Segundo a Organização Mundial da Saúde, o consumo de álcool no mundo tornou-se um problema de saúde pública. Neste contexto, o Brasil figura na 63ª posição no mundo em consumo de álcool per capita para pessoas com 15 anos ou mais de idade. Além de seus efeitos sócio-econômicos, o etanol constitui um importante fator de risco na ocorrência de isquemias cerebrais. O consumo exacerbado desta droga colabora para o mau prognóstico, assim como para o possível tratamento de morbidades relacionadas ao acidente vascular cerebral. O objetivo deste estudo foi avaliar alterações neuromotoras após bloqueio da ativação micróglial com minociclina em ratos submetidos à isquemia focal no córtex motor, quando tratadas cronicamente com etanol da adolescência a fase adulta. Ratos receberam durante 55 dias, por gavagem, etanol (6,5 g/kg/dia, 22,5 p/v). No término destes 55 dias os animais foram submetidos à microinjeções do peptídeo vasoconstritor endotelina-1 (40 pmol), para indução de lesão isquêmica focal córtex motor. Os animais isquemiados foram tratados com minociclina recebendo duas doses diárias de 50 mg/kg nos primeiros dois dias, e mais cinco aplicações diárias únicas de 25 mg/kg, por via intraperitoneal, até o sétimo dia pós-indução isquêmica. Os testes comportamentais realizados foram campo aberto, plano inclinado e rota-rod. Os resultados demonstraram que os animais isquemiados e os expostos ao etanol e isquemiados apresentaram déficits motores em todos os testes comportamentais. Entretanto, o tratamento com minociclina foi capaz de reverte-los, possibilitando melhor desempenho em todos os testes aplicados. Os resultados sugerem que a minociclina foi capaz de reverter os danos motores ocasionados pelo acidente vascular cerebral, mesmo em presença do etanol. O exato mecanismo envolvido neste processo necessita ser investigado em pesquisas futuras.

Introduction: Parkinson’s disease (PD) is a chronic disease of the nervous system, characterized by degeneration of neurons in the mesencephalic substantia nigra, leading to a clinical state of rest tremor, bradykinesia, muscular rigidity and postural instability. Physical therapy seeks to act by slowing the progression of the disease and when done in a group and maintain and / or improving the motor skills of the individual, can provide psychosocial benefi ts. Objective: examine the infl uence of the physical therapy group in balance, functional mobility and quality of life of individuals with PD. Method: participated in this study 04 subjects were female, mean age 67.75 (± 9.5) years, with medical diagnosis of PD, stages 1 to 3 of the Hoehn & Yahr. Before starting treatment, subjects underwent an assessment of the balance (BBS), functional mobility (TUG) and the quality of life (PDQ-39).The treatment was performed in groups, for a period of 10 weeks, lasting 60 minutes each session twice a week, totaling 20 sessions of physiotherapy. Upon completion of the treatment period the subjects were again assessed for balance, functional mobility and quality of life. The data were analyzed using the Student t-test, with signifi cance level of 5% (p ≤ 0.05). Results: statistical analysis showed signifi cant differences in three variables: equilibrium (p = 0.010), functional mobility (p = 0.029) and quality of life (p = 0.004), after physiotherapy intervention. Conclusion: physiotherapy treatment was group provides better balance, functional mobility and quality of life of patients with PD.

Introduction: Parkinson’s Disease (PD) is characterized by a set of four motor symptoms: tremor, rigidity, bradykinesia and postural instability. These defi cits may predispose individuals to limitations resulting from falls and their secondary consequences. Objective: To evaluate the functional balance and quality of life (QoL) in individuals with PD and determine whether there is correlation between performance on tests of balance with the QoL. Method: The project was referred to the Ethics Committee in Research of Universidade Estadual Paulista “Julio de Mesquita Filho” Campus de Marília and was approved under protocol number 1806/09. Participated in this study with individuals diagnosed with PD between levels one and four in the Hoen and Yahr scale. The subjects were evaluated according to functional balance and QoL, respectively by the instruments: Functional Balance Scale Berg (EEFB), Time Up and Go test (TUG), and Parkinson’s Disease Questionnaire-39 (PDQ-39). To perform the statistical analysis used the GraphPad Prism 5. To perform the correlation analysis for the variables passed normality by the Shapiro-Wilk. Since the variables were non-parametric test was used Spearman. During the analysis the statistical signifi cance level was considered p ≤ 0, 05. Results: We studied 25 individuals aged between 54 and 85 years (71.20 ± 8.50), time of diagnosis between one and 39 years (6.54 ± 7.71) disease. Moderate correlation was found between the EEFB with QoL (r =- 0.6), and TUG with QoL (r = 0.6836). Among the aspects of QoL balance showed a higher correlation with the domains “mobility” (TUG r = 0, 6768; EEFB r = -0.6155) and “Activities of daily living” (TUG r = 0, 7357, and EEFB r = -0.6521). Conclusion: Patients with Parkinson’s disease show defi cits in balance and QoL. The balance disorders have a high correlation among themselves and show how aspects of QoL.

Evolutionary algorithms have been widely used for Artificial Neural Networks (ANN) training, being the idea to update the neurons' weights using social dynamics of living organisms in order to decrease the classification error. In this paper, we have introduced Social-Spider Optimization to improve the training phase of ANN with Multilayer perceptrons, and we validated the proposed approach in the context of Parkinson's Disease recognition. The experimental section has been carried out against with five other well-known meta-heuristics techniques, and it has shown SSO can be a suitable approach for ANN-MLP training step.

Gait disorders are identified in people with Parkinson’s disease. The aim of this study was to investigate the effect of auditory cues and medication on kinematic, kinetic and EMG parameters, during different gait phases of people with PD and healthy elderly. Thirty subjects distributed in two groups (Group 1, PD patients off and on medication; Group 2, healthy elderly) participated in this study and were instructed to walk in two experimental conditions: non-cued and cued. Therefore, kinematic, kinetic and electromyography analyses were utilized to investigate the locomotor pattern. Changes in locomotor pattern (greater muscular activity) with auditory cue were observed for PD patients. Regarding the medication, locomotor parameter improvement was observed after levodopa intake in association with the auditory cue. These results confirm the hypothesis about the external cues therapy that could be used as a complement to drug therapy to achieve improvement in the locomotor pattern of PD patients.

Introdução: Até o presente momento, não existem observações com o intuito de comparar o andar (em terreno regular e durante a ultrapassagem de obstáculo) de pacientes com doença de Parkinson (DP) caidores e não caidores. O conhecimento mais detalhado das diferenças clínicas e locomotoras entre pacientes caidores e não caidores pode servir como ferramenta importante para o melhor delineamento de intervenções com o intuito de prevenir e/ou reduzir a ocorrência de quedas na DP. Objetivo: Comparar o quadro clínico e o comportamento locomotor durante o andar em terreno regular e a ultrapassagem de obstáculo de indivíduos com DP caidores e não caidores. Método: Participaram do estudo 36 indivíduos com diagnóstico de DP idiopática. Inicialmente, a ocorrência de quedas foi registrada durante quatro meses por meio de entrevista semanal (contato pessoal e telefônico). Estes dados foram utilizados para distribuir os indivíduos em dois grupos: caidores (n=12) e não caidores (n=24). Em seguida, os pacientes realizaram, em estado “on” do medicamento, a avaliação do quadro clínico e do comportamento locomotor. A avaliação clínica incluiu a aplicação da escala de Hoehn & Yahr (estágio da doença) e da subescala motora da Unified Parkinson’s Disease Rating Scale (comprometimento motor). A avaliação do andar consistiu em caminhar, em velocidade preferida, sobre uma passarela de oito metros de comprimento em condições com e sem obstáculo. Um sistema optoeletrônico para a análise do movimento (OPTOTRAK) foi utilizado para a coleta de dados do andar. As variáveis dependentes incluíram as pontuações nas escalas clínicas e os parâmetros espaciais e temporais do andar em terreno regular e da ultrapassagem de obstáculo. A doença de Parkinson (DP) é uma doença neurodegenerativa que afeta principalmente o controle motor com reflexos negativos no desempenho funcional de seus pacientes. Alterações no equilíbrio podem levar à diminuição da independência e funcionalidade. Alguns estudos evidenciam os benefícios do exercício físico, como alternativa nãofarmacológica para esses pacientes. Objetivo: O presente trabalho analisou e comparou os efeitos de dois programas de atividade física sobre o risco de quedas e o equilíbrio funcional em pacientes com doença de Parkinson. O presente estudo também teve como objetivo verificar a associação entre as variáveis clínicas e comportamentais. Método: Participaram do estudo 30 pacientes com DP idiopática entre os estágios I a III na escala de estagiamento clínico de Hoehn & Yahr, sendo distribuídos em três grupos: grupo de treinamento com pesos (GTP), atividade física generalizada (GAFG) e o grupo controle (GC). O período de intervenção para o GTP e o GAFG foi de quatro meses. As avaliações foram realizadas com os participantes na fase “on” da medicação. Para avaliar o equilíbrio dinâmico juntamente com o risco de quedas foi utilizado o teste Timed Up and Go (TUG) e, para analisar o risco de quedas foi utilizada a Escala de Equilíbrio Funcional de Berg (EEFB). As avaliações clínicas foram realizadas por meio da Unified Parkinson’s Disease Rating Scale (sub-escalas funcional e motora), escala de nível de gravidade da doença de Hohen & Yahr e Mini-Exame do Estado Mental (MEEM). O teste U de Mann-Whitney foi utilizado para comparação das variáveis analisadas entre os três grupos separadamente por momento do treinamento. Objective. This study aimed to evaluate the effect of a long-period multimodal exercise program on balance, mobility and clinical status of patients with Parkinson’s disease (PD). Methods. Thirty-three PD patients were assigned into two groups: a training group (TG—n = 22; aged 67.23 ± 8.39 years) and a control group (CG—n = 9; aged 71.56 ± 8.50 years). The TG patients were enrolled in a 6-month multimodal exercise program. This program was designed to improve physical capacity components and to reduce PD impairments. Balance and mobility were assessed immediately before and after the training protocol using the Berg Balance Scale (BBS), the “Timed up and go” (TUG), and the Posture Locomotion Test (PLM). Also, clinical variables were assessed (disease stage and impairments). Results. The TG showed an improvement in the TUG (P = 0.006) while CG were not influenced by the 6-months period. Both groups showed no differences for BBS and PLM and for their disease impairments—assessed through the Unified Parkinson’s disease Scale. Conclusions. Long-term multimodal exercise programs are able to improve mobility of patients with Parkinson’s disease and therefore should be used on clinical day life.

Rationale Cannabidiol, the main nonpsychotropic constituent of Cannabis sativa, possesses a large number of pharmacological effects including anticonvulsive, sedative, hypnotic, anxiolytic, antipsychotic, anti-inflammatory, and neuroprotective, as demonstrated in clinical and preclinical studies. Many neurodegenerative disorders involve cognitive deficits, and this has led to interest in whether cannabidiol could be useful in the treatment of memory impairment associated to these diseases. Objectives We used an animal model of cognitive impairment induced by iron overload in order to test the effects of cannabidiol in memory-impaired rats. Methods Rats received vehicle or iron at postnatal days 12-14. At the age of 2 months, they received an acute intraperitoneal injection of vehicle or cannabidiol (5.0 or 10.0 mg/kg) immediately after the training session of the novel object recognition task. In order to investigate the effects of chronic cannabidiol, iron-treated rats received daily intraperitoneal injections of cannabidiol for 14 days. Twenty-four hours after the last injection, they were submitted to object recognition training. Retention tests were performed 24 h after training. Results A single acute injection of cannabidiol at the highest dose was able to recover memory in iron-treated rats. Chronic cannabidiol improved recognition memory in iron-treated rats. Acute or chronic cannabidiol does not affect memory in control rats. Conclusions The present findings provide evidence suggesting the potential use of cannabidiol for the treatment of cognitive decline associated with neurodegenerative disorders. Further studies, including clinical trials, are warranted to determine the usefulness of cannabidiol in humans suffering from neurodegenerative disorders.

Objective: To validate the freezing of gait questionnaire (FOG-Q) for a Brazilian population of Parkinson's disease (PD) patients. Methods: One hundred and seven patients with a diagnosis of PD were evaluated by shortened UPDRS motor scale (sUPDRm), Hoehn and Yahr (HY), Schwab and England scale (SE), Berg balance scale (BBS), falls efficacy scale international (FES-I), gait and balance scale (GABS), and the FOG-Q Brazilian version. Results: 47.7% of PD patients had FOG episodes; this group had worse scores on sUPDRSm, FOGQ, FES-I, BBS, GABS and FOG item of UPDRS when compared to the PD group without FOG. The internal consistency was 0.86, intra-rater 0.82 and inter-rater 0.78. The FOG-Q Brazilian version was significantly correlated with items related to gait and balance. The ROC curve was 0.94, the sensitivity was 0.90 and specificity was 0.92. Conclusion: Our study suggests that the FOG-Q Brazilian version is a reliable and valid instrument for assessing FOG in PD patients.

The main clinical manifestations of the spinocerebellar ataxias (SCAs) result from the involvement of the cerebellum and its connections. Cerebellar activity has been consistently observed in functional imaging studies of olfaction, but the anatomical pathways responsible for this connection have not yet been elucidated. Previous studies have demonstrated olfactory deficit in SCA2, Friedreich's ataxia and in small groups of ataxia of diverse aetiology. The authors used a validated version of the 16-item smell identification test from Sniffin' Sticks (SS-16) was used to evaluate 37 patients with genetically determined autosomal dominant ataxia, and 31 with familial ataxia of unknown genetic basis. This data was also compared with results in 106 Parkinson's disease patients and 218 healthy controls. The SS-16 score was significantly lower in ataxia than in the control group (p<0.001, 95% CI for beta=0.55 to 1.90) and significantly higher in ataxia than in Parkinson's disease (p<0.001, 95% CI for beta=-4.58 to -3.00) when adjusted for age (p=0.001, 95% CI for beta=-0.05 to -0.01), gender (p=0.19) and history of tobacco use (p=0.41). When adjusted for general cognitive function, no significant difference was found between the ataxia and control groups. This study confirms previous findings of mild hyposmia in ataxia, and further suggests this may be due to general cognitive deficits rather than specific olfactory problems.

Background and Purpose-The pattern of antenatal brain injury varies with gestational age at the time of insult. Deep brain nuclei are often injured at older gestational ages. Having previously shown postnatal hypertonia after preterm fetal rabbit hypoxia-ischemia, the objective of this study was to investigate the causal relationship between the dynamic regional pattern of brain injury on MRI and the evolution of muscle tone in the near-term rabbit fetus. Methods-Serial MRI was performed on New Zealand white rabbit fetuses to determine equipotency of fetal hypoxia-ischemia during uterine ischemia comparing 29 days gestation (E29, 92% gestation) with E22 and E25. E29 postnatal kits at 4, 24, and 72 hours after hypoxia-ischemia underwent T2- and diffusion-weighted imaging. Quantitative assessments of tone were made serially using a torque apparatus in addition to clinical assessments. Results-Based on the brain apparent diffusion coefficient, 32 minutes of uterine ischemia was selected for E29 fetuses. At E30, 58% of the survivors manifested hind limb hypotonia. By E32, 71% of the hypotonic kits developed dystonic hypertonia. Marked and persistent apparent diffusion coefficient reduction in the basal ganglia, thalamus, and brain stem was predictive of these motor deficits. Conclusions-MRI observation of deep brain injury 6 to 24 hours after near-term hypoxia-ischemia predicts dystonic hypertonia postnatally. Torque-displacement measurements indicate that motor deficits in rabbits progressed from initial hypotonia to hypertonia, similar to human cerebral palsy, but in a compressed timeframe. The presence of deep brain injury and quantitative shift from hypo-to hypertonia may identify patients at risk for developing cerebral palsy. (Stroke. 2012;43:2757-2763.)

Pneumococcal meningitis is a life-threatening disease characterized by an acute infection affecting the pia matter, arachnoid and subarachnoid space. The intense inflammatory response is associated with a significant mortality rate and neurologic sequelae, such as, seizures, sensory-motor deficits and impairment of learning and memory. The aim of this study was to evaluate the effects of acute and extended administration of cannabidiol on pro-inflammatory cytokines and behavioral parameters in adult Wistar rats submitted to pneumococcal meningitis. Male Wistar rats underwent a cisterna magna tap and received either 10 mu l of sterile saline as a placebo or an equivalent volume of S. pneumoniae suspension. Rats subjected to meningitis were treated by intraperitoneal injection with cannabidiol (2.5, 5, or 10 mg/kg once or daily for 9 days after meningitis induction) or a placebo. Six hours after meningitis induction, the rats that received one dose were killed and the hippocampus and frontal cortex were obtained to assess cytokines/chemokine and brain-derived neurotrophic factor levels. On the 10th day, the rats were submitted to the inhibitory avoidance task. After the task, the animals were killed and samples from the hippocampus and frontal cortex were obtained. The extended administration of cannabidiol at different doses reduced the TNF-alpha level in frontal cortex. Prolonged treatment with canabidiol, 10 mg/kg, prevented memory impairment in rats with pneumococcal meningitis. Although descriptive, our results demonstrate that cannabidiol has anti-inflammatory effects in pneumococcal meningitis and prevents cognitive sequel. (C) 2012 Elsevier B.V. All rights reserved.
