Oleiomioma da uretra é uma patologia rara, estando descritos cerca de uma centena de casos na literatura. São tumores benignos, constituídos por músculo liso e surgem mais frequentemente na idade fértil.Otratamento é cirúrgico com excisão da massa, existindo contudo possibilidade de recidiva local. Apresenta-seumcaso clínico de leiomioma da uretra e realizou-se revisão da literatura.

Tal como os outros soberanos do Antigo Regime, D. João V procurava a legitimidade do Império Português a nível europeu, através dos grandes projetos arquitetónicos. O novo palácio real e a igreja patriarcal que Juavarra desenhou em 1719 para a cidade de Lisboa teve que representar e simbolizar o poder do rei. Um estudo pormenorizado sobre parte da arquitectura de Juvarra parece revelar uma estreita relação, não com a contemporânea Roma dos Papas, mas antes com a majestosa arquitectura da Roma Imperial. D. João V tinha o ensejo de ser considerado um imperador e a arquitectura de Juvarra demonstra-o claramente.

FUNDAMENTO: Documentos institucionais recomendam que variáveis hemodinâmicas - frequência cardíaca (FC) e pressão arterial sistólica (PAS) e diastólica (PAD) - sejam rotineiramente controladas na parte aeróbica de sessões de exercício supervisionado para cardiopatas. OBJETIVO: a) Determinar o comportamento e a reprodutibilidade da PA ao longo de 15 minutos de exercício de intensidade constante e moderada; b) comparar a medida de PA obtida com equipamentos digital e convencional no exercício. MÉTODOS: Trinta adultos de ambos os sexos (de 65 ± 11 anos) foram avaliados em 15 minutos no cicloergômetro de membros inferiores. A PA foi medida a cada dois minutos: entre o 3º e o 13º minutos, pelo esfigmomanômetro digital Tango (Suntech, Estados Unidos da América) e, no 14º minuto, pelo esfigmomanômetro de coluna de mercúrio. Sete dias depois e em horário similar, seis indivíduos repetiram o protocolo para avaliar a reprodutibilidade. RESULTADOS: Enquanto a PAD não variou ao longo do exercício (p > 0,05), a PAS aumentou do 3º para o 7º minuto (146±4,1 versus 158±4,5 mmHg, p < 0,05) e depois se manteve praticamente constante. As medidas digital e convencional correlacionaram-se fortemente - r = 0,83 para PAS e 0,84 para PAD -, sem diferenças para PAS (163±4,5 versus 162±4,3 mmHg; p > 0,05) e uma pequena diferença para a PAD (72±2,4 versus 78±2,3 mmHg; p < 0,05). CONCLUSÃO: Para exercícios de intensidade moderada e constante no cicloergômetro com 15 minutos de duração, a medida da PA deverá ser feita a partir do sétimo minuto. As medidas digitais com o Tango e convencionais de PA foram, para efeitos clínicos, muito similares e reprodutíveis.

BACKGROUND: Cone-beam computed tomography (CBCT) image-guided radiotherapy (IGRT) systems are widely used tools to verify and correct the target position before each fraction, allowing to maximize treatment accuracy and precision. In this study, we evaluate automatic three-dimensional intensity-based rigid registration (RR) methods for prostate setup correction using CBCT scans and study the impact of rectal distension on registration quality. METHODS: We retrospectively analyzed 115 CBCT scans of 10 prostate patients. CT-to-CBCT registration was performed using (a) global RR, (b) bony RR, or (c) bony RR refined by a local prostate RR using the CT clinical target volume (CTV) expanded with 1-to-20-mm varying margins. After propagation of the manual CT contours, automatic CBCT contours were generated. For evaluation, a radiation oncologist manually delineated the CTV on the CBCT scans. The propagated and manual CBCT contours were compared using the Dice similarity and a measure based on the bidirectional local distance (BLD). We also conducted a blind visual assessment of the quality of the propagated segmentations. Moreover, we automatically quantified rectal distension between the CT and CBCT scans without using the manual CBCT contours and we investigated its correlation with the registration failures. To improve the registration quality, the air in the rectum was replaced with soft tissue using a filter. The results with and without filtering were compared. RESULTS: The statistical analysis of the Dice coefficients and the BLD values resulted in highly significant differences (p<10(-6)) for the 5-mm and 8-mm local RRs vs the global, bony and 1-mm local RRs. The 8-mm local RR provided the best compromise between accuracy and robustness (Dice median of 0.814 and 97% of success with filtering the air in the rectum). We observed that all failures were due to high rectal distension. Moreover, the visual assessment confirmed the superiority of the 8-mm local RR over the bony RR. CONCLUSION: The most successful CT-to-CBCT RR method proved to be the 8-mm local RR. We have shown the correlation between its registration failures and rectal distension. Furthermore, we have provided a simple (easily applicable in routine) and automatic method to quantify rectal distension and to predict registration failure using only the manual CT contours.

Rubisco is responsible for the fixation of CO2 into organic compounds through photosynthesis and thus has a great agronomic importance. It is well established that this enzyme suffers from a slow catalysis, and its low specificity results into photorespiration, which is considered as an energy waste for the plant. However, natural variations exist, and some Rubisco lineages, such as in C4 plants, exhibit higher catalytic efficiencies coupled to lower specificities. These C4 kinetics could have evolved as an adaptation to the higher CO2 concentration present in C4 photosynthetic cells. In this study, using phylogenetic analyses on a large data set of C3 and C4 monocots, we showed that the rbcL gene, which encodes the large subunit of Rubisco, evolved under positive selection in independent C4 lineages. This confirms that selective pressures on Rubisco have been switched in C4 plants by the high CO2 environment prevailing in their photosynthetic cells. Eight rbcL codons evolving under positive selection in C4 clades were involved in parallel changes among the 23 independent monocot C4 lineages included in this study. These amino acids are potentially responsible for the C4 kinetics, and their identification opens new roads for human-directed Rubisco engineering. The introgression of C4-like high-efficiency Rubisco would strongly enhance C3 crop yields in the future CO2-enriched atmosphere.

Semiotics is hardly known in German business literature and management practice, despite the fact that its methodological approaches to discourse analysis have already been broadly absorbed by management theory in France and the United States. The present contribution points out why the general echo of semiotic theory and its numerous applications to business administration has remained limited so far, especially when compared with its potential for describing and explaining management problems. It is important, then, to show what the object of semiotic research is, independent of any semiotic orientation or school. What tools have been developed so far, and which discourses in business administration were chosen to apply these tools? The problems limiting a broader use of semiotic instruments in business administration are explained in detail, and the research perspectives are illustrated. Die in der deutschsprachigen Betriebswirtschaftslehre (BWL) noch wenig bekannte Semiotik und die in ihrem Rahmen entwickelten methodologischen Ansätze zur Analyse von Diskursen haben insbesondere in Frankreich und in den USA schon eine weite Verbreitung gefunden. Das Echo, auf welches die Semiotik trifft, bleibt im Vergleich zum Beitrag, den sie hinsichtlich der Beschreibung und Erklärung betriebswirtschaftlicher Tatbestände leisten könnte, noch sehr beschränkt - und dies, obwohl ihre grundsätzliche wissenschaftliche Leistungsfähigkeit inzwischen an Hand zahlreicher Beispiele aus dem betriebswirtschaftlichen Bereich belegt werden konnte. Es ist deshalb wichtig und interessant zu zeigen, was der Forschungsgegenstand der Semiotik ist - und zwar unabhängig von den verschiedenen Schulen der Semiotik. Welche Instrumente hat sie entwickelt? Auf welche betriebswirtschaftlichen Diskurse sind diese Instrumente bereits angewandt worden? Zum Schluss werden die Probleme dargestellt, die einen breiteren Einsatz semiotischer Instrumente in der Betriebswirtschaftslehre behindern und dabei auch die verschiedenen Forschungsrichtungen erläutert.

Background: Infection with EBV and a lack in vitamin D may be important environmental triggers of MS. 1,25-(OH)2D3 mediates a shift of antigen presenting cells (APC) and CD4+ T cells to a less inflammatory profile. Although CD8+ T cells do express the vitamin D receptor, a direct effect of 1,25(OH)2D3 on these cells has not been demonstrated until now. Since CD8+ T cells are important immune mediators of the inflammatory response in MS, we examined whether vitamin D directly affects the CD8+ T cell response, and more specifically if it modulates the EBV-specific CD8+ T cell response. Material and Methods: To explore whether the vitamin D status may influence the pattern of the EBV-specific CD8+ T cell response, PBMC of 10 patients with early MS and 10 healthy controls (HC) were stimulated with a pool of immunodominant 8-10 mer peptide epitopes known to elicit CD8+ T cell responses. PBMC were stimulated with this EBV CD8 peptide pool, medium (negative control) or anti- CD3/anti-CD28 beads (positive control). The following assays were performed: ELISPOT to assess the secretion of IFN-gamma by T cells in general; cytometric beads array (CBA) and ELISA to determine whichcytokines were released by EBV-specific CD8+ T cells after six days of culture; and intracellular cytokine staining assay to determine by which subtype of T cells secreted given cytokines. To examine whether vitamin D could directly modulate CD8+ T cell immune responses, we depleted CD4+ T cells using negative selection. Results: We found that pre-treatment of vitamin D had an antiinflammatory action on both EBV-specific CD8+ T cells and on CD3/ CD28-stimulated T cells: secretion of pro-inflammatory cytokines (IFNgamma and TNF-alpha) was decreased, whereas secretion of antiinflammatory cytokines (IL-5 and TGF-beta) was increased. At baseline, CD8+ T cells of early MS patients showed a higher secretion of TNFalpha and lower secretion of IL-5. Addition of vitamin D did not restore the same levels of both cytokines as compared to HC. Vitamin D-pretreated CD8+T cells exhibited a decreased secretion of IFN-gamma and TNF-alpha, even after depletion of CD4+ T cells from culture. Conclusion: Vitamin D has a direct anti-inflammatory effect on CD8+ T cells independently from CD4+ T cells. CD8+ T cells of patients with earlyMS are less responsive to the inflammatory effect of vitamin D than HC, pointing toward an intrinsic dysregulation of CD8+ T cells. The modulation of EBV-specific CD8+T cells by vitaminDsuggests that there may be interplay between these twomajor environmental factors of MS. The most frequent clinical manifestation of borreliosis in Switzerland is erythema migrans, with about 2500 patients each year. Neurological manifestations are rare, mostly hyperalgesic radiculitis (Bannwarth syndrome), aseptic meningitis or cranial nerve involvement. We report the first Swiss patient with meningovasculitis due to neuroborreliosis, with recurrent multiple ischemic strokes in multiple vascular territories. The treatment with ceftriaxone stopped the progression, but the patient is still suffering from severe invalidating cognitive disorders. We also comment on the pathophysiology and review the literature of other clinical cases.

BACKGROUND AND PURPOSE: Neuromyelitis optica (NMO) or Devic's disease is a rare inflammatory and demyelinating autoimmune disorder of the central nervous system (CNS) characterized by recurrent attacks of optic neuritis (ON) and longitudinally extensive transverse myelitis (LETM), which is distinct from multiple sclerosis (MS). The guidelines are designed to provide guidance for best clinical practice based on the current state of clinical and scientific knowledge. SEARCH STRATEGY: Evidence for this guideline was collected by searches for original articles, case reports and meta-analyses in the MEDLINE and Cochrane databases. In addition, clinical practice guidelines of professional neurological and rheumatological organizations were studied. RESULTS: Different diagnostic criteria for NMO diagnosis [Wingerchuk et al. Revised NMO criteria, 2006 and Miller et al. National Multiple Sclerosis Society (NMSS) task force criteria, 2008] and features potentially indicative of NMO facilitate the diagnosis. In addition, guidance for the work-up and diagnosis of spatially limited NMO spectrum disorders is provided by the task force. Due to lack of studies fulfilling requirement for the highest levels of evidence, the task force suggests concepts for treatment of acute exacerbations and attack prevention based on expert opinion. CONCLUSIONS: Studies on diagnosis and management of NMO fulfilling requirements for the highest levels of evidence (class I-III rating) are limited, and diagnostic and therapeutic concepts based on expert opinion and consensus of the task force members were assembled for this guideline.
