PURPOSE: To investigate prospectively the relative accuracy of computed tomographic (CT) angiography, calcium scoring (CS), and both methods combined in demonstrating coronary artery stenoses by using conventional angiography as the reference standard. MATERIALS AND METHODS: The study was approved by the institutional review board Human Research Ethics Committee, and all patients completed written informed consent. Fifty patients (40 men, 10 women) aged 62 years ± 11 (± standard deviation) who were suspected of having coronary artery disease underwent both conventional coronary angiography and multisection coronary CT angiography with CS. Sensitivity and specificity of CS, CT angiography, and both methods combined in demonstrating luminal stenosis greater than or equal to 50% were determined for each arterial segment, coronary vessel, and patient. Receiver operating characteristic (ROC) curves were generated for CS prediction of significant stenosis, and the Mann-Whitney U test was used for comparison of CS between groups. RESULTS: When used with segment-specific electrocardiographic phase reconstructions, CT angiography demonstrated stenosed segments with 79% sensitivity and 95% specificity. Mean calcium score was greater in segments, vessels, and patients with stenoses than in segments, vessels, and patients without stenoses (P < .001 for all); nine (16%) of 56 stenosed segments, however, had a calcium score of 0. The patient calcium score correlated strongly with the number of stenosed arteries (Spearman {rho} = 0.75, P < .001). CS was more accurate in demonstrating stenosis in patients than in segments (areas under ROC curve were 0.88 and 0.74, respectively). CT angiography, however, was more accurate than CS in demonstrating stenosis in patients, vessels, and segments. The sensitivity and specificity of CS varied according to the threshold used, but when the calcium score cutoff (ie, >150) matched the specificity of CT angiography (95%), the sensitivity of CS in demonstrating stenosed segments was 29% (compared with 79% for CT angiography). Combining CT angiography with CS (at threshold of 400) improved the sensitivity of CT angiography (from 93% to 100%) in demonstrating significant coronary disease in patients, without a loss of specificity (85%); this finding, however, was not statistically significant. CONCLUSION: CT angiography is more accurate than CS in demonstrating coronary stenoses. A patient calcium score of greater than or equal to 400, however, can be used to potentially identify patients with significant coronary stenoses not detected at CT angiography.

With the Twentieth World Youth Day in Cologne on 16-21 August, this year, scheduled to be the first overseas visit of Pope Benedict XVI, it is timely to consider the latest book from George Weigel, (perhaps best known for his Witness to Hope: The Biography of John Paul II) in which he composes 14 letters specifically aimed at young Catholics, who, like many of their older counterparts, may well be wondering what it means to be a Catholic today.

There is evidence that increasing carbohydrate (CHO) availability during exercise by raising preexercise muscle glycogen levels attenuates the activation of AMPK{alpha}2 during exercise in humans. Similarly, increasing glucose levels decreases AMPK{alpha}2 activity in rat skeletal muscle in vitro. We examined the effect of CHO ingestion on skeletal muscle AMPK signaling during exercise in nine active male subjects who completed two 120-min bouts of cycling exercise at 65 ± 1% VO2 peak. In a randomized, counterbalanced order, subjects ingested either an 8% CHO solution or a placebo solution during exercise. Compared with the placebo trial, CHO ingestion significantly (P < 0.05) increased plasma glucose levels and tracer-determined glucose disappearance. Exercise-induced increases in muscle-calculated free AMP (17.7- vs. 11.8-fold), muscle lactate (3.3- vs. 1.8-fold), and plasma epinephrine were reduced by CHO ingestion. However, the exercise-induced increases in skeletal muscle AMPK{alpha}2 activity, AMPK{alpha}2 Thr172 phosphorylation and acetyl-CoA Ser222 phosphorylation, were essentially identical in the two trials. These findings indicate that AMPK activation in skeletal muscle during exercise in humans is not sensitive to changes in plasma glucose levels in the normal range. Furthermore, the rise in plasma epinephrine levels in response to exercise was greatly suppressed by CHO ingestion without altering AMPK signaling, raising the possibility that epinephrine does not directly control AMPK activity during muscle contraction under these conditions in vivo.

A perceived opposition between 'culture' and 'nature', presented as a dominant, biased and antagonistic relationship, is engrained in the language of Western culture. This opposition is reflected in, and adversely influences, our treatment of the ecosphere. I argue that through the study of literature, we can deconstruct this opposition and that such an ‘ecocritical’ operation is imperative if we are to avoid environmental catastrophe. I examine the way language influences our relationship with the world and trace the historical conception of ‘nature’ and its influence on the English language. The whale is, for many people, an important symbol of the natural world, and human interaction with these animals is an indication of our attitudes to the natural world in general. By focusing on whale texts (including older narratives, whaling books, novels and other whale-related texts), I explore the portrayal of whales and the natural world. Lastly, I suggest that Schopenhaurean thought, which has affinities in Moby-Dick, offers a cogent approach to ecocritically reading literature.

Reduced activation of exercise responsive signalling pathways have been reported in response to acute exercise after training; however little is known about the adaptive responses of the mitochondria. Accordingly, we investigated changes in mitochondrial gene expression and protein abundance in response to the same acute exercise before and after 10-d of intensive cycle training.

Nine untrained, healthy participants (mean±SD; VO2peak 44.1±17.6 ml/kg/min) performed a 60 min bout of cycling exercise at 164±18 W (72% of pre-training VO2peak). Muscle biopsies were obtained from the vastus lateralis muscle at rest, immediately and 3 h after exercise. The participants then underwent 10-d of cycle training which included four high-intensity interval training sessions (6×5 min; 90–100% VO2peak) and six prolonged moderate-intensity sessions (45–90 min; 75% VO2peak). Participants repeated the pre-training exercise trial at the same absolute work load (64% of pre-training VO2peak). Muscle PGC1-α mRNA expression was attenuated as it increased by 11- and 4- fold (P<0.001) after exercise pre- and post-training, respectively. PGC1-α protein expression increased 1.5 fold (P<0.05) in response to exercise pre-training with no further increases after the post-training exercise bout. RIP140 protein abundance was responsive to acute exercise only (P<0.01). COXIV mRNA (1.6 fold; P<0.01) and COXIV protein expression (1.5 fold; P<0.05) were increased by training but COXIV protein expression was decreased (20%; P<0.01) by acute exercise pre- and post-training.

These findings demonstrate that short-term intensified training promotes increased mitochondrial gene expression and protein abundance. Furthermore, acute indicators of exercise-induced mitochondrial adaptation appear to be blunted in response to exercise at the same absolute intensity following short-term training.

A necessidade de estimar com precisão o gasto energético dos pacientes gravemente doentes é cada vez mais importante para que se possa planejar uma nutrição adequada. Está bem estabelecido que tanto a desnutrição quanto o excesso alimentar prejudicam a evolução favorável destes doentes, especialmente quando estão sob ventilação mecânica. O objetivo do presente estudo foi comparar o Gasto Energético Total (GET) dos pacientes ventilados mecanicamente nos modos controlado e assistido, através da calorimetria indireta, medidos pelos monitores de gases TEEM-100 e DATEX-OHMEDA, verificando a necessidade de ajuste no aporte calórico em cada modo, correlacionando-os com a equação de Harris-Benedict (H-B). Foram estudados 100 pacientes em que os gases exalados (CO2 e O2) foram medidos durante 20 minutos em cada modo ventilatório (controlado e assistido) e o gasto energético calculado pela fórmula de “Weir”, determinando o GET, em 24 horas e comparado com o GET estimado pela equação de H-B. A média do escore APACHE II foi 21,1± 8,3. A média dos valores estimados pela equação de H-B foi de 1853,87±488,67 Kcal/24 h, considerando os fatores de atividade e estresse. Os valores médios obtidos pela calorimetria indireta (CI) foram de 1712,76 ±491,95 Kcal/24 h para a modalidade controlada e de 1867,33±542,67 Kcal/24 h para a assistida. A média do GET na modalidade assistida foi de 10,71% maior do que na controlada (p<0,001). A comparação das médias do GET, obtidos por CI com a equação de H-B ajustada para fatores de atividade e estresse demonstraram que a equação superestimou em 141,10 Kcal/24 h (8,2%) (p=0,012), quando na modalidade controlada. Retirando-se os fatores de atividade, observou-se uma tendência não significativa a subestimar em 44,28 Kcal/24 h (2,6%) (p=0,399). Quando na modalidade assistida, a comparação com H-B sem o fator de atividade, a medida por CI subestima em 198,84 Kcal/24 h, (10,71%), (p=0,001), enquanto que com o fator de atividade também subestimam, mas em 13,46 Kcal/24 h (0,75%) sem significância estatística (p=0,829). As diferenças observadas com o uso ou não de vasopressor, presença ou não de infecção e sepse como causa de internação na UTI, o tipo de dieta parenteral ou enteral ou sem dieta, e as faixas etárias não tiveram significância estatística, portanto não tiveram influência no gasto energético entre os modos ventilatórios. Os homens quando ventilando no modo controlado gastaram mais energia em relação às mulheres (1838,08 vs. 1577,01; p=0,007). Concluindo-se, os dados sugerem que devemos considerar o fator de atividade de 10%, somente quando em VM assistida, uma vez que este fator de atividade determina hiperalimentação, quando no modo controlado.

Esta dissertação discute as imagens e representações encontradas na Literatura e as lutas pelo controle da cultura no exemplo da Festividade, da Irmandade e da Marujada de São Benedito, na cidade de Bragança, Estado do Pará, na Amazônia brasileira, a partir da década de 1930, no século XX. Analisando uma farta bibliografia nos temas Folclore, Memória, Tradição Popular e Antropologia, o estudo tenta explicar como se construíram as relações sociais entre os sujeitos históricos da Igreja Católica pela Prelazia do Guamá e da Irmandade do Glorioso São Benedito de Bragança, relacionando-os com o recurso literário e com os principais teóricos da historiografia, para entender o catolicismo popular e oficial em suas representações assim como os símbolos construídos no tempo, como elementos da História de tensão entre as ideias e regras de controle eclesiástico católico e a reação popular dos irmãos de São Benedito.
