"If you know the enemy and know yourself, you need not fear the result of a hundred battles. If you know yourself but not the enemy, for every victory gained you will also suffer a defeat" (SunTzu the Art of War, 544-496 BC). Although written for the managing of conflicts and winning clear victories, this basic guideline can be directly transferred to our battle against apicomplexan parasites and how to focus future basic research in order to transfer the gained knowledge to a therapeutic intervention stratey. Over the last two decades the establishment of several key-technologies, by different groups working on Toxoplasma gondii, made this important human pathogen accessible to modern approaches in molecular cell biology. In fact more and more researchers get attracted to this easy accessible model organism to study specific biological questions, unique to apicomplexans. This fascinating, unique biology might provide us with new therapeutic options in our battle against apicomplexan parasites by finding its Achilles' heel. In this article we argue that in the absence of a powerful high throughput technology for the characterisation of essential gene of interests a coordinated effort should be undertaken to convert our knowledge of the genome into one of the phenome.

Background: The exploratory study is part of an evaluation of the pre-graduate teaching of communication skills (Lausanne Medical School). It is based on the data of a project highlighting the impact of individualized vs. group training for medicine students in breaking bad news to simulated patients who are diagnosed with cancer. The analysis of the video-taped interviews of the students (N=63) with the RIAS has shown a current usage of utterances such as I don't know if -you have any plans for the future / you have already heard about chemotherapy / ... or I don't know how -you are feeling today after this surgery / you like that all this stuff takes place / ...Aim: The present study questions the specificity of these assertive utterances used as questions (indirect), the specificity of their content, and their intentionality - specific vs. exploratory.Methods: The mentioned utterances are qualitatively analyzed (content analysis, intentionality analysis, etc).Results: 26 students (41%) used 1 to 6 times I don't know utterances during the interviews that contain 53 of such utterances in total. In contrast, they are atypical in an oncologist sample who conducted similar interviews (N=31; 4 oncologist used them 1 to 2 times). In more than half of the cases (29/53), simulated patients interpret I don't know questions as giving them a space to speak (open responses). Conclusions: The atypicality of the I don't know utterances in the oncologist sample may have linguistic explanations in terms of generational marker, but the specificity of the content suggests psychological explanations in terms of defense mechanism as well (marker of "toning down" or insecurity as regards the discussed topic).Keywords: Breaking bad news, communication skills, oncology, pre-graduate medical education, indirect questioning

An 88 years old woman was admitted for muscular pain and weakness. She was under a treatment of simvastatin and was recently prescribed clarithromycin for a lung infection. The diagnosis of statin induced rhabdomyolysis by drug interaction was made. The evolution is good with eviction of the statin and aggressive hydratation. This case shows how important it is to know the risks factors and drug interactions predisposing to statin-induced myopathy.

A travelogue to serkamoan theater deals, just like its name implies, with the molding process of one theater opinion. It has been influenced by both Aristotle's poetry and Bertold Brecht's epic theater. Side by side with theater goes dance, whose development the thesis also considers briefly. Of today's theater makers, Päivi Ketonen (playback theater), Juha Hurme and Nigel Charnock (dance theater) have had a definitive impact on the Serkamoan theater view. Their relation to theater and dance is clarified in the interviews conducted by the writer, which can be found in their entirety among the appendices of this thesis. Serkamoan theater has also been influenced by the writer's studies in Portuguese theater school in 2004. In Portugal the writer had a chance to work a theater piece in a new cultural environment where, for example, a novel and paintings were used as inspiration. The strongest legacy from Portugal however was the desire to do theater that can be understood without words. Serkamoan theater considers the combinations of theater and dance in performing arts, agreeing with, among others, Pina Bausch's footsteps. Serkamoan theater ladles its inspiration from theater, dance, human being, human body and movement. It is theater that cannot be understood through intellect but through the heart.

Tutkimuksen tarkoituksena oli selvittää lääkelogistisen palvelumallin tuomaa muutosta Itä-Helsingin palvelutalojen hoitohenkilökunnan työajankäyttöön. Tavoitteena oli selvittää, miten lääkelogistinen palvelumalli muutti hoitohenkilökunnan työajankäyttöä suhteessa lääkehuoltoon, välittömään ja välilliseen hoitotyöhön, muuhun työajan käyttöön sekä toimintakykyä tukevaan työhön. Ennakko-oletuksena oli, että lääkehuollosta vapautuisi muutoksen myötä työaikaa, joka voitaisiin kohdentaa välittömään ja asiakkaiden toimintakykyä tukevaan työhön. Raportointien välillä palvelutaloista oli lääkelogistisen palvelumallin myötä siirtynyt osa asiakkaiden lääkehoitoon liittyvistä tehtävistä apteekeille. Tutkimus toteutettiin survey-tutkimuksena käyttäen kokonaisotantaa Itä-Helsingin neljässä palvelutalossa. Työajankäyttötiedot koottiin hoitohenkilökunnan itseraportoimana marraskuussa 2005 (n=65) ja syyskuussa 2006 (n=70). Työntekijät täyttivät viiden päivän ajan kyselylomaketta, jossa työajankäyttö raportoitiin kymmenen minuutin jaksoissa luokiteltujen koodien mukaisesti. Tulokset kuvattiin prosentteina, keskiarvoina sekä alku- ja lopputilanteiden välisiä eroja vertailemalla Excelin ja SPSS:n avulla. Tulosten tilastollisessa tarkastelussa käytettiin Wilcoxonin merkkitestiä. Tutkimusaikana hoitohenkilökunnan lääkehuoltoon käyttämä työaika väheni. Vapautunut työaika kohdentui välittömän ja toimintakykyä tukevan työn sijaan välilliseen hoitotyöhön. Myös välittömän työajan prosentuaalinen osuus väheni. Muutosta voitaneen osin selittää syksylle 2006 sijoittuneella sähköisen asiakastietojärjestelmän käyttöönotolla. Ammattiryhmittäin suurin muutos tapahtui sairaanhoitajien kohdalla, jossa sekä välillisen työajan että muun työajan käyttö lisääntyivät merkittävästi. Palvelutalojen kesken esiintyi myös eroavaisuuksia käytetyn työajan suhteen. Tämän tutkimuksen perusteella lääkelogististen tehtävien ulkoistaminen on mahdollista sekä järkevää henkilökunnan ajankäytön näkökulmasta. Tiettyjä palveluja, kuten esimerkiksi osa lääkelogistisista tehtävistä, ulkoistamalla voidaan kohdentaa hoitohenkilökunnan työaikaa välittömään hoitotyöhön. Tulevaisuuden haasteena on hoitohenkilökunnan riittävyys ja palvelujen turvaaminen asiakkaille. Työajan kohdentaminen hoitotyön kannalta keskeisiin asioihin on tärkeä haaste työyhteisöille ja edellyttää selkeää johtamista.

The purpose of the thesis was to explore expectations of elderly people on the nurse-client relationship and interaction in home care. The aim is to improve the quality of care to better meet the needs of the clients. A qualitative approach was adopted. Semi-structured theme interviews were used for data collection. The interviews were conducted during spring 2006. Six elderly clients of a private home care company in Southern Finland acted as informants. Content analysis was used as the method of data analysis. The findings suggest that clients expect nurses to provide professional care with loving-kindness. Trust and mutual, active interaction were expected from the nurse-client relationship. Clients considered it important that the nurse recognizes each client's individual needs. The nurse was expected to perform duties efficiently, but in a calm and unrushed manner. A mechanic performance of tasks was considered negative. Humanity was viewed as a crucial element in the nurse-client relationship. Clients expressed their need to be seen as human beings. Seeing beyond the illness was considered important. A smiling nurse was described to be able to alleviate pain and anxiety. Clients hoped to have a close relationship with the nurse. The development of a close relationship was considered to be more likely if the nurse is familiar and genuine. Clients wish the nurses to have a more attending presence. Clients suggested that the work areas of the nurses could be limited so that they would have more time to transfer from one place to another. Clients felt that they would benefit from this as well. The nurses were expected to be more considerate. Clients wished for more information regarding changes that affect their care. They wished to be informed about changes in schedules and plans. Clients hoped for continuity from the nurse-client relationship. Considering the expectations of clients promotes client satisfaction. Home care providers have an opportunity to reflect their own care behaviour on the findings. To better meet the needs of the clients, nurses could apply the concept of loving-kindness in their work, and strive for a more attending presence.

So far, cardiac arrest is still associated with high mortality or severe neurological disability in survivors. At the tissue level, cardiac arrest results into an acute condition of generalized hypoxia. A better understanding of the pathophysiology of ischemia-reperfusion and of the inflammatory response that develops after cardiac arrest could help to design novel therapeutic strategies in the future. It seems unlikely that a single drug, acting as a <magic bullet>, might be able to improve survival or neurological prognosis. Lessons learned from pathophysiological mechanisms rather indicate that combined therapies, involving thrombolysis, neuroprotective agents, antioxidants and anti-inflammatory molecules, together with temperature cooling, might represent helpful strategies to improve patient's outcome after cardiac arrest.

L’objectiu principal d’aquest treball és fer un programa que permeti portar la informació que un treballador autònom li interessa, d’acord amb les seves necessitats. En el nostre cas es tracta d’un centre d’estètica, que té més d’una seu, el qual li interessa portar tota la cartilla de clients, centres associats, proveïdors ... a més de poder fer les factures corresponents als centres associats, poder calcular en el moment que en el treballador li interessi, els ingressos realitzats durant un període de temps determinat i poder portar una agenda actualitzada dels dos centres, on es mostren totes les visites que hi ha en un dia. Per tal de realitzar el programa, s’han portat a terme mitjançant dos aplicacions, i connectant-los en una base de dades. Per una banda tenim una aplicació implementada amb C++, per l’altra, una pàgina web amb PHP, finalment com a sistema gestor de base de dades utilitzem el MySQL Server. El programa fet amb C++, consta de tota la part d’entrada i/o modificacions de dades, en aquesta part només hi pot accedir el treballador autònom, ja que és la única persona que pot fer aquesta feina. En la pàgina web, hi pot accedir qualsevol persona que tingui un nom d’usuari i una contrasenya. A través de la web es pot fer qualsevol tipus de consulta, fer tot el control de les agendes, portar a terme tot el tema de facturació i ingressos, i com a excepció l’entrada de dades de clients, ja que s’ha de poder realitzar en qualsevol moment i lloc. Per acabar, tenim la necessitat de tenir un servidor, aquest ha d’estar format, mínim, per la base de dades. Com que l’aplicació amb C++ i la base de dades han d’estar ubicades al mateix lloc. A més, necessitem un servidor web per tal de tenir la nostra pàgina a la xarxa, per aconseguir això, utilitzem un programa anomenat DynDNS, que es fa servir per a convertir una IP dinàmica en una IP estàtica i d’aquesta manera convertir un ordinador qualsevol amb un servidor web.

It offers descriptions of various landscape designs or planting styles used within or adjacent to Iowa’s highway rights-of-way, as well as various plant profiles. In addition, this guide will help you learn more about the value of plants and their contribution to our environment and society. This publication is written for persons having little or no formal training in botany, and technical terminology has been kept to the minimum necessary to maintain standards of accuracy and conciseness in the descriptions. Plants are known by common names and botanical names. Most people prefer to use common names because they are easier to spell and say. Both have been used in this publication. Botanical names are taken from Latin, Greek or “Latinized” words of other languages. Each plant species has a unique botanical name, consisting of the genus, followed by the species. Some botanical names contain additional words after the species name to designate cultivars or subspecies. Plant species are grouped into families by flower structure. Family names are Latin, so the associated common family names are included in parenthesis. Sources of information for this publication are not cited within the text to save space, avoid repetition and make it more readable. However, all references used are included in the bibliography at the end of this publication.
