Fungal pathogens are a frequent cause of opportunistic infections. They live as commensals in healthy individuals but can cause disease when the immune status of the host is altered. T lymphocytes play a critical role in pathogen control. However, specific Ags determining the activation and function of antifungal T cells remain largely unknown. By using an immunoproteomic approach, we have identified for the first time, to our knowledge, a natural T cell epitope from Candida albicans. Isolation and sequencing of MHC class II-bound ligands from infected dendritic cells revealed a peptide that was recognized by a major population of all Candida-specific Th cells isolated from infected mice. Importantly, human Th cells also responded to stimulation with the peptide in an HLA-dependent manner but without restriction to any particular HLA class II allele. Immunization of mice with the peptide resulted in a population of epitope-specific Th cells that reacted not only with C. albicans but also with other clinically highly relevant species of Candida including the distantly related Candida glabrata. The extent of the reaction to different Candida species correlated with their degree of phylogenetic relationship to C. albicans. Finally, we show that the newly identified peptide acts as an efficient vaccine when used in combination with an adjuvant inducing IL-17A secretion from peptide-specific T cells. Immunized mice were protected from fatal candidiasis. Together, these results uncover a new immune determinant of the host response against Candida ssp. that could be exploited for the development of antifungal vaccines and immunotherapies.

Objective: The vascular access steal syndrome is a complication occurring in 1-6% after native arterio-venous (AV) fistulas, often due to huge diameter of the vein. This results in very high flow, which could also be responsible for cardiac overload. The aim of this study is to evaluate the efficiency of a new approach in the treatment of this pathology using open-pore external scaffolding prosthesis.Methods: This a retrospective review of all patients presenting symptomatic high flow after native AV fistula between January 2007 and December 2009 in 3 vascular centers. Pre-operative duplex exam confirmed the diagnosis of high flow. The operation consisted in preparation of the whole fistula, measurement of the flow and section on the venous side. The vein was wrapped with this 6 to 8 mm open-pore external scaffolding prosthesis (ProVena, BBraun, Germany) according to its diameter and to the flow and then sutured. Measurement of the flow was repeated. Patients were followed by duplex exam at 1 week and at 1, 3, 6 and 12 months. Procedural success was defined as complete implantation of the prosthesis and reduction of the flow. Primary outcomes were reduction of the flow and recovery of the symptoms and secondary endpoint was patency of the fistula.Results: During the study period, 14 patients, with a mean age of 65・8 years old, have been operated with this technique.There were 2 native forearmfistulas and 12 on the armwith a mean pre-operative flow of 2600 ml/min (1800-3800). The mode of presentation was pain in 6 patients, neurological disorders in 10 and necrosis in 4. Moreover, 3 patients had cardiac insufficiency due to high flow in the fistula. The procedure was technically successful in 100% of cases. Re-intervention was necessary in 2 patients due to hematoma. Recovery of the initial symptoms occurred in 13 patients (93%). The mean flow reduction was 1200 ml/min (600-2000). In 1 patient, a persistent steal syndrome despite flow reduction to 1400 ml/min resulted in fistula closure 2 months later. At a mean follow-up of 22 months (4-35), all remaining patients (13/14) presented a patent fistula without recurrence.Conclusion: This new approach seems to be safe and effective in the treatment of symptomatic high flow native AV fistulas by significantly reducing the flow and avoiding closure of the vascular access. Longer follow-up with more patients are necessary to evaluate the risk of recurrence.

As respostas geoquímicas do ambiente são controladas, em primeiro lugar, pelos processos naturais, geológicos, pedológicos, climáticos e biológicos. Sobrepondo-se a essa herança, a presença do homem adquire papel importante pelas possibilidades de alteração que promove no ambiente geoquímico a partir das concentrações urbanas, actividades industriais e agrícolas. Nos últimos anos a cartografia geoquímica tem assumido uma importância relevante já que tem sido largamente reconhecido que para se identificar e quantificar a poluição antropogénica é fundamental a existência de mapas que apresentem os valores de fundo geoquímico natural. O principal objectivo deste estudo é o estabelecimento de uma base de dados geoquímicos da ilha de Santiago (Cabo Verde) utilizando os solos como meio amostral. Esta base de dados geoquímicos permitiu caracterizar o estado actual dos solos da ilha de Santiago e estabelecer os valores de fundo geoquímico. A ilha de Santiago, com uma área de 991km2, é a maior ilha do arquipélago representando cerca de 25% da área total. Tem forma adelgaçada na direcção Norte-Sul, apresentando um comprimento máximo de 54,9km entre a ponta Moreia, a Norte, e a ponta Mulher Branca, a Sul, e uma largura máxima de 29km entre a ponta Janela, a Oeste, e a ponta Praia Baixo, a Leste. Apresenta uma altitude máxima de 1392m. No presente trabalho foram seguidas as recomendações do projecto “IGCP 259 – International Geochemical Mapping” no que se refere aos procedimentos de amostragem, preparação física das amostras, análise química e controlo de qualidade dos resultados. Na ilha de Santiago foram colhidas 278 amostras de solos correspondendo a uma densidade de amostragem de 0.28 amostras por km2. As amostras foram decompostas com uma solução modoficada de áqua régia e analisadas para 37 elementos (Ag, Al, As, Au, B, Ba, Bi, Ca, Cd, Co, Cr, Cu,Fe, Hg, K, La, Mg, Mn, Mo, Na, Ni, P, Pb, S, Sb, Sc, Se, Sr, Te, Th, Ti, Tl, U, V, W and Zn) por ICP-ES. Foram ainda determinados 5 parâmetros característicos do solo (pH, condutividade, matéria orgânica, cor e textura). Os padrões geoquímicos obtidos atravs dos mapas de distribuição espacial foram correlacionados com vários factores designadamente a natureza da rocha mãe, o tipo de solo e ainda com algumas fontes de contaminação. A utilização da Análise em Componentes Principais permitiu distinguir diferentes tipos de associações de variáveis realçando a importância das associações do tipo geogénico relativamente às associações to tipo antropogénico. A associação Na-CE assinala as áreas onde o impacto das actividades humanas influência os valores de fundo geoquímico
