A new formula for glomerular filtration rate estimation in pediatric population from 2 to 18 years has been developed by the University Unit of Pediatric Nephrology. This Quadratic formula, accessible online, allows pediatricians to adjust drug dosage and/or follow-up renal function more precisely and in an easy manner.

When a new treatment is compared to an established one in a randomized clinical trial, it is standard practice to statistically test for non-inferiority rather than for superiority. When the endpoint is binary, one usually compares two treatments using either an odds-ratio or a difference of proportions. In this paper, we propose a mixed approach which uses both concepts. One first defines the non-inferiority margin using an odds-ratio and one ultimately proves non-inferiority statistically using a difference of proportions. The mixed approach is shown to be more powerful than the conventional odds-ratio approach when the efficacy of the established treatment is known (with good precision) and high (e.g. with more than 56% of success). The gain of power achieved may lead in turn to a substantial reduction in the sample size needed to prove non-inferiority. The mixed approach can be generalized to ordinal endpoints.

Childhood obesity and physical inactivity are increasing dramatically worldwide. Children of low socioeconomic status and/or children of migrant background are especially at risk. In general, the overall effectiveness of school-based programs on health-related outcomes has been disappointing. A special gap exists for younger children and in high risk groups. This paper describes the rationale, design, curriculum, and evaluation of a multicenter preschool randomized intervention study conducted in areas with a high migrant population in two out of 26 Swiss cantons. Twenty preschool classes in the German (canton St. Gallen) and another 20 in the French (canton Vaud) part of Switzerland were separately selected and randomized to an intervention and a control arm by the use of opaque envelopes. The multidisciplinary lifestyle intervention aimed to increase physical activity and sleep duration, to reinforce healthy nutrition and eating behaviour, and to reduce media use. According to the ecological model, it included children, their parents and the teachers. The regular teachers performed the majority of the intervention and were supported by a local health promoter. The intervention included physical activity lessons, adaptation of the built infrastructure; promotion of regional extracurricular physical activity; playful lessons about nutrition, media use and sleep, funny homework cards and information materials for teachers and parents. It lasted one school year. Baseline and post-intervention evaluations were performed in both arms. Primary outcome measures included BMI and aerobic fitness (20 m shuttle run test). Secondary outcomes included total (skinfolds, bioelectrical impedance) and central (waist circumference) body fat, motor abilities (obstacle course, static and dynamic balance), physical activity and sleep duration (accelerometry and questionnaires), nutritional behaviour and food intake, media use, quality of life and signs of hyperactivity (questionnaires), attention and spatial working memory ability (two validated tests). Researchers were blinded to group allocation. The purpose of this paper is to outline the design of a school-based multicenter cluster randomized, controlled trial aiming to reduce body mass index and to increase aerobic fitness in preschool children in culturally different parts of Switzerland with a high migrant population. La elaboración de un marco legal y normativo que fija el derecho de los ciudadanos a una información veraz y adecuada sobre salud y enfermedad implica tener en cuenta estas necesidades informativas desde la doble perspectiva del ¿consumer health¿ y el ¿patient education¿. Se defiende, desde el punto de vista de los profesionales de la información, la materialización de este derecho a la información y a la formación en temas relacionados con la salud y la enfermedad gracias a un mejor aprovechamiento de los recursos informativos disponibles en las bibliotecas médicas y a los profesionales que los gestionan.

Intervenció educativa amb dos objectius: primer, desenvolupar l'esperit crític dels ciutadans perquè seleccionin continguts de qualitat en les consultes sobre temes de salut a Internet i adoptin decisions responsables; i, segon, donar a conèixer un conjunt d'orientacions bàsiques per avaluar la informació sobre temes de salut a Internet. El contingut de la sessió va ser: breu introducció a Internet, criteris d'avaluació de pàgines web, segells de qualitat i prescripció web. Es lliurà als assistents un full amb una prescripció web i les icones d'alguns segells de qualitat, a més del Decàleg per a l'ús de webs de salut de la Web Mèdica Acreditada.

Proposta d'implantació a les consultes d'infermeria d'atenció primària d'un procediment relacionat amb la qualitat de la informació a Internet i l'aprofitament dels recursos d'informació i de salut disponibles, amb dos objectius: primer, proporcionar als pacients crònics criteris per identificar i seleccionar fonts d'informació de qualitat a Internet per tal que puguin gestionar la seva malaltia i ser autònoms en la seva autocura; i, segon, avaluar els coneixements extrets d'Internet que tenen els pacients crònics. Es proposa complementar el procediment amb activitats comunitàries en espais alternatius com els centres cívics, els casals de gent gran i, especialment, les biblioteques públiques.

Se pretende optimizar así los recursos de estos dos servicios públicos en beneficio del paciente crónico.

OBJECTIVE: To extract and to validate a brief version of the DISCERN which could identify mental health-related websites with good content quality. METHOD: The present study is based on the analysis of data issued from six previous studies which used DISCERN and a standardized tool for the evaluation of content quality (evidence-based health information) of 388 mental health-related websites. After extracting the Brief DISCERN, several psychometric properties (content validity through a Factor analysis, internal consistency by the Cronbach's alpha index, predictive validity through the diagnostic tests, concurrent validity by the strength of association between the Brief DISCERN and the original DISCERN scores) were investigated to ascertain its general applicability. RESULTS: A Brief DISCERN composed of two factors and six items was extracted from the original 16 items version of the DISCERN. Cronbach's alpha coefficients were more than acceptable for the complete questionnaire (alpha=0.74) and for the two distinct domains: treatments information (alpha=0.87) and reliability (alpha=0.83). Sensibility and specificity of the Brief DISCERN cut-off score > or =16 in the detection of good content quality websites were 0.357 and 0.945, respectively. Its predictive positive and negative values were 0.98 and 0.83, respectively. A statistically significant linear correlation was found between the total scores of the Brief DISCERN and those of the original DISCERN (r=0.84 and p<0.0005). CONCLUSION: The Brief DISCERN seems to be a reliable and valid instrument able to discriminate between websites with good and poor content quality. PRACTICE IMPLICATIONS: The Brief DISCERN is a simple tool which could facilitate the identification of good information on the web by patients and general consumers.

BACKGROUND: The perichondral cutaneous graft (PCCG) from the posterior conchal region is an elegant solution for the coverage of facial defects with particular stability requirements. The donor defect can easily be covered with a transposition flap from the postauricular region. Although this region is a common donor site for skin grafts and has an important supporting function for glasses or hearing aids, little is known about long-term morbidity after graft harvest. OBJECTIVE: To assess the morbidity of the posterior concha and the postauricular region in terms of pain, scar formation, and patient satisfaction. MATERIALS AND METHODS: A retrospective study of 16 patients who had a PCCG harvested from the posterior concha. RESULTS: Two patients presented with a postoperative wound dehiscence on the postauricular region and one with a keloid scar on the posterior concha. One case of transitory hyperesthesia and pain when sleeping on the operated site was observed. None had complaints related to wearing glasses or hearing aids. CONCLUSION: Donor site morbidity of the postauricular and posterior conchal region is minimal and associated with high patient satisfaction, excellent aesthetic results, and emotional detachment from the hidden donor site.

Suite à la demande du Président de la République de disposer d'indicateurs de mortalité en établissements de santé » (discours du 18 septembre 2008 sur le thème de la politique de la santé et de la réforme du système de soins), la Direction de la recherche, des études, de l'évaluation et des statistiques (DREES) a piloté en 2009, en lien avec la Direction générale de l'offre de soins (DGOS) et la Haute autorité de santé (HAS), l'instruction des aspects méthodologiques relatifs à la construction de ces indicateurs de mortalité. Cette revue de littérature portant sur les différentes méthodes d'ajustement a été réalisée dans ce cadre, à la demande conjointe de la DREES et de la HAS.Ce rapport décrit les méthodes et modèles d'estimation et d'ajustement de la mortalité hospitalière identifiées dans la littérature.Il ressort de cette revue, que d'une manière générale, trois questions méthodologiques majeures préoccupent les épidémiologistes, les chercheurs et les décideurs s'intéressant à l'indicateur de la mortalité hospitalière : i) premièrement, sur l'opportunité d'établir l'indicateur de mortalité hospitalière à partir de groupes de population de patients homogènes définis par des pathologies et/ou des procédures médicales/chirurgicales cibles ; ii) deuxièmement, sur le type d'approche analytique et de l'intérêt de prendre en compte plusieurs niveaux dans l'analyse statistique ; iii) enfin troisièmement, sur le choix des variables d'ajustement permettant de contrôler les différences de case-mix entre plusieurs établissements ou groupes de patients pour réaliser des comparaisons. At the University of Lausanne third-year medical students are given the task of spending a month investigating a question of community medicine. In 2009, four students evaluated the legitimacy of health insurers intervening in the management of depression. They found that health insurers put pressure on public authorities during the development of legislation governing the health system and reimbursement for treatment. This fact emerged during the scientific investigation led jointly by the team in the course of the "module of immersion in community medicine." This paper presents each step of their study. The example chosen illustrates the learning objectives covered by the module.

PURPOSE: To select and propose a set of knowledge, attitudes, and skills essential for the care of adolescents; to encourage the development of adolescent health multidisciplinary networks; and to set up training programs in as many European countries as possible. METHODS: The curriculum was developed by 16 physicians from 11 European countries with various professional specializations. In line with modern guidelines in medical education, it is a modular, flexible instrument which covers the main teaching areas in the field, such as basic skills (i.e. setting, rights and confidentiality, gender and cultural issues) as well as specific themes (i.e. sexual and reproductive health, eating disorders, chronic conditions). It consists of 17 thematic modules, each containing detailed objectives, learning approaches, examples, and evaluation methods. RESULT: Two international one-week summer schools were used to assess the feasibility and appropriateness of the curriculum. The overall evaluation was good, with most of the items surpassing three on a four-point Likert scale. However, it pointed to several aspects (process and content) which will need to be refined in the future, such as an increase in interactive sessions (role playing), and a better mix of clinical and public health issues.

Traducció comentada del pròleg d¿un llibre on la seva autora, amb motiu de presentar les propostes dels seus estudiants sobre activitats integrades de plàstica i música per a l¿escola i l'institut, ens parla de la necessitat d¿activar la situació creadora en els individus per millorar el seu excel-lent procés educatiu, a la manera com ho feien els antics.

Objetivo: Elaboración de un folleto informativo sobre la neumonía, en forma de tríptico, dirigido a pacientes ingresados por este motivo, así como a sus familiares y cuidadores y su posteriorevaluación. Material y método: El estudio consta de dos partes. 1ª Estudio observacional: revisión bibliografíca de la neumonía en bases de datos internacionales, elaboración de un tríptico estructurado y comprensible para el paciente que incluye definición, síntomas, cómo se diagnostica y tratamiento. 2ª Estudio experimental, cualitativo y prospectivo en dos grupos de pacientes ingresados con neumonía adquirida en la comunidad en un hospital de tercer nivel. En cada grupo entraron 38 pacientes. El grupo intervención recibió el folleto informativo en forma de tríptico y el grupo control no recibió ninguna intervención. A ambos grupos se les pasó un cuestionario al alta con preguntas relacionadas sobre la neumonía. Resultado: Muestra de 76 pacientes, 58% hombres con una edad media de 69 años y un nivel de estudios primarios de un 50%. El 79% era la primera vez que tenia neumonía. En el estudio comparativo de los dos grupos destaca que en el grupo intervención se le había explicado lo que era la neumonía al 100% de pacientes mientras que en el de control era del 52%. Referente a la pregunta de cuáles son los síntomas que aparecen en la neumonía en el grupo intervención el 99% es capaz de enumerarlos, sin embargo, en el grupo control solo un 56% es capaz. Conclusión: Consideramos que la importancia de la información facilitada por escrito, con un lenguaje claro, conciso y asequible para las diferentes edades en pacientes ingresados, aumenta sus conocimientos sobre su enfermedad, síntomas y pruebas, obteniendo más confort y tranquilidad durante su estancia

This article summarizes the different stages of research for the development of medical interventions and their specific characteristics in terms of design, population, resources, importance of results and scientific interest. The emphasis is focused on the two final stages of development, the effectiveness and the impact. An example from our own experience is given to illustrate the reduction of the effect of an intervention against malaria in young children at different stages of the development of the intervention, and the parallel decrease of the recognition by the scientific community of the importance of these results.
