Esta pesquisa buscou conhecer o trabalho desenvolvido no Centro de Cuidados a Dependentes Químicos - CCDQ, localizado no município de Belém, estado do Pará, único serviço público no Brasil dentro do Sistema Único de Saúde destinado a oferecer tratamento específico para dependência química de álcool e outras drogas em regime de internação em longa duração, seguida de acompanhamento ambulatorial. A escassez de pesquisas nesta área no contexto local, fomenta a importância da discussão sobre o uso de drogas e as formas de tratamento existentes. Essa nova possibilidade de tratamento surgida das demandas sociais do município, através de dados compilados por diversas pesquisas, que levaram o governo estadual a planejar e implementar o centro, tinha por objetivo respeitar a voluntariedade do sujeito, incentivar a construção de um ambiente humanizado, pautada na lógica da reforma psiquiátrica, no que concerne à participação da família e práticas ressocializantes. Examinamos algumas contribuições da gestalt-terapia, Psicologia de base fenomenológica – existencial, que orienta minha linha de pesquisa e da Terapia Ocupacional, minha área de atuação, correlacionando-as como ciências e intervenções que dialogam no campo da interdisciplinaridade, tendo em vista o tratamento da drogadicção. A coleta de dados se deu através de questionários com questões abertas, respondidos por representantes da coordenação do centro, da equipe psicossocial e usuários do serviço. A perspectiva fenomenológica-existencial gestáltica configurou a fundamentação teórica na condução desse estudo, em que procuramos colaborar com este campo de investigação, desvelando alguns sentidos contidos nos diferentes discursos dos atores que trabalham ou se tratam no CCDQ e que colaboraram para configurar este relatório. Observamos a partir desses discursos, transformados em unidades de significação, que tanto o trabalho voltado às famílias, quanto as atividades ressocializantes estão deficitárias, sendo apontada também a necessidade de capacitação da equipe e a ausência de suporte da rede de serviços de saúde dentre outras questões. Pela especificidade dos resultados, pretendemos contribuir com a fundamentação de estratégias locais de ações preventivas e terapêuticas em drogadicção, além de possibilitar o conhecimento do que já vem sendo construído neste campo de atuação, tendo em vista ainda que novas ações terapêuticas podem ser construídas a partir da avaliação dos serviços já existentes prestados à população, bem como suscitar reflexões sobre as políticas, as ações e os seres humanos.

Na segunda metade do século passado iniciava-se um movimento revolucionário no cenário mundial da saúde mental: a Reforma Psiquiátrica. No mesmo momento nascia a proposta das Comunidades Terapêuticas, que mais tarde se tornaria um modelo consagrado de atendimento para a dependência do álcool e outras drogas. Por outro lado, com o alarmante crescimento deste problema no Brasil, assim como pela ausência de políticas públicas que dessem conta do problema, houve uma indiscriminada proliferação de locais de internação para dependentes químicos que, mesmo se autodenominando como Comunidades Terapêuticas, em nada se assemelham ao modelo inicial proposto. Estes locais apresentam práticas desumanas e iatrogênicas, muito semelhantes às criticadas pelo movimento da Reforma Psiquiátrica, o que tem provocado o descrédito para com o modelo das Comunidades Terapêuticas. Este artigo tem como objetivo demonstrar, através de pesquisa bibliográfica, como as bases conceituais e metodológicas da Reforma Psiquiátrica se assemelham profundamente as do movimento das Comunidades Terapêuticas, tendo surgido na mesma época e pelo mesmo motivo, e como a falta de regulamentação dos locais de internação para dependentes químicos no Brasil tem contribuído com o atual descrédito deste modelo.

Introduction: The use of stabilizer bracelet is a frequent treatment for movement disorders to strengthen muscles and adjust coordination. Still questions remain regarding the benefi t of using loads to decrease involuntary movements and the best load and placement. Objective: To measure the infl uence of the stabilizer bracelet on the kinematics and spatiotemporal parameters in planar movements performed by the upper limb. Method: One child, who has the spastic diplegy type of cerebral palsy with choreoathetoid component, and a control child without cerebral palsy, both female and 7 years old, were subjected to analysis of movements in relation to displacement, velocity, linear acceleration, and the calculation of mean square error (MSE) with and without use of stabilizer bracelet with loads of 25, 50, and 75% of the supported maximum load. Results: After comparing data between subjects, a difference was found between patient and control in all situations and variables. An inter-individual comparison using 25% of the maximum load showed the smallest difference with the NDE. Discussion and Conclusion: This therapeutic option is low cost, easy to apply, and does not signifi cantly interfere in the aesthetic of the individual. Therefore, physiotherapists may prescribe this for activities that require greater control of the upper limb because for the case studied the upper limb movement was more effi cient with the use of the stabilizer bracelet.

The syndrome of Gilles de la Tourette (ST) can cause marked distress in several areas of functioning. The literature indicates that children with TS may present diffi culties in relationships with teachers and peers. This case study exploration aimed to discover and describe the perceptions of parents and teachers of child with TS enrolled in regular school, regarding the child-school relationships. Interviews were conducted with parents of a 8 years-old child, diagnosed with ST and three educators. Participant observation of everyday children’s school was also performed. The set of reports reveals doubts on the part of respondents to identify involuntary behaviors of the child or to identify intentional actions. It was observed that there is still no connection between parents and educators in meeting the needs of the child. It is assumed that intervention strategies in a multidisciplinary perspective could help parents, educators and health care professionals act in a coordinated way in order to improve the school and social interaction of the child.

Background: Urinary incontinence (UI) is defined as any involuntary loss of urine that can influence the quality of life, personal hygiene and social interaction. The types of UI that most affect women are stress urinary incontinence, urge incontinence and mixed urinary incontinence. There are several risk factors that result in specific treatments. We aimed to investigate the evolution of female urinary continence after physical therapy intervention and its associated factors. Method: A retrospective cross-sectional study was conducted with 71 participants who were discharged from physiotherapy sector from August 2006 to April 2012 and met the inclusion criteria. Results: Among the studied variables, the number of sessions and completion of home pelvic floor exercises showed a significant association. The urinary continence appeared in 43.7% of the cases, and factors, performance of home exercises, and number of sessions showed a significant association. Conclusion: The number of sessions and completion of home pelvic floor exercises showed a significant relationship with each other.

This is an exploratory descriptive study with a qualitative approach. It aimed at apprehending what users undergoing treatment at a mental health service know about their rights. A semistructured interview was used as a data collection strategy. Five users were interviewed. Thematic content analysis was used for data analysis, and three categories emerged: process of falling ill, experiences of treatment and users’ rights. The users reported voluntary and involuntary hospitalization experiences and that, after treatment commencement at the psychosocial care center, they perceived significant changes in mental health care. They reported that they did not know the mental health legislation, but they knew about the rights stemming from experiencing mental disorder. For users to be able to recognize and take advantage of their citizens’ rights, awareness must be developed by health care professionals, which also includes their academic education

Objective: To test the hypothesis that the extraocular muscles (EOMs) of patients with infantile nystagmus have muscular and innervational adaptations that may have a role in the involuntary oscillations of the eyes. Methods: Specimens of EOMs from 10 patients with infantile nystagmus and postmortem specimens from 10 control subjects were prepared for histologic examination. The following variables were quantified: mean myofiber cross-sectional area, myofiber central nucleation, myelinated nerve density, nerve fiber density, and neuromuscular junction density. Results: In contrast to control EOMs, infantile nystagmus EOMs had significantly more centrally nucleated myofibers, consistent with cycles of degeneration and regeneration. The EOMs of patients with nystagmus also had a greater degree of heterogeneity in myofiber size than did those of controls, with no difference in mean myofiber cross-sectional area. Mean myelinated nerve density, nerve fiber density, and neuromuscular junction density were also significantly decreased in infantile nystagmus EOMs. Conclusions: The EOMs of patients with infantile nystagmus displayed a distinct hypoinnervated phenotype. This represents the first quantification of changes in central nucleation and myofiber size heterogeneity, as well as decreased myelinated nerve, nerve fiber, and neuromuscular junction density. These results suggest that deficits in motor innervation are a potential basis for the primary loss of motor control.

Objectives. Adductor spasmodic dysphonia (ADSD) is a focal laryngeal dystonia, which compromises greatly the quality of life of the patients involved. It is a severe vocal disorder characterized by spasms of laryngeal muscles during speech, producing phonatory breaks, forced, strained and strangled voice. Its symptoms result from involuntary and intermittent contractions of thyroarytenoid muscle during speech, which causes vocal fold to strain, pressing each vocal fold against the other and increasing glottic resistance. Botulinum toxin injection remains the gold-standard treatment. However, as injections should be repeated periodically leading to voice quality instability, a more definitive procedure would be desirable. In this pilot study we report the long-term vocal quality results of endoscopic laser thyroarytenoid myoneurectomy. Study Design. Prospective study. Methods. Surgery was performed in 15 patients (11 females and four males), aged between 29 and 73 years, diagnosed with ADSD. Voice Handicap Index (VHI) was obtained before and after surgery (median 31 months postoperatively). Results. A significant improvement in VHI was observed after surgery, as compared with baseline values (P = 0.001). The median and interquartile range for preoperative VHI was 99 and 13, respectively and 24 and 42, for postoperative VHI. Subjective improvement of voice as assessed by the patients showed median improvement of 80%. Conclusions. Because long-term follow-up showed significant improvement of voice quality, this innovative surgical technique seems a satisfactory alternative treatment of ADSD patients who seek a definite improvement of their condition.
