maximum/mean gradients were 28.4±14.1/15.9±8.6 mm Hg, and 95% of patients had ≤+1 degree of aortic regurgitation. At 30-day follow-up, all-cause mortality was 8.4%, and 84.1% of patients were at New York Heart Association functional class I/II. One-year follow-up was obtained in 87 patients, with 85.8% survival of treated patients. CONCLUSIONS The valve-in-valve procedure is clinically effective in the vast majority of patients with degenerated bioprosthetic valves. Safety and efficacy concerns include device malposition, ostial coronary obstruction, and high gradients after the procedure.

Dilatation of the ascending aorta (AAD) is a prevalent aortopathy that occurs frequently associated with bicuspid aortic valve (BAV), the most common human congenital cardiac malformation. The molecular mechanisms leading to AAD associated with BAV are still poorly understood. The search for differentially expressed genes in diseased tissue by quantitative real-time PCR (qPCR) is an invaluable tool to fill this gap. However, studies dedicated to identify reference genes necessary for normalization of mRNA expression in aortic tissue are scarce. In this report, we evaluate the qPCR expression of six candidate reference genes in tissue from the ascending aorta of 52 patients with a variety of clinical and demographic characteristics, normal and dilated aortas, and different morphologies of the aortic valve (normal aorta and normal valve n = 30; dilated aorta and normal valve n = 10; normal aorta and BAV n = 4; dilated aorta and BAV n = 8). The expression stability of the candidate reference genes was determined with three statistical algorithms, GeNorm, NormFinder and Bestkeeper. The expression analyses showed that the most stable genes for the three algorithms employed were CDKN1β, POLR2A and CASC3, independently of the structure of the aorta and the valve morphology. In conclusion, we propose the use of these three genes as reference genes for mRNA expression analysis in human ascending aorta. However, we suggest searching for specific reference genes when conducting qPCR experiments with new cohort of samples.

OBJECTIVE To study the factors associated with choice of therapy and prognosis in octogenarians with severe symptomatic aortic stenosis (AS). STUDY DESIGN Prospective, observational, multicenter registry. Centralized follow-up included survival status and, if possible, mode of death and Katz index. SETTING Transnational registry in Spain. SUBJECTS We included 928 patients aged ≥80 years with severe symptomatic AS. INTERVENTIONS Aortic-valve replacement (AVR), transcatheter aortic-valve implantation (TAVI) or conservative therapy. MAIN OUTCOME MEASURES All-cause death. RESULTS Mean age was 84.2 ± 3.5 years, and only 49.0% were independent (Katz index A). The most frequent planned management was conservative therapy in 423 (46%) patients, followed by TAVI in 261 (28%) and AVR in 244 (26%). The main reason against recommending AVR in 684 patients was high surgical risk [322 (47.1%)], other medical motives [193 (28.2%)], patient refusal [134 (19.6%)] and family refusal in the case of incompetent patients [35 (5.1%)]. The mean time from treatment decision to AVR was 4.8 ± 4.6 months and to TAVI 2.1 ± 3.2 months, P < 0.001. During follow-up (11.2-38.9 months), 357 patients (38.5%) died. Survival rates at 6, 12, 18 and 24 months were 81.8%, 72.6%, 64.1% and 57.3%, respectively. Planned intervention, adjusted for multiple propensity score, was associated with lower mortality when compared with planned conservative treatment: TAVI Hazard ratio (HR) 0.68 (95% confidence interval [CI] 0.49-0.93; P = 0.016) and AVR HR 0.56 (95% CI 0.39-0.8; P = 0.002). CONCLUSION Octogenarians with symptomatic severe AS are frequently managed conservatively. Planned conservative management is associated with a poor prognosis.

A 49-year-old woman, without known cardiovascular risk factors. Hoarseness of voice caused by a paralysis of left vocal cord. She was admitted to hospital because of acute coronary syndrome, associated to resuscitated cardiac arrest (asystolia documented) without later neurology sequels. Physical examination was anodyne. Echocardiographic study demonstrated a compatible image with a large left sinus of Valsalva aneurysm (SVA) (Panel A) and mild aortic regurgitation. Cardiac catheterization confirmed the presence of left SVA (Panel B) that produced extrinsic compression of the left main coronary artery (Panels C and D). Repair surgery was made by means of closing the aneurysmal orifice with a patch of dacron. Intra-operatory echocardiographic control study found severe aortic regurgitation, so valvular replacement with 19 mm mechanical prosthesis and extension of the valve annulus with patch of dacron was performed, associated with bypass with safena vein graft to left coronary artery. SVA is a very infrequent cardiac anomaly, generally with silent clinical course until it ruptures. Myocardial ischaemia caused by coronary artery compression is unusual. We described the case of a patient diagnosed of left SVA, whose initial clinical manifestation was the appearance of resuscitated sudden cardiac death in the context of an acute coronary syndrome.

OBJECTIVE To evaluate immediate transcatheter aortic valve implantation (TAVI) results and medium-term follow-up in very elderly patients with severe and symptomatic aortic stenosis (AS). METHODS This multicenter, observational and prospective study was carried out in three hospitals. We included consecutive very elderly (> 85 years) patients with severe AS treated by TAVI. The primary endpoint was to evaluate death rates from any cause at two years. RESULTS The study included 160 consecutive patients with a mean age of 87 ± 2.1 years (range from 85 to 94 years) and a mean logistic EuroSCORE of 18.8% ± 11.2% with 57 (35.6%) patients scoring ≥ 20%. Procedural success rate was 97.5%, with 25 (15.6%) patients experiencing acute complications with major bleeding (the most frequent). Global mortality rate during hospitalization was 8.8% (n = 14) and 30-day mortality rate was 10% (n = 16). Median follow up period was 252.24 ± 232.17 days. During the follow-up period, 28 (17.5%) patients died (17 of them due to cardiac causes). The estimated two year overall and cardiac survival rates using the Kaplan-Meier method were 71% and 86.4%, respectively. Cox proportional hazard regression showed that the variable EuroSCORE ≥ 20 was the unique variable associated with overall mortality. CONCLUSIONS TAVI is safe and effective in a selected population of very elderly patients. Our findings support the adoption of this new procedure in this complex group of patients.

Cystatin C is considered the most important physiological inhibitor of endogenous cysteine proteases; the role of cystatin C is believed to be to modulate the activity of proteases secreted or released from damaged cells or in the process of necrosis, therefore cystatins being fundamental regulatory processes and a potential prevention of local proteolytic damage. Antiphospholipid antibodies are used to clarify the diagnosis of diseases like multiple sclerosis (MS) and other pathologies could present similar symptoms or paraclinical findings. The objective of the present work is to analyze the concentration of cystatin C and the presence or absence of antiphospholipid antibodies in patients diagnosed with relapsing remitting multiple sclerosis (RRMS) as markers of demyelization. This work was carried out jointly by the Vascular Risk Laboratory, the Laboratory of Autoimmunity and Multiple Sclerosis Unit, Hospital Universitario Virgen Macarena in Seville in one year. Two types of people were selected: Group 1 (n = 30) RRMS group and a control group, n = 30. Cystatin C and antiphospholipid antibodies IgG and IgM, IgG and IgM anticardiolipin, β2 glycoprotein IgG and IgM were determined. Patients showed negative titers of antiphospholipid antibodies IgG and IgM, IgG and IgM anticardiolipin, β2 glycoprotein IgG and IgM. Cystatin C concentration is lower in the group of patients diagnosed with MS, which could give rise to a decrease in the modulation of endogenous cysteine proteases. This would exacerbate the progress of demyelization in MS.

El presente trabajo muestra los datos preliminares de una estudio de cohortes prospectivo unicéntrico que pretende comparar el daño neurológico asociado a dos intervenciones cardiacas para el tratamiento de la estenosis aórtica severa. Concretamente se analiza la aparición de lesiones isquémica agudas cerebrales detectadas por RM tras los dos procedimientos y su posible asociación con alteraciones del estado neurocognitivo en la evolución. La presentación actual solo muestra los datos preliminares de los resultados de la RM cerebral. En el apartado métodos se describe también como se realizó la valoración del estado neurocognitivo, no obstante, los resultados de estas valoraciones y su posible correlación con las lesiones en la RM cerebral aún no estan analizados y por lo tanto no se presentan.

O tratamento das malformações venosas pode ser realizado por ressecção cirúrgica ou esclerose farmacológica. Este estudo mostra os resultados de 12 pacientes com angiodisplasia submetidos a múltiplas aplicações diretas de etanol na lesão, após exame clínico e angiografia venosa percutânea. Dez pacientes referiram melhora dos sintomas e diminuição do tamanho da lesão. Todos os pacientes queixaram-se de ardência durante a aplicação, que cessou no máximo em 24 horas. As complicações encontradas foram três casos de úlceras pequenas na pele e um caso de hiperemia no membro tratado, associada a linfonodomegalia dolorosa. Concluiu-se que o tratamento escleroterapêutico das malformações venosas com infusão de pequenos volumes de etanol apresenta as vantagens de ser um tratamento seguro, eficaz, simples, fácil e rápido, de baixo custo, bem tolerado e com poucas complicações.

Apresentamos um caso de uma criança com dois anos e oito meses de idade,acometida pela forma recessiva da displasia craniometafisária. São enfatizados os achados clínico-radiológicos, bem como seus principais diagnósticos diferenciais, baseados em dados de breve revisão literária.
