This study was designed to compare the writing motivation of students with specific language impairments with their non-disabled peers. Due to the cognitive and linguistic demands of the writing process, students with language impairments face unique difficulties during the writing process. It was hypothesized that students with specific language impairments will be more likely to report lower levels of perceived writing competence and be less autonomously motivated to write. Students in grades 3-5 in 11 schools (33 with specific language impairments, 242 non-disabled peers) completed self-report measures, designed from a Self-Determination Theory perspective, which measured the degree that students are intrinsically motivated to write as well as their perceived writing competence. Statistical analyses showed that (1) students with specific language impairments reported lower levels of perceived writing competence and autonomous writing motivation; (2) SLI status was a significant predictor of perceived writing competence after spelling, grade, and gender were controlled; and (3) when spelling, grade, and gender were controlled, perceived writing competence was a significant predictor of autonomous writing motivation, but SLI status was not. The results of this study are expected to inform the current understanding of the relationship between language ability and writing motivation in students with specific language impairments, as well as the design of future writing interventions.

In spite of improved antimicrobial therapy, bacterial meningitis still results in brain damage leading to significant long-term neurological sequelae in a substantial number of survivors, as confirmed by several recent studies. Meningitis caused by Streptococcus pneumoniae is associated with a particularly severe outcome. Experimental studies over the past few years have increased our understanding of the molecular mechanisms underlying the events that ultimately lead to brain damage during meningitis. Necrotic damage to the cerebral cortex is at least partly mediated by ischemia and oxygen radicals and therefore offers a promising target for adjunctive therapeutic intervention. Neuronal apoptosis in the hippocampus may represent the major pathological process responsible for cognitive impairment and learning disabilities in survivors. However, the mechanisms involved in causing this damage remain largely unknown. Anti-inflammatory treatment with corticosteroids aggravates hippocampal damage, thus underlining the potential shortcomings of current adjuvant strategies. In contrast, the combined inhibition of matrix metalloproteinase and tumour necrosis factor-alpha converting enzyme protected both the cortex and hippocampus in experimental meningitis, and may represent a promising new approach to adjunctive therapy. It is the hope that a more refined molecular understanding of the pathogenesis of brain damage during bacterial meningitis will lead to new adjunctive therapies.

Multiplication of bacteria within the central nervous system compartment triggers a host response with an overshooting inflammatory reaction which leads to brain parenchyma damage. Some of the inflammatory and neurotoxic mediators involved in the processes leading to neuronal injury during bacterial meningitis have been identified in recent years. As a result, the therapeutic approach to the disease has widened from eradication of the bacterial pathogen with antibiotics to attenuation of the detrimental effects of host defences. Corticosteroids represent an example of the adjuvant therapeutic strategies aimed at downmodulating excessive inflammation in the infected central nervous system. Pathophysiological concepts derived from an experimental rat model of bacterial meningitis revealed possible therapeutic strategies for prevention of brain damage. The insights gained led to the evaluation of new therapeutic modalities such as anticytokine agents, matrix metalloproteinase inhibitors, antioxidants, and antagonists of endothelin and glutamate. Bacterial meningitis is still associated with persistent neurological sequelae in approximately one third of surviving patients. Future research in the model will evaluate whether the neuroprotective agents identified so far have the potential to attenuate learning disabilities as a long-term consequence of bacterial meningitis.

As Henderson and Pochin point out in the introduction to their book, recent years have seen the concept of advocacy given increasing prominence in central and local government policy in the UK. It made an appearance in local community care and long-stay hospital closure plans. It features in reforms to the health service in England and Wales, in the form of the Patient Advocacy and Liaison Services (DoH 2000), while proposed changes to the mental health system also accord a key role to service users' advocates. In addition, Valuing People, central government's proposals on the future strategy for people with learning disabilities, promised the widespread development of advocacy services (DoH 2001). Advocacy, traditionally located on the margins of state activity in the UK, is experiencing something of an attempt to shift it into mainstream policy and service provision. This makes it a significant time to review the core values and practices that have distinguished advocacy from other forms of professional and voluntary intervention and to explore how these may be preserved and developed in the contemporary context.

OBJECTIVES Congenital portosystemic shunts (CPSSs) are rare but increasingly recognized as a cause of important multisystem morbidity. We present new cases and a systematic literature review and propose an algorithm for the identification and care of affected patients. METHODS We reviewed the charts of consecutive patients seen in our pediatric liver clinic between 2003 and 2010 and systematically reviewed the literature of cases with CPSS. RESULTS We identified 316 published cases and 12 patients in our own clinic. Of the published cases (177 male), 185 had an extrahepatic and 131 an intrahepatic portosystemic shunt. Diagnosis was made at any age, from prenatal to late adulthood. Cardiac anomalies were found in 22% of patients. The main complications were hyperammonemia/neurological abnormalities (35%), liver tumors (26%), and pulmonary hypertension or hepatopulmonary syndrome (18%). The spectrum of neurological involvement ranged from changes in brain imaging, subtle abnormalities on neuropsychological testing, through learning disabilities to overt encephalopathy. Spontaneous shunt closure occurred mainly in infants with intrahepatic shunts. Therapeutic interventions included shunt closure by surgery or interventional radiology techniques (35%) and liver transplantation (10%) leading to an improvement of symptoms in the majority. These findings mirror the observations in our own patients. CONCLUSIONS In this largest review of the reported clinical experience, we identify that children with CPSS may present with otherwise unexplained developmental delay, encephalopathy, pulmonary hypertension, hypoxemia, or liver tumors. When CPSS is diagnosed, children should be screened for all of these complications. Spontaneous closure of intrahepatic shunts may occur in infancy. Closure of the shunt is indicated in symptomatic patients and is associated with a favorable outcome.

Our last study with regularly developed children demonstrated a positive effect of working memory training on cognitive abilities. Building upon these findings, the aim of this multidisciplinary study is to investigate the effects of training of core functions with children who are suffering from different learning disabilities, like AD/HD, developmental dyslexia or specific language impairment. In addition to working memory training (BrainTwister), we apply a perceptual training, which concentrates on auditory-visual matching (Audilex), as well as an implicit concept learning task. We expect differential improvements of mental capacities, specifically of executive functions (working memory, attention, auditory and visual processing), scholastic abilities (language and mathematical skills), as well as of problem solving. With that, we hope to find further directions regarding helpful and individually adapted interventions in educational settings. Interested parties are invited to discuss and comment the design, the research question, and the possibilities in recruiting the subjects.

Gastroschisis is a birth defect in which an opening in the abdominal wall allows herniation of the viscera. Prenatal counseling regarding gastroschisis typically discusses that, although these infants often endure a difficult neonatal course, they experience few long-term complications. However, information regarding long-term outcomes is based on limited studies that lack specificity. Therefore, we aimed to study the long-term morbidity and quality of life in children born with gastroschisis in a large and diverse population drawn from the Texas Birth Defects Registry (TBDR). Study packets with informed consent, a questionnaire, and the Pediatric Quality of Life Inventory Generic Core Scale 4.0 (PedsQL 4.0) in English and Spanish were mailed to 1,112 parents of children born with isolated gastroschisis in Texas between 1999 and 2008 via the TBDR. Information was abstracted from the TBDR for 58 mothers of children with gastroschisis who returned study materials. Three hundred fifty five packets were returned to sender, giving a response rate of 7.7%. Children born with gastroschisis had quality of life scores that were not significantly different than expected (p = 0.981). However, factors such as having a learning disability (p = 0.001) and missing school due to gastrointestinal issues (p = 0.020) were found to significantly decrease quality of life. Overall, children with gastroschisis had a significantly increased risk for learning disabilities regardless of whether they were preterm (p = 0.021) or full term (p = 0.021). Additionally, there appeared to be an increased risk for auditory impairment in Caucasian children (p < 0.0005). Therefore, while overall long-term quality of life is not significantly altered for children born with gastroschisis, the previously unreported increased risk for learning disabilities and possible association with hearing impairment are important findings that should be conveyed to prospective parents.

El presente trabajo está basado en el recorte de algunos resultados y conclusiones de la investigación que desarrollé en el marco de la beca doctoral UBACyT (2003-2007) acerca de los "Aspectos histórico-libidinales en niños y niñas con problemas en sus aprendizajes que manifiestan dificultades atencionales". Se partió de la idea de que no existen niños que no atiendan en lo absoluto, que de lo que se tratará es de ubicar en qué tienen puesta su atención los niños desatentos. Las conclusiones principales fueron: La necesidad de plantear las dificultades atencionales como resultante de múltiples determinaciones y no tan sólo a causa de un supuesto déficit neurológico. La responsabilidad profesional de diferenciar diagnósticos descriptivos, de diagnósticos estructurales de sujetos en constitución. Se cuestionó la dicotomía cuerpo - mente, se propuso pensar la intersección entre “vulnerabilidad genética" y “potencialidad traumática". Se encontró una estrecha ligazón de las dificultades atencionales con las historias libidinales. Se hallaron situaciones silenciadas en todas las historias analizadas, modalidades a predominio de la desinvestidura y la preponderancia de situaciones traumáticas vivenciadas por las diferentes generaciones que el psiquismo prefiere mantener desligadas, incluso a costa del empobrecimiento subjetivo y las dificultades para simbolizar. A partir de este recorrido se proponen algunas herramientas para abordar la problemática tanto desde el espacio clínico, como escolar.

Surge de la consideración de niveles de rendimiento críticos en el dominio de la lectura y escritura en diferentes niveles educativos, señalados reiteradamente en la literatura especializada del país y del exterior. Con ese propósito se revisan las habilidades y conocimientos implicados a partir de la evidencia empírica surgida de múltiples investigaciones. En el estado actual de la investigación en curso es posible identificar la insuficiencia de disponibilidad y uso de instrumentos de evaluación en nuestro medio. Se desprenden de ello, por un lado, las dificultades para detectar la oportunidad y naturaleza de los problemas que presentan los lectores con dificultades y con trastornos específicos de aprendizaje, y por el otro, la insuficiencia de información sobre indicadores específicos que permitan diseñar las estrategias para optimizar el desempeño en lectura y escritura

Este trabajo de investigación presenta los resultados de una revisión sistemática realizada a partir de la recopilación, lectura y análisis de distintas fuentes bibliográficas dentro de un conjunto heterogéneo consistente de 175 estudios que forman la base bibliográfica actual del documento “Cognitive Accessibility User Research” (W3C, 2015a) del W3C. Esta base bibliográfica está compuesta por publicaciones científicas basadas en libros, artículos, conferencias y sitios Web especializados, en los cuales se potencia como objeto particular de análisis, la indagación en la búsqueda de pautas de accesibilidad en las tecnologías Web que apoyen la integración a personas con discapacidad cognitiva. Como parte de este proceso de investigación se ha recopilado y descrito la situación actual, particularmente, de los retos en la utilización de las tecnologías de la información y la comunicación (TIC) en relación a personas con dificultades de aprendizaje o discapacidades cognitivas, tales como la Dislexia, Afasia, Trastorno de Aprendizaje No verbal, Envejecimiento-Demencia, Trastornos por Déficit de Atención con o sin Hiperactividad, Autismo, Síndrome de Down y Discalculia. Como aporte primordial de este Trabajo Fin de Master (TFM), se intenta trazar una línea de criterios que permitan la evaluación objetiva de este tópico, con miras a ofrecer un enfoque práctico y reciente sobre ésta temática, mostrando de forma esquematizada las pautas existentes y sirviendo de síntesis orientativa para el diseño accesible de las TIC con la finalidad de promover un soporte real a personas con los tipo de discapacidad cognitiva en los que se ha enfocado esta investigación. Logrando obtener como resultado principal de este estudio, 36 pautas generales que agrupan las coincidencias del grupo de discapacidades estudiadas y que han sido distribuidas en categorías: texto, navegación y generales, para su mejor interpretación y manejo de la Accesibilidad en las TIC´S para Personas con Discapacidad Cognitiva.---ABSTRACT---This research presents the results of a systematic review from collecting, reading and analysis of different bibliographic sources within a heterogeneous group consisting of 175 studies that form the basis of current literature document "Accessibility User Cognitive Research" (W3C , 2015th) of the W3C. This bibliographic database is composed of scientific publications based on books, articles, lectures and specialized Web sites, in which is enhanced as a particular object of analysis, the inquiry into the search for accessibility guidelines for Web technologies to support integration of people with cognitive disabilities. As part of this research process, the current situation has been collected and described, particularly the challenges in the use of information and communications technology (ICT) in relation to people with learning disabilities or cognitive disabilities, such as Dyslexia, aphasia, nonverbal learning disorder, aging-Dementia, Attention Deficit Disorders with or without hyperactivity, autism, Down syndrome and dyscalculia. As primary contribution of this Master's Thesis (TFM), it tries to draw a line of criteria to allow an objective assessment of this topic, in order to provide a practical and recent focus on this theme, showing schematically existing guidelines and serving as guidance for accessible design of ICT in order to promote a real support to people with cognitive disabilities where this research has focused on. Managing to obtain the main result of this study, 36 general guidelines that group the set of disabilities studied and have been distributed in categories: text, navigation and general, for better interpretation and management of ICTs for Accessibility people with cognitive disabilities.

Dr. Zelma Lloyd Frank was very active in the Jefferson City community and received both her Bachelor’s and Master’s degrees from Lincoln University. She received her Doctorate in Education at The University of Missouri-Columbia. Dr. Frank is a member of Phi Delta Kappa, Kappa Delta Pi, and the International and State Association for Children with Learning Disabilities. She was a teaching and reading assistant for elementary students in Missouri, Massachusetts, and New York. She was very active with the Capital City Council on the Arts, Meals on Wheels, St. Mary’s Health Center Auxiliary, and the local High School PTA.

Este estudo investiga evidências da influência de uma intervenção psicomotora lúdica na construção do pensamento operatório concreto e desempenho neuromotor de alunos com dificuldade de aprendizagem da 2ª série do ensino fundamental, de escola pública do Estado de São Paulo. Utiliza-se de método experimental. Manipula a intervenção psicomotora lúdica, (Variável Independente-VI), com o objetivo de verificar a sua possível influência no desempenho cognitivo relativo a noções de Conservação, Classificação, Seriação e Aritmética, assim como no neuromotor relativo a Agilidade e Orientação Direita-Esquerda (Variáveis Dependentes - VDs) - totalizando 6 VDs. A amostra compõe-se por 18 escolares, na faixa etária de 7 a 11 anos, de ambos os sexos, organizados em dois grupos: experimental (N=9) e controle (N=9). O procedimento experimental desenvolve-se em 16 sessões, constando de três etapas. Aos dois grupos são aplicados, individualmente, pré-teste (1ª etapa) e pós-teste (3ª etapa), constando de duas sessões individuais em cada etapa, com a utilização dos seguintes instrumentos: Provas Operatórias de Piaget, teste Piaget-Head de Orientação Direita-Esquerda e subteste de Aritmética do teste de Desempenho Escolar de Stein, teste de Shuttle Run . A 2ª etapa, exclusiva do grupo experimental, consta da intervenção psicomotora lúdica, em 12 sessões grupais de 50 minutos cada. Utilizou-se da prova de Wilcoxon, para comparação dos resultados entre os grupos. Os resultados referentes às noções de Classificação ( p=0,010), Seriação (p=0,034), Aritmética (p=0,157) e Orientação Direita- Esquerda (p=0,007), indicam que ocorreu uma diferença significativa estatisticamente, considerando-se que os participantes apresentaram desempenho superior nos pós-testes. Nas demais provas, foi observado melhor desempenho nos dois grupos, o que, portanto, não pode ser atribuído à intervenção. Conclui-se que o objetivo do trabalho foi atingido, visto que o programa mostrou-se eficiente para desenvolver o pensamento operatório e o neuromotor em relação a Orientação Direita-Esquerda dos participantes.(AU)
