Patients with systemic lupus erythematosus (SLE) can be affected by a multitude of neurologic and psychiatric symptoms with a wide range of prevalence and severity. Irrespectively from attribution to SLE or other causes, neuropsychiatric (NP) symptoms strongly impact short-term and long-term outcomes,thus NP evaluation during routine clinical practice in SLE should be undertaken regularly. The assessment of NP involvement in SLE patients is challenging and the available diagnostic tools fail to guarantee optimal diagnostic accuracy, sensitivity to changes as well as feasibility in routine clinical care. Standardised questionnaires (both physician-administered and self-reported) can offer valuable help to the treating physician to capture all possible NP syndromes; few SLE-specific NP questionnaire have been developed but validation in large cohort or cross-cultural adaptations are still pending. On the other hand, general instruments have been largely applied to SLE patients. Both kinds of questionnaires can address all possible NP manifestations either globally or, more frequently, focus on specific NP symptoms. These latter have been mainly used in SLE to detect and classify mild and subtle symptoms, more likely to be overlooked during routine clinical assessment such as headache, cognitive impairment and psychiatric manifestations. In conclusion, this literature review highlights a clear case for validation studies in this area and the wider implementation of questionnaires to assess NP involvement is still warranted. The broader use of such instruments could have important consequences; first of all, by standardising symptom assessment, a better definition of the prevalence of NP manifestation across different centres could be achieved. Secondly, prospective studies could allow for the evaluation of clinical significance of mild symptoms and their impact on the patient’s function and quality of life.

OBJECTIVE: To assess the spectrum and prevalence of mutations in the GJB2 gene in Portuguese nonsyndromic sensorineural hearing loss (NSSHL) patients. DESIGN: Sequencing of the coding region, basal promoter, exon 1, and donor splice site of the GJB2 gene; screening for the presence of the two common GJB6 deletions. STUDY SAMPLE: A cohort of 264 Portuguese NSSHL patients. RESULTS: At least one out of 21 different GJB2 variants was identified in 80 (30.2%) of the 264 patients analysed. Two mutant alleles were found in 53 (20%) of these probands, of which 83% (44/53) harboured at least one c.35delG allele. Twenty-seven (10.2%) of the probands harboured only one mutant allele. Subsequent analysis revealed that the GJB6 deletion del(GJB6-D13S1854) was present in at least 7.4% (2/27) of the patients carrying only one mutant GJB2 allele. Overall, one in five (55/264) of the patients were diagnosed as having DFNB1-related NSSHL, of which the vast majority (53/55) harboured only GJB2 mutations. CONCLUSIONS: This study provides clear demonstration that mutations in the GJB2 gene are an important cause of NSSHL in Portugal, thus representing a valuable indicator as regards therapeutical and rehabilitation options, as well as genetic counseling of these patients and their families.

In August 1983,85 inhabitants of the municipality of Humaitá, Amazonas State, Brazil were studied to determine the prevalence of antigens to HLA-A, -B, -C and DR. Thirty-eight were sick with malaria due to Plasmodium falciparum. All subjects were examined for splenomegaly, blood parasitaemia and antibodies to malaria. They constituted three groups: 1) 25 subjects native to the Amazon region who had never had malaria; 2) 38 Amazonian subjects who had malaria in the past or currently had an infection; 3) 22 patients with malaria who had acquired the infection in the Amazon Region but came from other regions of Brazil. Blood was taken from each person, the lymphocytes were separated and typed by the test of microlymphocytotoxicity. There was a high frequency of antigens that could not be identified in the groups studied which suggests the existence of a homozygote or phenotype not identified in the population. There was a high frequency of the phenotype Ag(W24) (44.7%) in group 2 when compared with group 1 (32%) or group 3 (9%). Also the individuals in group 2 showed an elevated frequency of antigen DR(4)80%) when compared with group 1 (36.6%) or group 3 (16.6%). These observations suggest the possibility of a genetic susceptibility to malaria among Amazonian residents and indicate a necessity for more extensive studies of the frequency of HLA antigens among inhabitants of this endemic malarial zone.

The frequency of myocarditis associated with meningococcal disease in children was reported only in two autopsied series (United States and South Africa). Here we report the frequency of associated myocarditis in 31 children who died of meningoccal infection at Hospital Infantil N.S. da Glória in Vitória, Espirito Santo State, Brazil. The diagnosis was confirmed by isolation of Neisseria meningitidis . At least three sections of fragments of both atria and ventricles were studied using the Dallas Criteria for the morphologic diagnosis of myocarditis. The mean age was 47.6 ± 39.8 months and the mean survival time after the onset of symptoms was 46.1 ± 26.5h (12-112h). Myocarditis was present in 13 (41.9%) patients, being of minimal severity in 11 cases and of moderate severity in 2 cases. There were no cases with severe diffuse myocarditis. The frequency of myocarditis was not influenced by sex, presence of meningitis, survival time after the onset of symptoms or use of vasoactive drugs. The frequency of myocarditis reported here was intermediate between the values reported in the only two case series published in the literature (57% in the United States and 27% in South Africa). Although our data confirm the high frequency of myocaditis in meningoccal disease, further investigations are necessary to elucidate the contribution of myocarditis to myocardial dysfunction observed in cases of meningococcal infection in children.

Ureaplasma urealyticum (UU) and Mycoplasma hominis (MH) have been detected in the urine of women with systemic lupus erythematosus (SLE). We evaluated the presence of these mycoplasma in the endocervix of women presenting SLE. A total of 40 SLE patients (mean age 40.2 years), and 51 healthy women (mean age 30.9 years), were studied. Endocervical swabs were cultured in specific liquid media for MH or UU, detected by a quantitative color assay, and considered positive at >10³ dilutions. Statistical analysis was performed using the two-tailed Fisher test. UU was detected in 52.5 % of patients and in 11.8% of controls (p= 0.000059). MH was detected in 20% of patients and 2% controls (p=0.003905). Both mycoplasmas were detected in 7.3% patients and 0% controls (p<0.000001). The results reported here corroborate the association of the mycoplasma infection and SLE. Thus, these agents may stimulate the production of autoreactive clones.

In August/1999, a group of 14 adults from the staff of a private hospital in Contagem -- Minas Gerais State, Brazil, received unintentionally a 25 times concentrated dose of the 17-DD yellow fever vaccine (Bio-Manguinhos), due to a mistake at the reconstitution step. All patients were clinically and laboratorially evaluated at days 5, 13 and 35 post vaccination. Frequency of side effects and clinical observations of this group of individuals were not different from the observed in recipients immunized with normal doses of the vaccine. At the second and third evaluation none of the subjects reported symptoms. None of the patients presented abnormalities at the physical examination at none of the time points and in all cases the blood examination was normal, except for a reduced number of platelets that was detected in one subject at the first and second evaluation and reverted to normal at third evaluation. At the first evaluation point, 8 subjects were serum negative and 6 serum positive for yellow fever at the plaque reduction neutralization test. In 5 subjects the observed titre was 10 times higher as the baseline of 2.36 Log10 mUI/ml. The samples collected at second and third evaluation (13th and 35th days) demonstrated that all subjects responded to the vaccination with the exception of one that did not present a positive result in any of the samples collected. This evaluation confirms the safety of the 17-DD yellow fever vaccine.

Cash-in-advance models usually require agents to reallocate money and bonds in fixed periods, every month or quarter, for example. I show that fixed periods underestimate the welfare cost of inflation. I use a model in which agents choose how often they exchange bonds for money. In the benchmark specification, the welfare cost of ten percent instead of zero inflation increases from 0.1 percent of income with fixed periods to one percent with optimal periods. The results are robust to different preferences, to different compositions of income in bonds or money, and to the introduction of capital and labor.

Acute infections by the protozoan Toxoplasma gondii during pregnancy (gestational toxoplasmosis) are known to cause serious health problems to the fetus (congenital toxoplasmosis). In Brasília, there have been few studies on the incidence of toxoplasmosis. This report summarizes a retrospective study performed on 2,636 selected pregnant women attended by the public health system of Guará, a satellite-city of Brasília. In this survey, 17 cases of gestational toxoplasmosis were detected; 15 of which were primary maternal infection and the remaining 2 were consistent with secondary maternal infection. These results suggest an annual seroconversion rate of 0.64 percent (90 percent confidence interval: 0.38, 0.90).

The objective of the present study was to investigate the frequency and risk factors for developing multidrug-resistant tuberculosis in Cabo de Santo Agostinho, PE. This was a prospective study conducted from 2000 to 2003, in which suspected cases were investigated using bacilloscopy and culturing. Out of 232 confirmed cases of tuberculosis, culturing and antibiotic susceptibility tests were performed on 174. Thirty-five of the 174 cultures showed resistance to all drugs. The frequencies of primary and acquired resistance to any drug were 14% and 50% respectively, while the frequencies of primary and acquired multidrug resistance were 8.3% and 40%. Previous tuberculosis treatment and abandonment of treatment were risk factors for drug resistance. The high levels of primary and acquired resistance to the combination of isoniazid and rifampicin contributed towards the difficulties in controlling tuberculosis transmission in the city.

Blastocystis homins is a protozoan that causes an intestinal infection known as human blastocystosis. This infection is diagnosed by means of parasitological examination of stools and by permanent staining techniques. The present study was developed to evaluate the frequency of Blastocystis hominis infection among inhabitants of the Araraquara region, State of São Paulo, and to compare different methods for investigating this protozoan in feces samples. Evaluations on 503 stool samples were performed by means of direct fresh examination and using the techniques of Faust et al., Lutz and Rugai et al. In addition, the iron hematoxylin, trichrome and modified Kinyoun staining techniques were used. Out of the 503 samples examined, 174 (34.6%) were found to be positive for the presence of intestinal parasites. The most frequent protozoa and helminths were Entamoeba coli (14.6%) and Strongyloides stercoralis (6.7%), respectively. Blastocystis hominis was present in 23 (4.6%) fecal samples, with a predominately pasty consistency and without characterizing a condition of diarrhea. Despite the low frequency of Blastocystis hominis found in the Araraquara region, compared with other regions of Brazil, it is important to perform laboratory diagnostic tests for this protozoan. Its finding in fecal material is indicative of food and drinking water contamination. Since the transmission route for this parasite is accepted to be oral-fecal, this implies that the population needs guidance regarding hygiene and basic sanitation measures as a means for controlling health problems caused by enteroparasites.

Human Activity Recognition systems require objective and reliable methods that can be used in the daily routine and must offer consistent results according with the performed activities. These systems are under development and offer objective and personalized support for several applications such as the healthcare area. This thesis aims to create a framework for human activities recognition based on accelerometry signals. Some new features and techniques inspired in the audio recognition methodology are introduced in this work, namely Log Scale Power Bandwidth and the Markov Models application. The Forward Feature Selection was adopted as the feature selection algorithm in order to improve the clustering performances and limit the computational demands. This method selects the most suitable set of features for activities recognition in accelerometry from a 423th dimensional feature vector. Several Machine Learning algorithms were applied to the used accelerometry databases – FCHA and PAMAP databases - and these showed promising results in activities recognition. The developed algorithm set constitutes a mighty contribution for the development of reliable evaluation methods of movement disorders for diagnosis and treatment applications.

RESUMO - Introdução: Os problemas do sono, designadamente a insónia, os sintomas de insónia, os padrões de sono inadequados e a sonolência diurna, são frequentes na adolescência. Estes problemas estão frequentemente associados a múltiplos fatores, entre os quais estilos de vida e fatores ambientais, e apresentam consequências significativas na vida do adolescente e posteriormente na idade adulta. O sono e as suas perturbações deveriam constituir uma preocupação para os profissionais da saúde e da educação com o objetivo de tornar os hábitos de sono saudáveis num estilo de vida - com benefícios calculáveis como os associados a outros estilos de vida saudáveis (alimentação e exercício físico). Em Portugal, os estudos sobre problemas do sono em adolescentes são escassos, bem como as intervenções individuais e comunitárias no âmbito da higiene do sono. Os objetivos desta investigação foram estimar a prevalência de insónia e de sintomas de insónia em adolescentes, identificar fatores de risco e protetores dos sintomas de insónia, analisar as repercussões dos sintomas de insónia, caracterizar os padrões de sono dos adolescentes do distrito de Viseu e elaborar uma proposta de intervenção destinada à promoção da higiene do sono adaptada às características dos adolescentes do distrito de Viseu. Métodos: Realizou-se um estudo transversal onde se avaliaram alunos de vinte e seis escolas públicas do terceiro ciclo e secundário do distrito de Viseu, durante ano letivo 2011/2012. A recolha dos dados foi efetuada através de um questionário autoaplicado e respondido pelos alunos em sala de aula. Foram considerados elegíveis para participar no estudo todos os alunos que frequentassem entre o 7.º e o 12.º ano de escolaridade e tivessem idades entre os 12 e os 18 anos. Dos 9237 questionários distribuídos recolheu-se 7581 (82,1%). Foram excluídos da análise os questionários relativos a adolescentes com idade inferior a 12 ou superior a 18 anos e os questionários devolvidos por preencher. A amostra global foi constituída por 6919 adolescentes, sendo 3668 (53,2%) do sexo feminino. A insónia foi definida com base na presença, no mês prévio, dos sintomas de insónia definidos nos critérios do DSM-IV (dificuldade em adormecer, dificuldade em manter o sono, acordar muito cedo e ter dificuldade em voltar a adormecer e sono não reparador) com uma frequência de pelo menos três vezes por semana e associados a consequências no dia-a-dia. A qualidade de vida foi avaliada com recurso à escala de qualidade de vida SF-36; a sintomatologia depressiva através do Inventário de Depressão de Beck para adolescentes (BDI-II) e a sonolência diurna utilizando a Escala de Sonolência de Epworth (ESE). Para responder ao último objetivo foi elaborada uma proposta de intervenção individual e comunitária no âmbito da higiene do sono. A proposta resulta da evidência científica, dos resultados da presente investigação e de reuniões com profissionais da saúde e da educação. Resultados: No total da amostra, a prevalência de insónia foi de 8,3% e de sintomas de insónia foi de 21,4%. A prevalência de insónia foi superior no sexo feminino (10,1% vs. 5,9%; p<0,001) assim como a prevalência de sintomas de insónia (25,6% vs. 15,8%; p<0,001). Individualmente, todos os sintomas foram mais prevalentes no sexo feminino, sendo a diferença estatisticamente significativa (p<0,001). Em média os adolescentes dormiam, durante a semana, 8:04±1:13 horas. A prevalência de sono insuficiente (< 8 horas) foi de 29%. Apenas 6,4% dos adolescentes indicaram que se deitavam todas as noites à mesma hora. A prevalência de sintomatologia depressiva foi de 20,9% (26,0% nas raparigas e 15,1% nos rapazes, p<0,001). A prevalência de sonolência diurna foi de 33,1%, apresentando o sexo feminino um risco superior (OR=1,40; IC95%: 1,27-1,55). A prevalência de sintomatologia depressiva e de sonolência diurna foi superior entre os adolescentes com sintomas de insónia (48,2% vs. 18,8%, p<0,001 e 42,4% vs. 33,0%, p<0,001, respetivamente). Os adolescentes com sintomas de insónia apresentavam igualmente pior qualidade de vida. Em relação a outras repercussões no dia-a-dia, foram os adolescentes com sintomas de insónia que referiam mais vezes sentir dificuldade em levantar-se de manhã, acordar com cefaleias, acordar cansado e recorrer a medicação para dormir. Nos rapazes os sintomas de insónia associaram-se com o IMC. Após o ajustamento para o sexo e idade com recurso à regressão logística verificou-se uma associação entre sintomas de insónia e sexo feminino [OR ajustado(idade)= 1,82; IC95%: 1,56-2,13], idade ≥16 anos [OR ajustado(sexo)= 1,17; IC95%: 1,01-1,35], residência urbana (OR ajustado= 1,30; IC95%: 1,04-1,63), consumo de café (OR ajustado= 1,40; IC95%: 1,20-1,63), consumo de bebidas alcoólicas (OR ajustado= 1,21; IC95%: 1,03-1,41) e sintomatologia depressiva (OR ajustado= 3,59; IC95%: 3,04-4,24). Quanto à escolaridade dos pais, verificou-se uma redução do risco com o aumento da escolaridade dos pais (5º-6º ano OR ajustado= 0,82; IC95%: 0,64- 1,05; 7º-12º ano OR ajustado= 0,77; IC95%: 0,61-0,97; >12º ano OR ajustado= 0,64; IC95%: 0,47-0,87). Após uma análise multivariada, o modelo preditivo para a ocorrência de sintomas de insónia incluiu as variáveis sexo feminino, viver em meio urbano, consumir café e apresentar sintomatologia depressiva. Este modelo apresenta uma especificidade de 84,2% e uma sensibilidade de 63,6%. O sono insuficiente associou-se, após ajuste para o sexo e idade, com o ano de escolaridade, estado civil dos pais, determinados estilos de vida (consumo de café, tabagismo, consumo de álcool, consumo de outras drogas, sair à noite, presença de TV no quarto e número de horas despendido a ver televisão e no computador), latência do sono, sesta > 30 minutos, horários de sono irregulares e com a toma de medicamentos para dormir. Os resultados deste estudo constituem um diagnóstico de situação relativamente aos problemas de sono em adolescentes no distrito de Viseu. Tendo por base os princípios da Carta de Ottawa relativamente à promoção da saúde, a proposta elaborada visa a implementação de estratégias de prevenção agrupadas em intervenções individuais, comunitárias e sobre os planos curriculares. As intervenções baseiam-se na utilização das tecnologias da informação e comunicação, no contexto da nova arquitetura na esfera pública da saúde conducente aos sistemas personalizados de informação em saúde (SPIS). Conclusões: Registou-se uma elevada prevalência de insónia e sintomas de insónia entre os adolescentes do distrito de Viseu, superior no sexo feminino. A presença de sintomas de insónia esteve associada, sobretudo, a determinados estilos de vida e à ausência de higiene do sono. Os problemas de sono em adolescentes, devido à sua frequência e repercussões, devem constituir uma preocupação em termos de saúde pública e constituir uma prioridade nas estratégias de educação para a saúde. Os 9 princípios da intervenção delineada visam uma abordagem preventiva de problemas de sono - através da ação conjunta de profissionais da saúde e da educação, de elementos da comunidade e com o indispensável envolvimento dos adolescentes e da família -, procurando instituir os hábitos de sono saudáveis como um estilo de vida.

The objective of this study was to investigate whether malnourished autopsied adults would present higher frequency of pneumonitis than non-malnourished ones would. All of the autopsied adults (n = 175; age > 18 years) with complete records, including weight and height data, were included. Pneumonitis was observed more frequently in malnourished individuals (59.1%) than in non-malnourished individuals (41.3%). This study showed that the percentage of pneumonitis among autopsied adults was high, in addition to an increased risk of pneumonitis among these individuals.

RESUMO: Introdução: A prestação de cuidados a um familiar idoso dependente pode ser esgotante e interferir adversamente na saúde e bem-estar do cuidador familiar. A literatura tem privilegiado a análise da sobrecarga da prestação de cuidados em cuidadores familiares de idosos em situação de dependência, negligenciando a incidência de morbilidades físicas, como a lombalgia, que podem advir da prestação de cuidados. A lombalgia constitui um dos fatores mais importantes que afeta a saúde física das pessoas idosas e encontra-se associada à diminuição da função física geral. Objetivo: Avaliar a prevalência, as características e os fatores de risco da lombalgia em idosos cuidadores familiares de pessoas idosas com dependência. Metodologia: Foram avaliados trinta e um cuidadores principais de idosos com dependência, com idade ≥ 65 anos. A informação foi recolhida por entrevista através de um questionário geral e três questionários padronizados (Oswerty Disability Questionnaire – versão portuguesa 2.0, MOS SF36 V2.0 e Escala Visual Analógica) que avaliaram as características sociodemográficas, clínicas, antropométricas e comportamentais dos cuidadores familiares. A dependência dos idosos alvo de cuidados também foi avaliada pela Escala de Barthel Modificada. Resultados e conclusão: Os resultados deste estudo sugerem que a prevalência da dor lombar é elevada em idosos cuidadores principais (80,6%). A análise inferencial mostrou que os fatores relacionados com a lombalgia nos cuidadores foram a autoperceção do estado de saúde física e mental (ρS = -0,822, p<0.001 e ρS = -0.566, p=0.001, respetivamente) e a sua idade (p < 0,05). Mais estudos são necessários para melhor definir a frequência da lombalgia e explorar a relação destes fatores de risco.-----------ABSTRACT: Background: The care of a dependent elderly relative can be grueling and adversely affect the health and well-being of family caregivers. Previous research has focused on the analysis of the burden on family caregivers of elderly people in a situation of dependence, neglecting the incidence of physical morbidities, such as low back pain, which may arise from the provision of care. Low back pain is one of the most important factors that affects the physical health of older people and is associated with decreased overall physical function.Purpose: Evaluate the prevalence, features and risk factors of low back pain among old family caregivers of elderly with dependence. Methods: Thirty one primary caregivers of elderly with dependence, with 65 or more years old, were studied. Data were collected by interviews, through a general questionnaire and three standardized questionnaires (Oswestry Disability Questionnaire – Portuguese version 2.0, MOS SF36 V2.0, Visual Analogue Scale) to evaluate social, demographic, clinical, anthropometric and behavioral characteristics of family caregivers. Elderly dependence was also assessed by Modified Barthel Index in old people with disabilities. Results and conclusion: Results of this study suggest that prevalence of low back pain is high in old primary caregivers (80,6%). Forward inferential analysis showed that the factors related to low back pain in the caregivers were their physical and mental health perception (ρS = -0,822, p<0.001and ρS = -0.566, p=0.001, respectively) and age (p < 0,05). Further studies are needed to better define the frequency of low back pain and explore the relationship of these risk factos.
