Verificar experimentalmente la utilidad diagnóstica del test guestaltico visomotor para niños en sujetos afectados de disfunción cerebral mínima. Analizar la fundamentación diagnóstica del mismo, detallando los procesos psicológicos y los mecanismos neurológicos implicados en su ejecución por un niño. El trabajo se realizó con una muestra reducida; 84 sujetos afectados por diversos desórdenes cerebrales; dividida en dos grupos: 1. Niños de 6 a 11 años con lesiones o desórdenes graves. 2. Niños de 6 a 11 años con disfunción cerebral mínima, pacientes de la Residencia Ntra. Sra. Del Rosell (Cartagena). Se revalidó el test en niños afectados por desórdenes cerebrales graves (recomprobación de los resultados de Koppitz en sus diferentes investigaciones con niños de Educación Especial desde el año 1963). Los criterios diagnósticos del TGVM se trasladaron al campo de los niños afectados con disfunciones cerebrales mínimas para validarlos en este tipo de poblaciones. Desde el punto de vista neuropsicológico se realizó un estudio minucioso de los procesos y mecanismos neurológicos y psicológicos implicados en la ejecución del test, utilizando el metodo del análisis sistemático de tareas para deducir las implicaciones de aquellos en cada paso o fase de ejecución de la prueba. Electroencefalogramas. Test guestáltico visomotor para niños (versión E.M. Koppitz). Protocolo 'ad hoc' de diagnóstico de la disfunción cerebral mínima en niños según modelos de Crukshank (1980) y Vallet (1979). Análisis sistemático de tareas (modelo Riviére). Test Factor G de Catell (nivel I). Se emplearon dos metodologías: 1. Empírico-cuantitativa. 2. Cualitativa. Para el primero se empleó: A) Técnicas estadísticas bivariantes (recuento de frecuencias; Chi-square). B) Técnicas estadísticas multivariantes (análisis factoriales de correspondencias (modelo Benzecri). Para el método cualitativo se empleó: A) Análisis de tareas de ejecución. B) Observación sistemática de las ejecuciones. El primer método utilizado revela la buena disposición del TGVM en el diagnóstico neuropsicológico de la disfunción cerebral mínima. La aparición en el test de 4 o más de 4 indicadores de desorden cerebral es suficientemente predictiva de la existencia de DCM. El análisis de tareas, como eje central del segundo método, permitió descubrir la cadena procesal a que da lugar la ejecución del test y las funciones neuropsicológicas de la percepción visomotriz más afectadas por causa de la DCM. El TGVM es un buen predictor de las disfunciones cerebrales mínimas no aparentes en los estudios electroencefalográficos o aparentes sólo en términos dudosos e inespecíficos. Es un adecuado evaluador de la madurez en la percepción visomotriz en niños con DCM.

Tratar de buscar qué formas de comunicación utiliza el niño y qué implicaciones pudiera tener con el desarrollo sensoriomotor. Se pretende indagar en qué medida determinados procesos cognitivos básicos están ligados a la aparición de conductas comunicativas, para ello se analizan la relaciones existentes entre el uso de conductas individuales, aisladas, secuencias y señalizaciones a lo largo del desarrollo evolutivo del niño de 6 hasta 20 meses. Veintiocho niños con edades comprendidas entre 6 meses y 19 meses 26 días. Selecionados de una consulta pediátrica de la Seguridad Social de Elche (Alicante). Se distribuyen en cuatro grupos, basados en las etapas del desarrollo sensoriomotor, utilizando para esta clasificación la prueba de Caseti-Lezine. Los grupos obtenidos son: reacciones circulares secundarias (RCS); coordinación esquemas secundarios (CES); reacciones circulares terciarias (RCT); representación. Modelo no paramétrico. El plan de investigación es una estructura formal caracterizada por la presentación de unas pruebas con el objeto de clasificar, en un modelo preestablecido las respuestas de los sujetos. Se trata de encontrar una relación funcional entre el nivel cognitivo y las conductas lingüísticas o no lingüísticas asociadas a él. Grabación en vídeo de una experiencia que consta de 2 partes: primero, juego libre con la madre y segundo, separar un objeto del niño y dejarlo dentro de su campo visual, pero fuera de su alcance. Duración de la prueba: 25 a 30 minutos. Análisis cualitativo a través de porcentajes y representaciones gráficas. Análisis estadísticos utilizando Chi-Square de Pearson para la elaboración de las relaciones funcionales. Para las comparaciones Post-Hoc el modelo Scheffe. En el grupo RCS los niños utilizan principalmente conductas individuales y algunas secuencias, ausencia de señalizadores. En CES hay mayor utilización de secuencias y localizaciones acompañadas de acción. En RCT las secuencias priman sobre las conductas individuales aumentan las señalizaciones con intencionalidad. En el grupo de representación aumento de las conductas individuales y disminución de las secuencias, utilización de las palabras, en forma semejante a los adultos, para indicar lo que se desea. Existen diferencias significativas entre los grupos comparados. Durante RCS se destacan unos comportamientos que en principio debemos admitir como ligados a la propia acción: reacciones a la situación. En el grupo de CES los patrones de comportamiento aparecen como formando parte fundamental de las secuencias. En RCT aparece una nueva pauta de comportamiento (señalización), vocalizaciones más especializadas que van evolucionando hasta la aparición y utilización de la palabra.

Conocer los componentes en los que se estructuran los estilos educativos parentales a partir del estudio de 22 situaciones educativas en las que se pone de manifiesto el modo concreto de actuar de los padres. Analizar las percepciones de los hijos respecto de los estilos educativos que exhiben sus padres en situaciones cotidianas. Dos grupos diferentes: A. 311 padres y madres de preadolescentes. B. 311 preadolescentes (11-14 años) alumnos de ciclo superior de EGB en centros escolares de Murcia capital y provincia. Se exploraron estilos educativos de los padres para configurar la estructura interna de los mismos, sus componentes y factores explicativos. Análisis de la percepción, distorsionada o precisa que tienen los hijos sobre dichos estilos educativos para establecer la configuración interna y los componentes de los estilos educativos 'percibidos', así como ver la relevancia de dos variables cognitivas: la percepción social y el 'autoconcepto'. Cuestionario Cesed-1, para explorar los estilos educativos exhibidos por los padres. Cuestionario Cesed-2, para explorar las percepciones de los sujetos sobre los estilos educativos parentales. Ambos cuestionarios fueron pilotados y validados en la población de origen de la muestra y elaborados para los fines de esta investigación. Análisis factorial para estudiar los componentes de los estilos educativos y los de la percepción de dichos estilos. El estudio del grado de precisión perceptiva requirió además, la confrontación de sendos análisis factoriales y un análisis general de recuento de frecuencias absolutas y relativas. Cruzamientos de variables con análisis de significación de diferencias estadísticas por medio del Chi-Square. Se establecieron siete estilos educativos de los padres y otros siete percibidos. El primer grupo de estilos goza de alta coherencia interna, no ocurre así en los estilos percibidos por los hijos, éstos tienden a percibir los estilos educativos de sus padres con una configuración diferente de la que son emitidos. Por ello, se estableció la baja precisión perceptiva entre el modo de actuar de los padres y la manera en que los hijos perciben tales actuaciones. Ciertas variables configuradoras de los estilos educativos están influídas en su percepción por el 'autoconcepto' de los hijos. El análisis de la relación educativa debe contemplar el alcance de las variables cognitivas determinantes, de modo que pueda avanzarse en la propuesta de modelos educativos más coherentes con la dinámica educacional.

La nostra investigació s'inscriu en la concepció dinàmica de la intel·ligència, i concretament en el processos que configuren el processament cerebral en el Model d'integració de la informació descrit per Das, Kirby i Jarman (1979). Els dos processos cerebrals que constitueixen la base de la conducta intel·ligent són el processament simultani i el processament seqüencial; són les dues estratègies principals del processament de la informació. Tota classe d'estímul és susceptible d'ésser processat o bé seqüencialment (seriació, verbal, anàlisi), o be simultàniament (global, visual, síntesi). Basant-nos en el recull bibliogràfic i amb la convicció de que apropant-nos al coneixement de les peculiaritats del processament de la informació, ens endinsem en la comprensió del procés que mena a la conducta intel·ligent, i per tant, a l'aprenentatge, formulem la següent hipòtesi de treball: en els nens de preescolar (d'entre els 3 i els sis anys) es donaran aquest dos tipus de processament i variaran en funció de l'edat, el sexe, l'atenció, les dificultats d'aprenentatge, els problemes de llenguatge, el bilingüisme, el nivell sociocultural, la dominància manual, el nivell mental i de la presència de patologia. Les diferències que s'esdevinguin ens permetran de formular criteris i pautes per a la intervenció educativa. Els nostres objectius es refonen en mesurar el processament en nens de preescolar de les comarques gironines, verificar la relació de cada tipus de processament amb les variables esmentades, comprovar si s'estableix un paral·lelisme entre el processament i les aportacions de concepció localitzacionista de les funcions cerebrals en base als nostres resultats, i pautes per a la intervenció pedagògica. Quant al mètode, hem seleccionat una mostra representativa dels nens i nenes matriculats a les escoles publiques de les comarques gironines durant el curs 92/93, mitjançant un mostreig aleatori estratificat i per conglomerats. El tamany real de la mostra és de dos-cents seixanta un subjectes. Els instruments emprats han estat els següents: el Test K-ABC de Kaufman & Kaufman (1983) per a la avaluació del processament; un formulari dirigit als pares per a la recollida de la informació pertinent; entrevistes amb les mestres, i el Test de la Figura Humana de Goodenough. Pel que fa referència als resultats de la nostra recerca i en funció dels objectius proposats, constatem els fets següents. En els nens de preescolar, amb edats d'entre els tres i els sis anys, es constata l'existència dels dos tipus de processament cerebral, sense que es doni un predomini d'un sobre de l'altre; ambdós processaments actuen interrelacionadament. Ambdós tipus de processament milloren a mesura que augmenta l'edat, però es constaten diferències derivades del nivell mental: amb un nivell mental normal s'hi associa una millora d'ambdós processaments, mentre que amb un nivell mental deficient només millora fonamentalment el processament seqüencial. Tanmateix, el processament simultani està més relacionat amb les funcions cognitives complexes i és més nivell mental dependent que el processament seqüencial. Tant les dificultats d'aprenentatge com els problemes de llenguatge predominen en els nens i nenes amb un desequilibri significatiu entre ambdós tipus de processament; les dificultats d'aprenentatge estan més relacionades amb una deficiència del processament simultani, mentre que els problemes de llenguatge es relacionen més amb una deficiència en el processament seqüencial. Els nivells socioculturals baixos es relacionen amb resultats inferiors en ambdós tipus de processament. Per altra part, entre els nens bilingües és més freqüent el processament seqüencial significatiu. El test de la Figura Humana es comporta com un marcador de processament simultani i el nivell atencional com un marcador de la gravetat del problema que afecta al processament i en el següent ordre: nivell mental deficient, dificultats, d'aprenentatge i problemes de llenguatge . Les deficiències atencionals van lligades a deficiències en el processament simultani i a la presencia de patologia. Quant a la dominància manual no es constaten diferències en el processament. Finalment, respecte del sexe només podem aportar que quan un dels dos tipus de processament és deficitari,i es dóna per tant, un desequilibri en el processament, predomina significativament el nombre de nens afectats per sobre del de nenes.

Objectives We examined the characteristics and CHD risks of people who accessed the free Healthy Heart Assessment (HHA) service operated by a large UK pharmacy chain from August 2004 to April 2006. Methods Associations between participants’ gender, age, and socioeconomics were explored in relation to calculated 10-year CHD risks by cross-tabulation of the data. Specific associations were tested by forming contingency tables and using Pearson chi-square (χ2). Results Data from 8,287 records were analysable; 5,377 were at low and 2,910 at moderate-to-high CHD risk. The likelihood of moderate-to-high risk for a male versus female participant was significantly higher with a relative risk ratio (RRR) 1.72 (P < 0.001). A higher percentage of those in socioeconomic categories ‘constrained by circumstances’ (RRR 1.15; P < 0.05) and ‘blue collar communities’ (RRR 1.13; P < 0.05) were assessed with moderate-to-high risk compared to those in ‘prospering suburbs’. Conclusions People from ‘hard-to-reach’ sectors of the population, men and people from less advantaged communities, accessed the HHA service and were more likely to return moderate-to-high CHD risk. Pharmacists prioritised provision of lifestyle information above the sale of a product. Our study supports the notion that pharmacies can serve as suitable environments for the delivery of similar screening services.

Rationale: In UK hospitals, the preparation of all total parenteral nutrition (TPN) products must be made in the pharmacy as TPNs are categorised as high-risk injectables (NPSA/2007/20). The National Aseptic Error Reporting Scheme has been collecting data on pharmacy compounding errors in the UK since August 2003. This study reports on types of error associated with the preparation of TPNs, including the stage at which these were identified and potential and actual patient outcomes. Methods: Reports of compounding errors for the period 1/2004 - 3/2007 were analysed on an Excel spreadsheet. Results: Of a total of 3691 compounding error reports, 674 (18%) related to TPN products; 548 adult vs. 126 paediatric. A significantly higher proportion of adult TPNs (28% vs. 13% paediatric) were associated with labelling errors and a significantly higher proportion of paediatric TPNs (25% vs. 15% adult) were associated with incorrect transcriptions (Chi-Square Test; p<0.005). Labelling errors were identified equally by pharmacists (42%) and technicians (48%) with technicians detecting mainly at first check and pharmacists at final check. Transcription errors were identified mainly by technicians (65% vs. 27% pharmacist) at first check. Incorrect drug selection (13%) and calculation errors (9%) were associated with adult and paediatric TPN preparations in the same ratio. One paediatric TPN error detected at first check was considered potentially catastrophic; 31 (5%) errors were considered of major and 38 (6%) of moderate potential consequence. Five errors (2 moderate, 1 minor) were identified during or after administration. Conclusions: While recent UK patient safety initiatives are aimed at improving the safety of injectable medicines in clinical areas, the current study highlights safety problems that exist within pharmacy production units. This could be used in the creation of an error management tool for TPN compounding processes within hospital pharmacies.

Objective To assess the impact of a closed-loop electronic prescribing and automated dispensing system on the time spent providing a ward pharmacy service and the activities carried out. Setting Surgical ward, London teaching hospital. Method All data were collected two months pre- and one year post-intervention. First, the ward pharmacist recorded the time taken each day for four weeks. Second, an observational study was conducted over 10 weekdays, using two-dimensional work sampling, to identify the ward pharmacist's activities. Finally, medication orders were examined to identify pharmacists' endorsements that should have been, and were actually, made. Key findings Mean time to provide a weekday ward pharmacy service increased from 1 h 8 min to 1 h 38 min per day (P = 0.001; unpaired t-test). There were significant increases in time spent prescription monitoring, recommending changes in therapy/monitoring, giving advice or information, and non-productive time. There were decreases for supply, looking for charts and checking patients' own drugs. There was an increase in the amount of time spent with medical and pharmacy staff, and with 'self'. Seventy-eight per cent of patients' medication records could be assessed for endorsements pre- and 100% post-intervention. Endorsements were required for 390 (50%) of 787 medication orders pre-intervention and 190 (21%) of 897 afterwards (P < 0.0001; chi-square test). Endorsements were made for 214 (55%) of endorsement opportunities pre-intervention and 57 (30%) afterwards (P < 0.0001; chi-square test). Conclusion The intervention increased the overall time required to provide a ward pharmacy service and changed the types of activity undertaken. Contact time with medical and pharmacy staff increased. There was no significant change in time spent with patients. Fewer pharmacy endorsements were required post-intervention, but a lower percentage were actually made. The findings have important implications for the design, introduction and use of similar systems.

Introduction Health promotion (HP) aims to enhance good health while preventing ill-health at three levels of activity; primary (preventative), secondary (diagnostic) and tertiary (management).1 It can range from simple provision of health education to ongoing support, but the effectiveness of HP is ultimately dependent on its ability to influence change. HP as part of the Community Pharmacy Contract (CPC) aims to increase public knowledge and target ‘hard-to-reach’ individuals by focusing mainly on primary and tertiary HP. The CPC does not include screening programmes (secondary HP) as a service. Coronary heart disease (CHD) is a significant cause of morbidity and mortality in the UK. While there is evidence to support the effectiveness of some community pharmacy HP strategies in CHD, there is paucity of research in relation to screening services.2 Against this background, Alliance Pharmacy introduced a free CHD risk screening programme to provide tailored HP advice as part of a participant–pharmacist consultation. The aim of this study is to report on the CHD risk levels of participants and to provide a qualitative indication of consultation outcomes. Methods Case records for 12 733 people who accessed a free CHD risk screening service between August 2004 and April 2006 offered at 217 community pharmacies were obtained. The service involved initial self-completion of the Healthy Heart Assessment (HHA) form and measurement of height, weight, body mass index, blood pressure, total cholesterol and highdensity lipoprotein levels by pharmacists to calculate CHD risk.3 Action taken by pharmacists (lifestyle advice, statin recommendation or general practitioner (GP) referral) and qualitative statements of advice were recorded, and a copy provided to the participants. The service did not include follow-up of participants. All participants consented to taking part in evaluations of the service. Ethical committee scrutiny was not required for this service development evaluation. Results Case records for 10 035 participants (3658 male) were evaluable; 5730 (57%) were at low CHD risk (<15%); 3636 (36%) at moderate-to-high CHD risk (≥15%); and 669 (7%) had existing heart disease. A significantly higher proportion of male (48% versus 30% female) participants were at moderate- to-high risk of CHD (chi-square test; P < 0.005). A range of outcomes resulted from consultations. Lifestyle advice was provided irrespective of participants’ CHD risk or existing disease. In the moderate-to-high-risk group, of which 52% received prescribed medication, lifestyle advice was recorded for 62%, 16% were referred and 34% were advised to have a re-assessment. Statin recommendations were made in 1% of all cases. There was evidence of supportive and motivational statements in the advice recorded. Discussion Pharmacists were able to identify individuals’ level of CHD risk and provide them with bespoke advice. Identification of at-risk participants did not automatically result in referrals or statin recommendation. One-third of those accessing the screening service had moderate-to-high risk of CHD, a significantly higher proportion of whom were men. It is not known whether these individuals had been previously exposed to HP but presumably by accessing this service they may have contemplated change. As effectiveness of HP advice will depend among other factors on ability to influence change, future consultations may need to explore patients’ attitude towards change in relation to the Trans Theoretical Model4 to better tailor HP advice. The high uptake of the service by those at moderate-to-high CHD risk indicates a need for this type of screening programme in community pharmacy, perhaps specifically to reach men who access medical services less.

In order to identify the factors influencing adoption of technologies promoted by government to small-scale dairy farmers in the highlands of central Mexico, a field survey was conducted. A total of 115 farmers were grouped through cluster analysis (CA) and divided into three wealth status categories (high, medium and low) using wealth ranking. Chi-square analysis was used to examine the association of wealth status with technology adoption. Four groups of farms were differentiated in terms of farms’ dimensions, farmers’ education, sources of incomes, wealth status, management of herd, monetary support by government and technological availability. Statistical differences (p < 0.05) were observed in the milk yield per herd per year among groups. Government organizations (GO) participated little in the promotion of the 17 technologies identified, six of which focused on crop or forage production and 11 of which were related to animal husbandry. Relatives and other farmers played an important role in knowledge diffusion and technology adoption. Although wealth status had a significant association (p < 0.05) with adoption, other factors including importance of the technology to farmers, usefulness and productive benefits of innovations together with farmers’ knowledge of them, were important. It is concluded that the analysis of the information per group and wealth status was useful to identify suitable crop or forage related and animal husbandry technologies per group and wealth status of farmers. Therefore the characterizations of farmers could provide a useful starting point for the design and delivery of more appropriate and effective extension.

Active learning plays a strong role in mathematics and statistics, and formative problems are vital for developing key problem-solving skills. To keep students engaged and help them master the fundamentals before challenging themselves further, we have developed a system for delivering problems tailored to a student‟s current level of understanding. Specifically, by adapting simple methodology from clinical trials, a framework for delivering existing problems and other illustrative material has been developed, making use of macros in Excel. The problems are assigned a level of difficulty (a „dose‟), and problems are presented to the student in an order depending on their ability, i.e. based on their performance so far on other problems. We demonstrate and discuss the application of the approach with formative examples developed for a first year course on plane coordinate geometry, and also for problems centred on the topic of chi-square tests.

This thesis is concerned with development of improved management practices in indigenous chicken production systems in a research process that includes participatory approaches with smallholder farmers and other stakeholders in Kenya. The research process involved a wide range of activities that included on-station experiments, field surveys, stakeholder consultations in workshops, seminars and visits, and on-farm farmer participatory research to evaluate the effect of some improved management interventions on production performance of indigenous chickens. The participatory research was greatly informed from collective experiences and lessons of the previous activities. The on-station studies focused on hatching, growth and nutritional characteristics of the indigenous chickens. Four research publications from these studies are included in this thesis. Quantitative statistical analyses were applied and they involved use of growth models estimated with non-linear regressions for the growth characteristics, chi-square determinations to investigate differences among different reciprocal crosses of indigenous chickens and general linear models and covariance determination for the nutrition study. The on-station studies brought greater understanding of performance and production characteristics of indigenous chickens and the influence of management practices on these characteristics. The field surveys and stakeholder consultations helped in understanding the overarching issues affecting the productivity of the indigenous chickens systems and their place in the livelihoods of smallholder farmers. These activities created strong networking opportunities with stakeholders from a wide spectrum. The on-farm farmer participatory research involved selection of 200 farmers in five regions followed by training and introduction of interventions on improved management practices which included housing, vaccination, deworming and feed supplementation. Implementation and monitoring was mainly done by individual farmers continuously for close to one and half years. Six quarterly visits to the farms were made by the research team to monitor and provide support for on-going project activities. The data collected has been analysed for 5 consecutive 3-monthly periods. Descriptive and inferential statistics were applied to analyse the data collected involving treatment applications, production characteristics and flock demography characteristics. Out of the 200 farmers initially selected, 173 had records on treatment applications and flock demography characteristics while 127 farmers had records on production characteristics. The demographic analysis with a dissimilarity index of flock size produced 7 distinct farm groups from among the 173 farms. Two of these farm groups were represented in similar numbers in each of the five regions. The research process also involved a number of dissemination and communication strategies that have brought the process and project outcomes into the domain of accessibility by wider readership locally and globally. These include workshops, seminars, field visits and consultations, local and international conferences, electronic conferencing, publications and personal communication via emailing and conventional posting. A number of research and development proposals were also developed based on the knowledge and experiences gained from the research process. The thesis captures the research process activities and outcomes in 8 chapters which include in ascending order – introduction, theoretical concepts underpinning FPR, research methodology and process, on-station research output, FPR descriptive statistical analysis, FPR inferential statistical analysis on production characteristics, FPR demographic analysis and conclusions. Various research approaches both quantitative and qualitative have been applied in the research process indicating the possibilities and importance of combining both systems for greater understanding of issues being studied. In our case, participatory studies of the improved management of indigenous chickens indicates their potential importance as livelihood assets for poor people.

Poorer people are more likely to use antibiotics; inappropriate antibiotic use causes resistance, and health campaigns attempt to change behaviour through education. However, fuelled by the media, the public think antibiotic resistance is outside their control. Differences in the attribution of blame for antibiotic resistance in two genres of UK newspapers, targeting distinct socioeconomic groups, were examined using a mixed methods approach. Firstly, depiction of blame was categorised as either external to the lay public (outside their control) or internal (lay person accountable) and subjected to a chi-square test. Secondly, using critical discourse analysis, we examined the portrayal of the main agents through newspaper language. Data from 597 articles (307 broadsheets) analysed revealed a significant association between newspaper genre and attribution of blame for antibiotic resistance. While both newspaper types blamed antibiotic resistance predominantly on factors external to the lay public, broadsheets were more likely to acknowledge internal factors than tabloids. Tabloids provided a more skewed representation, exposing readers to inaccurate explanations about antibiotic resistance. They highlighted ineptitude in health professionals, victimising patients and blaming others, while broadsheets used less emotive language. Pharmacists should take special care to communicate the importance of appropriate antibiotic use against this backdrop of distortion.

OBJECTIVE: The aim of this study was to compare the knowledge and views of nursing staff on both acute elderly care and rehabilitation wards regarding elderly persons' oral care with that of carers in nursing homes. SUBJECTS: One hundred nurses working on acute, sub-acute and rehabilitation wards for elderly people (Group 1) and 75 carers in nursing homes (Group 2) were surveyed. DESIGN: A semi-structured questionnaire. RESULTS: Similar percentages of each group of nurses were registered with a dentist (86% and 88% respectively), although more hospital-based nurses were anxious about dental treatment compared with the nursing home group (40% and 28% respectively). More carers in nursing homes gave regular advice about oral care than the hospital-based nurses (54% and 43% respectively). Eighteen per cent of each group thought that edentulous individuals did not require regular oral care. Eighty-five per cent of hospital-based nurses and 95% of nursing home carers incorrectly thought that dentures were 'free' on the NHS. Although trends were observed between the two groups, no comparisons were statistically significant (Chi-square; level p < 0.05). CONCLUSIONS: Deficiencies exist in the knowledge of health care workers both in hospital and in the community setting, although the latter were less knowledgeable but more likely to give advice to older people.

Background: The validity of ensemble averaging on event-related potential (ERP) data has been questioned, due to its assumption that the ERP is identical across trials. Thus, there is a need for preliminary testing for cluster structure in the data. New method: We propose a complete pipeline for the cluster analysis of ERP data. To increase the signalto-noise (SNR) ratio of the raw single-trials, we used a denoising method based on Empirical Mode Decomposition (EMD). Next, we used a bootstrap-based method to determine the number of clusters, through a measure called the Stability Index (SI). We then used a clustering algorithm based on a Genetic Algorithm (GA)to define initial cluster centroids for subsequent k-means clustering. Finally, we visualised the clustering results through a scheme based on Principal Component Analysis (PCA). Results: After validating the pipeline on simulated data, we tested it on data from two experiments – a P300 speller paradigm on a single subject and a language processing study on 25 subjects. Results revealed evidence for the existence of 6 clusters in one experimental condition from the language processing study. Further, a two-way chi-square test revealed an influence of subject on cluster membership.

Aims: To assess the prevalence of primary headaches (HA) in adults with temporomandibular disorders (TMD) who were assessed in a specialty orofacial pain clinic, as well as in controls without TMD. Methods: The sample consisted of 158 individuals with TMD seen at a university-based specialty clinic, as well as 68 controls. The Research Diagnostic Criteria for TMD were used to diagnose the TMD patients. HAs were assessed using a structured interview and classified according to the Second Edition of the International Classification for Headache Disorders. Data were analyzed by chi-square tests with a significance level of 5% and odds ratio (OR) tests with a 95% confidence interval (CI). Results: HAs occurred in 45.6% of the control group (30.9% had migraine and 14.7% had tension-type headache [TTH]) and in 85.5% of individuals with TMD. Among individuals with TMD, migraine was the most prevalent primary HA (55.3%), followed by TTH (30.2%); 14.5% had no HA. In contrast to controls, the odds ratio (OR) for HA in those with TMD was 7.05 (95% confidence interval [CI] = 3.65-13.61; P = .000), for migraine, the OR was 2.76 (95% CI = 1.50-5.06; P = .001), and for TTH, the OR was 2.51 (95% CI = 1.18-5.35; P = .014). Myofascial pain/arthralgia was the most common TMD diagnosis (53.2%). The presence of HA or specific HAs was not associated with the time since the onset of TMD (P = .714). However, migraine frequency was positively associated with TMD pain severity (P = .000). Conclusion: TMD was associated with increased primary HA prevalence rates.
