The study undertakes a review of the Poverty Reduction Strategy Papers (PRSPs), and Interim Poverty Reduction Strategy Papers (I-PRSPs) of four selected Caribbean SIDS from the Eastern Caribbean. The countries had participated in the Poverty Reduction Strategy initiative formulated by the World Bank. The country papers reviewed were the Commonwealth of Dominica, Grenada, Saint Lucia and St. Vincent and the Grenadines. The review assesses and provides a comparative analysis of the general awareness of the notions of vulnerability and the inclusion and use of strategies that address the reduction of vulnerability and the strengthening of resilience by the countries reviewed. It also explores how vulnerability reduction strategies were applied to differing groups, as elaborated through the PRSPs.

Neste trabalho são analisadas as relações entre escolarização (configurada na Casa Familiar Rural) e as estratégias de reprodução das organizações sociais representativas do campesinato em interface com as famílias de agricultores na Transamazônica, frente pioneira de colonização no Oeste do Pará, particularmente no município de Medicilândia. Esta escola, pensada por estes agentes sociais e coletivos em um cenário nacional e regional de publicização dos quadros que fragilizam a agricultura de base camponesa, a partir de meados da década de 1990, tem sido instrumento da luta social. As tensões no espaço social, lidas como ‘crise da base’ e ‘crise dos sistemas de produção’, teriam desenhado simultaneamente uma ‘crise de formação’ na qual as finalidades da escola foram sendo construídas por desafios sócio-econômico e políticos. Este cenário teria constituído os jovens agricultores como categoria social, investidos da expectativa coletiva de tornarem-se, sob a mediação da CFR, técnicos agrícolas e/ou dirigentes, a fim de dar continuidade ao grupo (seja dos atores, nos campos das organizações sociais/sindicais e comunitário-religiosas; seja das famílias, na sucessão agrícola e na manutenção de sua posição social). As repercussões da CFR na condição camponesa destes jovens são analisadas a partir de dados qualitativos e quantitativos, tomando-se como referência os interesses e investimentos dos agentes sociais, das famílias, bem como as inserções sócio-profissionais no campo e/ou na cidade destes jovens após a escolarização. Os resultados da CFR, considerando-se esta escola como estratégia coletiva organizada que visa transformar para conservar o campo de lutas enquanto sistema de relações objetivas do grupo social que a constitui, revelam que a mesma tem possibilitado a permanência dos jovens agricultores no campo sob diversos arranjos em que se imbricam as relações com o campesinato, com a cidade, com o conhecimento escolar/técnico, e com uma ética de trabalho e relação com a terra/natureza “ambientalizada”. No âmbito dos grupos domésticos e da coletividade camponesa (nas quais se incluem as organizações representativas do grupo estudado), a posição social destes jovens caracteriza-se por formas de distinção social visíveis nas práticas sócio-produtivas intercedidas pelo capital escolar, bem como na posição de mediadores dirigentes e técnicos.

Professionals of Family Health Strategy (FHS) work in communities where there are complex medical social problems. These contexts may lead them to psychological suffering, jeopardizing their care for the users, and creating yet another obstacle to the consolidation of FHS as the primary health care model in Brazil. The study investigated the difficulties and coping strategies reported by health professionals of the FHS teams when they face medical social needs of the communities where they work. Focus groups and semi-structured interviews were carried out with 68 professionals of three primary care units in the city of Sao Paulo (Southeastern Brazil). Drug dealing and abuse, alcoholism, depression and domestic violence are the most relevant problems mentioned by the study group. Professionals reported lack of adequate training, work overload, poor working conditions with feelings of professional impotence and frustration. To overcome these difficulties, professionals reported collective strategies, particularly experience sharing during team meetings and matrix support groups. The results indicate that the difficulties may put the professionals in a vulnerable state, similar to the patients they care for. The promotion of specialized and long term support should be reinforced, as well as the interaction with the local network of services and communities leaders. That may help professionals to deal with occupational stress related to medical and social needs present in their routine work; in the end, it may as well contribute to the strengthening of FHS.

According to life-history theory age-dependent investments into reproduction are thought to co-vary with survival and growth of animals. In polygynous species, in which size is an important determinant of reproductive success, male reproduction via alternative mating tactics at young age are consequently expected to be the less frequent in species with higher survival. We tested this hypothesis in male Alpine ibex (Capra ibex), a highly sexually dimorphic mountain ungulate whose males have been reported to exhibit extremely high adult survival rates. Using data from two offspring cohorts in a population in the Swiss Alps, the effects of age, dominance and mating tactic on the likelihood of paternity were inferred within a Bayesian framework. In accordance with our hypothesis, reproductive success in male Alpine ibex was heavily biased towards older, dominant males that monopolized access to receptive females by adopting the 'tending' tactic, while success among young, subordinate males via the sneaking tactic 'coursing' was in general low and rare. In addition, we detected a high reproductive skew in male Alpine ibex, suggesting a large opportunity for selection. Compared with other ungulates with higher mortality rates, reproduction among young male Alpine ibex was much lower and more sporadic. Consistent with that, further examinations on the species level indicated that in polygynous ungulates the significance of early reproduction appears to decrease with increasing survival. Overall, this study supports the theory that survival prospects of males modulate the investments into reproduction via alternative mating tactics early in life. In the case of male Alpine ibex, the results indicate that their life-history strategy targets for long life, slow and prolonged growth and late reproduction.

STUDY QUESTION To what extent do the management of endometriosis and the symptoms that remain after treatment affect the quality of life in women with the disease? SUMMARY ANSWER Many women with endometriosis had impaired quality of life and continued to suffer from endometriosis-associated symptoms even though their endometriosis has been managed in tertiary care centres. WHAT IS KNOWN ALREADY The existing literature indicates that quality of life and work productivity is reduced in women with endometriosis. However, most studies have small sample sizes, are treatment related or examine newly diagnosed patients only. STUDY DESIGN, SIZE, DURATION A cross-sectional questionnaire-based survey among 931 women with endometriosis treated in 12 tertiary care centres in 10 countries. PARTICIPANTS/MATERIALS, SETTING, METHODS Women diagnosed with endometriosis who had at least one contact related to endometriosis-associated symptoms during 2008 with a participating centre were enrolled into the study. The study investigated the effect of endometriosis on education, work and social wellbeing, endometriosis-associated symptoms and health-related quality of life, by using questions obtained from the World Endometriosis Research Foundation (WERF) GSWH instrument (designed and validated for the WERF Global Study on Women's Health) and the Short Form 36 version 2 (SF-36v2). MAIN RESULTS AND THE ROLE OF CHANCE Of 3216 women invited to participate in the study, 1450 (45%) provided informed consent and out of these, 931 (931/3216 = 29%) returned the questionnaires. Endometriosis had affected work in 51% of the women and affected relationships in 50% of the women at some time during their life. Dysmenorrhoea was reported by 59%, dyspareunia by 56% and chronic pelvic pain by 60% of women. Quality of life was decreased in all eight dimensions of the SF-36v2 compared with norm-based scores from a general US population (all P < 0.01). Multivariate regression analysis showed that number of co-morbidities, chronic pain and dyspareunia had an independent negative effect on both the physical and mental component of the SF-36v2. LIMITATIONS, REASONS FOR CAUTION The fact that women were enrolled in tertiary care centres could lead to a possible over-representation of women with moderate-to-severe endometriosis, because the participating centres typically treat more complex and referred cases of endometriosis. The response rate was relatively low. Since there was no Institute Review Board approval to do a non-responder investigation on basic characteristics, some uncertainty remains regarding the representativeness of the investigated population. WIDER IMPLICATIONS OF THE FINDINGS This international multicentre survey represents a large group of women with endometriosis, in all phases of the disease, which increases the generalizability of the data. Women still suffer from frequent symptoms, despite tertiary care management, in particular chronic pain and dyspareunia. As a result their quality of life is significantly decreased. A patient-centred approach with extensive collaboration across disciplines, such as pain specialists, psychologists, sexologists and social workers, may be a valuable strategy to improve the long-term care of women with endometriosis. Many “workers” in north temperate colonies of the eusocial paper wasp Polistes fuscatus disappear within a few days of eclosion. We provide evidence that these females are pursuing an alternative reproductive strategy, i.e., dispersing to overwinter and become nest foundresses the following spring, instead of helping to rear brood on their natal nests. A female is most likely to stay and help at the natal nest (i.e., least likely to disperse) when it is among the first workers to emerge and when it emerges on a nest with more pupae (even though worker-brood relatedness tends to be lower in such colonies). The latter cause may result from the fact that pupae-laden nests are especially likely to survive, and thus any direct or indirect reproductive payoffs for staying and working are less likely to be lost. Disappearing females are significantly smaller than predicted if dispersal tendency was independent of body size (emergence order-controlled), suggesting that the females likely to be most effective at challenging for reproductive rights within the natal colony (i.e., the largest females) are also most likely to stay. Thus, early dispersal is conditional on a female’s emergence order, the maturity of its natal nest, and its body size. Finally, we present evidence that foundresses may actively limit the sizes of first-emerging females, perhaps to decrease the probability that the latter can effectively challenge foundresses for reproductive rights. The degree to which foundresses limit the size of first-emerging females accords well with the predictions of the theory of staying incentives.
