FUNDAMENTO: A monitorização ambulatorial da pressão arterial (MAPA) é considerada o padrão-ouro para a confirmação diagnóstica da hipertensão arterial resistente (HAR). No entanto, a monitorização residencial da pressão arterial (MRPA) tem sido considerada uma opção, pelo seu custo menor e maior conforto. OBJETIVO: Comparar os valores obtidos pela MRPA com os obtidos pela MAPA na identificação de pacientes hipertensos resistentes. MÉTODOS: Foram selecionados consecutivamente 51 pacientes hipertensos resistentes, adultos de ambos os sexos, em tratamento ambulatorial de referência, de jan/2007 a set/2009. A medida da pressão arterial (PA) casual de consultório, MAPA de 24 horas e MRPA foram realizadas conforme as diretrizes vigentes, com intervalo máximo de duas semanas entre os métodos. RESULTADOS: Ao comparar a MAPA (média de vigília) e MRPA, foi obtida boa correlação entre ambas, tanto para a pressão arterial sistólica (PAS) quanto para a diastólica (PAD): PAS r = 0,70, IC = 0,51-0,82; PAD r = 0,69, IC = 0,52-0,81. A HAR foi confirmada pela MAPA em 33 pacientes e pela MRPA em 37 desses, não havendo diferença significante entre os dois métodos. CONCLUSÃO: De acordo com os resultados obtidos, conclui-se que a MRPA é um exame que pode ser utilizado como alternativa à MAPA para a confirmação diagnóstica da HAR.

FUNDAMENTO: A medida da pressão arterial (PA) pelo próprio paciente, sem protocolos rígidos, com treinamento adequado, aparelhos validados e no próprio domicílio, é definida como automedida da pressão arterial (AMPA). OBJETIVO: Avaliar a interferência da AMPA na adesão ao tratamento e no controle pressórico. MÉTODOS: Participaram do estudo 57 pacientes, 38 no grupo de estudo (GE) e 19 no grupo de controle (GC). Esses pacientes foram seguidos por 12 meses e avaliados na randomização (V1), bem como no sexto (V2) e no décimo segundo mês (V3). Comparadas as médias da PA pela medida casual, pela AMPA e pela monitorização ambulatorial da pressão arterial (MAPA), exames laboratoriais e as respostas ao questionário sobre o estilo de vida. Os aparelhos utilizados foram: OMRON HEM 714, para a AMPA; OMRON 705 CP, para a medida casual; e Monitor SPACELABS 9002, para a MAPA. RESULTADOS: A idade média em anos foi de 62,05 ± 10,78 e de 55,42 ± 11,87 no GE e no GC (p = 0,03), respectivamente. Os valores da pressão arterial sistólica (PAS) pela medida casual no GE e no GC foram: 140,01 ± 16,73 mmHg e 141,79 ± 23,21 mmHg em V1 (p = 0,72); 135,49 ± 12,73 mmHg e 145,69 ± 19,31 mmHg em V2 (p = 0,02); 131,64 ± 19,28 mmHg e 134,88 ± 23,21 mmHg em V3 (p = 0,59). Os valores da pressão arterial diastólica (PAD) foram: 84,13 ± 10,71 mmHg e 86,29 ± 10,35 mmHg em V1 (p = 0,47); 81,69 ± 10,88 mmHg e 89,61 ± 11,58 mmHg em V2 (p = 0,02); 80,31 ± 11,83 mmHg e 86 ± 13,38 mmHg em V3 (p = 0,12). CONCLUSÃO: Os pacientes do GE apresentaram adesão ao tratamento não farmacológico semelhante ao GC, mas tiveram maior adesão ao tratamento medicamentoso e utilizaram menor número de drogas anti-hipertensivas. Não houve diferença entre os grupos na comparação do perfil metabólico e da função renal. (Arq Bras Cardiol. 2011; [online].ahead print, PP.0-0)

Background: Studies have shown sodium restriction to have a beneficial effect on blood pressure (BP) of hypertensive patients. Objective: To evaluate the impact of light salt substitution for regular salt on BP of hypertensive patients. Methods: Uncontrolled hypertensive patients of both sexes, 20 to 65 years-old, on stable doses of antihypertensive drugs were randomized into Intervention Group (IG - receiving light salt) and Control Group (CG - receiving regular salt). Systolic BP (SBP) and diastolic BP (DBP) were analyzed by using casual BP measurements and Home Blood Pressure Monitoring (HBPM), and sodium and potassium excretion was assessed on 24-hour urine samples. The patients received 3 g of salt for daily consumption for 4 weeks. Results: The study evaluated 35 patients (65.7% women), 19 allocated to the IG and 16 to the CG. The mean age was 55.5 ± 7.4 years. Most participants had completed the Brazilian middle school (up to the 8th grade; n = 28; 80.0%), had a family income of up to US$ 600 (n = 17; 48.6%) and practiced regular physical activity (n = 19; 54.3%). Two patients (5.7%) were smokers and 40.0% consumed alcohol regularly (n = 14). The IG showed a significant reduction in both SBP and DBP on the casual measurements and HBPM (p < 0.05) and in sodium excretion (p = 0.016). The CG showed a significant reduction only in casual SBP (p = 0.032). Conclusions: The light salt substitution for regular salt significantly reduced BP of hypertensive patients.

Abstract Background: Blood pressure is directly related to body mass index, and individuals with increased waist circumference have higher risk of developing hypertension, insulin resistance, and other metabolic changes, since adolescence. Objective: to evaluate the correlation of blood pressure with insulin resistance, waist circumference and body mass index in adolescents. Methods: Cross-section study on a representative sample of adolescent students. One group of adolescents with altered blood pressure detected by casual blood pressure and/or home blood pressure monitoring (blood pressure > 90th percentile) and one group of normotensive adolescents were studied. Body mass index, waist circumference were measured, and fasting glucose and plasma insulin levels were determined, using the HOMA-IR index to identify insulin resistance. Results: A total of 162 adolescents (35 with normal blood pressure and 127 with altered blood pressure) were studied; 61% (n = 99) of them were boys and the mean age was 14.9 ± 1.62 years. Thirty-eight (23.5%) adolescents had altered HOMA-IR. The group with altered blood pressure had higher values of waist circumference, body mass index and HOMA-IR (p<0.05). Waist circumference was higher among boys in both groups (p<0.05) and girls with altered blood pressure had higher HOMA-IR than boys (p<0.05). There was a significant moderate correlation between body mass index and HOMA-IR in the group with altered blood pressure (ρ = 0.394; p < 0.001), and such correlation was stronger than in the normotensive group. There was also a significant moderate correlation between waist circumference and HOMA-IR in both groups (ρ = 0.345; p < 0.05). Logistic regression showed that HOMA-IR was as predictor of altered blood pressure (odds ratio - OR = 2.0; p = 0.001). Conclusion: There was a significant association of insulin resistance with blood pressure and the impact of insulin resistance on blood pressure since childhood. The correlation and association between markers of cardiovascular diseases was more pronounced in adolescents with altered blood pressure, suggesting that primary prevention strategies for cardiovascular risk factors should be early implemented in childhood and adolescence.

Abstract Casual blood pressure measurements have been extensively questioned over the last five decades. A significant percentage of patients have different blood pressure readings when examined in the office or outside it. For this reason, a change in the paradigm of the best manner to assess blood pressure has been observed. The method that has been most widely used is the Ambulatory Blood Pressure Monitoring - ABPM. The method allows recording blood pressure measures in 24 hours and evaluating various parameters such as mean BP, pressure loads, areas under the curve, variations between daytime and nighttime, pulse pressure variability etc. Blood pressure measurements obtained by ABPM are better correlated, for example, with the risks of hypertension. The main indications for ABPM are: suspected white coat hypertension and masked hypertension, evaluation of the efficacy of the antihypertensive therapy in 24 hours, and evaluation of symptoms. There is increasing evidence that the use of ABPM has contributed to the assessment of blood pressure behaviors, establishment of diagnoses, prognosis and the efficacy of antihypertensive therapy. There is no doubt that the study of 24-hour blood pressure behavior and its variations by ABPM has brought more light and less darkness to the field, which justifies the title of this review.

Com o objetivo de averiguar a viabilidade de utilização de colostro fermentado naturalmente e colostro preservado com formaldeído em aleitamento de bezerros, através de consumo de alimento, desenvolvimento ponderal , conversão alimentar e estado sanitário, trinta bezerros de ambos os sexos, da raça Holandesa malhada de preto, foram submetidos a três sistemas de aleitamento na fase de 1 a 35 dias de idade: 1º) sucedâneo de leite; 2º) colostro fermentado e 3º) colostro preservado com formal deído. As dietas líquidas foram suplementadas à vontade com ração inicial de 18% de proteína bruta e feno de Kikuiu (Pennisetum clandestinum Hochst), de 19,8% de proteína bruta. Os animais foram distribuídos nos referidos tratamentos conforme delineamento de "experimentos inteiramente casual izados". Os consumos médios diários, ganhos médios diários de peso e conversão alimentar até os animais completarem 35 dias de idade foram, para os respectivos tratamentos, de 0,820 kg, 0,281 kg e 3,27; 0,796 kg, 0,396 kg e 2,23; 0,750 kg, 0,284 kg e 3,13. Da desmama até a 26ª semana, os ganhos diários médios foram de 0,655, 0,605 e 0,533 kg. Em relação a problemas sanitários, observou-se que quatro bezerros dos dez alimentados com sucedâneo apresentaram diarréia, contra dois em cada grupo de animais sob,regime de colostro. Os resultados indicaram que colostro fermentado e o preservado por formaldeído são excelentes alimentos substitutivos do leite integral ou de sucedâneo de leite. Apesar de não ter havido diferença significativa entre ambos os sistemas de aproveitamento, o colostro fermentado parece levar vantagem sobre o preservado por formal deído pela sua mais fácil preparação e melhor utilização pelo animal.

Els bacteris són la forma dominant de vida del planeta: poden sobreviure en medis molt adversos, i en alguns casos poden generar substàncies que quan les ingerim ens són tòxiques. La seva presència en els aliments fa que la microbiologia predictiva sigui un camp imprescindible en la microbiologia dels aliments per garantir la seguretat alimentària. Un cultiu bacterià pot passar per quatre fases de creixement: latència, exponencial, estacionària i de mort. En aquest treball s’ha avançat en la comprensió dels fenòmens intrínsecs a la fase de latència, que és de gran interès en l’àmbit de la microbiologia predictiva. Aquest estudi, realitzat al llarg de quatre anys, s’ha abordat des de la metodologia Individual-based Modelling (IbM) amb el simulador INDISIM (INDividual DIScrete SIMulation), que ha estat millorat per poder fer-ho. INDISIM ha permès estudiar dues causes de la fase de latència de forma separada, i abordar l’estudi del comportament del cultiu des d’una perspectiva mesoscòpica. S’ha vist que la fase de latència ha de ser estudiada com un procés dinàmic, i no definida per un paràmetre. L’estudi de l’evolució de variables com la distribució de propietats individuals entre la població (per exemple, la distribució de masses) o la velocitat de creixement, han permès distingir dues etapes en la fase de latència, inicial i de transició, i aprofundir en la comprensió del que passa a nivell cel•lular. S’han observat experimentalment amb citometria de flux diversos resultats previstos per les simulacions. La coincidència entre simulacions i experiments no és trivial ni casual: el sistema estudiat és un sistema complex, i per tant la coincidència del comportament al llarg del temps de diversos paràmetres interrelacionats és un aval a la metodologia emprada en les simulacions. Es pot afirmar, doncs, que s’ha verificat experimentalment la bondat de la metodologia INDISIM.

Report for the scientific sojourn at the Stanford University from January until June 2007. Music is well known for affecting human emotional states, yet the relationship between specific musical parameters and emotional responses is still not clear. With the advent of new human-computer interaction (HCI) technologies, it is now possible to derive emotion-related information from physiological data and use it as an input to interactive music systems. Providing such implicit musical HCI will be highly relevant for a number of applications including music therapy, diagnosis, nteractive gaming, and physiologically-based musical instruments. A key question in such physiology-based compositions is how sound synthesis parameters can be mapped to emotional states of valence and arousal. We used both verbal and heart rate responses to evaluate the affective power of five musical parameters. Our results show that a significant correlation exists between heart rate and the subjective evaluation of well-defined musical parameters. Brightness and loudness showed to be arousing parameters on subjective scale while harmonicity and even partial attenuation factor resulted in heart rate changes typically associated to valence. This demonstrates that a rational approach to designing emotion-driven music systems for our public installations and music therapy applications is possible.

La industria de los videojuegos crece exponencialmente y está ya superando a otras industrias punteras del ocio. En este proyecto, nos hemos planteado la realización de un videojuego con visualización en el espacio real 3D. Para la realización del videojuego se ha usado el siguiente software: Blender para diseñar los modelos 3D, C++ como lenguaje de programación para desarrollar el código y un conjunto de librerías básicas para desarrollar un videojuego llamadas Ogre3d (Motor Gráfico). La lógica del movimiento 3D y los choques entre las partículas del juego ha sido diseñada enteramente en este proyecto acorde con las necesidades del videojuego, y de forma compatible a los ficheros de Blender y a las librerías OGRE3D.

El món del videojoc està en constant evolució. Les últimes tendències marquen que els jocs online són els més acceptats per gran part dels usuaris. Aquest projecte forma part d’un videojoc orientat tant a la part més lúdica dels videojocs (visió clàssica), com a les xarxes socials i els negocis, combinat tot en una mateixa plataforma. Tot això integrat en un videojoc executable des del navegador i sense necessitat de cap tipus d’instal·lació. S’han desenvolupat una sèrie de mòduls per a satisfer les necessitats i els requeriments d’aquesta plataforma.

Alcohol, used responsibly and in moderation, is a normal and enjoyable aspect of everyday life for most people. Everyone is familiar with the wide variety of social settings in which it is consumed, ranging from casual enjoyment or relaxation to celebratory and ceremonial occasions. This widespread use of alcohol means that the manufacture and retailing of alcohol products, and its distribution through bars, clubs, restaurants and off-licences involves an estimated 32,000 jobs, both full and part time. This study on determinants of sexual protection behavior among HIV-positive gay men used the empirically tested information-motivation-behavioral skills (IMB) model. HIV-specific variables were added to the model to determine factors decisive for condom use with steady and casual partners. Data were collected using an anonymous, standardized self-administered questionnaire. Study participants were recruited at HIV outpatient clinics associated with the Eurosupport Study Group and the Swiss HIV Cohort Study. To identify factors associated with condom use, backward elimination regression analyses were performed. Overall, 838 HIV-infected gay men from 14 European countries were included in this analysis. About 53% of them reported at least one sexual contact with a steady partner; 62.5% had sex with a casual partner during the last 6 months. Forty-three percent always used condoms with steady partners and 44% with casual partners. High self-efficacy and subjective norms in favor of condom-use were associated with increased condom use with casual and steady partners, whereas feeling depressed was associated with decreased condom use with casual partners. Condoms were used less often with HIV-positive partners. Self-efficacy as an important behavioral skill to perform protection behavior was influenced by lower perceived vulnerability, higher subjective norms, and more positive safer sex attitudes. The IMB-model constructs appeared to be valid; however, not all the model predictors could be determined as hypothesized. Besides the original IMB constructs, HIV-specific variables, including sexual partners' serostatus and mental health, explained condom use. Such factors should be considered in clinical interventions to promote "positive prevention."

BACKGROUND The re-emergence of tuberculosis (TB) in low-incidence countries and its disproportionate burden on immigrants is a public health concern posing specific social and ethical challenges. This review explores perceptions, knowledge, attitudes and treatment adherence behaviour relating to TB and their social implications as reported in the qualitative literature. METHODS Systematic review in four electronic databases. Findings from thirty selected studies extracted, tabulated, compared and synthesized. FINDINGS TB was attributed to many non-exclusive causes including air-born transmission of bacteria, genetics, malnutrition, excessive work, irresponsible lifestyles, casual contact with infected persons or objects; and exposure to low temperatures, dirt, stress and witchcraft. Perceived as curable but potentially lethal and highly contagious, there was confusion around a condition surrounded by fears. A range of economic, legislative, cultural, social and health system barriers could delay treatment seeking. Fears of deportation and having contacts traced could prevent individuals from seeking medical assistance. Once on treatment, family support and "the personal touch" of health providers emerged as key factors facilitating adherence. The concept of latent infection was difficult to comprehend and while TB screening was often seen as a socially responsible act, it could be perceived as discriminatory. Immigration and the infectiousness of TB mutually reinforced each another exacerbating stigma. This was further aggravated by indirect costs such as losing a job, being evicted by a landlord or not being able to attend school. CONCLUSIONS Understanding immigrants' views of TB and the obstacles that they face when accessing the health system and adhering to a treatment programme-taking into consideration their previous experiences at countries of origin as well as the social, economic and legislative context in which they live at host countries- has an important role and should be considered in the design, evaluation and adaptation of programmes.

BACKGROUND: Annual syphilis testing was reintroduced in the Swiss HIV Cohort Study (SHCS) in 2004. We prospectively studied occurrence, risk factors, clinical manifestations, diagnostic approaches and treatment of syphilis. METHODS: Over a period of 33 months, participants with positive test results for Treponema pallidum hemagglutination assay were studied using the SHCS database and an additional structured case report form. RESULTS: Of 7244 cohort participants, 909 (12.5%) had positive syphilis serology. Among these, 633 had previously been treated and had no current signs or symptoms of syphilis at time of testing. Of 218 patients with newly detected untreated syphilis, 20% reported genitooral contacts as only risk behavior and 60% were asymptomatic. Newly detected syphilis was more frequent among men who have sex with men (MSM) [adjusted odds ratio (OR) 2.8, P < 0.001], in persons reporting casual sexual partners (adjusted OR 2.8, P < 0.001) and in MSM of younger age (P = 0.05). Only 35% of recommended cerebrospinal fluid (CFS) examinations were performed. Neurosyphilis was diagnosed in four neurologically asymptomatic patients; all of them had a Venereal Disease Research Laboratory (VDRL) titer of 1:>or=32. Ninety-one percent of the patients responded to treatment with at least a four-fold decline in VDRL titer. CONCLUSION: Syphilis remains an important coinfection in the SHCS justifying reintroduction of routine screening. Genitooral contact is a significant way of transmission and young MSM are at high risk for syphilis. Current guidelines to rule out neurosyphilis by CSF analysis are inconsistently followed in clinical practice. Serologic treatment response is above 90% in the era of combination antiretroviral therapy.

Este estudo constitui-se numa parte inicial de um Projeto Integrado que visa analisar os recursos estruturais das UTIs do Município de São Paulo, incluindo o ambiente físico, recursos humanos, equipamentos e materiais. Os aspectos analisados neste artigo referem-se às características destas Unidades quanto à sua quantidade e distribuição geográfica, número de leitos, entidade mantenedora, tipo de atendimento, de clientela e de tratamento, assim como as médias de ocupação e de permanência. Por amostragem casual estratificada, foram selecionados 40% dos hospitais com UTI de cada uma das regiões integrantes do SUS no Município. A amostra estudada foi de 43 UTIs e o instrumento para coleta de dados foi um questionário respondido pelo enfermeiro responsável pela Unidade. O número de UTIs nos hospitais variou de 1 a 4 , sendo mais frequentes aqueles com urna única Unidade (68,8%). 79,2% das UTIs pertenciam a hospitais particulares e 51,2% localizavam-se na região centro-oeste do Município. Os leitos de terapia intensiva corresponderam a 8,0% do total de leitos hospitalares. Cada Unidade tinha, em média, 10 leitos. Predominaram as UTIs gerais (60,5%), as destinadas somente a pacientes adultos (51,2%) e as que atendiam tanto a pacientes clínicos como cirúrgicos ( 95,3%). Na maioria das Unidades, a porcentagem de ocupação mensal dos leitos foi de 80 a 100% e a média de permanência, de 4,5 dias.
