Os cuidados em neonatologia têm se intensificado nos últimos anos no que tange as condições de prematuridade e do baixo peso ao nascimento, especialmente em relação à saúde, crescimento e desenvolvimento das crianças submetidas a esta condição de risco. Um dos pontos relevantes das pesquisas em neonatologia condiz à observação comportamental dos sinais emitidos pelo funcionamento do organismo do prematuro, que possivelmente revelam o nível de organização ou de desorganização do seu organismo como um todo, tornando possível predizer futuros déficits. Desde 1978, uma pesquisadora americana, Heidelise Als, juntamente com um grupo de colaboradores, iniciaram diversas pesquisas sobre o comportamento e assistência ao prematuro de baixo peso, as quais enfatizam uma prática diferenciada de cuidados para o desenvolvimento utilizando como base a Teoria Síncrono Ativa do Desenvolvimento (TSAD), que permitiu o registro e elaboração de cuidados individualizados para promoção do desenvolvimento de bebês prematuro, refletindo em ganhos para um desenvolvimento efetivo das diferentes funções do organismo, em particular as áreas cognitivo-motora e comportamental. Desta forma, o presente estudo tem como objetivo quantificar os comportamentos que são mais evidentes no contexto de uma UTIN, e quais os mecanismos ambientais que favorecem a manifestação dos mesmos, a fim de traçar os cuidados para o desenvolvimento de acordo com a realidade apresentada na UTIN. A pesquisa foi planejada a partir de um pré-projeto, o qual possibilitou o registro em 19 amostras de crianças para 30 sessões de observação, sendo desenvolvida no período de abril à julho de 2005. A coleta de dados foi realizada em bebês prematuros com baixo peso ao nascimento e com idade gestacional entre 26 a 37 semanas. A mesma utilizou um etograma padrão, baseado na observação comportamental de H. Als, assinalando os eventos ambientais que promoviam determinados comportamentos. A observação foi realizada em um período de 15 minutos por sujeito de cada sessão. Os dados tabulados resultaram em: dados gerais, que constituem as variáveis maternas e dos bebês, e dados específicos, referindo os comportamentos e as interferências ambientais observados. Os dados gerais mantiveram a similaridade com outras pesquisas, apontando as condições sociais desfavoráveis e a predisposição materna a infecções como possíveis fatores predisponentes ao parto prematuro, assim como a situação de prematuridade como agravante da condição de saúde e bem-estar do bebê. Entre os dados específicos, foi possível registrar uma prevalência dos comportamentos de retraimento, que revelam a desorganização do bebê, mediante aos excessos de interferências ambientais, como ruído, manipulação e luminosidade, muito comuns em UTIN. Mediante estes fatos, sugerimos nesta pesquisa as possíveis soluções para amenizar o excesso de manifestações comportamentais desorganizadoras, buscando priorizar os cuidados para o desenvolvimento por meio de práticas utilizadas por H. Als e sua equipe.

Objetivo: Analisar os desfechos aumento/diminuição da pressão intracraniana e/ou queda da pressão de perfusão cerebral, proporcionados pela fisioterapia respiratória em pacientes graves assistidos em unidade de terapia intensiva. Métodos: Por meio de uma revisão sistemática da literatura, foram selecionados ensaios clínicos publicados entre 2002 e 2012. A busca envolveu as bases de dados LILACS, SciELO, MedLine e PEDro, usando os descritores physical therapy, physiotherapy, respiratory therapy e randomized controlled trials em cruzamento com o descritor intracranial pressure. Resultados: Foram incluídos 5 estudos, que somaram 164 pacientes, com média de idade entre 25 e 65 anos, e que indicaram que as manobras de fisioterapia respiratória aumentaram significativamente a pressão intracraniana, sem alterar a pressão de perfusão cerebral. Os artigos abordaram as técnicas de vibração, vibrocompressão, tapotagem, drenagem postural, além da manobra de aspiração intratraqueal. Todos os pacientes estavam sob ventilação mecânica invasiva. Conclusão: A fisioterapia respiratória promove aumento da pressão intracraniana. Os estudos sugerem que não há repercussões hemodinâmicas e respiratórias a curto prazo ou alteração da pressão de perfusão cerebral. Entretanto, não há estudos que avaliem desfechos clínicos e que assegurem a segurança das manobras.

Health care waste (HCW) is the type of waste that results from activities performed in health care services during care provision to humans or animals. Presently, according to RDC 306/04, issued in 2004 by Anvisa, and Resolution no. 358/05, by CONAMA, waste groups have the following classification: Group A (biological waste), Group B (chemical waste), Group C (waste containing radionucleotides), Group D (common waste) and Group E (piercing and cutting waste). In Brazil, 149 tons of wastes are collected every day, and HCW corresponds to approximately 1% to 3 % of that total. An efficient way to adequately manage HCW is through the Health Care Waste Management Plan (HCWMP), and it is possible to reduce the risk posed by certain materials in addition to ensuring disposal in an ecologically correct and economical fashion. According to the Pan-American Health Organization (PAHO), the management process enables health care establishments to adequately manage waste. Hence, there is greater control and reduction in the health risks caused by infectious or special waste, in addition to facilitated recycling, treatment, storage, transport and final disposal of solid hospital waste in an environmentally safe fashion. To evaluate the management of HCW of Groups A and D from the Intensive Care Unit of the University Emergency Hospital - FMB - UNESP in the city of Botucatu according to the guidelines presently in force. The waste flow was followed up, and during four random days in the month of September 2011, waste was quantified by estimating daily and monthly values, according to its classification. : In 2011, the University hospital has produced an average of 57,676.8 kg/month of biological and common waste. Due to the complexity and instability of clinical conditions of ICU patients, the drug therapy applied in this type of environment requires a combination of several prescribed drugs, which is a favorable condition for drug interaction, toxic synergism and possible iatrogenia. In the possible universe of ICU occurrences, this study aimed at identifying and evaluating the incidence of adverse events in drug therapy at the Intensive Care Therapy Service (SETI) in wards I and II. It is a cross-sectional, descriptive, prospective and quantitative study conducted from August to September, 2011 in the Intensive Care Service of the Botucatu School of Medicine University Hospital - UNESP. The population consisted of fifteen clinical nurses, including those in the Improvement and Volunteer Internship Programs, who contributed to the investigation after signing an informed consent form and according to approval by the Research Ethics Committee number 10711/CE - FMB. The data were entered on a form and analyzed. Results showed that, on average, 8.9% of events/day occurred, and the highest frequency was observed on August 04, 2011. 63% and 22% were respectively observed in the morning and afternoon shifts, and 15% in the night shift. 48% of these were due to administration time errors, followed by drug prescription and dispensation errors, with percentages of 22% and 18%, respectively. Antibiotics showed the highest frequency of adverse events - 18%, which was followed by 13% for anticoagulant, 11% for antiemetic and 10% for antiulcerative drugs. As regards the occurrence of adverse events related to hospitalization time, the highest frequency occurred in patients who were hospitalized for 10 days. The goal of this study is to characterize the clinical outline of patients in postoperative of bariatric surgery attended in an Intensive Care Unit and identify the nursing diagnosis presented by these patients in accordance to Taxonomy II of North American Nursing Diagnosis Association (NANDA). This is a transversal, descriptive and prospective study that was desenvolved at the Intensive Care Unit of the Clinical Hospital of Botucatu Medical School – UNESP. The population was composed by all the patients who were submitted to the bariatric surgery and were attended at the Intensive Care Unit in the period between June and August of 2010, totalizing 13 patients. The data were collected based on the Nursing History that is composed by the Anamnesis, Physical Examination and Complementary Exams. The results were worked out: a) among the 13 patients studied, 10 are women (76,9%) and 3 men (23,1%); the majority is between 30 and 40 years old; 11 people of 13 are morbid obese, which means that they have Body Mass Index higher than 40 kg/m2; b) It was identified 22 nursing diagnosis and 14 of these were found in 100% of the patients. They belong to the dominions of Activity/Rest, Comfort, Elimination and Changing, Security and Protection, Functions and Relationships. Therefore, in relation to the factors, it was conclude that nursing diagnosis help nurses to understand the unsettled human answers or the risks to disturbance in an individual way, which contributes to plan specific interventions to these patients. It was possible to verify that the stage of the nursing diagnosis that is part of the nursing process should be executed by the nurses systematically, because it has been realized by the nurses’ clinical idea, but not in a systematical way

Admission blood lactate concentration has been shown to be a useful indicator of disease severity in human medicine and numerous studies have associated hyperlactatemia with patients at high risk of death who should be treated aggressively regardless of the cause of the lactate generation. The degree and duration of hyperlactacidaemia also have been correlated with the subsequent development of organ failure. Similarly, in a small number of studies about equine colic, blood lactate concentration has been investigated as a useful prognostic variable . In neonatal foals blood lactate was studied first by Magdesian (2003) who described venous blood lactate concentration in 14 normal foals during the initial 48 hours post-partum. A preliminary study about lactate concentration in foals presenting to a neonatal intensive care unit reported that surviving foals had earlier lactate clearance. The measurement of blood lactate concentration is traditionally available with a wet chemistry laboratory method or with blood-gas analyzers, for clinicians working at university or large private hospital. But this methods may not be easily accessible to many practitioners in field conditions. Several relatively inexpensive, easy to use and rapid pocket size monitors to measure lactate concentration have been validated in human patients and athletes. None of these portable lactate analyzer have been evaluated in clinically normal neonatal foals or in foals referred to a neonatal intensive care unit. The aims of this study were to validate the Lactate Scout analyzer in neonatal foals, investigating the correlation between lactate concentration in whole blood measured with the portable monitor and measured in plasma with the reference laboratory analyzer. The effect of hematocrit (Hct) on the accuracy of Lactate Scout was also evaluated. Further, we determined the utility of venous lactate measurement in critically-ill foals, describing lactate values in the most frequent neonatal pathologies, evaluating serial blood lactate measurements during hospitalization and investigating its prognostic value. The study also describes normal range for lactate in healthy neonatal foals during the first 72 hours of life.

Phase 1: To validate Near-Infrared Reflectance Analysis (NIRA) as a fast, reliable and suitable method for routine evaluation of human milk’s nitrogen and fat content. Phase 2: To determine whether fat content, protein content and osmolality of HM before and after fortification may affect gastroesophageal reflux (GER) in symptomatic preterm infants. Patients and Methods: Phase 1: 124 samples of expressed human milk (55 from preterm mothers and 69 from term mothers) were used to validate NIRA against traditional methods (Gerber method for fat and Kjeldhal method for nitrogen). Phase 2: GER was evaluated in 17 symptomatic preterm newborns fed naïve and fortified HM by combined pH/intraluminal-impedance monitoring (pH-MII). HM fat and protein content was analysed by a Near-Infrared-Reflectance-Analysis (NIRA). HM osmolality was tested before and after fortification. GER indexes measured before and after fortification were compared, and were also related with HM fat and protein content and osmolality before and after fortification. Results: Phase 1: · A strong agreement was found between traditional methods’ and NIRA’s results (expressed as g/100 g of milk), both for fat and nitrogen content in term (mean fat content: NIRA=2.76; Gerber=2.76; mean nitrogen content: NIRA=1.88; Kjeldhal =1.92) and preterm (mean fat content: NIRA=3.56; Kjeldhal=3.52; mean nitrogen content: NIRA=1.91; Kjeldhal =1.89) mother’s milk. · Nitrogen content of the milk samples, measured by NIRA, ranged from 1.18 to 2.71 g/100 g of milk in preterm milk and from 1.48 to 2.47 in term milk; fat content ranged from 1.27 to 6.23 g/100 g of milk in preterm milk and from 1.01 to 6.01 g/100 g of milk in term milk. Phase 2: · An inverse correlation was found between naïve HM protein content and acid reflux index (RIpH: p=0.041, rho=-0.501). · After fortification, osmolality often exceeded the values recommended for infant feeds; furthermore, a statistically significant (p<.05) increase in non acid reflux indexes was observed. Conclusions: NIRA can be used as a fast, reliable and suitable tool for routine monitoring of macronutrient content of human milk. Protein content of naïve HM may influence acid GER in preterm infants. A standard fortification of HM may worsen non acid GER indexes and, due to the extreme variability in HM composition, may overcome both recommended protein intake and HM osmolality. Thus, an individualized fortification, based on the analysis of the composition of naïve HM, could optimize both nutrient intake and feeding tolerance.

NAVA®, dall’inglese Neurally Adjusted Ventilatory Assist, è una tecnica di monitoraggio e di ventilazione e rappresenta una sofisticata innovazione tecnologica in quanto consente un’assistenza ventilatoria costantemente in armonia con le esigenze del paziente, grazie alla rilevazione diretta dell’attività elettrica del diaframma. Da un punto di vista pratico-operativo, NAVA è un modulo integrativo che si inserisce nelle apparecchiature di ventilazione già esistenti integrandone al meglio le funzionalità. A tale modulo è collegato un sondino nasogastrico nella cui parte distale vi sono elettrodi bipolari. Il sondino viene inserito nell’esofago vicino al diaframma e permette l’acquisizione del segnale Edi (Diaphragmatic Electrical Activity), relativo all’attività elettrica del diaframma. L’attività di quest ultimo è strettamente correlata alla dinamica respiratoria del paziente, in quanto il diaframma, contraendosi, determina il flusso d’aria all’interno delle vie aree. Il segnale Edi acquisito viene usato per interfacciarsi con il ventilatore e grazie speciali algoritmi, il segnale guida il ventilatore permettendo un’assistenza ventilatoria proporzionale e sincrona agli sforzi respiratori del paziente. NAVA è tra le nuove apparecchiature sanitarie ed elettromedicali che la Banca Popolare dell’Emilia Romagna ha recentemente donato al Centro Grandi Ustionati dell’Ospedale Bufalini di Cesena per un valore complessivo di oltre 120.000 euro. NAVA e gli altri strumenti donati sono apparecchiature di ultima generazione destinate a migliorare le possibilità di sopravvivenza dei pazienti più critici, con ustioni e ferite alla cute molto gravi, che necessitano di un’assistenza intensiva. In questo elaborato, nel Capitolo 1, viene presentata la Fisiologia dell’apparato respiratorio e a seguire, nel Capitolo 2, viene descritta la Ventilazione meccanica convenzionale, ancora oggi molto utilizzata. Successivamente, nel Capitolo 3 è illustrata la nuova modalità NAVA. Proseguendo, nel Capitolo 4 si apre un confronto tra le principali differenze tra la NAVA e le precedenti modalità di ventilazione. L’elaborato si conclude con la speranza che NAVA, un’innovazione senza precedenti, non sia limitata ad un investimento potenzialmente utile nel presente della terapia intensiva, ma che la ricerca ad essa correlata possa, in un imminente futuro, aprire la strada a nuove tecnologie ancora più efficienti nella salvaguardia dei pazienti in terapia intensiva.
