A case series to study factors related to family expectation regarding schizophrenic patients was conducted in an out-patient setting in the city of S. Paulo, Brazil. Patients diagnosed as presenting schizophrenia by the ICD 9th Edition and having had the disease for more than four years were included in the study. Family Expectation was measured by the difference between the Katz Adjustment Scale (R2 and R3) scores based on the relative's expectation and the socially expected activities of the patient (Discrepancy Score), and social adjustment was given by the DSM-III-R Global Assessment Scale (GAS) . Outcome assessments were made independently, and 44 patients comprised the sample (25 males and 19 females). The Discrepancy mean score was twice as high for males as for females (p < 0.02), and there was an inverse relationship between the discrepancy score and social adjustment (r =-0.46, p < 0.001). Moreover, sex and social adjustment exerted independent effects on the discrepancy score when age, age at onset and number of psychiatric admissions were controlled by means of a multiple regression technique. There was an interaction between sex and social adjustment, the inverse relationship between social adjustment and discrepancy score being more pronounced for males. These findings are discussed in the light of the potential association between the family environment, gender and social adjustment of schizophrenic patients, and the need for further research, i.e. ethnographic accounts of interactions between patient and relatives sharing households particularly in less developed countries.

OBJETIVO: Discutir aspectos metodológicos da estratégia de identificação de casos psiquiátricos, em duas etapas, em estudos epidemiológicos. MÉTODOS: Analisa-se a metodologia utilizada no Estudo Multicêntrico de Morbidade Psiquiátrica do Adulto realizado em três cidades brasileiras, entre 1990-1991. Na primeira etapa do citado estudo, uma amostra aleatória (6.740 indivíduos) da população foi selecionada e submetida a um rastreamento com o Questionário de Morbidade Psiquiátrica do Adulto -- QMPA. Na segunda etapa, uma subamostra (775 indivíduos) foi selecionada e submetida ao Inventário de Sintomas do DSM-III (Diagnostic and Statistical Manual of Mental Disorders), aplicado por psiquiatras e psicólogos treinados, para confirmação-diagnóstica. RESULTADOS: São descritos os procedimentos empregados para a estimativa das prevalências, mostrando que o fraco desempenho da escala de rastreamento não compromete o método. CONCLUSÃO: A vantagem da metodologia é a de corrigir as distorções apresentadas pelos instrumentos atuais de identificação de casos psiquiátricos.

OBJECTIVE: To investigate the relevance of subjective criteria adopted by a psychiatry and psychology consultation-liaison service, and their suitability in the evaluation of case registries and objective results. METHODS: Semi-structured interviews were conducted and all supervisors of the university hospital service were interviewed. Routinely collected case registries were also reviewed. Standardized assessment with content analysis for each category was carried out. RESULTS: The results showed distortions in the adopted service focus (doctor-patient relationship) and consultant requests. This focus is more on consulting physician-oriented interventions than on patients. DISCUSSION: Evaluation of the relevance of service criteria could help promoting quality assessment of the services provided, mainly when objective criteria have not yet been established to assure their suitability.

Este trabalho tem como objectivo a determinação do teor crítico de cloretos no betão, ou seja, o teor de cloretos à profundidade da armadura que é necessário para sustentar uma quebra localizada do filme de passivação e, por isso, iniciar a corrosão activa. Uma vez que não existe consenso quanto ao seu valor, este trabalho pretendia dar um contributo no estudo deste parâmetro, utilizando um procedimento que modele realisticamente a corrosão das armaduras em estruturas de betão armado expostas directamente a água do mar. Realizaram-se três séries de testes com cimentos de fontes diferentes. Utilizaram-se varões nervurados e incorporados no betão. Os iões cloreto foram introduzidos por absorção capilar e difusão e a despassivação foi detectada por medição do potencial de corrosão do aço. O teor crítico de cloretos foi determinado em relação à massa de cimento por titulação potenciométrica obtendo-se para a amassadura 1 um intervalo entre 1.0% e 1.6%, para a amassadura 2, um teor entre 0.5% e 0.9% e para a amassadura 3 obtiveram-se teores críticos entre 0.6% e 1.0%. Comparando com o teor máximo de cloretos definido pela norma NP EN 206-1, 0.4%, conclui-se que este valor não é conservador e é um valor para o qual efectivamente ainda não se iniciou a corrosão por cloretos podendo ser utilizado em modelos de previsão do início da corrosão das armaduras. Verificou-se que a não protecção dos varões nas amassaduras, principalmente na amassadura 1, levam a uma maior acessibilidade de oxigénio, provocando “crevice” e corrosão na armadura exposta ao ar, levando a teores críticos de cloretos erróneos. Efectuaram-se registos fotográficos e observações à lupa das armaduras bem como micrografias em MEV e microanálises por dispersão de Raios-X no betão adjacente à armadura. Verificou-se a existência de produtos de corrosão na armadura os quais continham iões cloreto.

OBJETIVO: Examinar a influência do bem-estar espiritual na saúde mental de estudantes universitários. MÉTODOS: Estudo transversal com 464 universitários das áreas de medicina e de direito, de Pelotas, RS. A coleta de dados foi realizada em grupos na sala de aula. Os alunos ausentes foram localizados para responderem individualmente; entretanto, 43 não foram encontrados (9,3% de perda). Utilizou-se um questionário auto-aplicável contendo: escala de bem-estar espiritual (SWBS), SRQ-20 (Self-Reporting Questionnaire) e informações sociodemográficas sobre práticas religiosas/espirituais e sobre a ocorrência de eventos de vida produtores de estresse. Para análise estatística, foram utilizados os testes de qui-quadrado e regressão logística. RESULTADOS: A maioria dos alunos (80%) afirma possuir uma crença espiritual ou religião. O escore médio de bem-estar espiritual foi de 90,4, sendo de 45,6 e 45,1 para as sub-escalas existencial e religiosa, respectivamente. A SWBS apresentou associação com a freqüência a serviços religiosos e práticas espirituais, e não demonstrou ser influenciada por variáveis sociodemográficas e culturais. Indivíduos com bem-estar espiritual baixo e moderado apresentaram o dobro de chances de possuir transtornos psiquiátricos menores (TPM) (OR=0,42; IC95% 0,22-0,85). Sujeitos com bem-estar existencial baixo e moderado apresentaram quase cinco vezes mais TPM (OR=0,19; IC95% 0,08-0,45). CONCLUSÕES: O presente estudo mostrou que o bem-estar espiritual atua como fator protetor para transtornos psiquiátricos menores, sendo a sub-escala de bem-estar existencial a maior responsável pelos resultados obtidos.

The objective of this descriptive study was to map mental health research in Brazil, providing an overview of infrastructure, financing and policies mental health research. As part of the Atlas-Research Project, a WHO initiative to map mental health research in selected low and middle-income countries, this study was carried out between 1998 and 2002. Data collection strategies included evaluation of governmental documents and sites and questionnaires sent to key professionals for providing information about the Brazilian mental health research infrastructure. In the year 2002, the total budget for Health Research was US$101 million, of which US$3.4 million (3.4) was available for Mental Health Research. The main funding sources for mental health research were found to be the São Paulo State Funding Agency (Fapesp, 53.2%) and the Ministry of Education (CAPES, 30.2%). The rate of doctors is 1.7 per 1,000 inhabitants, and the rate of psychiatrists is 2.7 per 100,000 inhabitants estimated 2000 census. In 2002, there were 53 postgraduate courses directed to mental health training in Brazil (43 in psychology, six in psychiatry, three in psychobiology and one in psychiatric nursing), with 1,775 students being trained in Brazil and 67 overseas. There were nine programs including psychiatry, neuropsychiatry, psychobiology and mental health, seven of them implemented in Southern states. During the five-year period, 186 students got a doctoral degree (37 per year) and 637 articles were published in Institute for Scientic Information (ISI)-indexed journals. The investment channeled towards postgraduate and human resource education programs, by means of grants and other forms of research support, has secured the country a modest but continuous insertion in the international knowledge production in the mental health area.

OBJECTIVE: To estimate the direct costs of schizophrenia for the public sector. METHODS: A study was carried out in the state of São Paulo, Brazil, during 1998. Data from the medical literature and governmental research bodies were gathered for estimating the total number of schizophrenia patients covered by the Brazilian Unified Health System. A decision tree was built based on an estimated distribution of patients under different types of psychiatric care. Medical charts from public hospitals and outpatient services were used to estimate the resources used over a one-year period. Direct costs were calculated by attributing monetary values for each resource used. RESULTS: Of all patients, 81.5% were covered by the public sector and distributed as follows: 6.0% in psychiatric hospital admissions, 23.0% in outpatient care, and 71.0% without regular treatment. The total direct cost of schizophrenia was US$191,781,327 (2.2% of the total health care expenditure in the state). Of this total, 11.0% was spent on outpatient care and 79.2% went for inpatient care. CONCLUSIONS: Most schizophrenia patients in the state of São Paulo receive no regular treatment. The study findings point out to the importance of investing in research aimed at improving the resource allocation for the treatment of mental disorders in Brazil.

OBJECTIVE: To assess the extent of mental health scientific production in Brazil from 1999 to 2003, and to identify the nature of the publications generated, their sources of finance and the ways of publicly disseminating the research findings. METHODS: Searches for publications were conducted in the Medline and PsychInfo databases for the period 1999-2003. A semi-structured questionnaire developed by an international team was applied to 626 mental health researchers, covering each interviewee's educational background, research experience, access to funding sources, public impact and research priorities. The sample was composed by 626 mental health researchers identified from 792 publications indexed on Medline and PsychInfo databases for the period above, and from a list of reviewers of Revista Brasileira de Psiquiatria. RESULTS: In Brazil, 792 publications were produced by 525 authors between 1999 and 2003 (441 indexed in Medline and 398 in the ISI database). The main topics were: depression (29.1%), substance misuse (14.6%), psychoses (10%), childhood disorders (7%) and dementia (6.7%). Among the 626 Brazilian mental health researchers, 329 answered the questionnaire. CONCLUSIONS: There were steadily increasing numbers of Brazilian articles on mental health published in foreign journals from 1999 to 2003: the number of articles in Medline tripled and it doubled in the ISI database. The content of these articles corresponded to the priorities within mental health, but there is a need for better interlinking between researchers and mental health policymakers.

OBJETIVO: Analisar o significado da depressão para mulheres diagnosticadas com o transtorno e o contexto do atendimento realizado pelos psiquiatras que as acompanham. MÉTODOS: Estudo qualitativo realizado no município de Embu, na Grande São Paulo, entre agosto de 2002 e janeiro de 2003. Foram realizadas observação etnográfica e entrevistas em profundidade com 16 mulheres diagnosticadas com depressão, pacientes de uma Unidade Básica de Saúde, e quatro psiquiatras. Após a leitura exaustiva, os dados foram agrupados em categorias e analisados. A interpretação dos resultados baseou-se no conceito de cultura. RESULTADOS: As entrevistadas tinham ampla noção do transtorno, aceitando o tratamento com medicação. Para os psiquiatras, a depressão é um termo assimilado pelo senso-comum. Todas as entrevistadas identificaram a origem da doença em eventos passados, como: morte de filho, episódios violentos ligados ao tráfico de drogas, desemprego e agressividade do companheiro. A violência era comum no cotidiano das entrevistadas, tanto fora como dentro de casa. CONCLUSÕES: Para essas mulheres, a depressão era uma forma de expressar sentimentos, como a infelicidade num contexto de pobreza e violência. Os psiquiatras extrapolam as suas funções clínicas e têm um papel na reorganização do cotidiano dessas mulheres.

OBJECTIVE: To analyze the scoring obtained by an instrument, which evaluates the ability to read and understand items in the health care setting, according to education and age. METHODS: The short version of the Test of Functional Health Literacy in Adults was administered to 312 healthy participants of different ages and years of schooling. The study was conducted between 2006 and 2007, in the city of São Paulo, Southeastern Brazil. The test includes actual materials such as pill bottles and appointment slips and measures reading comprehension, assessing the ability to read and correctly pronounce a list of words and understand both prose passages and numerical information. Pearson partial correlations and a multiple regression model were used to verify the association between its scores and education and age. RESULTS: The mean age of the sample was 47.3 years(sd=16.8) and the mean education was 9.7 years(sd=5; range: 1 - 17). A total of 32.4% of the sample showed literacy/numeracy deficits, scoring in the inadequate and marginal functional health literacy ranges. Among the elderly (65 years or older) this rate increased to 51.6%. There was a positive correlation between schooling and scores (r=0.74; p<0.01) and a negative correlation between age and the scores (r=-0.259; p<0.01). The correlation between the scores and age was not significant when the effects of education were held constant (rp=-0.031, p=0.584). A significant association (B=3.877, Beta =0.733; p<0.001) was found between schooling and scores. Age was not a significant predictor in this model (B=-0.035, Beta=-0.22; p=0.584). CONCLUSIONS: The short version of the Test of Functional Health Literacy in Adults was a suitable tool to assess health literacy in the study population. The high number of individuals classified as functional illiterates in this test highlights the importance of special assistance to help them properly understand directions for healthcare.

La tragi-comédie de Corneille, «Le Cid» (1636-7), est davantage une pièce d‘amour qui illustre bien la condition de la femme au XVIIème siècle. Le triangle amoureux constitué par l‘Infante, Chimène et Rodrigue est l‘un des fondements de la pièce. Cependant, comme les personnages sont insérés dans un système féodal basé sur une idéologie aristocratique, l‘amour ne se présente pas comme une jouissance paisible. De cette façon, il y a une lutte acharnée entre l‘amour, le devoir et l‘honneur, ce qui est frappant au niveau lexical. Les personnages les plus jeunes utilisent beaucoup de mots associés au thème de l‘amour et qui ont le radical amour-, tandis que les plus âgés emploient très souvent des mots qui appartiennent au champ lexical de l‘honneur et de la gloire. Selon D. Diègue, l‘honneur a plus de puissance que l‘amour, et il trouve que l‘homme qui s‘endort dans l‘amour oublie ses devoirs. Pour l‘Infante, Chimène et Rodrigue, l‘amour est lié à la souffrance. L‘Infante souffre, mais elle se conforme, parce qu‘étant donné qu‘elle est fille de roi, elle ne peut pas aimer Rodrigue qui appartient à un rang inférieur. Chimène est le revers de l‘Infante, c‘est une femme rebelle qui veut mouler sa destinée. Cependant, tout au long de la pièce, l‘héroïne est assujettie à une force mâle: si parfois l‘amour entre elle et Rodrigue ressemble à l‘amour courtois du Moyen âge quand il se met à la disposition de sa maîtresse et la place au-dessus de lui, la plupart du temps il met la passion en dessous de l‘honneur, parce que c‘est un homme et descend d‘une famille de guerriers vaillants. Selon le code chevaleresque dans lequel il est inséré la femme est inférieure à l‘homme. À la fin de la pièce, il y a une victoire de l‘amour sur l‘honneur et le devoir : Chimène abdique de son honneur en pardonnant et en prenant pour mari l‘assassin de son père. Le monde de l‘élément masculin triomphe et elle devient le prix de la victoire de Rodrigue. Mais si son statut de femme l‘empêche de sortir victorieuse, elle ne se conforme pas et dans sa dernière tirade elle met en question la justice et défie la société féodale.

OBJECTIVE: To assess direct medical costs associated with schizophrenia relapses in mental health services. METHODS: The study was conducted in three health facilities in the city of São Paulo: a public state hospital; a Brazilian National Health System (SUS)-contracted hospital; and a community mental health center. Medical records of 90 patients with schizophrenia who received care in 2006 were reviewed. Information on inpatient expenditures was collected and used for cost estimates. RESULTS: Mean direct medical cost of schizophrenia relapses per patient was US$ 4,083.50 (R$ 8,167.58) in the public state hospital; US$ 2,302.76 (R$ 4,605.46) in the community mental health center; and US$ 1,198.50 (R$ 2,397.74) in the SUS-affiliated hospital. The main component was daily inpatient room rates (87% - 98%). Medication costs varied depending on the use of typical or atypical antipsychotic drugs. Atypical antipsychotic drugs were more often used in the community mental health center. CONCLUSIONS: Costs associated with schizophrenia relapses support investments in antipsychotic drugs and strategies to reduce disease relapse and the need for mental health inpatient services. Treating patients in a community mental health center was associated with medium costs and added the benefit of not depriving these patients from family life.

ABSTRACT OBJECTIVE : To analyze if the demographic and socioeconomic variables, as well as percutaneous coronary intervention are associated with the use of medicines for secondary prevention of acute coronary syndrome. METHODS : In this cohort study, we included 138 patients with acute coronary syndrome, aged 30 years or more and of both sexes. The data were collected at the time of hospital discharge, and after six and twelve months. The outcome of the study was the simultaneous use of medicines recommended for secondary prevention of acute coronary syndrome: platelet antiaggregant, beta-blockers, statins and angiotensin-converting-enzyme inhibitor or angiotensin receptor blocker. The independent variables were: sex, age, education in years of attending, monthly income in tertiles and percutaneous coronary intervention. We described the prevalence of use of each group of medicines with their 95% confidence intervals, as well as the simultaneous use of the four medicines, in all analyzed periods. In the crude analysis, we verified the outcome with the independent variables for each period through the Chi-square test. The adjusted analysis was carried out using Poisson Regression. RESULTS : More than a third of patients (36.2%; 95%CI 28.2;44.3) had the four medicines prescribed at the same time, at the moment of discharge. We did not observe any differences in the prevalence of use in comparison with the two follow-up periods. The most prescribed class of medicines during discharge was platelet antiaggregant (91.3%). In the crude analysis, the demographic and socioeconomic variables were not associated to the outcome in any of the three periods. CONCLUSIONS : The prevalence of simultaneous use of medicines at discharge and in the follow-ups pointed to the under-utilization of this therapy in clinical practice. Intervention strategies are needed to improve the quality of care given to patients that extend beyond the hospital discharge, a critical point of transition in care.
