This article investigates to what extent the worldwide increase in body mass index (BMI) has been affected by economic globalization and inequality. We used time-series and longitudinal cross-national analysis of 127 countries from 1980 to 2008. Data on mean adult BMI were obtained from the Global Burden of Metabolic Risk Factors of Chronic Diseases Collaborating Group. Globalization was measured using the Swiss Economic Institute (KOF) index of economic globalization. Economic inequality between countries was measured with the mean difference in gross domestic product per capita purchasing power parity in international dollars. Economic inequality within countries was measured using the Gini index from the Standardized World Income Inequality Database. Other covariates including poverty, population size, urban population, openness to trade and foreign direct investment were taken from the World Development Indicators (WDI) database. Time-series regression analyses showed that the global increase in BMI is positively associated with both the index of economic globalization and inequality between countries, after adjustment for covariates. Longitudinal panel data analyses showed that the association between economic globalization and BMI is robust after controlling for all covariates and using different estimators. The association between economic inequality within countries and BMI, however, was significant only among high-income nations. More research is needed to study the pathways between economic globalization and BMI. These findings, however, contribute to explaining how contemporary globalization can be reformed to promote better health and control the global obesity epidemic. © 2013 Copyright Taylor and Francis Group, LLC.

Emerging research provides substantial evidence to classify strawberries as a functional food with several preventive and therapeutic health benefits. Strawberries, a rich source of phytochemicals (ellagic acid, anthocyanins, quercetin, and catechin) and vitamins (ascorbic acid and folic acid), have been highly ranked among dietary sources of polyphenols and antioxidant capacity. It should however be noted that these bioactive factors can be significantly affected by differences in strawberry cultivars, agricultural practices, storage, and processing methods: freezing versus dry heat has been associated with maximum retention of strawberry bioactives in several studies. Nutritional epidemiology shows inverse association between strawberry consumption and incidence of hypertension or serum C-reactive protein; controlled feeding studies have identified the ability of strawberries to attenuate high-fat diet induced postprandial oxidative stress and inflammation, or postprandial hyperglycemia, or hyperlipidemia in subjects with cardiovascular risk factors. Mechanistic studies have elucidated specific biochemical pathways that might confer these protective effects of strawberries: upregulation of endothelial nitric oxide synthase (eNOS) activity, downregulation of NF-kB activity and subsequent inflammation, or inhibitions of carbohydrate digestive enzymes. These health effects may be attributed to the synergistic effects of nutrients and phytochemicals in strawberries. Further studies are needed to define the optimal dose and duration of strawberry intake in affecting levels of biomarkers or pathways related to chronic diseases.

Aim: To critically appraise recent research into associations between periodontal disease and systemic diseases and conditions specifically respiratory disease, chronic kidney disease, rheumatoid arthritis, cognitive impairment, obesity, metabolic syndrome and cancer.

Methods: A MEDLINE literature search of papers published between 2002 and April 2012 was conducted. Studies that included periodontitis as an exposure were identified. Cross-sectional epidemiological investigations on large samples, prospective studies and systematic reviews formed the basis of the narrative review. A threshold set for the identification of periodontitis was used to identify those studies that contributed to the conclusions of the review.

Results: Many of the investigations were cross-sectional secondary analyses of existing data sets in particular the NHANES studies. There were a small number of systematic reviews and prospective studies. There was substantial variability in the definitions of exposure to periodontitis. A small number of studies met the threshold set for periodontitis and supported associations; however, in some of the chronic diseases there were no such studies. There was strong evidence from randomized controlled trials that interventions, which improve oral hygiene have positive effects on the prevention of nosocomial pneumonias.

Conclusions: There was substantial heterogeneity in the definitions used to identify periodontitis and very few studies met a stringent threshold for periodontitis. Published evidence supports modest associations between periodontitis and some, although not all, of the diseases and conditions reviewed. There is a need to reach a consensus on what constitutes periodontitis for future studies of putative associations with systemic diseases.

A high intake of fruit and vegetables (FV) has been shown to be associated with reduced risk of a number of chronic diseases, including CVD. This review aims to provide an overview of the evidence that increased FV intake reduces risk of CVD, focusing on studies examining total FV intake. This evidence so far available is largely based on prospective cohort studies, with meta-analyses demonstrating an association between increased FV intake and reduced risk of both CHD and stroke. Controlled intervention trials examining either clinical or cardiovascular risk factor endpoints are scarce. However, such trials have shown that an increase in FV consumption can lower blood pressure and also improve microvascular function, both of which are commensurate with a reduced risk of CVD. The effects of increased FV consumption on plasma lipid levels, risk of diabetes and body weight have yet to be firmly established. In conclusion, evidence that FV consumption reduces the risk of CVD is so far largely confined to observational epidemiology, with further intervention studies required.

The objective of the present paper was to review the literature investigating the potential relationship between fruit and vegetables (FV) and psychological well-being. The rising prevalence of mental ill health is causing considerable societal burden. Inexpensive and effective strategies are therefore required to improve the psychological well-being of the population, and to reduce the negative impact of mental health problems. A growing body of literature suggests that dietary intake may have the potential to influence psychological well-being. For example, studies have suggested that particular dietary constituents, including vitamins and minerals, might be beneficial to psychological health. However, in order to better reflect normal dietary intake, health-based research has increasingly begun to focus on whole foods and dietary patterns, rather than individual nutrients. One food group that has received increasing attention with regard to psychological health is FV. This is probably a result of the strong evidence base, which exists in relation to their protective association with a number of chronic diseases, as well as the fact that they are a rich source of some of the nutrients which have been linked to psychological health. While some promising findings exist with regards to FV intake and psychological well-being, overall, results are inconsistent. Possible reasons for this, such as methodological issues related to study design and the measurement of psychological well-being and FV intake, are discussed within this review. Based on the predominantly observational nature of existing literature, the present paper concludes that future well-designed randomised controlled trials are required to investigate the relationship further.

BACKGROUND: Cardiovascular disease (CVD) represents a leading cause of mortality worldwide, especially in the elderly. Lowering the number of CVD deaths requires preventive strategies targeted on the elderly.

OBJECTIVE: The objective was to generate evidence on the association between WHO dietary recommendations and mortality from CVD, coronary artery disease (CAD), and stroke in the elderly aged ≥60 y.

DESIGN: We analyzed data from 10 prospective cohort studies from Europe and the United States comprising a total sample of 281,874 men and women free from chronic diseases at baseline. Components of the Healthy Diet Indicator (HDI) included saturated fatty acids, polyunsaturated fatty acids, mono- and disaccharides, protein, cholesterol, dietary fiber, and fruit and vegetables. Cohort-specific HRs adjusted for sex, education, smoking, physical activity, and energy and alcohol intakes were pooled by using a random-effects model.

RESULTS: During 3,322,768 person-years of follow-up, 12,492 people died of CVD. An increase of 10 HDI points (complete adherence to an additional WHO guideline) was, on average, not associated with CVD mortality (HR: 0.94; 95% CI: 0.86, 1.03), CAD mortality (HR: 0.99; 95% CI: 0.85, 1.14), or stroke mortality (HR: 0.95; 95% CI: 0.88, 1.03). However, after stratification of the data by geographic region, adherence to the HDI was associated with reduced CVD mortality in the southern European cohorts (HR: 0.87; 95% CI: 0.79, 0.96; I(2) = 0%) and in the US cohort (HR: 0.85; 95% CI: 0.83, 0.87; I(2) = not applicable).

CONCLUSION: Overall, greater adherence to the WHO dietary guidelines was not significantly associated with CVD mortality, but the results varied across regions. Clear inverse associations were observed in elderly populations in southern Europe and the United States.

Background: LL-37, an anti-microbial peptide belonging to the cathelicidin family, derives its name from two N-terminal leucine residues and the 37 amino acids comprising the peptide. LL-37 is the only known cathelicidin to exist in humans. It exhibits both anti-bacterial and immunomodulatory properties. Objectives: In the current study, LL-37 was quantified in GCF from periodontitis patients. Previous studies have relied on qualitative results from Western blotting to detect LL-37 in GCF. This study aims to quantitatively determine LL-37 levels in GCF. Methods: GCF and bacterial plaque samples, pre- and post non-surgical periodontal treatment, were collected from 4 sites in 12 patients presenting with advanced periodontitis. Plaque samples were analysed by QPCR for the presence or absence of the periodontopathic bacterium Porphyromonas gingivalis (P. gingivalis). The concentrations of LL-37 in patient samples pre- and post-treatment were deduced by indirect enzyme linked immunosorbent assay (ELISA). Results: Concentrations of LL-37 in samples varied between a minimum and maximum of 1 and 40 ng/ml. LL-37 levels were shown, pre-treatment, to be higher in deep pockets (6-9 mm) compared with shallower pockets (3-5 mm) and highest in those sites which were positive for P. gingivalis. Non-surgical therapy resulted in a significant improvement in clinical indices while expression levels of P. gingivalis were reduced. Following treatment, LL-37 levels in GCF decreased from an average of 6.5 ± 1 - 5.8 ± 1.2 ng/ml. The most interesting observation however was the reduction in LL-37 levels, from an average of 7 ± 1.3 – 2.5 ± 1.1 ng/ml in those sites where P. gingivalis infection was eradicated post-treatment. Conclusions: LL-37 levels are increased at sites showing advanced periodontal disease, reduce following treatment and appear to be linked to the presence of P. gingivalis. This study will further our knowledge of host defence in chronic diseases such as periodontitis.

Background: Epididymal protease inhibitor (eppin) is a dual motif protein belonging to the whey acidic protein (WAP) family. Although expressed in numerous different tissues, to date, its functional characterisation is limited. It has been shown to exhibit antibacterial activity against Gram-negative bacteria (Escherichia coli) and antiprotease activity against some proteases of the serine protease family. We are interested in determining the role of eppin in innate immune defence. Objectives: This study aims to determine eppin's potential function in the innate immune response in the oral cavity by investigating the antimicrobial activity of eppin against relevant oral pathogens. Methods: Eppin was recombinantly expressed in E. coli cells and purified by immobilised metal affinity chromatography (IMAC). The antimicrobial effects of the protein were then assessed against two oral pathogens, Fusobacterium nucleatum and Candida albicans, using a double layer radial diffusion assay. Results: Eppin displayed antimicrobial activities against both oral pathogens tested and these activities were shown to be comparable to the well characterised antimicrobial peptide, LL-37. The antifungal effects of eppin were shown to be more potent than those of the human cathelicidin, LL-37. Conclusions: Eppin has been shown to possess both antibacterial and antifungal properties against oral pathogens, suggesting an important role for this protein in the innate immune response in the oral cavity. This study furthers our knowledge of the physiological role exerted by eppin and its possible role in the modulation of chronic diseases such as periodontitis and oral candidiasis.

A substantial proportion of aetiological risks for many cancers and chronic diseases remain unexplained. Using geochemical soil and stream water samples collected as part of the Tellus Project studies, current research is investigating naturally occurring background levels of potentially toxic elements (PTEs) in soils and stream sediments and their possible relationship with progressive chronic kidney disease (CKD). The Tellus geological mapping project, Geological Survey Northern Ireland, collected soil sediment and stream water samples on a grid of one sample site every 2 km2 across the rural areas of Northern Ireland resulting in an excess of 6800 soil sampling locations and more than 5800 locations for stream water sampling. Accumulation of several PTEs including arsenic, cadmium, chromium, lead and mercury have been linked with human health and implicated in renal function decline. The hypothesis is that long-term exposure will result in cumulative exposure to PTEs and act as risk factor(s) for cancer and diabetes related CKD and its progression. The ‘bioavailable’ fraction of total PTE soil concentration depends on the ‘bioaccessible’ proportion through an exposure pathway. Recent work has explored this bioaccessible fraction for a range of PTEs across Northern Ireland. In this study the compositional nature of the multivariate geochemical PTE variables and bioaccessible data is explored to augment the investigation into the potential relationship between PTEs, bioaccessibility and disease data.

BACKGROUND: Physical inactivity has been associated with obesity and related chronic diseases. Understanding built environment (BE) influences on specific domains of physical activity (PA) around homes and workplaces is important for public health interventions to increase population PA.

PURPOSE: To examine the association of home and workplace BE features with PA occurring across specific life domains (work, leisure, and travel).

METHODS: Between 2012 and 2013, telephone interviews were conducted with participants in four Missouri metropolitan areas. Questions included sociodemographic characteristics, home and workplace supports for PA, and dietary behaviors. Data analysis was conducted in 2013; logistic regression was used to examine associations between BE features and domain-specific PA.

RESULTS: In home neighborhoods, seven of 12 BE features (availability of fruits and vegetables, presence of shops and stores, bike facilities, recreation facilities, crime rate, seeing others active, and interesting things) were associated with leisure PA. The global average score of home neighborhood BE features was associated with greater odds of travel PA (AOR=1.99, 95% CI=1.46, 2.72); leisure PA (AOR=1.84, 95% CI=1.44, 2.34); and total PA (AOR=1.41, 95% CI=1.04, 1.92). Associations between workplace neighborhoods' BE features and workplace PA were small but in the expected direction.

CONCLUSIONS: This study offers empirical evidence on BE supports for domain-specific PA. Findings suggest that diverse, attractive, and walkable neighborhoods around workplaces support walking, bicycling, and use of public transit. Public health practitioners, researchers, and worksite leaders could benefit by utilizing worksite domains and measures from this study for future BE assessments.

Os factores de risco nos adultos jovens são fortes preditores da incidência de doença cardiovascular e mortalidade na idade mais avançada. Em Portugal, desconhecem-se estudos que avaliem os factores de risco para as doenças crónicas, em jovens adultos, na transição do ensino secundário para o universitário. Este estudo pretendeu contribuir para a promoção do conhecimento dos determinantes sócioculturais e ambientais no diagnóstico e detecção de factores de risco para as doenças crónicas, nomeadamente as doenças cardiovasculares, em estudantes universitários. Teve como objectivo principal a investigação do efeito da vida académica nos factores de risco modificáveis, estilos de vida e determinantes da saúde. Teve como objectivos específicos identificar a(s) prevalência(s) dos factores de risco cardiovascular numa população universitária, a identificação dos intervalos de referência para a homocisteína total no soro de adultos jovens portugueses, a determinação do perfil lípidico, comportamentos de saúde e dieta alimentar de tipo mediterrânico entre os estudantes universitários de acordo com o género e a área científica de frequência e a avaliação longitudinal do impacto da exposição à vida académica no estado de saúde dos estudantes universitários Participaram no estudo 781 estudantes sendo a média de idades de 20,6. Os factores de risco estudados para as doenças crónicas, foram o hábito tabágico, a pressão arterial, o índice de massa corporal, a composição do sangue (lípidos, homocisteina e glicose), a alimentação e a actividade física. O estudo mostra que a prevalência de: sedentarismo é significativamente mais elevada nos rapazes (p<0,001); dislipidemia e a hipertrigliceridemia é significativamente mais elevada nas raparigas. Mais de um quarto dos estudantes tem colesterol elevado sendo a hipercolesterolemia significativamente mais elevada nas raparigas (p<0,001); a hipertensão verificou-se em ambos os sexos (6,0%) mas foi significativamente mais elevada nos rapazes (p=0,001). O estudo identificou o intervalo de referência para a homocisteína em adultos jovens portugueses independentemente do sexo (6,2 a 11,6 μmol/) sendo que, acima de 11,6 μmol/l é condição para vigilância médica em populações jovens adultas. Quando se estudou a exposição à vida académica comparada com aqueles que acabaram de entrar na universidade, verificou-se uma associação significativa no que respeita às concentrações de lípidos no sangue, à pressão arterial sistólica e à actividade física, tendo sido as raparigas aquelas que mais se afastavam dos padrões saudáveis (p<0,001). No que respeita à adesão à dieta mediterranica, não foram encontradas associações entre este tipo de alimentação e os vários factores de risco independentemente do género. Os resultados forneceram, evidências empíricas acerca da importância da detecção dos principais factores de risco na idade adulta (jovem) na prevenção das doenças cardiovasculares e vieram corroborar as orientações do Plano de Desenvolvimento Estratégico do Instituto Nacional de Saúde Português para as doenças crónicas, nomeadamente o estabelecimento de valores de referência nacionais para análises biológicas e as orientações do Plano de Acção Estratégica Global para a Prevenção e Controle das Doenças Não-Transmissíveis-2008/2013 da Organização Mundial de Saúde.

A obesidade infantil é um importante problema de saúde pública, não só pelos efeitos adversos durante a infância mas porque tende a persistir na idade adulta, constituindo um factor de risco para diversas doenças crónicas. Os alicerces de uma vida saudável estruturam-se na vida pré-natal e sedimentam-se nos seis primeiros anos de vida, sendo o crescimento da criança fortemente influenciado pelo seu contexto ambiental familiar. Foi neste âmbito que emergiu como objectivo geral deste estudo explorar as relações existentes entre os determinantes infantis (antecedentes obstétricos e peri-natais) e parentais (práticas alimentares, conhecimento dos pais sobre alimentação infantil, percepção parental de competência e percepção do peso da criança) e o desenvolvimento de excesso de peso em crianças pré-escolares. Este estudo, de carácter observacional e transversal, foi realizado com 792 crianças pré-escolares, idade M= 4,39 anos (±0,91Dp) e seus pais, residentes num concelho pertencente às NUTs III Dão-Lafões, sendo efectuada a avaliação antropométrica e classificação nutricional das crianças com base no referencial NCHS (CDC, 2000) e da OMS nos pais. O protocolo de pesquisa incluiu instrumentos de medida que validamos para a população portuguesa e a construção do Questionário de Conhecimentos sobre Alimentação Infantil (QAI) cujas propriedades psicométricas certificam a sua qualidade (Alfa de Cronbach = 0,942; Alfa de Cronbach teste re-teste = 0,977). Nas crianças, 31,3% apresentavam excesso de peso (12,4% obesidade), assim como 41,1% das mães (10,2% obesidade) e 64,4% dos pais (14,8% obesidade), sendo mais evidente nas mães o risco metabólico associado ao Perímetro da Cintura. As mães revelam mais conhecimentos sobre alimentação e sentimentos de eficácia mais elevados com o papel parental, enquanto os pais manifestaram mais sentimentos de motivação e satisfação. Os resultados obtidos corroboram existir efeito significativo dos determinantes infantis e parentais no excesso de peso da criança, designadamente: (i) do peso ao nascer, com impacto dos nascidos grandes; (ii) da higiene do sono especificamente dos que dormem menos de 11horas; (iii) dos que não brincam na rua, (iv) das mães mais jovens, do IMC e risco metabólico dos Pais; (v) da percepção parental da imagem corporal dos filhos, verificando-se que quanto mais elevado o IMC das crianças, mais distorcida é esta percepção dos pais; (vi) das crenças, atitudes e práticas alimentares e que permitem inferir que uma maior preocupação com o peso da criança, maior controlo, restrição e menor pressão para comer se associa a maior excesso de peso. As inferências evidenciam que, na vigilância de saúde periódica se torna imprescindível a valorização dos determinantes de risco biológicos e familiares do excesso de peso infantil, considerando programas de intervenção centrados na família, num processo que encontre sentido a partir daquilo que as famílias experienciam, de forma a ajudá-las a criar recursos fortalecedores de competência para uma parentalidade mais positiva.

Este trabalho procura averiguar o impacte das doenças crónicas no ajustamento psicológico das crianças, tendo em conta diferentes tipos de doenças, as suas características e a perceção dos pais acerca das mesmas. Para além disso procura perceber a perceção dos pais e dos profissionais de saúde em relação à importância atribuída ao brincar em contexto hospitalar. A amostra é constituída por 176 crianças, dos 3 aos 10 anos, distribuídas por quatro grupos: crianças com asma, crianças com cancro, crianças com patologia uro-nefrológica e crianças sem doença. A recolha de dados teve lugar nas salas de espera de consulta externa de Pediatria do Hospital Infante D. Pedro e de Oncologia Médica do Hospital Pediátrico de Coimbra. Este estudo recorreu a metodologia quantitativa e qualitativa. Desta forma os instrumentos utilizados foram a Escala de Observação do Brincar (POS), alguns itens do Revised Illness Perception Questionnaire (IPQ-R), o Questionário de Capacidades e de Dificuldades (SDQ) e a entrevista semi-estruturada. O ajustamento psicológico foi avaliado através de questionários aplicados aos pais mas também através da observação direta do brincar da criança, colmatando assim uma das principais lacunas nesta área – o acesso a uma única fonte de informação e forma de avaliação. A análise dos resultados permitiu perceber que não existe uma relação linear entre o ajustamento psicológico das crianças e a presença de uma doença crónica e que a avaliação do ajustamento da criança através da observação direta do brincar nem sempre é coincidente com a perspetiva dos pais acerca desse ajustamento. Tanto os pais como os profissionais de saúde reconhecem ainda inúmeras vantagens na utilização do brincar em crianças com doença crónica.
