Promoting and protecting the health of Iowans is the mission of the Iowa Department of Public Health (IDPH). Surveillance of notifiable health conditions is essential in establishing what, how, and when events impact the public’s health. Multiple divisions and bureaus are dedicated to accomplishing the goals of surveillance. In 2012, in addition to 850 cases reported with no lab results, there were more than 79,000 laboratory results of infectious diseases and conditions submitted to IDPH disease surveillance programs. IDPH also investigates non-infectious conditions related to lead, occupational, and environmental hazards like carbon monoxide. Approximately 100,000 blood lead test results were reported to IDPH in 2012.

Hepatocellular carcinoma (HCC) is one of the most frequent malignant tumors worldwide and its incidence has increased over the last years in most developed countries. The majority of HCCs occur in the context of liver cirrhosis. Therefore, patients with cirrhosis and those with hepatitis B virus infection should enter a surveillance program. Detection of a focal liver lesion by ultrasound should be followed by further investigations to confirm the diagnosis and to permit staging. A number of curative and palliative treatment options are available today. The choice of treatment will depend on the tumor stage, liver function and the presence of portal hypertension as well as the general condition of the patient. A multidisciplinary approach is mandatory to offer to each patient the best treatment.

Les fluides de coupe sont principalement utilisés pour leurs propriétés lubrifiantes et/ou pour leur qualité de refroidissement des pièces en métallurgie. Ils sont utilisés pour faciliter l'opération d'usinage et contribue à améliorer la durée de vie de l'outil utilisé. Ils permettent d'éloigner les copeaux de la zone de coupe, ce qui contribue à l'obtention d'un état de surface de qualité, et de refroidir le copeau et la pièce usinée afin de contribuer à la précision dimensionnelle de la pièce finie. Les usages des fluides de coupe sont en évolution depuis les années 2000. Il y a environ 15-30 ans, seules les huiles entières étaient utilisées. Aujourd'hui, l'utilisation des huiles varie selon le type d'application mais il est à noter toutefois que l'usage des huiles entières est en pleine décroissance depuis 2001, contrairement à celui des fluides aqueux (émulsions) en pleine expansion. Les fluides de coupe exposeraient selon l'étude SUMER plus de 1 million de travailleurs. Il existe deux grands secteurs principaux utilisateurs de ces fluides : le travail des métaux (qui expose selon l'INRS le plus grand nombre de salariés) et l'automobile (plus grand secteur utilisateur selon la CSNIL, Chambre Syndical Nationale de l'Industrie des Lubrifiants). De nombreux éléments existent autour de la prévention des risques liés à l'utilisation des fluides de coupe mais comme le rappelait l'INRS dans son rapport publié en 2002, la plupart des méthodes de prévention, collectives ou individuelles, bien que déjà connues, ne sont pas toujours appliquées, et notamment dans les petites entreprises. De plus les moyens de surveillance, bien que largement détaillés dans de nombreux guides, sont rarement mis en oeuvre. Néanmoins, des substances dangereuses peuvent se retrouver soit au sein de la formulation des fluides de coupe (comme certains additifs, biocides..), soit se former au cours du stockage (comme c'est le cas de pour la NDELA, N-nitroso diéthanolamine). Certaines substances ont déjà vu leur usage en tant que biocide être interdit en France, tel que le formaldéhyde ou encore l'acide borique. Des solutions de substitution existent déjà, telles que la micro-lubrification ou l'usinage à sec. Mais la mise en place ou la recherche de substituts s'accompagne souvent de difficultés en ce qui concerne l'adaptation du processus industriel. La mise en oeuvre des collaborations avec les fournisseurs doit donc être fortement encouragée. Enfin, il existe des zones d'ombre concernant les risques associés au développement microbiologique intervenant lors du vieillissement des fluides. L'utilisation grandissante de ces fluides de type aqueux est en lien avec ces problèmes de contamination microbienne des fluides ainsi que les pathologies respiratoires qui semblent y être associées. Cependant l'absence de référentiel exclut toute interprétation des résultats relatifs à ces agents microbiologiques retrouvés à la fois dans les fluides et les aérosols. Conclusions : En raison de la complexité de la formulation des fluides de coupe, il n'existe pas aujourd'hui de méthode de mesure fiable pour évaluer l'exposition à ces produits. Ainsi la mise en place d'une évaluation quantitative des risques sanitaires complète semble prématurée voire impossible. Il apparaît cependant nécessaire d'aller plus loin en matière de prévention : - une valeur limite pourrait être imposée concernant les quantités en amines secondaires précurseurs de nitrosamines, telle que développée dans le modèle allemand, le respect de cette réglementation allemande est déjà un argument mis en place par certains fournisseurs français ; - La micro-lubrification et la recherche de substitution doivent être encouragées ; - L'élaboration d'un référentiel microbiologique qui puisse tenir compte de la qualité des fluides et de la protection des travailleurs devrait être soutenue. OBJECTIVE: Few epidemiological studies have addressed the health of workers exposed to novel manufactured nanomaterials. The small current workforce will necessitate pooling international cohorts. METHOD: A road map was defined for a globally harmonized framework for the careful choice of materials, exposure characterization, identification of study populations, definition of health endpoints, evaluation of appropriateness of study designs, data collection and analysis, and interpretation of the results. RESULTS: We propose a road map to reach global consensus on these issues. The proposed strategy should ensure that the costs of action are not disproportionate to the potential benefits and that the approach is pragmatic and practical. CONCLUSIONS: We should aim to go beyond the collection of health complaints, illness statistics, or even counts of deaths; the manifestation of such clear endpoints would indicate a failure of preventive measures.

BACKGROUND: Community-acquired respiratory viral infections (RVIs) are common in lung transplant patients and may be associated with acute rejection and bronchiolitis obliterans syndrome (BOS). The use of sensitive molecular methods that can simultaneously detect a large panel of respiratory viruses may help better define their effects. METHODS: Lung transplant recipients undergoing serial surveillance and diagnostic bronchoalveolar lavages (BALs) during a period of 3 years were enrolled. BAL samples underwent multiplex testing for a panel of 19 respiratory viral types/subtypes using the Luminex xTAG respiratory virus panel assay. RESULTS: Demographics, symptoms, and forced expiratory volume in 1 sec were prospectively collected for 93 lung transplant recipients enrolled. Mean number of BAL samples was 6.2+/-3.1 per patient. A respiratory virus was isolated in 48 of 93 (51.6%) patients on at least one BAL sample. Of 81 positive samples, the viruses isolated included rhinovirus (n=46), parainfluenza 1 to 4 (n=17), coronavirus (n=11), influenza (n=4), metapneumovirus (n=4), and respiratory syncytial virus (n=2). Biopsy-proven acute rejection (> or =grade 2) or decline in forced expiratory volume in 1 sec > or =20% occurred in 16 of 48 (33.3%) patients within 3 months of RVI when compared with 3 of 45 (6.7%) RVI-negative patients within a comparable time frame (P=0.001). No significant difference was seen in incidence of acute rejection between symptomatic and asymptomatic patients. Biopsy-proven obliterative bronchiolitis or BOS was diagnosed in 10 of 16 (62.5%) patients within 1 year of infection. CONCLUSION: Community-acquired RVIs are frequently detected in BAL samples from lung transplant patients. In a significant percentage of patients, symptomatic or asymptomatic viral infection is a trigger for acute rejection and obliterative bronchiolitis/BOS.

BACKGROUND: Surveillance of multiple congenital anomalies is considered to be more sensitive for the detection of new teratogens than surveillance of all or isolated congenital anomalies. Current literature proposes the manual review of all cases for classification into isolated or multiple congenital anomalies. METHODS: Multiple anomalies were defined as two or more major congenital anomalies, excluding sequences and syndromes. A computer algorithm for classification of major congenital anomaly cases in the EUROCAT database according to International Classification of Diseases (ICD)v10 codes was programmed, further developed, and implemented for 1 year's data (2004) from 25 registries. The group of cases classified with potential multiple congenital anomalies were manually reviewed by three geneticists to reach a final agreement of classification as "multiple congenital anomaly" cases. RESULTS: A total of 17,733 cases with major congenital anomalies were reported giving an overall prevalence of major congenital anomalies at 2.17%. The computer algorithm classified 10.5% of all cases as "potentially multiple congenital anomalies". After manual review of these cases, 7% were agreed to have true multiple congenital anomalies. Furthermore, the algorithm classified 15% of all cases as having chromosomal anomalies, 2% as monogenic syndromes, and 76% as isolated congenital anomalies. The proportion of multiple anomalies varies by congenital anomaly subgroup with up to 35% of cases with bilateral renal agenesis. CONCLUSIONS: The implementation of the EUROCAT computer algorithm is a feasible, efficient, and transparent way to improve classification of congenital anomalies for surveillance and research.
