Inflammatory fibroid polyp (IFP) is a benign uncommon lesion (1%-4% of gastric benign lesions), originated from the submucosa of the gastrointestinal tract. Its origin is controversial and immunohistochemical studies of lesions have largely refuted the possible vascular, neural or smooth muscle origin. Recent studies suggest a neoplastic etiology due to a mutation, in some cases, in the alpha-type platelet-derived growth factor receptor gene (PDGFRa). Our objective is to report the case of a patient aged 70 years, with gastric IFP, comparing her immunohistochemical profile with those of other studies, and a brief review of the literature.

Based on the lack of information regarding the morphology of marsh deer, this work aims to describe some morphological aspects of the gastric chamber in this species, collaborating with future investigations, mainly related to rational handling in this cervid. This work aimed to describe the morphology of the gastric chamber of the marsh deer, characterizing the external and internal macroscopical details and the microscopical architecture of these structures by light microscopy. Macroscopically, the marsh deer stomach is formed by the rumen, reticulum, omasum and abomasum similar to the domestic ruminants. Microscopically, rumen and abomasum are similar to the domestic ruminants. The reticulum and the omasum, however, present specific characteristics such as keratin on the top of the reticulum, small epithelial projections and omasum folds covered with discrete papillae.

The association of Helicobacter pylori with different gastric diseases has been continuously discussed in the literature, whereas the last 20 years a large body of publications was intended to characterize the ecological niches and habitats of this pathogen being the mouth a possible habitat for that rod. The present study aimed to discuss the occurrence of this organism as part of the supplemental or transient microbiota of the mouth. Thus, an extensive review of literature covering the period 1990-2014 was carried out with different databases, yielding 5024 articles discussing transmission and occurrence of H. pylori, in English or Portuguese. Articles were selected according to the inclusion and exclusion criteria and data were tabulated and analyzed. Despite the significant heterogeneity of the literature, it was found this microaerophilic has a worldwide distribution, even in the mouth, which behaves as a reservoir for gastric reinfection. The role of oral hygiene and periodontal disease in the distribution of this microorganism remains unclear, but most studies support the hypothesis that oral biofilms and inflammation of periodontium may facilitate the installation of this pathogen in the mouth. Studies suggest that this microorganism could collaborate as a predisposing factor for oral cancer.

Use of ring in Roux-en-Y gastric bypass is still a matter of controversy among bariatric surgeons. There is no consensus on its impact in relation to weight loss and weight maintenance in the long term. AIM: To evaluate the influence of the ring on the evolution of body weight over four years after bariatric surgery. METHODS: Retrospective analyzis of 143 women who underwent laparoscopic Roux-en-Y gastric bypass paired on the use or not use of Silastic® ring. Follow-up time was 48 months. Inclusion criteria were age over 18 years, primary bariatric operation and regular attendance at the clinic during the period of interest for research. The technique kept small gastric reservoir estimated in a volume of 30 ml. The food limb had in average 150 cm and the bile one 40 cm from the duodenojejunal angle. The group "ring" used Silastic® device with length of 6.5 cm, placed 2 cm from gastrojejunal anastomosis. The ring was closed for five polypropylene surgical thread sutures. In the morning after surgery the patients received isotonic fluids; on the second day salty liquid diet and were discharged on the third day. Semisolid diet started from the 20th day and solid on the 30th, with daily tablet of polivitamins. RESULTS: The weight loss was larger on the ring than without ring groups in all periods, respectively 10% and only 5% in the third postoperative year. The proportion of not having reached the 50% excess weight loss expectative was significantly higher in the group without ring than in the group with the ring (31% and 8% respectively in the fourth year). There was no difference between groups in delayed recovery of weight lost with the operation. CONCLUSIONS: The results were favorable to use the ring exclusively when it is analyzed only the weight loss.

The Casearia genus (Salicaceae) is well known because of the medicinal use of its species. Among them, a noteworthy one is the C. sylvestris specie because it already has studies concerning its antiproliferative and/or cytotoxic activity in tumor cells. Furthermore, this specie is popularly used against snake bites, in gastric ulcers treatment and as anti-inflammatory. As well as this, there are other species from this same genus which have been poorly studied, such as the following species: C. decandra, C. grandiflora, C. javitensis, C. arborea, C. lasiophylla and C. ulmifolia. However, several biological activities have been reported for them. In this context, the aim of this project, besides of contributing to the Casearia genus studies, is to study those six species through the analysis and documentation of their leaves' chemical composition (aqueous, ethanolic and hexanic extracts), using analytical separation techniques coupled with spectroscopic techniques, such as UHPLC-DAD, GC-MS and NMR 1H, which will assist the identification of new secondary metabolites in this genus. Moreover, another goal of this present work is aiming the bioprospection of substances with medicinal and economical potential and finally promote the systematic study of some biological activities, such as antimicrobial and cytotoxicity bioassays. A wide variety of metabolites was identified in those three types of extracts, being most of them detected for the first time in Casearia genus, highlighting C. lasiophylla and C. decandra for featuring antimicrobial activity against Staphylococcus aureus.

CONTEXT: Esophageal dysphagia is the sensation that the ingested material has a slow transit or blockage in its normal passage to the stomach. It is not always associated with motility or transit alterations. OBJECTIVES: To evaluate in normal volunteers the possibility of perception of bolus transit through the esophagus after swallows of liquid and solid boluses, the differences in esophageal contraction and transit with these boluses, and the association of transit perception with alteration of esophageal contraction and/or transit. METHODS: The investigation included 11 asymptomatic volunteers, 4 men and 7 women aged 19-58 years. The subjects were evaluated in the sitting position. They performed swallows of the same volume of liquid (isotonic drink) and solid (macaroni) boluses in a random order and in duplicate. After each swallow they were asked about the sensation of bolus passage through the esophagus. Contractions and transit were evaluated simultaneously by solid state manometry and impedance. RESULTS: Perception of bolus transit occurred only with the solid bolus. The amplitude and area under the curve of contractions were higher with swallows of the solid bolus than with swallows of the liquid bolus. The difference was more evident in swallows with no perception of transit (n = 12) than in swallows with perception (n = 10). The total bolus transit time was longer for the solid bolus than for the liquid bolus only with swallows followed by no perception of transit. CONCLUSION: The results suggest that the perception of esophageal transit may be the consequence of inadequate adaptation of esophageal transit and contraction to the characteristics of the swallowed bolus.

BACKGROUND: MYC deregulation is a common event in gastric carcinogenesis, usually as a consequence of gene amplification, chromosomal translocations, or posttranslational mechanisms. FBXW7 is a p53-controlled tumor-suppressor that plays a role in the regulation of cell cycle exit and reentry via MYC degradation. METHODS: We evaluated MYC, FBXW7, and TP53 copy number, mRNA levels, and protein expression in gastric cancer and paired non-neoplastic specimens from 33 patients and also in gastric adenocarcinoma cell lines. We also determined the invasion potential of the gastric cancer cell lines. RESULTS: MYC amplification was observed in 51.5% of gastric tumor samples. Deletion of one copy of FBXW7 and TP53 was observed in 45.5% and 21.2% of gastric tumors, respectively. MYC mRNA expression was significantly higher in tumors than in non-neoplastic samples. FBXW7 and TP53 mRNA expression was markedly lower in tumors than in paired non-neoplastic specimens. Moreover, deregulated MYC and FBXW7 mRNA expression was associated with the presence of lymph node metastasis and tumor stage III-IV. Additionally, MYC immunostaining was more frequently observed in intestinal-type than diffuse-type gastric cancers and was associated with MYC mRNA expression. In vitro studies showed that increased MYC and reduced FBXW7 expression is associated with a more invasive phenotype in gastric cancer cell lines. This result encouraged us to investigate the activity of the gelatinases MMP-2 and MMP-9 in both cell lines. Both gelatinases are synthesized predominantly by stromal cells rather than cancer cells, and it has been proposed that both contribute to cancer progression. We observed a significant increase in MMP-9 activity in ACP02 compared with ACP03 cells. These results confirmed that ACP02 cells have greater invasion capability than ACP03 cells. CONCLUSION: In conclusion, FBXW7 and MYC mRNA may play a role in aggressive biologic behavior of gastric cancer cells and may be a useful indicator of poor prognosis. Furthermore, MYC is a candidate target for new therapies against gastric cancer.
