Proteasome inhibitors, used in cancer treatment for their proapoptotic effects, have anti-inflammatory and antifibrotic effects on animal models of various inflammatory and fibrotic diseases. Their effects in cells from patients affected by either inflammatory or fibrotic diseases have been poorly investigated. Nasal polyposis is a chronic inflammatory disease of the sinus mucosa characterized by tissue inflammation and remodeling. We tested the hypothesis that proteasome inhibition of nasal polyp fibroblasts might reduce their proliferation and inflammatory and fibrotic response. Accordingly, we investigated the effect of the proteasome inhibitor Z-Leu-Leu-Leu-B(OH)2 (MG262) on cell viability and proliferation and on the production of collagen and inflammatory cytokines in nasal polyp and nasal mucosa fibroblasts obtained from surgery specimens. MG262 reduced the viability of nasal mucosa and polyp fibroblasts concentration- and time-dependently, with marked effects after 48 h of treatment. The proteasome inhibitor bortezomib provoked a similar effect. MG262-induced cell death involved loss of mitochondrial membrane potential, caspase-3 and poly(ADP-ribose) polymerase activation, induction of c-Jun phosphorylation, and mitogen-activated protein kinase phosphatase-1 expression. Low concentrations of MG262 provoked growth arrest, inhibited DNA replication and retinoblastoma phosphorylation, and increased expression of the cell cycle inhibitors p21 and p27. MG262 concentration-dependently inhibited basal and transforming growth factor-β-induced collagen mRNA expression and interleukin (IL)-1β-induced production of IL-6, IL-8, monocyte chemoattractant protein-1, regulated on activation normal T cell expressed and secreted, and granulocyte/macrophage colony-stimulating factor in both fibroblast types. MG262 inhibited IL-1β/tumor necrosis factor-α-induced activation of nuclear factor-κB. We conclude that noncytotoxic treatment with MG262 reduces the proliferative, fibrotic, and inflammatory response of nasal fibroblasts, whereas high MG262 concentrations induce apoptosis.

OBJETIVO: Analisar os aspectos tomográficos dos tumores malignos da cavidade nasal. MATERIAIS E MÉTODOS: Foram estudados 18 pacientes - dez homens e oito mulheres - com tumor da cavidade nasal, os quais realizaram tomografia computadorizada da face. RESULTADOS: Dos tumores, seis eram casos de carcinoma epidermóide, três melanomas, dois carcinomas adenóides císticos, um adenocarcinoma polimórfico de baixo grau, um carcinoma indiferenciado, um carcinoma neuroendócrino, um linfoma não-Hodgkin, um rabdomiossarcoma alveolar, um sarcoma fusocelular grau II e um estesioneuroblastoma. As lesões foram mais freqüentes (p > 0,05) no lado esquerdo e no andar médio. CONCLUSÃO: Os carcinomas epidermóides apresentam grau de destruição correspondente ao seu volume, semelhante aos tumores epidermóides de outros sítios. O septo nasal foi acometido de maneira diferente, de acordo com os tipos histológicos.

OBJETIVO: Avaliar a translucência nucal, o ducto venoso, o osso nasal e a idade materna > 35 anos como testes de rastreamento para aneuploidias entre 12 e 14 semanas de gestação em pacientes de alto risco. MATERIAIS E MÉTODOS: Estudo prospectivo observacional envolvendo 92 gestantes entre 12 e 14 semanas submetidas a biópsia de vilo corial por alto risco de trissomia, baseado na medida da translucência nucal (17,4%) e idade materna >35 anos (78,3%). Antes da biópsia de vilo corial, realizaram-se medida da translucência nucal, avaliação de fluxo no ducto venoso e identificação do osso nasal. Calcularam-se a sensibilidade, a especificidade, o valor preditivo positivo e o valor preditivo negativo para testes realizados em paralelo e em seqüência. RESULTADOS: Encontrou-se alteração cromossômica em 12 (13,5%) fetos; 7 (58,3%) apresentavam trissomia 21. Osso nasal foi identificado em todos os fetos com trissomia. Translucência nucal, ducto venoso e idade materna isolados mostraram baixa sensibilidade (41,67-58,33%) e baixo valor preditivo positivo (10-45,45%). A associação translucência nucal + ducto venoso + idade materna apresentou o melhor resultado (sensibilidade: 100%; especificidade: 6,49%; valor preditivo positivo: 14,29%; valor preditivo negayivo: 100%). CONCLUSÃO: Em gestantes com idade > 35 anos, a associação translucência nucal + ducto venoso mostra-se como a mais sensível para a indicação de procedimento invasivo.

OBJETIVO: Avaliar a profundidade das fossas olfatórias e a freqüência de assimetria na altura e na inclinação lateral do contorno do teto etmoidal. MATERIAIS E MÉTODOS: Estudo retrospectivo de 200 tomografias computadorizadas dos seios da face no plano coronal realizadas no período de agosto a dezembro de 2006. As profundidades das fossas olfatórias foram classificadas segundo Keros. O teto etmoidal foi avaliado quanto à simetria entre os lados. RESULTADOS: O tipo de Keros mais encontrado foi o tipo II (73,3%), seguido do tipo I (26,3%) e do tipo III (0,5%). Em 12% (24 exames) havia assimetria entre os lados quanto à altura do teto etmoidal, e em 48,5% (97 exames) observou-se assimetria do contorno do teto, com inclinação lateral da lâmina crivosa de um dos lados. CONCLUSÃO: Em relação à profundidade das fossas olfatórias, o tipo II de Keros foi o mais freqüente. Verificou-se que a assimetria do teto do seio etmoidal, na maioria dos casos, estava relacionada com a inclinação lateral da lamela lateral da lâmina crivosa.

OBJETIVO: Determinar valores de referência para o comprimento do osso nasal entre 11 e 15 semanas de gestação em uma população brasileira. MATERIAIS E MÉTODOS: Realizou-se estudo de corte transversal com 171 gestantes normais entre 11 e 15 semanas completas. O osso nasal foi medido por via transabdominal em todos os casos. Foram calculados os percentis 5 a 95 para o comprimento do osso nasal pela fórmula: média ± 1,645 desvio-padrão. Para avaliar a correlação do comprimento do osso nasal com parâmetros antropométricos fetais utilizou-se o coeficiente de correlação de Spearman, com intervalo de confiança de 95%. RESULTADOS: O osso nasal foi mensurado em todos os casos, sendo que o comprimento médio variou de 1,69 mm a 2,94 mm. O comprimento do osso nasal mostrou-se fortemente correlacionado com todos os parâmetros antropométricos fetais (p < 0,001) e com a idade gestacional (R² = 0,59). CONCLUSÃO: Apesar de ser um estudo preliminar, a curva de referência do comprimento do osso nasal foi estabelecida.

BACKGROUND: Transmission of mucosal pathogens relies on their ability to bind to the surfaces of epithelial cells, to cross this thin barrier, and to gain access to target cells and tissues, leading to systemic infection. This implies that pathogen-specific immunity at mucosal sites is critical for the control of infectious agents using these routes to enter the body. Although mucosal delivery would ensure the best onset of protective immunity, most of the candidate vaccines are administered through the parenteral route. OBJECTIVE: The present study evaluates the feasibility of delivering the chemically bound p24gag (referred to as p24 in the text) HIV antigen through secretory IgA (SIgA) in nasal mucosae in mice. RESULTS: We show that SIgA interacts specifically with mucosal microfold cells present in the nasal-associated lymphoid tissue. p24-SIgA complexes are quickly taken up in the nasal cavity and selectively engulfed by mucosal dendritic cell-specific intercellular adhesion molecule 3-grabbing nonintegrin-positive dendritic cells. Nasal immunization with p24-SIgA elicits both a strong humoral and cellular immune response against p24 at the systemic and mucosal levels. This ensures effective protection against intranasal challenge with recombinant vaccinia virus encoding p24. CONCLUSION: This study represents the first example that underscores the remarkable potential of SIgA to serve as a carrier for a protein antigen in a mucosal vaccine approach targeting the nasal environment.

A concha nasal média secundária é uma rara variação anatômica na cavidade nasal, descrita pela primeira vez por Khanobthamchai et al. como uma estrutura óssea revestida por partes moles originária da parede lateral do meato médio. Na maioria dos casos relatados na literatura ocorre bilateralmente, sem complicações associadas. Neste artigo descrevemos um caso encontrado em nosso serviço, com tal variação anatômica incomum.

Superior vena cava syndrome is defined by a set of signs and symptoms secondary to superior vena cava obstruction caused principally by malignant diseases. The present report describes the case of an unusual clinical manifestation of this syndrome with bilateral breast swelling, and emphasizes the relevance of knowledge on mammographic signs of systemic diseases.

Sleep-disordered breathing (SDB) is underdiagnosed in women, probably due to the different gender-related manifestation. We investigated the differences in presentation, symptoms and co-morbidities of SDB in men and in pre- and postmenopausal women by a clinical, retrospective, cross-sectional study of 601 consecutively referred women and 233 age- and BMI-matched male-female pairs studied with the static-chargesensitive bed (SCSB) and an oximeter. Data on the use of nasal CPAP were gathered from the Paimio hospital database, and the co-morbidity information was based on reimbursed medication data from the National Agency for Medicines and the Social Insurance institution. The abnormal breathing episodes at night were more frequent in men than in women, and in postmenopausal women compared to premenopausal ones. Partial upper airway obstruction was the most common type of SDB in both genders but especially in females. BMI and the major symptoms of SDB were similar in pre- and postmenopausal women, and a menopause effect on symptoms was not found. CPAP adherence did not differ between symptomatic patients with partial upper airway obstruction and those presenting with conventional obstructive sleep apnea. Comorbidities were more frequent in SDB patients than in the general Finnish population. Compared to sleep apnea, partial upper airway obstruction was associated with a threefold prevalence of asthma and/or COPD in both genders, and with a 60% reduced prevalence of hypertension in females matched for age and BMI. Our results emphasize that partial upper airway obstruction is not a milder form of SDB but a different entity, the severity of which is underestimated when using the conventional apnea-hypopnea index. It seems clinically relevant to diagnose and treat the co-morbidities and SDB also in patients with partial upper airway obstruction, especially in elderly and symptomatic women.

Background: Pacemaker implantation (PMI) may predispose to venous thromboembolism (VTE) and obstruction (VO). This prospective study aimed at quantifying changes in venous calibers, and at determining the incidence of symptomatic and asymptomatic VTE/VO after PMI. Further goals included an assessment of the role of transesophageal echocardiography (TEE) in the diagnosis of lead-related central venous thrombi (CVT), and determination of predictors for VTE/VO. Methods: 150 (mean age 67; 61% male) consecutive patients with first PMI were enrolled and followed for 6 months. Contrast venography was performed at baseline and 6 months after PMI to measure venous diameters, and to detect stenosis, total occlusions and thrombi. TEE was conducted in 66 patients. Based on clinical suspicion, work-up for pulmonary embolism (PE) or acute deep vein thrombosis (DVT) were performed as needed. A total of 50 cases underwent longer-term (mean 2.4 years) follow-up venography. All cases with VTE/VO during the initial 6 months, and their matched controls, were selected for a case-control study focused on possible predictive role of laboratory and patient-related factors for the development of VTE/VO. Results: 10 (7 %) patients were found to have baseline venous abnormalities (e.g. 8 obstructions). Mean venous diameters diminished significantly during the first 6 months, but no further reduction occurred in late follow-up. New VO was discovered in 19 patients (14 %; 14 stenosis, 5 total occlusions; all asymptomatic). Small non-obstructive thrombi were found in 20/140 (14 %) 6-month venograms. TEE at 6 months disclosed CVT in 6 (9 %) patients. One (0.7 %) patient had acute symptomatic upper-extremity DVT, and PE was discovered in 5/150 (3.3 %) patients during the first 6 months with no further cases thereafter. At 6 months, the total number of cases with VTE/VO amounted to 47 (31.3 %). Additionally, the later 2-year venograms (n=50) disclosed 4 (8 %) total occlusions and 1 (2 %) stenosis. In the case-control study, no parameter was predictive of venous end-points as a single variable, but there appeared to be significant clustering of traditional VTE risk-factors among the cases. Laboratory parameters showed a definite acute hypercoagulative state induced by PMI, but its degree did not predict subsequent development of VTE/VO. Conclusions: This study shows that VTE/VO is relatively common after PMI with an overall incidence of at least 30 %. Although the majority of the lesions are asymptomatic and clinically benign, cases of PE were also encountered, and totally occluded veins may hamper future upgrading or replacement of pacing system. Venous complications seem difficult to prognosticate as firm predictors were not identified from a wide range of parameters analyzed in this study, although clustering of classic VTE risk factors may be a predisposing factor. Parameters related to implantation procedure or pacing systems and the severity of implantation-induced trauma did not emerge as predictors.

This study analyzed the feasibility and efficacy of surgical therapies in patients with sleep-disordered breathing ranging from partial upper airway obstruction during sleep to severe obstructive sleep apnea syndrome. The surgical procedures evaluated were tracheostomy, laser-assisted uvulopalatoplasty (LUPP) and uvulopalatopharyngoplasty (UPPP) with laser or ultrasound scalpel. Obstructive sleep apnea and partial upper airway obstruction during sleep were measured with the static charge-sensitive bed (SCSB) and pulse oximeter. The patients with severe obstructive sleep apnea syndrome were treated with tracheostomy. Palatal surgery was performed only if the upper airway narrowing occurred exclusively at the soft palate level in patients with partial upper airway obstruction during sleep. The ultrasound scalpel technique was compared to laser-assisted UPPP. The efficacy of LUPP to reduce partial upper airway obstruction during sleep was assessed and histology of uvulopalatal specimen was compared to body fat distributional parameters and sleep study findings. Tracheostomy was effective therapy in severe obstructive sleep apnea. Partial upper airway obstruction and arterial oxyhemoglobin desaturation index during sleep decreased significantly after LUPP. The minimal retropalatal airway dimension increased and soft palate collapsibility decreased at the level where the velopharyngeal obstruction had occurred before the surgery. Ultrasound scalpel did not offer any significant benefits over the laser-assisted technique, except fewer postoperative haemorrhage events. The loose connective tissue as a manifestation of edema was the only histological finding showing correlation with partial upper airway obstruction parameters of SCSB. Tracheostomy remains a life-saving therapy and also long-term option when adherence to CPAP fails in patients with obstructive sleep apnea syndrome. LUPP effectively reduces partial upper airway obstruction during sleep provided that obstruction at the other levels than the soft palate and uvula were preoperatively excluded. Technically the ultrasound scalpel or laser surgeries are equal. In patients with partial upper airway obstruction the loose connective tissue is more important than fat accumulation in the soft palate. This supports the hypothesis that edema is a primary trigger for aggravation of upper airway narrowing during sleep at the soft palate level and evolution towards partial or complete upper airway obstruction during sleep.

Plug dynamics of non compensate drip tubes were evaluated, by the precipitation of moisturized whitewash [Ca(OH)2], which is used in the fertigation for the bulb pH control of the trademarks Azud, Amanco, Naandan, Netafim, Petroisa, Queen Gil, with flow rate varying between 0.4 to 3.0 L h-1 usually used in the country. For this matter, increasing doses of Ca(OH)² were applied in the irrigation water, from 0.01 g L-1 to 1.84 g L-1. The flow rate of each drip tube was measured in intervals of time initially of 7 days, later of 15 days of system operation, totaling a time of 100 days of operation, corresponding to nine applications or 432 hours. The coefficient of variation (CV), and relative rate flow (Qr) were evaluated. The results pointed differences among the evaluated emitter regarding the occurrence of the clogging, and the models G2 and G5 presented the smallest levels of flow rate variation comparing to the models G6, G7 and G9.
