The aim of the present study was to evaluate the efficacy of QMiX, SmearClear, and 17% EDTA for the debris and smear layer removal from the root canal and its effects on the push-out bond strength of an epoxy-based sealer by scanning electron microscopy (SEM). Forty extracted human canines (n = 10) were assigned to the following final rinse protocols: G1-distilled water (control), G2–17% EDTA, G3-SmearClear, and G4-QMiX. The specimens were submitted to a SEM analysis to evaluate the presence of debris and smear layer, respectively, in the apical or cervical segments. In sequence, forty extracted human maxillary canines with the root canals instrumented were divided into four groups (n = 10) similar to the SEM analysis study. After the filling with AH Plus, the roots were transversally sectioned to obtain dentinal slices. The specimens were submitted to a push-out bond strength test using an electromechanical testing machine. The statistical analysis for the SEM and push-out bond strength studies were performed using the Kruskal–Wallis and Dunn tests (α = 5%). There was no difference among the G2, G3, and G4 efficacy in removing the debris and smear layer (P > 0.05). The efficacy of these groups was superior to the control group. The push-out bond strength values of G2, G3, and G4 were superior to the control group. The ability to remove the debris and smear layer by SmearClear and QMiX was as effective as the 17% EDTA. The final rinse with these solutions promoted similar push-out bond strength values.

This study sought to evaluate the efficacy of passive ultrasonic irrigation (PUI) on removing the smear layer and debris from root dentin using scanning electron microscopy (SEM). Twenty-five bovine incisors were manually prepared and divided into three groups according to the final irrigation protocol: EDTA, final irrigation with 12 mL of 17% EDTA for 3 minutes followed by 5 mL of 2.5% NaOCl; EDTA=PUI, final flush with 4 mL of 17% EDTA and PUI for 30 seconds. These procedures were repeated three times to standardize the volume of the irrigant. Control group, after preparation, the specimens were irrigated only with 17 mL of 2.5% NaOCl. The roots were fractured and analyzed using SEM. The intragroup analysis revealed that the EDTA=PUI protocol removed a higher amount of debris at the cervical third (P 5 0.03). The intergroup analysis revealed that EDTA=PUI presented the lowest amount of debris at the cervical third (P 5 0.007). Smear layer scores were higher in the control group compared with the EDTA and EDTA=PUI groups, but only at the cervical third (P 50.02). None of the final irrigant protocols completely removed the smear layer and debris. EDTA=PUI only improved the removal of debris at the cervical third.

Um questionamento muito frequente: qual o tempo que se deve esperar para movimentar um dente submetido a tratamento endodôntico, inclusive os de perfuração radicular? A extrapolação dos fenômenos observados em outras regiões da raiz e a fundamentação experimental com base em situações correlatas permitem afirmar que 30 dias correspondem a um período mais do que razoável para o reparo periapical estar em fase avançada de maturação e síntese. As forças ortodônticas são muito leves e dissipantes - muito mais do que o traumatismo dentário, o trauma oclusal e as forças mastigatórias normais -, e não devem interferir na patogenicidade e virulência das microbiotas envolvidas nas necroses e lesões periapicais crônicas, assim como não devem interferir nos fenômenos celulares e teciduais durante a reorganização dos tecidos apicais e periapicais.

The aim of the study was to determine if the increase in radiopacity provided by bismuth oxide is related to the color alteration of calcium silicate-based cement. Calcium silicate cement (CSC) was mixed with 0%, 15%, 20%, 30% and 50% of bismuth oxide (BO), determined by weight. Mineral trioxide aggregate (MTA) was the control group. The radiopacity test was performed according to ISO 6876/2001. The color was evaluated using the CIE system. The assessments were performed after 24 hours, 7 and 30 days of setting time, using a spectrophotometer to obtain the ΔE, Δa, Δb and ΔL values. The statistical analyses were performed using the Kruskal-Wallis/Dunn and ANOVA/ Tukey tests (p < 0.05). The cements in which bismuth oxide was added showed radiopacity corresponding to the ISO recommendations ( > 3 mm equivalent of Al). The MTA group was statistically similar to the CSC / 30% BO group (p > 0.05). In regard to color, the increase of bismuth oxide resulted in a decrease in the ΔE value of the calcium silicate cement. The CSC group presented statistically higher ΔE values than the CSC / 50% BO group (p < 0.05). The comparison between 24 hours and 7 days showed higher ΔE for the MTA group, with statistical differences for the CSC / 15% BO and CSC / 50% BO groups (p < 0.05). After 30 days, CSC showed statistically higher ΔE values than CSC / 30% BO and CSC / 50% BO (p < 0.05). In conclusion, the increase in radiopacity provided by bismuth oxide has no relation to the color alteration of calcium silicate-based cements.

Introduction: The sealers can be in direct contact with the periapical tissues. Thus, these materials must have appropriate physical and biological properties, providing conditions for repair to occur. Objective: The aim of this study was to evaluate the response of rat subcutaneous tissue to endodontics sealers. Material and methods: Three materials comprised the groups: group I – Zinc Oxide, Eugenol and Iodoform paste, group II – Portland cement with propylene glycol, and group III – MTA Fillapex® (Angelus). These materials were placed in polyethylene tubes and implanted into dorsal connective tissue of Wistar rats for seven and 15 days. The specimens were stained with hematoxylin and eosin and evaluated regarding to inflammatory reaction parameters through a light microscope. The data were compared using Kruskal-Wallis test with significance level of 5%. The intensity of inflammatory response against the sealers was analyzed by two blinded and previously calibrated observers for all experimental periods. Results: The histological evaluation showed that all the materials caused a moderated inflammatory reaction at seven days which decreased with time. A greater inflammatory reaction was observed at seven days in group I. The other specimens had significantly less inflammatory cells when compared to this group. Tubes with MTA Fillapex® presented some giant cells, macrophages and lymphocytes after seven days. At 15 days, the presence of fibroblasts and collagen fibers was observed indicating normal tissue healing. The group II showed similar results to those observed in MTA Fillapex® already at seven days. At 15 days the inflammatory reaction presented was almost absent at the tissue, with many collagen fibers indicating normal tissue healing. Objective: The aim of this study was to evaluate, ex vivo, the precision of five electronic root canal length measurement devices (ERCLMDs) with different operating systems: the Root ZX, Mini Apex Locator, Propex II, iPex, and RomiApex A-15, and the possible influence of the positioning of the instrument tips short of the apical foramen. Material and Methods: Forty-two mandibular bicuspids had their real canal lengths (RL) previously determined. Electronic measurements were performed 1.0 mm short of the apical foramen (-1.0), followed by measurements at the apical foramen (0.0). The data resulting from the comparison of the ERCLMD measurements and the RL were evaluated by the Wilcoxon and Friedman tests at a significance level of 5%. Results: Considering the measurements performed at 0.0 and -1.0, the precision rates for the ERCLMDs were: 73.5% and 47.1% (Root ZX), 73.5% and 55.9% (Mini Apex Locator), 67.6% and 41.1% (Propex II), 61.7% and 44.1% (iPex), and 79.4% and 44.1% (RomiApex A-15), respectively, considering ±0.5 mm of tolerance. Regarding the mean discrepancies, no differences were observed at 0.0; however, in the measurements at -1.0, the iPex, a multi-frequency ERCLMD, had significantly more discrepant readings short of the apical foramen than the other devices, except for the Propex II, which had intermediate results. When the ERCLMDs measurements at -1.0 were compared with those at 0.0, the Propex II, iPex and RomiApex A-15 presented significantly higher discrepancies in their readings. Conclusions: Under the conditions of the present study, all the ERCLMDs provided acceptable measurements at the 0.0 position. However, at the -1.0 position, the ERCLMDs had a lower precision, with statistically significant differences for the Propex II, iPex, and RomiApex A-15.

Introduction: A growing interest to preserve teeth into the mouth by patients resulted in the increasing number of endodontic retreatments, and when these happen, many different types of irritants are extruded through the foramen. Objective: This study analyzed in vitro the amount of debris extruded through the foramen using four instrumentation techniques during endodontic retreatment. Material and methods: Forty mesial-buccal roots of first molars were selected, instrumented with anatomical diameter up to size #30 ISO file and then obturated with gutta-percha and grossman sealer by lateral condensation. After, they were separated and randomly allocated into four groups with 10 teeth each for the endodontic retreatment procedure: G1 – conventional technique + solvent, G2 – conventional technique without solvent, G3 – ProTaper retreatment + solvent, G4 – ProTaper retreatment without solvent. In all groups, gutta-percha in the coronal portion was removed by using size 1-3 Gates Glidden drills. All teeth were irrigated with distilled water. The debris extruded through the foramen were collected and weighed by an analytical balance. Results: Group 4 had the lowest average for material extrusion through the foramen followed by groups 2, 3 and 1. When Tukey test for statistical analysis was applied, no significant difference among groups were found (p = 0.5664). Conclusion: We conclude that all instrumentation techniques used in this study produced debris which goes beyond the foramen.

O tratamento endodôntico de dentes com forame apical amplo requer consideração especial, devido ao risco de extravasamento do material obturador nos tecidos periapicais durante a obturação, de maneira relevante em áreas próximas às fissuras labiopalatinas. A criação de uma barreira apical é indicada em casos de difícil travamento do cone principal e consequente deficiência do selamento apical. O MTA em decorrência de suas excelentes propriedades biológicas, é o material de escolha para permanecer em contato com tecidos periodontais e periapicais, fazendo parte do arsenal endodôntico especificamente nestes casos de ápice aberto para a confecção de plug apical. Relato clínico: Indivíduo do gênero feminino, com fissura labiopalatina bilateral transforame, a distal dos dentes 11 e 21; compareceu no setor de endodontia do Hospital de Reabilitação de Anomalias Craniofaciais (HRAC-USP), apresentando necrose pulpar do dente 21. Na primeira sessão foi realizado o acesso cirúrgico, neutralização do conteúdo séptico/tóxico em sentido corono-apical, odontometria, biomecânica, curativo intracanal (pasta Calen) selamento provisório com cimento de ionômero de vidro. Na segunda sessão, foi realizada a obturação onde ocorreu extravasamento de material obturador, verificado através da radiografia comprobatória; justificando sua remoção e nova obturação, com prévia confecção de plug apical com MTA. Foi realizada proservação aos 6 meses pós tratamento endodôntico. Conclusão: neste caso, o plug apical com MTA possibilitou a obturação final do canal radicular com segurança, pela confecção de um anteparo apical, evitando extravasamento de material obturador e preservando a área de fissura labiopalatina.

A displasia cementária periapical (DCP) é considerada uma alteração pseudotumoral de etiologia desconhecida. Essa patologia é assintomática e tem crescimento autolimitante. Os dentes afetados apresentam normalmente vitalidade pulpar e integridade da lâmina dura. Objetivo: Apresentar dois casos clínicos que demonstram a importância do conhecimento e correto diagnóstico da DCP. Relatos clínicos: Paciente M.I.S, de 61 Anos, Gênero Feminino, Leucoderma. Apresentou-se ao setor de Endodontia, para avaliação, ao exame radiográfico foi constatada imagem radiopaca nos ápices de incisivos inferiores. Ao teste de sensibilidade ao frio houve resposta negativa, então foi realizado o teste em outras áreas da mandíbula, também com resposta negativa, tornando o teste inconclusivo. Com base nas informações obtidas pelos exames clínico e radiográfico, foi diagnosticada a DCP. Realizado controle clínico e radiográfico, não houve alterações significativas. Paciente H.N.P.N, de 22 Anos, Gênero Feminino, Leucoderma. Apresentou-se ao setor de Endodontia, para avaliação, ao exame radiográfico foi constatada imagem radiopaca na região perirradicular de canino e primeiro pré-molar inferiores do lado esquerdo. Ao teste de sensibilidade ao frio, houve resposta positiva. Com base nas informações obtidas pelos exames clínico e radiográfico, foi diagnosticada a DCP. Realizado controle clínico e radiográfico, não houve alterações significativas. Conclusão: Devido as suas características radiográficas, a DCP pode ser confundida com lesões inflamatórias de origem endodôntica. Intervenções endodônticas desnecessárias podem ocorrer por isso o cirurgião-dentista deve estar atento quanto à vitalidade pulpar e a integridade da lâmina dura dos dentes envolvidos, para o correto diagnóstico.

Objetivo: Dente Duplo (“double tooth”) é uma anomalia de forma que ocorre pela fusão de dois ou mais dentes ou geminação de um único dente, geralmente durante a fase de morfodiferenciação do germe dentário. A incidência de dente duplo varia 0,14-5,0% da população mundial, sem predileção pelo gênero. Apresentação unilateral é mais comum do que a bilateral. No geral, as anomalias dentarias em pacientes com fissura labiopalatina ocorrem próximas à região da fissura. O objetivo deste trabalho é relatar o tratamento endodôntico não cirúrgico de um caso de dentes duplos em um paciente com fissura labiopalatina. Relato de Caso: Paciente de gênero feminino de 10 anos com fissura labiopalatina matriculado no Hospital de Reabilitação de Anomalias Craniofaciais (HRAC-USP), encaminhado ao setor de Endodontia do HRAC-USP para realizar o tratamento endodôntico dos dentes 11 e 12 para ser re-anatomizados. Pela superposição dos dentes 11, 12, e presença de uma cúspide acessória (“talon cusp”) além de um dente supranumerário, as radiografia panorâmica e a radiografia periapical não mostram claramente a anatomia externa do dente e a interna dos condutos radiculares, motivo pelo qual foi solicitada uma tomografia computadorizada Cone-Beam para determinar o comprimento de trabalho para preparo biomecânico e obturação dos condutos radiculares. Serão apresentadas as etapas de diagnóstico e tratamento realizados no caso. Conclusão: Pela morfologia anormal da coroa e complexidade dos canais radiculares, o tratamento endodôntico apresentaria dificuldades, por isso o exame clínico e radiográfico cuidadoso é essencial para o sucesso do tratamento endodôntico. Desta forma concluise que a imagem fornecida pela tomografia computadorizada Cone-Beam foi útil no planejamento e tratamento deste caso.
