Ocular neovascularizations are responsible for irreversible loss of vision in various diseases, including age-related macular degeneration. Treatments have changed greatly, and photodynamic therapy with verteporfin has come into common use. However, the visual prognosis remains poor. The recent approval of new antiangiogenic molecules such as ranibizumab and pegaptanib should allow for new therapeutical possibilities. The unapproved ophthalmological use of bevacizumab requires further studies. This paper updates what is known about old and new neovascularization treatments: their mechanism of action, their efficacy, and their toxicity. It reviews the principal clinical studies, and concludes with the recognized recommendations. For the first time, ophthalmologists can hope not only to stabilize loss of vision, but also to improve visual acuity. Complementary treatments can now be tested in associations, concomitantly or not, with the hope of improving visual results.

Issue ownership theory argues that when a voter considers a party to be the most competent amongst others to deal with an issue (that is, the party "owns" the issue), chances are the voter will vote for that party. Recent work has shown that perceptions of issue ownership are dynamic: they are affected by the media coverage of party messages. However, based on the broad literature on partisan bias, we predict that parties' efforts to change issue ownership perceptions will have a difficult time breaching the perceptual screen created by a voter's party preference. Using two separate experiments with a similar design we show that the effect of partisan issue messages on issue competence is moderated by party preference. The effect of issue messages is reinforced when people already like a party, and blocked when people dislike a party.

The liquid and plastic limits of a soil are consistency limits that were arbitrarily chosen by Albert Atterberg in 1911. Their determination is by strictly empirical testing procedures. Except for the development of a liquid limit device and subsequent minor refinements the method has remained basically unchanged for over a half century. The empirical determination of an arbitrary limit would seem to be contrary to the very foundations of scientific procedures. However, the tests are relatively simple and the results are generally acceptable and valuable in almost every conceivable use of soil from an engineering standpoint. Such a great volume of information has been collected and compiled by application of these limits to cohesive soils, that it would be impractical and virtually impossible to replace the tests with a more rational testing method. Nevertheless, many believe that the present method is too time consuming and inconsistent. Research was initiated to investigate the development of a rapid and consistent method by relating the limits to soil moisture tension values determined by porous plate and pressure membrane apparatus. With the moisture tension method, hundreds of samples may be run at one time, operator variability is minimal, results are consistent, and a high degree of correlation to present liquid limit tests is possible.

Soils consist largely of mineral particles in a wide range of sizes. It is advantageous to assign names, such as "sand", etc., to describe particles which lie between certain size limits. These names are convenient to use and give more information than merely stating that the particles fit certain size limitations. Many systems of particle-size limits have been proposed and used, and have many discrepancies. For example, depending on the system used, a term such as "sand" may designate very different materials. Since no clear-cut divisions can be made between members of a continuous series all particle-size limit schemes are arbitrary. The originators of the various systems were influenced by many factors: convenience of investigation, methods and equipment available for analysis, ease of presenting data, convenience for statistical analysis, previous work, and systems in use. The complications were further compounded because of widely varying fields of endeavor with varying background, outlook, and goals. For example, many inconsistencies are found in engineering depending on whether the size limits are used to differentiate soils, or characterize aggregates for concrete. Some of the investigators have tried to place limits to correspond with the various properties of the soil components; others were more interested in the ease and convenience of obtaining and presenting data. The purpose of this paper is to review many of the systems which have been proposed and used, and if possible, to suggest what may have been the reasons for the selection of the particle-size limits.

The electrical stimulation of the dorsal columns of the spinal cord exerts a dual analgesic and vasodilatory effect on ischemic tissues. It is increasingly considered a valuable method to treat severe and otherwise intractable coronary and peripheral artery disease. The quality of the results depends from both a strict selection of the patients by vascular specialists and the frequency and quality of the follow-up controls. However the indications, limits, mode of action and results of spinal cord stimulation are still poorly understood. This article, based on a personal experience of 164 implantations for peripheral and coronary artery disease, aims to draw attention to this technique and to provide information on recent and future developments.

BACKGROUND: A major goal of antiretroviral therapy (ART) for HIV-1-infected persons is the recovery of CD4 T lymphocytes, resulting in thorough protection against opportunistic complications. Interruptions of ART are still frequent. The long-term effect on CD4 T-cell recovery and clinical events remains unknown. METHODS: Immunological and clinical endpoints were evaluated in 2491 participants of the Swiss HIV Cohort Study initiating ART during a mean follow-up of 7.1 years. Data were analysed in persons with treatment interruptions (n = 1271; group A), continuous ART, but intermittent HIV-1 RNA at least 1000 copies/ml (n = 469; group B) and continuous ART and HIV-1 RNA constantly less than 1000 copies/ml (n = 751; group C). Risk factors for low CD4 T-cell counts and clinical events were analysed using Cox proportional hazards models. RESULTS: In groups A-C, CD4 T lymphocytes increased to a median of 427, 525 and 645 cells/μl at 8 years. In group A, 63.0 and 37.2% reached above 350 and 500 CD4 T cells/μl, whereas in group B 76.3 and 55.8% and in group C 87.3 and 68.0% reached these thresholds (P < 0.001). CD4 T-cell recovery directly depended on the cumulative duration of treatment interruptions. In addition, participants of group A had more Centers for Disease Control and Prevention B/C events, resulting in an increased risk of death. Major risk factors for not reaching CD4 T cells above 500 cells/μl included lower baseline CD4 T-cell count, higher age and hepatitis C virus co-infection. CONCLUSION: In persons receiving continuous ART larger CD4 T-cell recovery and a reduced risk for opportunistic complications and death was observed. CD4 T-cell recovery was smaller in persons with treatment interruptions more than 6 months.
