This paper analyses the demand for private health care by Spanishhouseholds using a micro budget survey. The methodology used takescare of the three part decision process involved in this type ofbehaviour, namely the decision to use private health care, howoften to do so and how much to spend each time and also the effectsof unobserved heterogeneity. Since the theoretical frameworkcorresponds to the Grossman model of health investment, the resultsalso provide a test of the theory when these issues are considered.Finally, the obtained evidence also suggest that the current systemof tax deductions for private health care expenditures is regressive.

Despite clear evidence of correlations between financial and medical statuses and decisions, most models treat financial and health-related choices separately. This article bridges this gap by proposing a tractable dynamic framework for the joint determination of optimal consumption, portfolio holdings, health investment, and health insurance. We solve for the optimal rules in closed form and capitalize on this tractability to gain a better understanding of the conditions under which separation between financial and health-related decisions is sensible, and of the pathways through which wealth and health determine allocations, welfare and other variables of interest such as expected longevity or the value of health. Furthermore we show that the model is consistent with the observed patterns of individual allocations and provide realistic estimates of the parameters that confirm the relevance of all the main characteristics of the model.

Iowa faces a growing crisis in providing an adequate number of direct care workers for its again population. Direct care workers caring for the aging and disabled population are among those in our work force with the lowest wages and lack of access to health insurance. A survey of direct care workers conducted by the Iowa Caregivers Association in June of 2066 indicated that benefits ranked second only to wages in the reasons for job-hoping. A study of the direct care workforce in Vermont healthy insurance, ranked second only to wages, as important to attracting and keeping direct care workers.

OBJECTIVE: To assess social, economic and medical data concerning children without a resident permit taken into care by the Children's Hospital of Lausanne (HEL) in order to evaluate their specific needs. METHODS: Prospective exploratory study by a questionnaire including the socio-demographic, medical and education data of 103 children without a resident permit, who consulted the HEL for the first time between August 2003 and March 2006. These children were then recalled for a second check-up one year later in order to allow a regular monitoring. RESULTS: Eighty-seven percent of the children were native of Latin America, 36% being less than two years old. This population of children lived in precarious conditions with a family income lower than the poverty level (89% of the families with less than 3100 CHF/month). Forty-five percent of the children had a health insurance. The main reasons for consultation were infectious diseases, a check-up requested by the school or a check-up concerning newborn children. Most of them were in good health and the others were affected by illnesses similar to those found in other children of the same age. At least 13% of the children were obese and 27% were overweight. All children who were of educational age went to school during the year after the first check-up and 48% were affiliated to a health insurance. CONCLUSIONS: The majority of the children from Latin America lived in very precarious conditions. Their general health status was good and most of them could benefit from regular check-ups. Prevention, focused on a healthier life style, was particularly important among this population characterised by a high incidence of overweight and obesity.

According to the Centers for Disease Control and Prevention (CDC), the number of adults reporting a disability is expected to increase, along with the need for appropriate medical and public health services. People with disabilities (PWD) face many barriers to good heath, including having overall poorer health, less access toadequate health care, limited access to health insurance, skipping mediacl care because of cost, and engaging in risky health behaviors including smoking and physical inactivity. The goals of the Iowa public health needs assessment were to assess the burden of disability in Iowa counties including health risk factors such as chronic conditions, determine access to preventive health care, and determine the effect of socioeconomic conditions. The state level assessment was based on th 2009-2012 American Community Survey (ACS) and publiccally available Behavioral Risk FactorSurveillance System (BRFSS) 2011 survey. The 2001-2010 combined BRFSS data was used for county level assessment. The needs assessment led us to conclude that adult Iowans with idsabilities face several challenges compared to non-disabled adults. They are more likely to suffer from debilitating chronic conditions and social disparities. Counties with the higher levels of poverty were more likely to have PWD with higher levels of disability related disparities.

In 2002, Senate File 2318, Insurance Premium Tax Reduction Act, the Iowa General Assembly approved a reduction in the insurance premium tax rate from 2 percent to 1 percent. The reduction was phased in at the rate of 1/4 percent annual increments over a five-year period, beginning with life and health insurance policy payments made on or after January 1st, 2003. Calendar year 2007 was the first year all premiums were taxed at 1 percent and fiscal year 2008 was the first full fiscal year at the 1 percent rate. Insurance premiums tax is paid only by insurance companies. The companies that benefit from the tax reduction are uniquely identified in government employment statistics. This allows for a unique opportunity to evaluate both the revenue impact and the employment impact of the tax rate reduction in Senate File 2318. This issue review focuses on the General Fund revenue impact of the rate reduction and the Iowa employment trends for the insurance industry since the rate reduction legislation was approved.

Although the performance of the Swiss health system is high, one out of ten patients in general practitioner's (GP) office declares having foregone care in the previous twelve months for economic reasons. Reasons for foregoing care are several and include a lack of knowledge of existing social aids in getting health insurance, unavailability of GPs and long waiting lists for various types of care. Although long term knowledge of patients or a psychosocial history of deprivation or poverty may help identify individuals at risk of foregoing care, many may remain undetected. We propose then a few instruments to help GPs to identify, in a simple and structured approach, patients at risk of forgoing care for economic reasons; these patients are frequently deprived and sometimes poor.

The mission of the Iowa Department of Public Health (IDPH) is “Promoting and Protecting the Health of Iowans.” In addition to its larger role in population health preparedness, surveillance, and response, IDPH has historically funded a broad array of health-related services to a “covered population” of approximately 1,000,000 Iowa residents through a varied network of local community-based “safety-net” provider contractors. Those health-related services range from funding direct healthcare services like immunizations and vision screening to providing or funding facilitative services like transportation and care coordination. While all Iowans may be eligible for some IDPH-funded direct healthcare service, such as smoking cessation, the individuals most often eligible for these services have traditionally been the uninsured and under-insured. As uninsured Iowans become enrolled in health plan options available through the Iowa Health and Wellness Plan (IHAWP) and the Marketplace, IDPH anticipates that many direct healthcare services funded by IDPH will become covered benefits or services under new plans, changing the demand for IDPH-funded services.

As the population ages, many of us will be faced with the prospect of moving either ourselves or a loved one into a long-term care setting (nursing home, assisted living facility or elder group home). Whether the decision comes up suddenly following a hospitalization or gradually as care needs evolve, the question of how to pay for long-term care is certain to arise. Some people mistakenly believe that Medicare will pay for their long-term care stay, but while Medicare will pay for hospital costs and skilled nursing facility stays, it does not pay for long-term care. Rather, possible payment options for long-term care include private pay, Medicaid or long-term care insurance or veterans benefits.

BACKGROUND: In most of the emergency departments (ED) in developed countries, a subset of patients visits the ED frequently. Despite their small numbers, these patients are the source of a disproportionally high number of all ED visits, and use a significant proportion of healthcare resources. They place a heavy economic burden on hospital and healthcare systems budgets overall. Several interventions have been carried out to improve the management of these ED frequent users. Case management has been shown in some North American studies to reduce ED utilization and costs. In these studies, cost analyses have been carried out from the hospital perspective without examining the costs induced by healthcare consumed in the community. However, case management might reduce ED visits and costs from the hospital's perspective, but induce substitution effects, and increase health service utilization outside the hospital. This study examined if an interdisciplinary case-management intervention-compared to standard ED care -reduced costs generated by frequent ED users not only from the hospital perspective, but also from the healthcare system perspective-that is, from a broader perspective taking into account the costs of healthcare services used outside the hospital. METHODS: In this randomized controlled trial, 250 adult frequent emergency department users (5 or more visits during the previous 12 months) who visited the ED of the University Hospital of Lausanne, Switzerland, between May 2012 and July 2013 were allocated to one of two groups: case management intervention (CM) or standard ED care (SC), and followed up for 12 months. Depending on the perspective of the analysis, costs were evaluated differently. For the analysis from the hospital's perspective, the true value of resources used to provide services was used as a cost estimate. These data were obtained from the hospital's analytical accounting system. For the analysis from the health-care system perspective, all health-care services consumed by users and charged were used as an estimate of costs. These data were obtained from health insurance providers for a subsample of participants. To allow comparisons in a same time period, individual monthly average costs were calculated. Multivariate linear models including a fixed effect "group" were run using socio-demographic characteristics and health-related variables as controlling variables (age, gender, educational level, citizenship, marital status, somatic and mental health problems, and risk behaviors).

L’asthme et la maladie pulmonaire obstructive chronique (MPOC) se classent au premier rang parmi les maladies respiratoires les plus fréquentes au Québec. Une mauvaise maîtrise de l’asthme et de la MPOC entraîne d’importantes répercussions sur la santé et la qualité de vie des patients et sur les coûts associés au système de santé canadien, dues à de fréquentes consultations médicales, des visites à l’urgence, des hospitalisations et des décès précoces. Il est donc très important d’évaluer l’usage optimal des médicaments dans le traitement de ces maladies afin de réduire la morbidité et la mortalité Cette thèse vise à comparer dans un premier temps l’efficacité des médicaments en situation réelle dans le traitement de la MPOC sur le taux d’exacerbations et la mortalité puisque les études observationnelles publiées à ce sujet comportaient des biais majeurs dus à une mauvaise mesure de l’exposition au traitement. Ainsi, dans le cadre de cette thèse, deux banques de données administratives québécoises ont été appariées pour créer une cohorte de 36 492 patients âgés de 50 ans ou plus atteints de MPOC (1995-1999) Dans cette cohorte, la théophylline diminuait davantage les exacerbations que les β2-agonistes à longue durée d’action (BALA, RR = 0,89; IC 95 % : 0,84-0,95), mais elle était moins efficace en situation réelle que les corticostéroïdes inhalés (CSI, RR = 1,07; IC 95 % : 1,04-1,10). Un devis cas-témoins niché dans cette cohorte a permis de vérifier que les CSI seuls ou combinés avec un BALA étaient plus efficients pour réduire la mortalité comparativement aux BALA seuls (RR = 0,69; IC 95 % : 0,53-0,88 et RR = 0,73; IC 95 % : 0,56-0,96, respectivement). L’efficacité des CSI dans le traitement de l’asthme pour réduire les exacerbations et la mortalité n’est plus à prouver, cependant la non-adhésion et la non-persistance aux CSI sont grandement problématiques. À notre connaissance, aucune étude n’a évalué l’impact du type d’assurance médicaments sur l'adhésion et la persistance des Québécois aux CSI en raison de l’absence des personnes qui ont une assurance médicaments privée dans la banque de données des services pharmaceutiques de la Régie de l’assurance maladie du Québec. Afin de combler ce manque, une des parties intégrantes de cette thèse a été de développer le registre reMed. Par la suite, une cohorte d’utilisateurs de CSI âgés de 20 à 64 ans a été sélectionnée à partir de reMed (2008-2010) et ces sujets ont été appariés à des utilisateurs de CSI sélectionnés à partir de la banque de données des services pharmaceutiques de la Régie de l’assurance maladie du Québec (RAMQ). Les résultats de cette dernière étude indiquent que même si l’adhésion était faible dans les deux cohortes, les patients ayant une assurance médicaments privée étaient moins adhérant que les patients couverts par l’assurance médicaments publique de la RAMQ (différence moyenne d’adhésion de -9,7 %; IC 95 % : -13,2 % à -6,5 %). De plus, ces patients couverts par une assurance médicaments privée étaient aussi 52 % plus susceptibles d'arrêter leur traitement de CSI au cours d’une année (HR = 1,52; IC 95 % : 1,16-2,00). En conclusion, selon les travaux de cette thèse, la théophylline peut être considérée comme une thérapie efficace en situation réelle pour prévenir les exacerbations aiguës de la MPOC d’autant plus qu’elle est moins dispendieuse que les traitements en inhalations et que sa formulation orale procurerait, selon la littérature, une meilleure adhésion que les médicaments en inhalation. Quant aux CSI, ils ont un rôle important dans le traitement de l’asthme, mais aussi dans le traitement de la MPOC, puisque selon les résultats de cette thèse, ils procureraient une plus grande diminution du risque d’exacerbations aiguës de la MPOC et de la mortalité par rapport aux autres traitements. Par contre, il a aussi été démontré que l'adhésion et la persistance aux CSI étaient très faibles, particulièrement dans le traitement de l’asthme. Le type d’assurance médicaments serait un facteur déterminant de l’adhésion et de la persistance aux CSI. D’autres études seront nécessaires pour évaluer si les différences d’adhésion et de persistance observées dans cette étude se traduisent par des différences sur l’utilisation et les coûts des soins de santé. De plus, il sera nécessaire d’étudier si les différences observées se limitent aux CSI ou si le type d’assurance médicaments a impact sur la prise d’autres médicaments indiqués dans le traitement des maladies chroniques.
