BACKGROUND: Patients with rare diseases such as congenital hypogonadotropic hypogonadism (CHH) are dispersed, often challenged to find specialized care and face other health disparities. The internet has the potential to reach a wide audience of rare disease patients and can help connect patients and specialists. Therefore, this study aimed to: (i) determine if web-based platforms could be effectively used to conduct an online needs assessment of dispersed CHH patients; (ii) identify the unmet health and informational needs of CHH patients and (iii) assess patient acceptability regarding patient-centered, web-based interventions to bridge shortfalls in care. METHODS: A sequential mixed-methods design was used: first, an online survey was conducted to evaluate health promoting behavior and identify unmet health and informational needs of CHH men. Subsequently, patient focus groups were held to explore specific patient-identified targets for care and to examine the acceptability of possible online interventions. Descriptive statistics and thematic qualitative analyses were used. RESULTS: 105 male participants completed the online survey (mean age 37 ± 11, range 19-66 years) representing a spectrum of patients across a broad socioeconomic range and all but one subject had adequate healthcare literacy. The survey revealed periods of non-adherence to treatment (34/93, 37%) and gaps in healthcare (36/87, 41%) exceeding one year. Patient focus groups identified lasting psychological effects related to feelings of isolation, shame and body-image concerns. Survey respondents were active internet users, nearly all had sought CHH information online (101/105, 96%), and they rated the internet, healthcare providers, and online community as equally important CHH information sources. Focus group participants were overwhelmingly positive regarding online interventions/support with links to reach expert healthcare providers and for peer-to-peer support. CONCLUSION: The web-based needs assessment was an effective way to reach dispersed CHH patients. These individuals often have long gaps in care and struggle with the psychosocial sequelae of CHH. They are highly motivated internet users seeking information and tapping into online communities and are receptive to novel web-based interventions addressing their unmet needs.

Els programes Leader de la Unió Europea tenen com a objectiu el desenvolupament de territoris rurals.La comarca catalana de l’Alt Urgell es va beneficiar anteriorment de l’aplicació del programa Leader 2en set municipis del sud de la comarca. L’actual convocatòria anomenada Leader Plus té en canvi unàmbit d’aplicació que afecta a la totalitat del territori de l’Alt Urgell.A més de l’àmbit territorial, també els objectius estratègics són diferents. Leader 2 pretenia corregir lesconseqüències negatives de la crisi industrial de Taurus. Per aquest motiu, el seu objectiu fonamentalconsistia en aturar la pèrdua de població del territori i va donar molta importància a la creaciód’ocupació i les inversions en la indústria. En canvi, el programa Leader Plus té com a objectiu bàsicestabilitzar la població i millorar el benestar econòmic en els nuclis rurals. Per aquesta raó prioritza lesinversions en sectors con el turisme o altres serveis que ajudin a millorar la qualitat de vida de les zonesrurals.Les conclusions d’aquest treball han servit com a referència per a la redacció de les bases que hauran deservir per a la determinació de quins tipus d’inversions es poden acollir a la iniciativa Leader Plus al’Alt Urgell i quines subvencions es poden concedir.

The fact that most of new Personal Data Assistant (PDA) devices and smartphones have the ability to communicate via different wireless technologies has made several new applications possible. While traditional network model is based on the idea of static hosts, mobile devices can create decentralized, self-organizing ad-hoc networks and act as peers in the network. This kind of adapting network is suitable for mobile devices which can freely join and leave the networks. Because several different wireless communication technologies are involved, flexible changing of the networking technology must be handled in order to enable seamless communication between these networks. This thesis presents a transparent network interface to mobile Peer-to-Peer environment which is named as Virtual PeerHood. Different wireless technologies and aspects of providing a seamless connectivity between these technologies are explored. The result is a middleware platform for mobile Peer-to-Peer environment, capable of handling several networking technologies.

During the last half decade the popularity of different peer-to-peer applications has grown tremendously. Traditionally only desktop-class computers with fixed line network connections have been powerful enough to utilize peer-to-peer. However, the situation is about to change. The rapid development of wireless terminals will soon enable peer-to-peer applications on these devices as well as on desktops. Possibilities are further enhanced by the upcoming high-bandwidth cellular networks. In this thesis the applicability and implementation alternatives of an existing peer-to-peer system are researched for two target platforms: Linux powered iPaq and Symbian OS based smartphone. The result is a peer-to-peer middleware component suitable for mobile terminals. It works on both platforms and utilizes Bluetooth networking technology. The implemented software platforms are compatible with each other and support for additional network technologies can be added with a minimal effort.
