There are few other areas in family law where incongruence between the legal and social positions is as evident as that concerning parenthood. Recent cases involving lesbian couples and known sperm donors serve to highlight the increasing tension between the respective roles of biology, intention and functional parenting in the attribution of legal parental status. As both legislative and case-law developments have shown, intention is central in some circumstances, but not in others. The main claim of this paper is that this ad hoc approach leads to incoherent and unsatisfactory law: instead of striving to identify a status, what we are really looking to do is to identify the people who assume responsibility for a child. Drawing upon recent case-law, this paper explores how a conceptual reform of the law could result in a principled framework which would place formally recognised intention at the heart of parental status in order to reconnect legal duty with social reality for as many children and parents as possible. Moreover, it would ensure that parental status would not be dictated by the mode of conception of the child (natural or assisted). The analysis identifies the objectives of reform before proposing a new model which, while recognising the social importance of the biological parentage link, would reserve legal status for functional parenthood.

A well-known histopathological feature of diseased skin in Buruli ulcer (BU) is coagulative necrosis caused by the Mycobacterium ulcerans macrolide exotoxin mycolactone. Since the underlying mechanism is not known, we have investigated the effect of mycolactone on endothelial cells, focussing on the expression of surface anticoagulant molecules involved in the protein C anticoagulant pathway. Congenital deficiencies in this natural anticoagulant pathway are known to induce thrombotic complications such as purpura fulimans and spontaneous necrosis. Mycolactone profoundly decreased thrombomodulin (TM) expression on the surface of human dermal microvascular endothelial cells (HDMVEC) at doses as low as 2ng/ml and as early as 8hrs after exposure. TM activates protein C by altering thrombin’s substrate specificity, and exposure of HDMVEC to mycolactone for 24 hours resulted in an almost complete loss of the cells’ ability to produce activated protein C. Loss of TM was shown to be due to a previously described mechanism involving mycolactone-dependent blockade of Sec61 translocation that results in proteasome-dependent degradation of newly synthesised ER-transiting proteins. Indeed, depletion from cells determined by live-cell imaging of cells stably expressing a recombinant TM-GFP fusion protein occurred at the known turnover rate. In order to determine the relevance of these findings to BU disease, immunohistochemistry of punch biopsies from 40 BU lesions (31 ulcers, nine plaques) was performed. TM abundance was profoundly reduced in the subcutis of 78% of biopsies. Furthermore, it was confirmed that fibrin deposition is a common feature of BU lesions, particularly in the necrotic areas. These findings indicate that there is decreased ability to control thrombin generation in BU skin. Mycolactone’s effects on normal endothelial cell function, including its ability to activate the protein C anticoagulant pathway are strongly associated with this. Fibrin-driven tissue ischemia could contribute to the development of the tissue necrosis seen in BU lesions.

Existe uma preocupação crescente da sociedade com a ética, devida em parte aos numerosos escândalos que surgiram no mundo corporativo ultimamente, em particular no setor financeiro em que as repercussões de malversações têm geralmente um impacto maior para a sociedade do que podem ter quando acontecem em outro setor de atividade. Outra consequência é o interesse cada vez maior dos acadêmicos em estudar ética dos negócios. Porém, a pesar do uso frequente do termo ética e dos conceitos relacionados, não existe ainda um consenso sobre uma definição comum de ética dos negócios. Pertence a cada corporação definir o que considera ser ética dos negócios. Portanto, para entender melhor o que tem por trás do termo ética, é preciso voltar a sua definição, explicar a sua evolução, colocá-lo dentro do nosso contexto contemporâneo, para depois aplicá-lo à área de interesse. Para o setor bancário, ética se refere mais ao aspecto legal, mas também faz referência ao desenvolvimento da responsabilidade social corporativa dentro das corporações através de diferentes práticas. A literatura a respeito ressalta que a abordagem da ética pelas corporações bancárias tradicionais desconecta a ética do foco de negócio, considerando que não pertence ao centro da sua atividade. De fato, as corporações respondem aos mesmos mecanismos isomorfos institucionais, os quais as levam a adotar práticas com o objetivo de conseguir legitimidade dentro da sociedade. Ao contrário dessas corporações bancárias tradicionais, os bancos qualificados como éticos pela literatura, tem em comum o fato de estruturar as suas atividades sobre ou em volta da ética, diferenciando-se dos outros bancos por ter como principal objetivo ser ético. O banco Brasileiro Itaú Unibanco, um dos maiores bancos tradicionais do mundo em termos de valor de mercado, foi percebido aos poucos como um banco muito preocupado com a ética, e foi recentemente reconhecido como o banco mais sustentável do mundo pela revista Financial Times. O Itaú 5 Unibanco desenhou e implementou um programa de ética, o qual junta um componente teórico (com diretrizes, regras e princípios e um código de ética), usado como um suporte para o componente prático (estrutura organizacional particular e práticas cotidianas). Esse programa levou o Banco Itaú a ser campeão inovador na institucionalização da ética dentro do setor bancário.

The concept of physical education emerged in the XIX century through the European gymnastic methods, initially for the purpose of asserting the sovereignty of the nationality of countries, by means of exaltation of patriotism of the masses and the physical preparation for military wars. Nowadays, the cult of the ideal body has changed, becoming the unceasing desire within each society, according to its characteristics of established standards, most of the time denying the genetic determinations of each ethnicity. This social phenomenon has generated openness and encouragement to the fitness market on the rise, eager in supporting the aspirations of the cult of the body”, representing the huge seek of the idea of a healthy body and an aesthetic appearance acceptable in today's society. Parallel to this, the Body Awareness Gymnastics, also known as Technical or Complementary Practices, were introduced in Brazil in the 70s, presenting as main themes the body awareness, the mind-body relationship, and against the ideals of body training. According to authors, the pioneering technique of the movements of alternative practices in Brazil was Antiginástica of Thérèse Bertherat. Therefore, this study aims to harmonize the complementary body practices of the physical education area through literature review of published works by the author Thérèse Bertherat, entitled The body has its reasons, The body communication system, Stations of the body, The Tiger´s Den and When the body is consent. The work also includes the contribution of a particular reading of the researcher, from the experiences of the practices carried out in order to enrich the reporting work through their records, the results obtained from the experiential process

Bakgrund: För att vården kring ett barn ska bli så bra som möjlig är föräldrarnas närvaro avgörande. Vårdpersonalen får arbeta mycket utifrån sin egen kunskap och erfarenhet i arbetet med familjer. Målsättningen inom barnsjukvården är att involvera familjen så mycket som möjligt och utgå från varje familjs behov runt den aktuella situationen. Syftet: Studiens syfte var att utifrån ett familjeperspektiv belysa upplevelsen av att ha ett barn inlagt på sjukhus. Metod: Denna litteraturstudie innehåller sammanlagt 14 studier inom det valda området och dessa har analyserats med en innehållsanalys. Resultat: Föräldrarna upplevde att god kommunikation hade en positiv inverkan på relationen mellan familjen och vårdgivare under sjukhusvistelsen. Under sjukhusvistelsen uppstod psykiska, fysiska och sociala behov som familjen behövde få stöd med. Familjen upplevde många känslor och hela vardagen blev påverkad. Slutsats: Denna litteraturstudie indikerar att det finns ett stort behov av en personlig relation mellan sjukvårdsgivare och familj. Familjen är även i behov av få stöd från sjuksköterskan för att motverka osäkerhet hos föräldrarna. Nyckelord: Delaktighet, familj, litteraturstudie, omvårdnad, upplevelser

The presence of endothelial cell (EC)-derived surface molecules in the circulation is among hallmarks of endothelial activation and damage in vivo. Previous investigations suggest that upregulation of T-cadherin (T-cad) on the surface of ECs may be a characteristic marker of EC activation and stress. We investigated whether T-cad might also be shed from ECs and in amounts reflecting the extent of activation or damage.

Although reactive hyperemia index (RHI) predicts future coronary events, associations with intravascular ultrasound (IVUS)-assessed coronary plaque structure have not been reported. This study therefore investigated associations between RHI and IVUS-assessed coronary plaques. In 362 patients RHI was measured by noninvasive peripheral arterial tonometry and coronary plaque components (fibrous, fibrofatty, necrotic core, and dense calcium) were identified by IVUS in 594 vessel segments of the left anterior descending, circumflex, and/or right coronary arteries. RHI values <1.67 were considered abnormal. Analysis of variance was used to detect independent associations between RHI and plaque composition. Patients with an abnormal RHI had greater plaque burden (41% vs 39% in patients with normal RHI, p = 0.047). Compared to patients with normal RHI, plaque of patients with abnormal RHI had more necrotic core (21% vs 17%, p <0.001) and dense calcium (19% vs 15%, p <0.001) and less fibrous (49% vs 54%, p <0.001) and fibrofatty (11% vs 14%, p = 0.002) tissue. After adjustment for age, gender, cardiovascular risk factors, and drug therapy, abnormal RHI remained significantly associated with fibrous (F ratio 14.79, p <0.001), fibrofatty (F ratio 5.66, p = 0.018), necrotic core (F ratio 14.47, p <0.001), and dense calcium (F ratio 10.80, p = 0.001) volumes. In conclusion, coronary artery plaques of patients with abnormal RHI had a larger proportion of necrotic core and dense calcium. The association of an abnormal RHI with a plaque structure that is more prone to rupture may explain why these patients exhibit a greater risk of coronary events.

Murray's law describes the optimal branching anatomy of vascular bifurcations. If Murray's law is obeyed, shear stress is constant over the bifurcation. Associations between Murray's law and intravascular ultrasound (IVUS) assessed plaque composition near coronary bifurcations have not been investigated previously.
