Incidentes com medicamentos geram problemas aos pacientes e custos adicionais ao sistema de saúde. A variedade de termos utilizada para comunicá-los propicia divergências nos resultados de pesquisas e confundem notificadores. Objetivou-se revisar os termos utilizados para descrever estes incidentes confrontando-os com as conceituações/definições oficiais disponíveis. Pesquisaram-se as bases PubMed, MEDLINE, IPA e LILACS para selecionar estudos publicados entre janeiro de 1990 e dezembro de 2005. Selecionaram-se 33 publicações. Verificou-se que a terminologia supranacional recomendada para descrever incidentes com medicamentos é insuficiente, mas que há consenso de uso das expressões em função do gênero do incidente. O termo Reação Adversa a Medicamento é mais utilizado quando não se verifica intencionalidade. A expressão Evento Adverso a Medicamento foi mais usada quando se descreviam incidentes durante a hospitalização; e Problema Relacionado a Medicamento foi mais utilizada em estudos que avaliaram atenção/cuidados farmacêuticos (uso/falta do medicamento). Ainda assim, a linha divisória entre essas três categorias não é clara e simples. Futuros estudos das relações entre as categorias e investigações multidisciplinares sobre erro humano podem subsidiar a proposição de novas conceituações

Scorpion stings are a public health problem in Brazil, with most incidents involving the species Tityus serrulatus. Some T serrulatus toxins may act as immunogens for the production of a specific anti-venom, but many of the component toxins remain poorly characterized. Here, we describe the immunological characteristics of the toxin Ts1 (also known as TsVII and Ts-gamma) and evaluate production of neutralizing antibodies against the crude venom of T serrulatus. Recombinant Ts1 with one copy (Ts1((1))) or two copies in tandem (Ts1((2))) was expressed in BL21 (DE3) cells. Rabbits and mice were immunized with the recombinant proteins (inclusion bodies) and then tested for production of neutralizing antibodies. Neutralization assays showed that anti-Ts1((1)) and anti-Ts1((2)) protected animals challenged with T serrulatus crude venom and native Ts1 Thus, Ts1 could be used in a mixed ""cocktail"" of immunogens for T serrulatus anti-venom production. This paper discusses a multi-layer feedforward (MLF) neural network incident detection model that was developed and evaluated using field data. In contrast to published neural network incident detection models which relied on simulated or limited field data for model development and testing, the model described in this paper was trained and tested on a real-world data set of 100 incidents. The model uses speed, flow and occupancy data measured at dual stations, averaged across all lanes and only from time interval t. The off-line performance of the model is reported under both incident and non-incident conditions. The incident detection performance of the model is reported based on a validation-test data set of 40 incidents that were independent of the 60 incidents used for training. The false alarm rates of the model are evaluated based on non-incident data that were collected from a freeway section which was video-taped for a period of 33 days. A comparative evaluation between the neural network model and the incident detection model in operation on Melbourne's freeways is also presented. The results of the comparative performance evaluation clearly demonstrate the substantial improvement in incident detection performance obtained by the neural network model. The paper also presents additional results that demonstrate how improvements in model performance can be achieved using variable decision thresholds. Finally, the model's fault-tolerance under conditions of corrupt or missing data is investigated and the impact of loop detector failure/malfunction on the performance of the trained model is evaluated and discussed. The results presented in this paper provide a comprehensive evaluation of the developed model and confirm that neural network models can provide fast and reliable incident detection on freeways. Instantaneous outbursts in underground coal mines have occurred in at least 16 countries, involving both methane (CH4) and carbon dioxide (CO2). The precise mechanisms of an instantaneous outburst are still unresolved but must consider the effects of stress, gas content and physico-mechanical properties of the coal. Other factors such as mining methods (e.g., development heading into the coal seam) and geological features (e.g., coal seam disruptions from faulting) can combine to exacerbate the problem. Prediction techniques continue to be unreliable and unexpected outburst incidents resulting in fatalities are a major concern for underground coal operations. Gas content thresholds of 9 m(3)/t for CH4 and 6 m(3)/t for CO2 are used in the Sydney Basin, to indicate outburst-prone conditions, but are reviewed on an individual mine basis and in mixed as situations. Data on the sorption behaviour of Bowen Basin coals from Australia have provided an explanation for the conflicting results obtained by coal face desorption indices used for outburst-proneness assessment. A key factor appears to be different desorption rates displayed by banded coals, which is supported by both laboratory and mine-site investigations. Dull coal bands with high fusinite and semifusinite contents tend to display rapid desorption from solid coal, for a given pressure drop. The opposite is true for bright coal bands with high vitrinite contents and dull coal bands with high inertodetrinite contents. Consequently, when face samples of dull, fusinite-or semifusinite-rich coal of small particle size are taken for desorption testing, much gas has already escaped and low readings result. The converse applies for samples taken from coal bands with high vitrinite and/or inertodetrinite contents. In terms of outburst potential, it is the bright, vitrinite-rich and the dull, inertodetrinite-rich sections of a coal seam that appear to be more outburst-prone. This is due to the ability of the solid coal to retain gas, even after pressure reduction, creating a gas content gradient across the coal face sufficient to initiate an outburst. This study provided information about how individual workers perceive, describe and interpret episodes of problematic communication. Sixteen full-time workers (5 males, 11 females) were interviewed in depth about specific incidents of problematic communication within their workplace. Their descriptions of the attributed causes of the incidents were coded using a categorisation scheme developed from Coupland, Wieman, and Giles' (1991) model of sources of problematic communication. Communication problems were most commonly attributed to individual deficiency and group membership, although there were differences depending on the direction of communication. The most negative attributions (to personality flaws, to lack of skills, and to negative stereotypes of the outgroup) were most commonly applied by individuals to their supervisors, whilst attributions applied to co-workers and subordinates tended to be less negative, or even positive in some instances (where individuals attributed the fault to themselves). Overall, results highlighted distinctions between the perceptions of communication problems with supervisors and with subordinates, and are interpreted with reference to social identity theory.

Workplace injuries at the University of Queensland Dental School during the period 1992-1994 were assessed to determine their incidence, and the associated indirect costs, causal factors, and appropriate preventive strategies. Overall, dental chairside assistants experienced a higher incidence of injuries than students both on a per worker and per time basis. Of the injuries with a low risk of cross-infection, burns and scalds from sterilizing equipment, and eye injuries in laboratories were the most common. This emphasizes the importance of wearing appropriate protective equipment in areas outside the treatment zone, and the need for signage and education. Common causes of sharps injuries were burs left in handpieces, two-handed needle recapping, and cleaning of probes in the sterilizing room. Changes to techniques and equipment would prevent such incidents. A range of factors which contribute to the calculation of indirect costs following injuries in the dental workplace are identified.

Objective: The Traffic Engineering Company of the City of Sao Paulo (Brazil) observed a decrease in productivity, and an increase in sick leave, accidents and psychological distress among their parking inspection agents. To document this situation, qualitative research was undertaken to obtain an in-depth comprehension of work activity. Participants: Workers, managers and health and safety professionals contributed to the documentation of the problem and to the proposal of possible solutions. Methods: Ergonomic work analysis focusing on real work activity, as well as interviews with individual or groups of stakeholders, were conducted. Results: This research revealed that political-economic factors gradually contributed to: 1) an increasing work load; 2) growing fatigue throughout the day, increasing the workers` vulnerability to incidents and accidents and their tendency to react inappropriately to violence experienced on the street; and 3) excessive individual responsibility to manage dangerous situations. Conclusions: Recommendations to ameliorate the situation are proposed. These suggestions are discussed in terms of feasibility given the impact of macro social factors upon micro work activity, and the associated potential expansion of the ergonomist`s role.

Police call data for domestic violence incidents in the city of Brisbane were used to further explore the locational disadvantage thesis. It was hypothesised that the supposed additional burdens and stresses on disadvantaged families living in the outer suburbs may be reflected in significantly higher rates of reported domestic violence. Using an index of relative socioeconomic disadvantage and employing Analysis of Variance (ANOVA), this research shows that significantly higher rates of reported domestic violence occur in the inner suburbs relative to the middle or outer suburbs of Brisbane. This finding adds further doubt as to the magnitude of locational disadvantage impacts on outer suburban low income family households.

Hypersensitivity to external stimuli, progressing in some animals to manic behaviour, occurred in a cattle herd that grazed a crop of field peas (Pisum sativum var arvense) in the pre-flowering stage. Haematological and biochemical analyses eliminated hypomagnesaemia and ketosis as diagnoses. Other than two steers euthanased due to injuries sustained during manic episodes, all affected animals survived, recovering over 3 days when moved to alternative pasture. No necropsies were conducted. No microbial pathogens or endophytes were found on or in the plants. A previously reported incident in Victoria in 1987 in cattle grazing peas appeared to be of a similar nature. Environmental factors leading to these incidents were not clearly identified.

The six-month period from July to December 2001 was an extraordinary phase in Australia’s political history. Until 26 August, when 460 asylum seekers were rescued from their sinking vessel off the Western Australian coast, there was nothing to suggest that politics in Australia would be any different from the usual round of policy pronouncements, routine attacks by the opposition, and the occasional headlinegrabbing scandal. It seemed, as the Australian’s international editor Paul Kelly wrote, that tax would dominate the 2001 federal election campaign (21-22 July 2001). Once the asylum seekers had been rescued by the Norwegian freighter Tampa and refused entry to Australia, however, tax declined in significance. The ramifications of the Tampa incident and its aftermath were still engaging the Australian polity when, on 11 September, terrorists hijacked four passenger jets in the United States and flew two of them into the World Trade Centre in New York, one into the Pentagon, and crashed another in Pennsylvania, killing, it was estimated, more than 6000 people. Both incidents changed Australian politics. The extent and significance of the changes remain to be fully seen, but one short-term effect was to boost significantly the government’s chances of re-election.

Reassuring visitors about their health and safety is particularly important for tourist destinations since the terrorist attacks of 11 September 2001. This study examined the deaths of 1513 overseas visitors to Australia over a four-year period, and found that most deaths (76%) were due to natural causes.Among the accidental deaths, the main causes were motor vehicle crashes and water-related incidents. The study findings support a widely held view that Australia is a safe destination for overseas visitors. It also provides a safety benchmark for other tourist destinations.

Acts of Corporate Social Responsibility are more often than not portrayed as success-story narratives. A quasi-ethnographic study in Senegal shatters the underlying assumptions of these accounts. First a computer donation from a Northern country is described with all the usual incidents and related vocabulary. Later, during a visit to a Senegalese rubbish dump, the story starts to falter, as countless questions arise about what is actually going on there, and how we can know and represent it (both as a portrayal and as a voice). A tipping point in an interview serves to reveal the ambiguities of the position adopted and the difficulties of expressing oneself in a different language. The CSR language is described as a kind of monolingualism, which employs a particular vocabulary and grammar for what is said or thought. This is the monolingualism of the other. Some of the implications of this monolingualism are examined, and this leads to the conclusion that there is a need to take measures with regard to the CSR language and that more than one language is needed to speak about CSR.

As wind power generation undergoes rapid growth, lightning and overvoltage incidents involving wind power plants have come to be regarded as a serious problem. Firstly, lightning location systems are discussed, as well as important parameters regarding lightning protection. Also, this paper presents a case study, based on a wind turbine with an interconnecting transformer, for the study of adequate lightning and overvoltage protection measures. The electromagnetic transients circuit under study is described, and computational results are presented.

O estudo que ora se apresenta avalia a percepção de risco por parte dos profissionais de educação em jardins-de-infância do concelho da Maia. A avaliação da percepção foi realizada através de inquérito por questionário. No questionário são abordados temas como a formação que os inquiridos obtiveram, a tipologia dos acidentes ocorridos com as crianças, as condições estruturais do jardim-de-infância, os recreios, materiais e equipamentos, riscos eléctricos e organização de emergência. O grupo referente à segurança, higiene e saúde nos jardins-de-infância, que se encontra no questionário, foi estruturado com base na legislação e documentação de referência. A amostra recolhida diz respeito a 8 jardins-de-infância, tendo sido respondido por 34 educadoras de infância e 34 auxiliares de acção educativa. Constituíram objectivos deste estudo, verificar a percepção de risco por parte das educadoras de infância e das auxiliares de acção educativa; verificar diferenças de percepção de risco entre as educadoras de infância e as auxiliares de acção educativa sobre os diferentes temas; analisar a influência da formação recebida, idade, experiência profissional e categoria profissional na percepção do risco; Identificar qual o tema em que a amostra demonstra menos domínio. Após a análise dos resultados verificou-se que percepção das educadoras de infância e auxiliares de acção educativa era elevada, não se verificando diferenças significativamente entre as duas categorias profissionais, nem entre quem teve formação com não teve.
