Background: Cardiovascular disease is the leading cause of death in Brazil, and hypertension is its major risk factor. The benefit of its drug treatment to prevent major cardiovascular events was consistently demonstrated. Angiotensin-receptor blockers (ARB) have been the preferential drugs in the management of hypertension worldwide, despite the absence of any consistent evidence of advantage over older agents, and the concern that they may be associated with lower renal protection and risk for cancer. Diuretics are as efficacious as other agents, are well tolerated, have longer duration of action and low cost, but have been scarcely compared with ARBs. A study comparing diuretic and ARB is therefore warranted.Methods/design: This is a randomized, double-blind, clinical trial, comparing the association of chlorthalidone and amiloride with losartan as first drug option in patients aged 30 to 70 years, with stage I hypertension. The primary outcomes will be variation of blood pressure by time, adverse events and development or worsening of microalbuminuria and of left ventricular hypertrophy in the EKG. The secondary outcomes will be fatal or non-fatal cardiovascular events: myocardial infarction, stroke, heart failure, evidence of new subclinical atherosclerosis and sudden death. The study will last 18 months. The sample size will be of 1200 participants for group in order to confer enough power to test for all primary outcomes. The project was approved by the Ethics committee of each participating institution.Discussion: The putative pleiotropic effects of ARB agents, particularly renal protection, have been disputed, and they have been scarcely compared with diuretics in large clinical trials, despite that they have been at least as efficacious as newer agents in managing hypertension. Even if the null hypothesis is not rejected, the information will be useful for health care policy to treat hypertension in Brazil. The objective of this study was to estimate the prevalence of adverse drug reactions (ADR) related to hospital admission of elderly people, identifying the use of potentially inappropriate medication (PIM), the ADR and the risk factors associated with the hospitalization. A cross-sectional study was conducted in a private hospital of São Paulo State, Brazil. All patients aged ≥ 60 years, admitted in the general practice ward in May 2006 were interviewed about the drugs used and the symptoms/complaints that resulted in hospitalization. More than a half (54.5 %) of elderly hospitalizations were related with ADR. The therapeutic classes involved with ADR were: cardiovascular (37.7 %), central nervous (34.6 %) and respiratory (5.7 %). The ADR observed were disorders in circulatory (28.4 %), digestive (20.0 %) and respiratory (18.9 %) tracts. 27 elderly had made PIM and in 20 of them this was the cause of hospitalization. Polypharmacy was an ADR risk factor (p = 0.021).These data allows the healthcare professionals upgrade, qualifying them in pharmcovigilance.
American visceral leishmaniasis: Factors associated with lethality in the state of São Paulo, Brazil

Objectives. To identify factors associated with death in visceral leishmaniasis (VL) cases. Patients and Methodology. We evaluated prognostic factors for death from VL in São Paulo state, Brazil, from 1999 to 2005. A prognostic study nested in a clinical cohort was carried out by data analysis of 376 medical files. A comparison between VL fatal cases and survivors was performed for clinical, laboratory, and biological features. Association between variables and death was assessed by univariate analysis, and the multiple logistic regression model was used to determine adjusted odds ratio for death, controlling confounding factors. Results. Data analysis identified 53 fatal cases out of 376 patients, between 1999 and 2005 in São Paulo state. Lethality was 14.1 (53/376), being higher in patients older than fifty years. The main causes of death were sepsis, bleeding, liver failure, and cardiotoxicity due to treatment. Variables significantly associated with death were severe anemia, bleeding, heart failure, jaundice, diarrhea, fever for more than sixty days, age older than fifty years, and antibiotic use. Conclusion. Educational health measures are needed for the general population and continuing education programs for health professionals working in the affected areas with the purpose of identifying and treating early cases, thus preventing the disease evolution towards death. © 2012 Geraldine Madalosso et al.

Constant rate infusion (CRI) shows several advantages in balanced anesthesia, such as reduction of requirement for inhaled anesthetics and control of pain. The most commonly used drugs in these protocols are local anesthetics, dissociative, and opioids, which may be administered alone or in combinations. We evaluated the records of 200 dogs that underwent various surgical procedures with anesthetic or analgesic CRI in the perioperative period during 2011 and 2012 at the Veterinary Hospital of Franca University (Unifran), and identified possible complications during the transoperative period. Records evaluated included clinical state, laboratory tests, drugs used in premedication and induction, and CRI protocol. Acepromazine and morphine were the main drugs used in premedication. Propofol was used to induce anesthesia alone or in combination with other agents. We evaluated records of the 25 different CRI protocols. Fentanyl was the main drug employed in CRI, either alone or in combination. There were 128 episodes of anesthetic complications during CRI;the most common were hypotension, hypertension, and tachycardia, which occurred in 43 (32%), 35 (26.3%), and 19 (14.2%) dogs, respectively. Cardiac arrhythmia was reported in only 4 dogs. Signs of respiratory depression were present in dogs treated with 6 different CRI protocols. The consumption of isoflurane (vol %) reduced between 15.7% and 21.05% after 30minutes of the CRI in the fentanyl and fentanyl-lidocaine-ketamine CRI groups (p<0.05). In conclusion, CRI is a valid component of balanced anesthesia in dogs, safe, and has a low incidence of adverse effects. However, future studies are warranted to describe the results of the clinical use of CRI to better characterize and refine this technique.

Em uma propriedade no município de Roseira Velha, São Paulo, com histórico de doença cardíaca, abortos e sinais nervosos em bovinos, associados à ingestão de Tetrapterys multiglandulosa, foi observada fibrose cardíaca em um bezerro de uma semana de idade que apresentava apatia, fraqueza e insuficiência respiratória. Os objetivos desse trabalho foram determinar se a espécie ovina é sensível à intoxicação por T. multiglandulosa, descrever as alterações clínicas e patológicas da intoxicação e avaliar a utilização dessa espécie como modelo experimental para a intoxicação. Para determinar a toxicidade de T. multiglandulosa a ser utilizada em ovinos, foi realizado um experimento prévio em um bovino, sendo administrado 22g por kg de peso vivo (g/kg/pv) da planta verde durante 9 dias. Após 9 dias de ingestão o animal apresentou sinais nervosos e no 12º dia foi eutanasiado. Na necropsia não foram observadas lesões. No estudo histológico observou-se vacuolização (status spongiosus, espongiose) da camada profunda da substância cinzenta do córtex cerebral e da substância branca subcortical. Para a reprodução da enfermidade em ovinos foram utilizados 6 ovinos, machos, divididos em 3 grupos de 2 animais cada. Os Ovinos 1 e 2 do Grupo 1 receberam doses diárias de 6 g/kg/pv da planta seca por um período de 30 dias; os Ovinos 3 e 4 do Grupo 2 receberam doses diárias de 3 g/kg/pv por um período de 60 dias; e os Ovinos 5 e 6 do Grupo 3 serviram como controle. O Ovino 1 foi sacrificado aos 30 dias de administração da planta. Apresentou somente arritmia cardíaca e não foram observadas lesões significativas na necropsia. Os Ovinos 2, 3 e 4 apresentaram arritmia a partir dos dias 9, 12 e 18 do início do experimento, respectivamente. A partir do 52º dia iniciaram a apresentar depressão, relutância em locomover-se e incoordenação. Esses sinais foram se agravando e os ovinos foram sacrificados, com sinais clínicos acentuados, aos 60, 70 e 80 dias após o início do experimento, respectivamente. Na necropsia apresentaram hidropericárdio, ascite, hidrotórax, fígado em noz moscada e miocárdio endurecido e esbranquiçado, especialmente no septo interventricular e ventrículo esquerdo. Microscopicamente, o coração dos Ovinos 2, 3 e 4 apresentava áreas de fibrose associadas a infiltrado inflamatório mononuclear. Não foram observadas lesões cardíacas no coração do Ovino 1. No cérebro e tronco encefálico de todos os animais que receberam a planta observou-se espongiose, principalmente na camada profunda da córtex e da substância branca subcortical. No cerebelo observou-se espongiose da substância branca e na medula cervical havia espongiose da substância branca e espongiose discreta da substância cinzenta. Essa lesões eram discretas no Ovino 1 e moderadas a acentuadas nos Ovinos 2, 3 e 4. Na microscopia eletrônica da substância branca cerebelar foi observado que o status spongiosus observado na microscopia de luz é causado por edema intramielínico. Os dois ovinos do grupo controle, sacrificados aos 80 dias após o início do experimento, não apresentaram sinais clínicos nem lesões macroscópicas ou histológicas significativas.
