Nas últimas décadas, Enterococcus resistentes à vancomicina tem se destacado no cenário hospitalar em todo mundo. A emergência da resistência à vancomicina no Estado do Rio de Janeiro foi registrada em 2000, na espécie Enterococcus faecalis. Em seguida, Enterococcus faecium passou a ser prevalente. O objetivo deste estudo foi avaliar a diversidade genética das amostras de E. faecium sensíveis (VSEfm) e resistentes (VREfm) à vancomicina, isoladas em diferentes instituições de saúde do Estado do Rio de Janeiro no período de 1993 a 2008. As amostras bacterianas foram avaliadas por metodologia de eletroforese em campo pulsado (PFGE), após restrição com Smal, análise do polimorfismo numérico de segmentos repetitivos (MLVA) e tipagem por sequenciamento de múltiplos loci (MLST); além da detecção de marcadores fenotípicos, como a resistência à ampicilina e à ciprofloxacina, e genotípicos, como determinantes genéticos de virulência, que estão vinculados a um complexo clonal globalmente disperso (CC17). A diversidade de Tn1546 (que alberga o conjunto gênico vanA) foi determinada por amplificação e restrição com ClaI. Um grupo clonal prevalente foi observado pela metodologia de PFGE e agrupou amostras isoladas, principalmente, no período de 2004 a 2006, estando disseminado por diversas instituições de saúde do Estado, indicando transmissão inter- e intra-hospitalar. A avaliação por MLVA identificou dois MTs prevalentes dentre as amostras VREfm: MT12 relacionado à maioria das amostras dos principais grupos clonais identificados por PFGE e, o MT159 que apresentou frequência aumentada no ano de 2008. A análise por MLST destacou o ST78 associado aos principais grupos clonais definidos por PFGE, bem como, ao MT12. Também foi observada correlação entre o ST412 associado ao grupo clonal formado por apenas amostras de 2008 e o MT159. Foi notório por MLST que as amostras VREfm do Estado do Rio de Janeiro, no período do estudo, estiveram relacionadas CC17, juntamente com algumas amostras VSEfm. Entretanto, os principais marcadores de resistência e de virulência, característicos de CC17, estiveram mais relacionados as amostras VREfm do que as VSEfm, como resistência à ampicilina e à ciprofloxacina; e a presença do gene esp e do alelo purK1. A identificação de VSEfm pertencentes ao CC17 sugerem que amostras já adaptadas ao ambiente hospitalar, podem ter adquirdo o determinante de resistência vanA, facilitando a emergência de amostras de VREfm. Adicionalmente, nossos dados sugerem uma modificação no cenário de amostras VREfm no Rio de Janeiro. Pois a dominância de um grupo clonal prevalente (PFGE- X; MT12; ST78) pode estar sendo substituída pela possível emergência de um novo grupo clonal, que foi característico de amostras mais recentes (PFGE-XV; MT159; ST412), apontando assim, para um novo curso na epidemiologia dos E. faecium no Estado. Um perfil de restrição prevalente de Tn1546 idêntico ao protótipo foi observado, apontando que a disseminação da resistência à vancomicina ocorreu por também por transferência horizontal. Entretanto, alterações do tipo deleção e presença de elementos de inserção com aproximadamente 2 kb foram observadas em um número reduzido das amostras. A incongruência no genótipo vanA, em relação ao fenótipo, foi observado para uma amostra. Considera-se fundamental o acompanhamento da disseminação de VREfm, particularmente diante da elevada frequência de amostras pertencentes a um complexo clonal que conjuga multirresistência com virulência, com elevado potencial de adaptabilidade e disseminação.

Amplified fragment length polymorphisms (AFLP) were used to study the inheritance of shell color in Argopecten irradians. Two scallops, one with orange and the other with white shells, were used as parents to produce four F-1 families by selfing and outcrossing. Eighty-eight progeny, 37 orange and 51 white, were randomly selected from one of the families for segregation and mapping analysis with AFLP and microsatellite markers. Twenty-five AFLP primer pairs were screened, yielding 1138 fragments, among which 148 (13.0%) were polymorphic in two parents and segregated in progeny. Six AFLP markers showed significant (P < 0.05) association with shell color. All six loci were mapped to one linkage group. One of the markers, F1f335, is completely linked to the gene for orange shell, which we designated as Orange1, without any recombination in the progeny we sampled. The marker was amplified in the orange parent and all orange progeny, but absent in the white parent and all the white progeny. The close linkage between F1f335 and Orange1 was validated using bulk segregation analysis in two natural populations, and all our data indicate that F1f335 is specific for the shell color gene, Orange1. The genomic mapping of a shell color gene in bay scallop improves our understanding of shell color inheritance and may contribute to the breeding of molluscs with desired shell colors.

Caballeria liewi Lim, 1995, uses adhesive secretions from the head organs and posterior secretory systems to assist in locomotion and attachment. Ultrastructural investigations show that the head organs of C. liewi consist of three pairs of antero-lateral pit-like openings bearing microvilli and ducts leading from two types of uninucleated gland cells (located lateral to the pharynx), one type producing rod-like (S1) bodies with an electron-dense matrix containing less electron-dense vesicles and the second type producing oval (S2) bodies with a homogeneous electron-dense matrix. Interlinking band-like structures are observed between S1 bodies and between S2 bodies. S1 body is synthesised in the granular endoplasmic reticulum, transported to a Golgi complex to be packaged into vesicles and routed into ducts for exudation. The synthesis of the S2 body is unresolved. Haptoral secretions manifested externally as net-like structures are derived from dual electron-dense (DED) secretory body produced in the peduncular gland cells. The DED body consists of a less electron-dense oval core in a homogeneous electron-dense matrix. On exocytosis into the pyriform haptoral reservoir, DED bodies are transformed into a secretion with two types of inclusions (less electron-dense oval and electron-dense spherical inclusions) in an electron-dense matrix. The secretions are further transformed (as small, oval, electron-dense bodies) when transported to the superficial anchor grooves, and on exudation into the gill tissues, the secretions become an electron-dense matrix. Secretory bodies associated with uniciliated structures, anchor sleeves and marginal hooks are also observed.

Purpose: To describe sequential phacoemulsification-intraocular lens (IOL) implantation-posterior capsulorhexis-anterior vitrectomy in the management of phakic malignant glaucoma. Methods: Twenty consecutive patients (25 eyes) with phakic malignant glaucoma were enrolled at the Zhongshan Ophthalmic Center, Sun Yat-sen University. All patients underwent phacoemulsification, IOL implantation and posterior capsulorhexis together with anterior vitrectomy via a clear corneal paracentesis. Visual acuity, intraocular pressure (IOP), anterior chamber depth (ACD), surgical complications and medications required after the surgery were recorded. Results: After surgery, the mean LogMAR visual acuity and ACD increased significantly (visual acuity from -1.56 ± 1.17 to -0.54 ± 0.81, p < 0.001; ACD from 0.367 ± 0.397 mm to 2.390 ± 0.575 mm, p < 0.001), and mean IOP decreased significantly (from 39.6 ± 10.6 mm Hg to 14.5 ± 4.1 mmHg, p < 0.001). No serious perioperative complications occurred, and only five eyes required topical glaucoma medications after surgery. Conclusion: Combined phacoemulsification-IOL implantation-posterior capsulorhexis-anterior vitrectomy surgery is a safe and effective method for treating patients with phakic malignant glaucoma. © 2012 The Authors. Realizou-se um estudo das alturas faciais anterior e posterior em 79 crianças brasileiras, sendo 46 do gênero feminino e 33 do masculino, com idades de 8 a 11 anos, portadoras de má oclusão Classe I de Angle, na fase de dentadura mista, não submetidas a tratamento ortodôntico. Esta investigação utilizou duas radiografias cefalométricas em norma lateral, de cada criança, com intervalo de 8 a 16 meses entre uma radiografia e outra, para comparação dos dados cefalométricos. Foram estudadas as proporções faciais anteriores, ou seja, a relação entre as dimensões N-Me (AFT), N-ENA (AFS), ENA-Me (AFI) e o relacionamento entre as dimensões Ar-GO (AFP) e a distância entre o plano palatino ao mento (AFA) , isto é, o Índice da Altura Facial (IAF). Os resultados obtidos, possibilitaram constatar que as tendências de crescimento foram equilibradas em todas as situações estudadas; a dimensão altura facial inferior, contribuiu de forma mais significativa, nas alterações observadas na altura facial total; as proporções faciais anteriores se mantiveram em torno de 42% para a altura facial superior e 58% para a altura facial inferior; o índice da altura facial ou se manteve constante em torno de 0,66 ou apresentou uma tendência a aumentar com a idade; não ocorreu dimorfismo sexual com relação às proporções faciais e índice da altura facial.

As mordidas cruzadas anteriores e posteriores podem ser tratadas desde as dentaduras decídua e mista. Embora a literatura descreva diversas técnicas para a correção individualizada desses problemas, o tratamento de pacientes com as duas alterações é pouco documentado. O presente artigo relata o tratamento desses problemas com o expansor tipo Haas associado a molas digitais. Essa alternativa apresenta boa previsibilidade, fácil confecção e instalação, e ótima eficiência.

ln order to predict the facial growth using Jarahak's analysis, cephalometric radiographies of 120 subjects during the mixed dentition and between 6 and 10 years of age were studied. From the total, 60 subjects (30 males and 30 females) were classified as Angle's Class I and 60 subjects (30 males and 30 females) were classified as Angle's Class II, Division 1. AIl subjects did not receive any orthodontic treatment. The proportion between anterior and posterior facial heigth (SGo-NMe) was studied, which is determined by the formula PFH x 100/ AFH = %, neither being straight (> 62% < 65), clockwise ( > 58% < 62%) and anti-clockwise ( > 65% - 80%). Mean average, standard deviation and facial height proportion (SGo - NMe) were determined for males and females according to Angle's classification of malocc!usion. Sexual dimorphism was examined with regard to different types of Angle's malocclusion and the direction of the facial growth, and the correlation of four cephalometrics measurements were analvsed

Physicians treating patients with posterior circulation strokes (PCS) tended to debate more on whether or not to introduce anticoagulation rather than performing investigations to identify stroke aetiology, as in patients with anterior circulation strokes (ACS). Recent findings suggest that stroke aetiologies of PCS and ACS are more alike than dissimilar, suggesting that PCS deserve the same investigations as ACS. The characteristics and current diagnostic evaluation between patients with PCS and ACS were compared.

Intravenous thrombolysis is an approved treatment for anterior (ACS) and posterior (PCS) circulation stroke. However, no randomized controlled trial has investigated safety and efficacy of intravenous thrombolysis according to stroke territory, although PCS is assumed to differ from ACS in many ways. We aimed to compare the safety and clinical outcome of intravenous thrombolysis applied to patients with PCS and ACS.

The relative advantages of cruciate retaining or cruciate resecting total knee replacement are still controversial. If the posterior cruciate ligament (PCL) is preserved, it should be properly balanced. In a previous study, it was demonstrated that increasing the flexion gap leads to an anterior translation of the tibia relative to the femur. Based on these results, we hypothesized that cutting the PCL increases the flexion gap and lessens anterior tibial translation.

INTRODUCTION: Urogenital prolapse is a very common condition in women with a prevalence of 30%. If conservative therapy fails or is not desired by the patient, prolapse repair is usually performed under general or regional anaesthetic. The aim of the study was to evaluate feasibility, efficacy and functional outcome after fascial prolapse repairs under local anaesthetic (LA). PATIENTS AND METHODS: Between November 1999 and December 2000, 130 consecutive patients presenting with anterior or posterior prolapse or both were invited to have their procedure performed under LA. All patients with a symptomatic minimum stage II prolapse were included. Prior to surgery all women completed a standardized questionnaire examining the specific and non-specific symptoms of prolapse and their situation was classified using the ICS Pelvic Organ Prolapse (POP-Q) system. Follow up was 30 months. Objective success was defined as a stage 1 or less and no symptoms of bulge, subjective success was defined as lack of specific or non-specific symptoms of prolapse. RESULTS: There were 128 patients who agreed to have their operations performed under LA: 68 in the anterior group, 52 in the posterior group and 8 with a combined anterior and posterior repair. Objective cure rate was 88% for posterior repair, 87% for anterior repair and 63% for combined repair. Success rates were no different in primary from recurrent cases. There were no intraoperative complications and operating time was 21 min (anterior repair) or 23 min (posterior repair). There was no de novo postoperative urinary or stool incontinence and all patients but two would have the operation performed again under the same circumstances. The two remaining refused due to embarrassment but for no other reason. CONCLUSION: Local anaesthetic prolapse repair is feasible and effective in middle term results. It is well accepted by the patients who benefit from less side effects and short hospital stay.
