Work-related stress of the oncology clinician is not only due to heavy clinical and administrative duties, but also arises when breaking bad news. However, there is important interindividual variation in stress levels during patient encounters, mainly due to the significance the situation represents for the oncologist. A reflection on his own development, his professional identity, and ways of dealing with the patient's suffering can reduce his levels of stress and distress and prevent burnout and other psychiatric disturbances. This chapter summarizes the psychological challenges the oncology clinician is facing when he announces the diagnosis of cancer, deals with the deception of relapse, discusses the transition to palliative care, copes with progression of the disease and uncertainty, and cares for the dying who is facing the unknown. Ways of refiecting on and dealing with these situations from a psychological and communicational perspective are described and illustrated by case vignettes.

En aquest treball ens hem proposat els segents objectius: Explicitar els valors tics que permeten lexcellncia professional dels jutges; Conixer lestat de la qesti, esbrinar la conscincia que els futurs professionals tenen dels valors requerits per dur a terme una funci de qualitat i daquells que ja lhan exercit durant 8 anys; Proposar mesures organitzatives per promoure i dinamitzar lexcellncia professional dels jutges. El treball consta de dues parts. En una primera expliquem qu s tica professional i explicitem aquells valors que necessriament ha dencarnar un jutge en el seu exercici professional perqu aquest sigui, ms enll de no negligent i acomplir la deontologia, una aposta per lexcellncia. Al llarg daquesta part albirem poltiques organitzatives, recomanacions, suggeriments (codis tics, comit dtica professional, etc.) per dinamitzar, des de lautoregulaci, la vigncia dels valors tics de les professions jurdiques que afavoreixen lexcellncia professional. En una segona part, es presenten les conclusions de lestudi de camp a partir duna enquesta sobre aquells valors realitzada, el 10 de febrer del 2004, als jutges que estan a lEscola Judicial de Barcelona rebent la formaci inicial. Amb aquesta segona part exposem el grau de conscincia i comproms en la responsabilitat inherents a la professi de jutge.

Introduction.The new demands of a fast changing world necessitate expanding the traditional concepts of nursing, extending the classical aspects to cover new areas. Purpose. Based on their professional duties, the nursing team in the pharmacy of a second level hospital aimed to establish a theoretical and situational framework for nurses working in the central services. Material and Methods. Application of the nursing process to nursing work in an area with no direct contact with patients. Results and Discussion. The application of the NANDA diagnoses to professional practice enabled the establishment of a nursing diagnosis with the implementation of measures designed to overcome a stressful situation with a risk of becoming unmotivated. Main Conclusion. The capacity to adapt the nursing profession to undertake new roles in the field of healthcare and the power of nursing own methodological resources permit the indirect care of faceless patients to be complemented with the inclusion of nurses from other services as clients, forming the focus of care, who can thus be helped with their daily care work.

PRINCIPLES: International guidelines for heart failure (HF) care recommend the implementation of inter-professional disease management programmes. To date, no such programme has been tested in Switzerland. The aim of this randomised controlled trial (RCT) was to test the effect on hospitalisation, mortality and quality of life of an adult ambulatory disease management programme for patients with HF in Switzerland.METHODS: Consecutive patients admitted to internal medicine in a Swiss university hospital were screened for decompensated HF. A total of 42 eligible patients were randomised to an intervention (n = 22) or usual care group (n = 20). Medical treatment was optimised and lifestyle recommendations were given to all patients. Intervention patients additionally received a home visit by a HF-nurse, followed by 17 telephone calls of decreasing frequency over 12 months, focusing on self-care. Calls from the HF nurse to primary care physicians communicated health concerns and identified goals of care. Data were collected at baseline, 3, 6, 9 and 12 months. Mixed regression analysis (quality of life) was used. Outcome assessment was conducted by researchers blinded to group assignment.RESULTS: After 12 months, 22 (52%) patients had an all-cause re-admission or died. Only 3 patients were hospitalised with HF decompensation. No significant effect of the intervention was found on HF related to quality of life.CONCLUSIONS: An inter-professional disease management programme is possible in the Swiss healthcare setting but effects on outcomes need to be confirmed in larger studies.

BACKGROUND AND OBJECTIVES: Urinary steroid profiling is used in doping controls to detect testosterone abuse. A testosterone over epitestosterone (T/E) ratio exceeding 4.0 is considered as suspicious of testosterone administration, irrespectively of individual heterogeneous factors such as the athlete's ethnicity. A deletion polymorphism in the UGT2B17 gene was demonstrated to account for a significant part of the interindividual variability in the T/E between Caucasians and Asians. Here, the variability of urinary steroid profiles was examined in a widely heterogeneous cohort of professional soccer players. Method: The steroid profile of 57 Africans, 32 Asians, 50 Caucasians and 32 Hispanics was determined by gas chromatography-mass spectrometry. RESULTS: Significant differences have been observed between all ethnic groups. After estimation of the prevalence of the UGT2B17 deletion/deletion genotype (African: 22%; Asian: 81%; Caucasian: 10%; Hispanic: 7%), ethnic-specific thresholds were developed for a specificity of 99% for the T/E (African: 5.6; Asian: 3.8; Caucasian: 5.7; Hispanic: 5.8). Finally, another polymorphism could be hypothesised in Asians based on specific concentration ratio of 5alpha-/5beta-androstane-3alpha,17beta-diol in urine. CONCLUSION: These results demonstrate that a unique and non-specific threshold to evidence testosterone misuse is not fit for purpose. An athlete's endocrinological passport consisting of a longitudinal follow-up together with the ethnicity and/or the genotype would strongly enhance the detection of testosterone abuse. Finally, additional genotyping studies should be undertaken to determine whether the remaining unexplained disparities have an environmental or a genetic origin.

El projecte que sollicitem s el resultat natural duna recerca iniciada lany 2008 pel grup de recerca MIRAS (Mediaci i Interpretaci: Recerca en lmbit Social) de la Universitat Autnoma de Barcelona. La recerca del grup MIRAS ha tingut com a objectiu general, durant els darrers dos anys, la definici del perfil professional del traductor/intrpret als serveis pblics. Aquesta figura emergent i encara poc reconeguda al nostre pas va comenar a sorgir a partir del boom migratori de les darreres dcades a Catalunya com a resposta als problemes de comunicaci freqents entre la poblaci nouvinguda i els serveis pblics locals. Per a assolir lobjectiu esmentat, la primera fase de la recerca del grup MIRAS sha centrat en el disseny, distribuci, buidatge, anlisi i creuament de dades de 250 qestionaris semioberts dirigits als tres principals agents implicats en la interacci comunicativa que estudiem: els traductors/intrprets, els usuaris i els provedors dels serveis. Els resultats daquest estudi han demostrat, entre daltres qestions rellevants, la manca duna formaci especfica per als traductors/intrprets als serveis pblics, aix com una absncia de regulaci de la professi que aquests exerceixen. Un cop realitzat aquest primer estudi pilot, i en vista dels resultats obtinguts, ens sembla doncs evident que el segent pas de la nostra recerca ha danar orientat cap a la formaci i regulaci professional dels traductors/intrprets als serveis pblics. En concret, en el projecte que sollicitem ens proposem dissenyar una formaci especialitzada per a traductors/intrprets als serveis pblics que compleixi amb els segents tres prerequisits: a) estar oberta a tot tipus de persones, independentment del seu nivell destudis; b) oferir un ampli ventall de llenges, principalment les llenges de la immigraci a Catalunya (per a una primera selecci daquestes llenges, vegeu informe demogrfic de la SIM de l1 de gener de 2010 i resultats de lestudi pilot de MIRAS), i c) conduir a una acreditaci nacional que reguli lexercici de la professi de traductor/intrpret als serveis pblics de Catalunya. La implementaci dun sistema dacreditaci, segon objectiu del projecte que presentem, s essencial ja que no noms permetr la regulaci duna professi fins avui poc o mal definida, sin que tamb garantir la qualitat dels serveis oferts per aquests professionals en el futur.

Leducaci dels immigrants s un tema prioritari a les agendes poltiques de molts pasos de la OCDE. En molts casos, els governs shan preocupat especialment de la seva integraci al mn del treball per creix linters en els seus resultats al sistema educatiu i en la revisi de les poltiques adreades a respondre a les seves necessitats educatives i formatives. La majoria daquests estudis es situen a lesfera de leducaci infantil, primria, secundaria i formaci professional, per aix resulta prcticament impossible trobar estudis que analitzin el baix percentatge daccs destudiants immigrants extracomunitaris a la universitat (al voltant de 3,3% en el cas de Catalunya); que contribueixin a entendre els factors que configuren les trajectries dxit escolar i dintegraci laboral dels estudiants immigrants que accedeixen i completen els estudis universitaris; que puguin donar pautes per desenvolupar poltiques educatives que millorin els resultats daprenentatge dels estudiants immigrants; i que puguin servir com a mirall i incentiu per a altres persones immigrants i, perqu no, tamb del pas. Aquest projecte ha estudiat el conjunt delements que condicionen laccs dels joves immigrants als estudis universitaris, i a lestatus laboral que els hauria de possibilitar la seva formaci universitria. Lestudi ha consistit en una metaanlisi dels documents existents sobre el tema i en la realitzaci de 8 narratives biogrfiques; quatre de persones cursant diferents estudis a les universitats catalanes i quatre que ja shan incorporat al mn del professional. Aquestes narratives venen acompanyades de 8 vdeos que exploren noves maneres de visibilitzaci daquesta poblaci i es transformen en una prctica dautoria. Aquest material visual pot servir com a recurs educatiu, en la mida que pugui ser un mirall i un incentiu per a altres persones immigrants i del pas. Finalment, presenta un seguit de recomanacions per als responsables de les poltiques i dels centres educatius.

Trata-se de um estudo descritivo, desenvolvido junto a dezesseis equipes de Estratgia de Sade da Famlia de Santa Maria (RS), que objetivou identificar os trabalhadores com a Sndrome de Burnout e as variveis associadas a este distrbio. A amostra foi composta por 86 trabalhadores, representando 86,3% dos profissionais (mdico, enfermeiro, tcnico de enfermagem, odontlogo e auxiliar de consultrio dentrio) e 30,2% dos agentes comunitrios de sade, os quais responderam ao Maslach Inventory Burnout. A idade mdia do grupo foi de 36,949,3 anos, com predominncia do sexo feminino (84,9%). A maioria possui companheiro (68,2%), tem filhos (69,4%), trabalha, em mdia, 3,381,9 anos na equipe e no realiza atividades fsicas (62,8%). Identificaram-se seis trabalhadores (6,9%) com a Sndrome de Burnout, a qual teve associao estatstica significativa (p= 0,034) com a varivel idade jovem. Os mais jovens obtiveram escores superiores nas subescalas de desgaste emocional e despersonalizao do Inventrio de Burnout.

Enfermeiros Residentes podem experimentar desgaste fsico e emocional decorrentes do cotidiano da Residncia de Enfermagem. O objetivo deste estudo foi determinar a incidncia do Burnout em Residentes de Enfermagem. Realizou-se um estudo exploratrio, descritivo, analtico e longitudinal-prospectivo com 16 Residentes, no perodo de dois anos. Utilizou-se o Maslach Burnout Inventory traduzido e validado para o Brasil e um instrumento de dados sociodemogrficos/ocupacionais. Dos Residentes, 17.2% mostraram valores elevados em Exausto Emocional e Despersonalizao; 18.8% comprometimento em Incompetncia/falta de Realizao Profissional, dos quais 75% pertenciam s especialidades de Pronto Socorro, Unidade de Terapia Intensiva Adulto e Peditrica. Idade e especialidade correlacionaram-se positivamente com a Incompetncia/falta de Realizao Profissional. Identificou-se um Residente de Enfermagem com alterao nas trs subescalas do Maslach Burnout Inventory, sendo caracterizado como portador da Sndrome de Burnout. Os Residentes de enfermagem possuem perfis de adoecimento. Conhecer esses fatores pode minimizar os agravos sade desse trabalhador.

En el marco del Espacio Europeo de Educacin Superior 1 se inscriben innumerables acciones que estn desarrollando todas las universidades europeas con la intencin de llegar al 2010 con un bagaje lo suficientemente importante como para afrontar este cambio con entereza y sobretodo conservando (o incrementando) la calidad de los procesos de enseanza- aprendizaje que hasta el momento han predominado. Dentro de este amplio proceso de transformacin se encuentra el diseo de los nuevos Grados que brinda la oportunidad de replantear los planes de estudios, por tanto, la organizacin de las asignaturas, estructura de los contenidos, metodologas y sistemas de evaluacin. Toda esta reflexin debe girar, a nuestro entender, alrededor de tres ncleos que se encuentran en estado deinterdependencia: los escenarios profesionales, los perfiles profesionales y las competencias que en ellos se inscriben.Para poder plantear los nuevos Grados en coherencia con la era del EEES debe hacerse un anlisis minucioso de los perfil profesionales demandados por el mercado de trabajo que, al fin y al cabo ser el destino de los profesionales que se forman en nuestras universidades, es por ello, que la tarea de definir el perfil profesional a priori del diseo de los nuevos Grados resulta una mxima para garantizar la calidad de estos. En este trabajo se presenta un ejemplo de metodologa a seguir para la definicin de un perfil profesional en Educacin Superior, concretamente, el del Ingeniero TIC mediante el Anlisis Funcional.
