Objective To investigate the relationship between skipping meals and biochemical variables in obese children and adolescents.Study design The sample was composed of 174 obese children and adolescents, aged between 6 and 16 years (80 male and 94 female). Body composition was assessed by dual-energy x-ray absorptiometry, fasting blood glucose, and lipid profile were measured after 12 hours fasting. The frequency of skipping breakfast, lunch, or dinner was assessed through a face-to-face interview carried out with the parents.Results The prevalence of eating breakfast daily was low in boys (47.5%) and girls (44.7%). A higher frequency of eating breakfast was negatively correlated with glucose (r = -0.16; P = .026), triglycerides (r = -0.19; P = .011), and very low density lipoprotein cholesterol (r = -0.21; P = .005). In the multivariate model, the weekly frequency of eating breakfast remained negatively associated with glucose (beta = -0.975; P = .017), triglycerides (beta = -7.792; P = .017), and very low density lipoprotein cholesterol (beta = -1.870; P = .009) independent of age, sex, trunk fatness, and parents' education.Conclusion Skipping meals, mainly breakfast, is associated with glucose and lipid levels in obese children and adolescents. PURPOSE: To evaluate the frequency of obstructive sleep apnea (OSA) in obese patients scheduled for bariatric surgery and their identification for risk of OSA by Berlin Questionnaire (BQ) and excessive daytime sleepiness by Epworth Sleepiness Scale (ESS). METHODS: Fifty nine patients were evaluated by BQ and ESS. Out of these individuals, 35 performed a full-night sleep study using a type 3 portable monitoring (PM). The questionnaire results were compared for gender and BMI. The presence and severity of OSA was correlated with gender and both questionnaires. RESULTS: 94.75% of the respondents presented high risk for OSA by BQ and 59.65% presented positivity by ESS. Taking into account the AHI> 5 per hour for OSA diagnosis, all of them presented OSA, average AHI of 45.31±26.3 per hour and 68.6% have severe OSA (AHI>30). The male patients had a higher AHI (p<0.05). There was a positive correlation between the positivity in both questionnaires as well as the severity of OSA measured by AHI (p<0.05). CONCLUSION: The frequency and severe obstructive sleep apnea in the studied group is high. The Berlin Questionnaire and Epworth Sleepiness Scale had a positive correlation with the diagnosis of OSA in the group studied.

CONTEXTO E OBJETIVO: A disfunção pulmonar no obeso pode estar associada a comprometimento muscular respiratório e também pode ser influenciada pelo predomínio de distribuição de gordura corporal na região toraco-abdominal. O objetivo foi avaliar a força dos músculos respiratórios em obesos e analisar a influência da distribuição do tecido adiposo. TIPO DE ESTUDO E LOCAL: Estudo transversal no período pré-operatório de Cirurgia Bariátrica. Estudo desenvolvido no Programa de Pós-Graduação em Bases Gerais da Cirurgia da Universidade Estadual Paulista (Unesp) - Faculdade de Medicina de Botucatu. MÉTODO: Mensuração da força dos músculos respiratórios através das medidas das pressões inspiratórias e expiratórias máximas (PImax e PEmax) em obesos candidatos à cirurgia bariátrica. Avaliar a distribuição do tecido adiposo através da relação entre as circunferências da cintura e quadril (RC/Q). Comparar esses atributos com os valores de referência de normalidade e também entre grupos com diferentes índices de massa corpórea (IMC). RESULTADOS: Foram avaliados 23 homens e 76 mulheres. Todos foram submetidos à avaliação de PImax e 86 realizaram a PEmax. O IMC médio foi de 44,42 kg/m². Os valores de PImax e de PEmax estavam dentro dos padrões de normalidade, a relação cintura-quadril mostrou distribuição do tecido adiposo na porção superior corporal e não houve correlação entre as variáveis estudadas. CONCLUSÃO: Na população de obesos estudada, o excesso de peso não provocou alterações na força dos músculos respiratórios, e as modificações não foram influenciadas pela distribuição de gordura predominante em porção superior corporal.

The mechanism by which adiposity can raise blood pressure is not fully understood. Leptin has been suggested as a possible cause of the arterial hypertension in obese subjects because leptin induces an increase in sympathetic activity. The aim of the present study is to evaluate serum leptin level, blood pressure, lipid profile, blood glucose, and insulin in obese women. Leptin, total cholesterol, low-density lipoprotein (LDL) cholesterol, and triglycerides were measured. Serum leptin was markedly higher in hypertensive obese women (92.3 +/- 22 ng/mL, n = 7) as compared with normotensive obese women (37.7 +/- 11 ng/mL, n = 7). Similarly, total cholesterol and LDL cholesterol were significantly elevated in the hypertensive group. No changes were observed in triglycerides and high-density lipoprotein cholesterol concentration between the 2 groups. There were no significant differences in plasma insulin concentration or blood glucose in both groups. In conclusion, our findings suggest a link among dyslipidemia, leptin level, and hypertension that might be relevant to the development of cardiovascular disease in obese subjects.

We have described previously the prophylactic and therapeutic effect of a DNA vaccine encoding the Mycobacterium leprae 65 kDa heat shock protein (DNA-HSP65) in experimental murine tuberculosis. However, the high homology of this protein to the corresponding mammalian 60 kDa heat shock protein (Hsp60), together with the CpG motifs in the plasmid vector, could trigger or exacerbate the development of autoimmune diseases. The non-obese diabetic (NOD) mouse develops insulin-dependent diabetes mellitus (IDDM) spontaneously as a consequence of an autoimmune process that leads to destruction of the insulin-producing beta cells of the pancreas. IDDM is characterized by increased T helper 1 (Th1) cell responses toward several autoantigens, including Hsp60, glutamic acid decarboxylase and insulin. In the present study, we evaluated the potential of DNA-HSP65 injection to modulate diabetes in NOD mice. Our results show that DNA-HSP65 or DNA empty vector had no diabetogenic effect and actually protected NOD mice against the development of severe diabetes. However, this effect was more pronounced in DNA-HSP65-injected mice. The protective effect of DNA-HSP65 injection was associated with a clear shift in the cellular infiltration pattern in the pancreas. This change included reduction of CD4(+) and CD8(+) T cells infiltration, appearance of CD25(+) cells influx and an increased staining for interleukin (IL)-10 in the islets. These results show that DNA-HSP65 can protect NOD mice against diabetes and can therefore be considered in the development of new immunotherapeutic strategies.

This study investigated the effects of growth hormone therapy on energy expenditure, lipid profile, oxidative stress and cardiac energy metabolism in aging and obesity conditions. Life expectancy is increasing in world population and with it, the incidence of public health problems such as obesity and cardiac alterations. Because growth hormone (GH) concentration is referred to be decreased in aging conditions, a question must be addressed: what is the effect of GH on aging related adverse changes? To investigate the effects of GH on cardiac energy metabolism and its association with calorimetric parameters, lipid profile and oxidative stress in aged and obese rats, initially 32 male Wistar rats were divided into 2 groups (n = 16), C: given standard-chow and water; H: given hypercaloric-chow and receiving 30 % sucrose in its drinking water. After 45 days, both C and H groups were divided into 2 subgroups (n = 8), C + PL: standard-chow, water, and receiving saline subcutaneously; C + GH: standard-chow, water, and receiving 2 mg/kg/day rhGH subcutaneously; H + PL: hypercaloric-chow, 30 % sucrose, receiving saline subcutaneously; H + GH: hypercaloric-chow, 30 % sucrose, receiving rhGH subcutaneously. After 30 days, C + GH and H + PL rats had higher body mass index, Lee-index, body fat content, percent-adiposity, serum triacylglycerol, cardiac lipid-hydroperoxide, and triacylglycerol than C + PL. Energy-expenditure (RMR)/body weight, oxygen consumption and fat-oxidation were higher in H + GH than in H + PL. LDL-cholesterol was highest in H + GH rats, whereas cardiac pyruvate-dehydrogenase and phosphofrutokinase were higher in H + GH and H + PL rats than in C + PL. In conclusion, the present study brought new insights on aging and obesity, demonstrating for the first time that GH therapy was harmful in aged and obesity conditions, impairing calorimetric parameters and lipid profile. GH was disadvantageous in control old rats, having undesirable effects on triacylglycerol accumulation and cardiac oxidative stress.
