A prospective study was conducted from June 2001 to May 2002 at the Burns Unit of Hospital Regional da Asa Norte, Brasília, Brazil. During the period of the study, 252 patients were treated at the Burns Unit, 49 (19.4%) developed clinically and microbiologically proven sepsis. Twenty-six (53.1%) were males and 23 (46.9%) females with a mean age of 22 years (range one to 89 years) and mean burned body surface area of 37.7 ± 18.4% (range 7 to 84%). Forty-three patients had flame burns, five a scald and one an electric burn. These 49 patients had a total of 62 septic episodes. Forty (81.6%) patients had only one and nine (18.4%) had up to three episodes of sepsis. Thirty (61.2%) patients had their first septicemic episode either earlier or by one week postburn. Out of 62 septic episodes, 58 were due to bacteria and four due to Candida sp. The most common bacteria isolated from blood culture were Staphylococcus aureus, coagulase-negative Staphylococcus, Acinetobacter baumannii, Enterobacter cloacae and Klebsiella pneumoniae. Eleven (18.9%) episodes were due to oxacillin resistant Staphylococcus aureus. Acinetobacter baumannii was sensitive to ampicillin/sulbactam in 71.4% and to imipenem in 85.7% of the cases. The primary foci of sepsis were the burn wound in 15 ( 24.2% ) episodes. The most common clinical findings of sepsis in these patients were fever, dyspnea, hypotension and oliguria. The most common laboratory findings of these patients were anemia, leukocytosis, hypoalbuminemia and thrombocytopenia. Twelve (24.5%) patients died. The appropriate knowledge of clinical, epidemiological, laboratorial and microbiological aspects of sepsis in burned patients permits an adequate diagnosis and treatment of this complication.

A manutenção de culturas de Malassezia pachydermatis em micotecas é importante para estudos retrospectivos e prospectivos. O objetivo deste trabalho foi avaliar o comportamento de Malassezia pachydermatis frente a diferentes métodos de conservação de culturas. Para tanto, após o processo de identificação, essa levedura foi estocada, por seis e nove meses, em salina e salina com óleo mineral a 28°C, bem como, em ágar Dixon, ágar Dixon acrescido de glicerol e ágar Dixon acrescido de dimetil-sulfóxido (DMSO) a -20°C. Os meios de Dixon e Dixon acrescido de glicerol foram os métodos mais adequados (p< 0,05) para manter a viabilidade das cepas, em seis e nove meses de estoque. Qualquer dos métodos utilizados foi conveniente para manutenção da positividade na prova da urease em seis meses de estocagem, sendo o ágar Dixon e o ágar Dixon acrescido de glicerol, os melhores (p< 0,05) para nove meses. Portanto, para a recuperação e manutenção das características de Malassezia pachydermatis, recomenda-se o emprego do meio de Dixon ou do meio de Dixon acrescido de glicerol.

Um estudo prospectivo, sobre a sensibilidade antimicrobiana da flora bacteriana em úlceras cutâneas leishmanióticas, foi realizado em pacientes portadores de leishmaniose tegumentar, em Corte de Pedra, Bahia. Foram estudados 84 pacientes, principalmente adolescentes e adultos dedicados à lavoura, apresentando lesão cutânea única. Staphylococcus aureus predominou (83%) nas culturas, sendo sensível à maioria dos antibióticos testados. Flora bacteriana mista esteve presente na úlcera em 37 (44,1%) pacientes. Entre as bactérias Gram-negativas isoladas, foram mais freqüentes Enterobacter sp (13,1%), Proteus sp (8,3%), Pseudomonas aeruginosa (7,1%) e Klebsiella sp (7,1%), sendo sensíveis principalmente à ciprofloxacina, aminoglicosídeos, cefalosporinas de terceira geração e carbapenêmicos.

A case of primary amoebic meningoencephalitis produced by Naegleria fowleri was diagnosed in the Independencia county of Anzoategui State, Venezuela. This case motivated the realization of the present epidemiological study with the aim of identifying free-living amoebae in this area. Representative water samples were taken and physicochemical and microbiologic analyses were carried out. Trophozoites and cysts of Naegleria spp, were detected in 44.4% (n=4). An excellent concordance was found among the observations of free-living amoebae in smears and those of monoxenic cultures in non nourishing agar with Klebsiella pneumoniae (Kappa=1; p= 0.003). A variable load of aerobic mesophils was obtained. Moulds and yeast averages presented 3.0 CFU/ml (SD± 2.0) and 102.9 CFU/ml (SD± 32.2), respectively. One hundred per cent of the samples presented a most probable number of total and fecal coliforms of 240,000 NMP/100mL. Naegleria spp was present in waters of the Independence county of Anzoategui state, which constitutes a risk for people that use these sources.

O ecossistema marinho é o habitat natural de bactérias como Vibrio parahaemolyticus, um importante patógeno causador de gastrenterite humana associada ao consumo de alimentos marinhos. Na presente investigação, foi avaliada a presença de V. parahaemolyticus a partir de 86 amostras de mexilhões in natura e pré-cozidos. Vibrio parahaemolyticus foi isolado a partir de 11,6% dos mexilhões in natura e pré-cozidos avaliados. Todas as cepas avaliadas demonstraram-se urease positivas e 28,5% Kanagawa positivas sugerindo um potencial patogênico para o homem. Houve a predominância do sorotipo O10:K52 e a identificação da cepa emergente O3:K6. Esses resultados apontam para a relevância epidemiológica de V. parahaemolyticus em casos de gastrenterite humana após consumo de mexilhões sem cozimento adequado (100ºC/15min). Além disso, é importante alertar as autoridades de Vigilância Sanitária no Brasil quanto a sua presença na cadeia alimentar e seus riscos para a Saúde Pública.

A infecção do trato urinário é uma das afecções mais comuns da clínica médica, sendo mandatório o conhecimento epidemiológico da mesma e do perfil de sensibilidade dos agentes etiológicos. O estudo teve como objetivo identificar os agentes etiológicos mais freqüentes e o perfil de sensibilidade aos antimicrobianos das bactérias isoladas de uroculturas de pacientes ambulatoriais atendidos no Hospital Universitário de Brasília no período de 2001 a 2005. Foram analisadas 2.433 uroculturas positivas realizadas no laboratório de microbiologia do Hospital Universitário de Brasília. A Escherichia coli foi a bactéria mais isolada (62,4%), seguida de Klebsiella pneumoniae (6,8%) e Proteus mirabillis (4,7%). A Escherichia coli apresentou maior sensibilidade à amicacina (98,6%), gentamicina (96,2%), nitrofurantoína (96,3%), e às quinolonas ciprofloxacina (90,9%) e norfloxacina (89,8%), com baixa sensibilidade ao sulfametoxazol-trimetoprima (50,6%). As outras bactérias apresentaram similar padrão de sensibilidade. Em conclusão, a Escherichia coli foi a bactéria mais isolada, sendo altamente sensível aos amiglicosídeos, nitrofurantoína e quinolonas.

Foi realizado um estudo retrospectivo, baseado no banco de dados eletrônico de um hospital universitário, com objetivo de investigar a prevalência dos germes causadores e suas suscetibilidades aos antibióticos em adultos (idade >18 anos), com infecção do trato urinário atendidos ambulatorialmente. Foram identificados 957 exames de urocultura positiva no período entre janeiro de 2000 e dezembro de 2004. Escherichia coli, Proteus mirabillis e Klebsiella sp foram três principais bactérias causadoras. Sulfametoxazol-trimetropim apresentou a maior (46,9%) prevalência de resistência bacteriana seguida por cefalotina (46,7%), ácido nalidíxico (27,6%) e nitrofurantoína (22,3%). Durante o período estudado, o ácido nalidíxico apresentou um aumento anual de 5,9% na taxa de resistência bacteriana (p= 0,02). Ciprofloxacina mostrou também a tendência de aumento, com um crescimento anual de 3,3% (p= 0,07). Este estudo demonstrou que os antibióticos amplamente recomendados no tratamento empírico da infecção do trato urinário em adultos apresentaram altas taxas de resistência bacteriana na população estudada.

A etiologia do processo diarréico na AIDS pode ser causada por vírus, bactérias, fungos, protozoários e helmintos, assim como pelo próprio HIV. Este trabalho avaliou enteropatogenos relacionados à diarréia em pacientes HIV que fazem uso de terapia anti-retroviral. Os métodos parasitológicos utilizados foram Faust, Hoffmann e Kinyoun. O isolamento e cultura dos fungos foram realizados conforme metodologia recomendada por NCCLS M27-A standard. A identificação das espécies de leveduras foi realizada através da reação em cadeia da polimerase. O isolamento de bactérias, foi feito em agar Mac Conkey e agar SS, a identificação das espécies através do Enterokit B (Probac do Brasil) e métodos bioquímicos. Foram avaliados 49 pacientes, 44,9% apresentaram enteroparasitas, 48,1% Candida sp com 61,5% Candida albicans, 7,6% Candida sp e 30,7% Candida não- albicans. Foram isoladas bactérias de 72% dos pacientes, 49% Escherichia coli, 13% Salmonella parathyphi, Klebsiella sp ou Proteus e 6% Citrobacter freundii ou Yersinia sp. Houve alta prevalência de Candida sp nos pacientes HIV com diarréia e foram isoladas espécies não albicans cuja presença pode ser entendida como cúmplice ou causa da infecção.

INTRODUÇÃO: O Cryptococcus neoformans é uma levedura capsulada, agente etiológico da criptococose em humanos e animais, encontrado em fontes ambientais, incluindo excretas de pombos, é uma importante causa de mortalidade em indivíduos imunodeprimidos em todo o mundo. MÉTODOS: Com o objetivo de verificar a ocorrência do Cryptococcus neoformans, em excretas de pombos, na Cidade de Pelotas, pesquisou-se 70 ambientes, incluindo prédios, praças e locais ao ar livre, da Cidade de Pelotas, RS. Após a coleta, os excrementos foram adicionados de salina com cloranfenicol, homogeneizados, semeados em ágar Sabouraud com cloranfenicol e ágar Níger e incubados a 32ºC. Para identificação, realizou-se exame direto, prova da fenoloxidase, urease, assimilação de carboidratos e cultura em meio CGB. RESULTADOS: Dos locais estudados (nº =70), em 26 (37,1%) havia excretas de pombos. Estes lugares foram representados por prédios históricos (nº =8), torre de igreja (nº =1), engenhos e armazéns de arroz (nº =7), praça (nº =1) e locais ao ar livre (nº =9), o isolamento de Cryptococcus neoformans ocorreu em 26,9% (nº =7/26), destes locais. CONCLUSÕES: O estudo chama a atenção, para o isolamento do fungo em áreas urbanas, que apresentavam grande acúmulo de excretas, indicando um risco para a saúde pública, especialmente para indivíduos imunocomprometidos.

INTRODUCTION: Extended spectrum β-lactamases (ESBLs) are enzymes that degrade β-lactam antibiotics and have been reported to be an important cause of nosocomial infection in worldwide. METHODS: During 2009, 659 enterobacteria strains were isolated from different clinical specimens and tested for ESBL production. The disk approximation test, combined disk method and addition of clavulanic acid were used for phenotypic detection of the ESBL-producing strains and PCR for detection of the blaTEM and blaCTX-M genes. RESULTS: Among the isolates, 125 were ESBL producers. The blaCTX-M and blaTEM genes were detected in 90.4% and 75% of the strains, respectively. Most strains were isolated from urine. Klebsiella pneumoniae was the most prevalent organism. Microorganisms presented high resistance to the antibiotics. CONCLUSIONS: These results support the need for extending ESBL detection methods to different pathogens of the Enterobacteriaceae family because these methods are only currently standardized by the CLSI for Escherichia coli, Klebsiella pneumoniae, Klebsiella oxytoca and Proteus mirabilis. Carbapenems were the antibiotic class of choice for the treatment of infections caused by ESBL-producing Enterobacteriaceae.

INTRODUCTION: Enterobacteriaceae strains are a leading cause of bloodstream infections (BSI). The aim of this study is to assess differences in clinical outcomes of patients with BSI caused by Enterobacteriaceae strains before and after introduction of an automated microbiologic system by the microbiology laboratory. METHODS: We conducted a retrospective cohort study aimed to evaluate the impact of the introduction of an automated microbiologic system (Phoenix(tm) automated microbiology system, Becton, Dickinson and Company (BD) - Diagnostic Systems, Sparks, MD, USA) on the outcomes of BSIs caused by Enterobacteriaceae strains. The study was undertaken at Hospital São Paulo, a 750-bed teaching hospital in São Paulo, Brazil. Patients with BSI caused by Enterobacteriaceae strains before the introduction of the automated system were compared with patients with BSI caused by the same pathogens after the introduction of the automated system with regard to treatment adequacy, clinical cure/improvement and 14- and 28-day mortality rates. RESULTS: We evaluated 90 and 106 patients in the non-automated and automated testing periods, respectively. The most prevalent species in both periods were Klebsiella spp. and Proteus spp. Clinical cure/improvement occurred in 70% and 67.9% in non-automated and automated period, respectively (p=0.75). 14-day mortality rates were 22.2% and 30% (p=0.94) and 28-day mortality rates were 24.5% and 40.5% (p= 0.12). There were no significant differences between the two testing periods with regard to treatment adequacy, clinical cure/improvement and 14- and 28-day mortality rates. CONCLUSIONS: Introduction of the BD Phoenix(tm) automated microbiology system did not impact the clinical outcomes of BSIs caused by Enterobacteriaceae strains in our setting.

Introduction In this study, the clinical features, underlying diseases and clinical outcomes of patients with cryptococcosis were investigated. In addition, a molecular analysis of the Cryptococcus neoformans species complex isolated from these patients was performed. Methods A prospective study of 62 cases of patients with cryptococcal infection was conducted at the Hospital de Doenças Tropicais de Goiás Dr. Anuar Auad from 2009-2010. Cryptococcal meningitis cases were diagnosed by direct examination and cerebrospinal fluid (CSF) sample culture. The profiling of these patients was assessed. The CSF samples were submitted to India ink preparation and cultured on Sabouraud dextrose agar, and C. neoformans was identified by the production of urease, a positive phenoloxidase test and assimilation of carbohydrates. C. neoformans and C. gattii isolates were distinguished by growth on L-canavanine-glycine-bromothymol blue medium, and molecular analysis was conducted via PCR fingerprinting reactions using M13 and (GACA)4 primers. Results From the 62 patients with cryptococcosis, 71 isolates of CSF were obtained; 67 (94.4%) isolates were identified as C. neoformans var. grubii/VNI, and 4 (5.6%) were identified as C. gattii/VGII. Of these patients, 53 had an HIV diagnosis. The incidence of cryptococcosis was higher among patients 20-40 years of age, with 74.2% of the cases reported in males. Cryptococcus-related mortality was noted in 48.4% of the patients, and the symptoms were altered sensorium, headache, fever and stiff neck. Conclusions The high morbidity and mortality observed among patients with cryptococcosis demonstrate the importance of obtaining information regarding the epidemiological profile and clinical course of the disease in the State of Goiás, Brazil.

Lithiasic cholecystitis is classically associated with the presence of enterobacteria, such as Escherichia coli, Enterococcus, Klebsiella, and Enterobacter, in the gallbladder. Cholecystitis associated with fungal infections is a rare event related to underlying conditions such as diabetes mellitus, steroid use, and broad-spectrum antibiotic use for prolonged periods, as well as pancreatitis and surgery of the digestive tract. Here, we present the first reported case of a gallbladder infection caused by Candida famata.

INTRODUCTION : Infections caused by Klebsiella pneumoniae carbapenemase (KPC)-producing isolates pose a major worldwide public health problem today. METHODS : A carbapenem-resistant Proteus mirabilis clinical isolate was investigated for plasmid profiles and the occurrence of β-lactamase genes. RESULTS : The isolate exhibited resistance to ertapenem and imipenem and was susceptible to meropenem, polymyxin, and tigecycline. Five plasmids were identified in this isolate. DNA sequencing analysis revealed the presence of bla KPC-2 and bla TEM-1 genes. An additional PCR using plasmid DNA confirmed that bla KPC-2 was present in one of these plasmids. Conclusions: We report the detection of bla KPC-2 in P. mirabilis in Brazil for the first time. This finding highlights the continuous transfer of bla KPC between bacterial genera, which presents a serious challenge to the prevention of infection by multidrug-resistant bacteria.

INTRODUCTION: Epidemiological data on the prevalence of extended-spectrum β-lactamases (ESBLs) are scarce in Brazil despite the fact that these data are essential for empirical treatment and control measures. The objective of this study was to evaluate the prevalence of different ESBLs by type and distribution in a tertiary hospital in southern Brazil. METHODS: We evaluated 1,827 enterobacterial isolates between August 2003 and March 2008 isolated from patients at a tertiary hospital. Samples were identified using a Vitek automated system and were confirmed by biochemical testing. The identified ESBL strains were characterized by phenotypic methods, polymerase chain reaction (PCR), and sequencing. Genetic similarities were evaluated by pulsed-field gel electrophoresis. RESULTS: It was 390 (21.3%) ESBL-producing strains, which expressed the ESBLs CTX-M (292), SHV (84), CTX and SHV (10), TEM (2), and PER (2). CONCLUSIONS: The prevalence of ESBL-expressing strains was high, especially in Klebsiella pneumoniae and Enterobacter spp. CTX-M was the predominant type of ESBL observed, and its genetic variability indicates a polyclonal distribution.
