The aim of this study was to evaluate the incidence of and mortality due to meningitis and compare data according to microbiological diagnosis. This was a ten-year retrospective study conducted at a neonatal intensive care unit (NICU). Newborns with meningitis confirmed by positive CSF culture were included; those with congenital infection or malformations that made lumbar puncture impossible were excluded. The variables investigated were birth weight, gestational and postnatal age, procedures, hematological and CSF parameters, and complications. Parametric and non-parametric tests were used (statistical value p<0.05). The incidence of meningitis was 0.6% and mortality was 27%. of the 22 cases, 59% involved Gram-negative bacteria; 36% Gram-positive and 5% fungi. The groups did not differ in relation to birth weight, gestational and postnatal age, procedures or hematological and CSF parameters. Sepsis, convulsions and deaths were frequent in both groups, without statistical difference. Gram-negative cases showed abscesses and higher frequency of ventriculitis and hydrocephaly. Meningitis was infrequent, but presented high mortality and frequent complications.

The nursing care for patients who are pronounced brain-dead but kept alive to serve as organ donors demands technical-scientific skills and the ability to handle situations that are often in conflict with the traditional concepts of nursing care. Based on the phenomenological approach in this article, essential themes of the lived experience of caring for these patients, including the technical and specific nursing care, the relationship with organ donors and their families, and the nurses' perception of themselves in this professional situation are described. The results point to the contradictions and ambiguities of this type of nursing, especially in regards to the affective and philosophical aspects.

We studied a nonconcurrent cohort of 582 patients admitted to a medical-surgical intensive care unit. Use of antimicrobials (imipenem and metronidazole) was a risk factor for acquisition of imipenem-resistant Acinetobacter baumannii only for the subcohort of patients admitted in months in which colonization pressure was lower than the median value. © 2013 by the Association for Professionals in Infection Control and Epidemiology, Inc. Os cuidados em neonatologia têm se intensificado nos últimos anos no que tange as condições de prematuridade e do baixo peso ao nascimento, especialmente em relação à saúde, crescimento e desenvolvimento das crianças submetidas a esta condição de risco. Um dos pontos relevantes das pesquisas em neonatologia condiz à observação comportamental dos sinais emitidos pelo funcionamento do organismo do prematuro, que possivelmente revelam o nível de organização ou de desorganização do seu organismo como um todo, tornando possível predizer futuros déficits. Desde 1978, uma pesquisadora americana, Heidelise Als, juntamente com um grupo de colaboradores, iniciaram diversas pesquisas sobre o comportamento e assistência ao prematuro de baixo peso, as quais enfatizam uma prática diferenciada de cuidados para o desenvolvimento utilizando como base a Teoria Síncrono Ativa do Desenvolvimento (TSAD), que permitiu o registro e elaboração de cuidados individualizados para promoção do desenvolvimento de bebês prematuro, refletindo em ganhos para um desenvolvimento efetivo das diferentes funções do organismo, em particular as áreas cognitivo-motora e comportamental. Desta forma, o presente estudo tem como objetivo quantificar os comportamentos que são mais evidentes no contexto de uma UTIN, e quais os mecanismos ambientais que favorecem a manifestação dos mesmos, a fim de traçar os cuidados para o desenvolvimento de acordo com a realidade apresentada na UTIN. A pesquisa foi planejada a partir de um pré-projeto, o qual possibilitou o registro em 19 amostras de crianças para 30 sessões de observação, sendo desenvolvida no período de abril à julho de 2005. A coleta de dados foi realizada em bebês prematuros com baixo peso ao nascimento e com idade gestacional entre 26 a 37 semanas. A mesma utilizou um etograma padrão, baseado na observação comportamental de H. Als, assinalando os eventos ambientais que promoviam determinados comportamentos. A observação foi realizada em um período de 15 minutos por sujeito de cada sessão. Os dados tabulados resultaram em: dados gerais, que constituem as variáveis maternas e dos bebês, e dados específicos, referindo os comportamentos e as interferências ambientais observados. Os dados gerais mantiveram a similaridade com outras pesquisas, apontando as condições sociais desfavoráveis e a predisposição materna a infecções como possíveis fatores predisponentes ao parto prematuro, assim como a situação de prematuridade como agravante da condição de saúde e bem-estar do bebê. Entre os dados específicos, foi possível registrar uma prevalência dos comportamentos de retraimento, que revelam a desorganização do bebê, mediante aos excessos de interferências ambientais, como ruído, manipulação e luminosidade, muito comuns em UTIN. Mediante estes fatos, sugerimos nesta pesquisa as possíveis soluções para amenizar o excesso de manifestações comportamentais desorganizadoras, buscando priorizar os cuidados para o desenvolvimento por meio de práticas utilizadas por H. Als e sua equipe.

Coagulase-negative staphylococci (CoNS) are the microorganisms most frequently isolated from clinical samples and are commonly found in neonatal blood cultures. Oxacillin is an alternative treatment of choice for CoNS infections; however, resistance to oxacillin can have a substantial impact on healthcare by adversely affecting morbidity and mortality. The objective of this study was to detect and characterise oxacillin-resistant CoNS strains in blood cultures of newborns hospitalised at the neonatal ward of the University Hospital of the Faculty of Medicine of Botucatu. One hundred CoNS strains were isolated and the mecA gene was detected in 69 of the CoNS strains, including 73.2% of Staphylococcus epidermidis strains, 85.7% of Staphylococcus haemolyticus strains, 28.6% of Staphylococcus hominis strains and 50% of Staphylococcus lugdunensis strains. Among these oxacillin-resistant CoNS strains, staphylococcal cassette chromosome mec (SCCmec) type I was identified in 24.6%, type II in 4.3%, type III in 56.5% and type IV in 14.5% of the strains. The data revealed an increase in the percentage of CoNS strains isolated from blood cultures from 1991-2009. Furthermore, a predominant SCCmec profile of the oxacillin-resistant CoNS strains isolated from neonatal intensive care units was identified with a prevalence of SCCmec types found in hospital-acquired strains.

Introduction: Scientific evidence indicates that neonatal exposure to ototoxic drugs cause hearing loss in newborns. Objective: To characterize the use of ototoxic antibiotics in newborns (NB), treated in the Neonatal Intensive Care Unit (NICU) and evaluate possible hearing modifications. Methods: A descriptive cross-sectional quantitative approach, using data from medical records of infants who were at some time in the NICU and used antibiotics, including ototoxic, from January to June 2004 as much as 2010, and the data were compared and analyzed. Parents/guardians of infants born in 2004 were contacted and applied a questionnaire containing questions about the children’s hearing. These children were submitted to audiological evaluation. Results: There was significant reduction in the time of use, the amount of antibiotics prescribed to newborns and Vancomycin prescription in 2010 compared to 2004. The hearing tests of 13 born in 2004 showed: sensorineural hearing loss in only 2 (one with moderate hearing loss and descending configuration in pure tone audiometry and the other with bilateral cochlear impairment); audiometric thresholds within the normal range in 11 patients, and the presence of otoacoustic emissions in 9. In Evoked Auditory Brainstem Response (ABR) no changes were observed. Conclusion: The reduction in the time of use, the amount and types of antibiotics observed may be related to the adoption of a Protocol in 2008, by the service. In contrast, auditory alterations may be related to a neonatal exposure to antibiotics in 2004.

Background: Late-onset sepsis (LOS) is an important cause of morbidity and mortality in very low birth weight (VLBW) infants.Aim: To determine the incidence, risk factors and etiology of LOS.Methods: LOS was investigated in a multicenter prospective cohort of infants at eight public university neonatal intensive care units (NICUs). Inclusion criteria included inborn, 23-33 weeks of gestational age, 400-1499 g birth weight, who survived >3 days.Results: Of 1507 infants, 357 (24%) had proven LOS and 345 (23%) had clinical LOS. Infants with LOS were more likely to die. The majority of infections (76%) were caused by Gram-positive organisms. Independent risk factors for proven LOS were use of central venous catheter and mechanical ventilation, age at the first feeding and number of days on parenteral nutrition and on mechanical ventilation.Conclusion: LOS incidence and mortality are high in Brazilian VLBW infants. Most risk factors are associated with routine practices at NICU.

Objective. To identify the perception of nurses with regard to the process of providing care to patients in the context of hospice care. Method. Qualitative study using the methodological framework Collective Subject Discourse. A total of 18 nursing professionals of the adult intensive care unit of a public hospital in São Paulo, Brazil were interviewed between June and August 2012. Results. The process of providing care to terminal patients is permeated by negative, conflictive and mixed feelings. As regards communication, while the participants acknowledge its importance as a therapeutic resource, they also admit a lack of professional qualification. Conclusion. The interviewees have difficulties to deal with care provided to terminal patients. The qualification of these professionals needs to be improved, starting in the undergraduate program.
