PURPOSE: The aim of this prospective case series study was to evaluate the short-term success rates of titanium screw-type implants with a chemically modified sand-blasted and acid-etched (mod SLA) surface after 3 weeks of healing. MATERIAL AND METHODS: A total of 56 implants were inserted in the posterior mandible of 40 partially edentulous patients exhibiting bone densities of class I to III. After a healing period of 3 weeks, all implants were functionally loaded with a screw-retained crown or fixed dental prosthesis. The patients were recalled at weeks 4, 7, 12, and 26 for monitoring and assessment of clinical and radiological parameters, including implant stability quotient (ISQ) measurements. RESULTS: None of the implants failed to integrate. However, two implants were considered "spinners" at day 21 and left unloaded for an extended period. Therefore, 96.4% of the inserted implants were loaded according to the protocol tested. All 56 implants including the "spinners" showed favorable clinical and radiographic findings at the 6-month follow-up examination. The ISQ values increased steadily throughout the follow-up period. At the time of implant placement, the range of ISQ values exhibited a mean of 74.33, and by week 26, a mean value of 83.82 was recorded. Based on strict criteria, all 56 implants were considered successfully integrated, resulting in a 6-month survival and success rate of 100.0%. CONCLUSION: This prospective study using an early-loading protocol after 3 weeks of healing demonstrated that titanium implants with the modified SLA surface can achieve and maintain successful tissue integration over a period of at least 6 months. The ISQ method seems feasible to monitor implant stability during the initial wound-healing period.

The aim of this research was to study the impact of loading on partial dentures within the supporting soft tissue with respect to different attachment techniques. A finite element model was developed to calculate the stress and strain distribution in this tissue. The model consisted of the left half of a mandible with three remaining teeth that had suffered an atrophy in the anterior region, and a partial denture over the toothless area that was connected at the left mandibular canine using an attachment system. Resulting stress/strain distributions are presented for different load cases using a commercially available prefabricated attachment system.

Introduction: Chronic atypical neutrophilic dermatosis with lipodystrophy and elevated temperature (CANDLE) is an auto inflammatory syndrome caused by an autosomal recessive gene mutation. This very rare syndrome has been reported in only 14 patients worldwide. A number of clinical signs have been reported including joint contractures, muscle atrophy, microcytic anaemia, and panniculitis-induced childhood lipodystrophy. Further symptoms include recurrent fevers, purpuric skin lesions, periorbital erythema and failure to thrive. This is the first reported case of periodontal manifestations associated with CANDLE syndrome.
Case Presentation: An 11 year old boy was referred to Cork University Dental School and Hospital with evidence of severe periodontal destruction. The patient’s medical condition was managed in Great Ormond Street Children’s Hospital, London. The patient’s dental management included initial treatment to remove teeth of hopeless prognosis followed by prosthodontic rehabilitation using removable partial dentures. This was followed by further non-surgical periodontal treatment and maintenance. In the long term, the potential definitive restorative options, including dental implants, will be evaluated in discussion with the patient’s medical team.
Conclusion: Periodontitis as a manifestation of systemic disease is one of seven categories of periodontitis as defined by the American Academy of Periodontology 1999 classification system. A number of systemic diseases have been associated with advanced periodontal destruction including Diabetes Mellitus, Leukaemia and Papillon-Lefevre Syndrome. In the case described, treatment necessitated a multidisciplinary approach with input from medical and dental specialities for a young patient with severe periodontal destruction associated with CANDLE syndrome.
La relación entre la calidad de la enseñanza y la mala práctica odontológica: relato de caso clínico

Las actividades desarrolladas en el área de la salud son de gran importancia, pues tienen como finalidad preservar la vida de los individuos y para eso deben ser desempeñadas por personas habilitadas. El aumento del número de facultades de Odontología, la disminución en la calidad de la enseñanza y el mayor ingreso de alumnos con baja capacidad para ejercer la profesión son hechos que traen consecuencias desastrosas para la sociedad. Estos hechos también se reflejan en el desempeño moral, ético y técnico-científico del profesional. El objetivo de ese relato de caso clínico es enseñar que, aunque hay una considerable oferta de instituciones de enseñanza, existe un desconocimiento científico y de técnicas adecuadas por parte de los cirujanos dentistas graduados. La paciente MSL, 17 años, sexo femenino, compareció al consultorio odontológico usando un molde tipo Vernis, sujeto al arco inferior. Tras evaluación clínica, se observó la utilización de un material incorrecto para la toma de impresión, siendo imposible su retiro con el método convencional. La planificación para su retiro se hizo por medio de seccionamiento. De esta manera, la consecuencia de la falta de conocimiento en el uso de materiales para la impresión causó gran trastorno a la paciente. Se puede concluir que el índice de impericia está relacionado directamente con la formación profesional, evidenciando la importancia de la enseñanza odontológica de calidad para una práctica clínica responsable.

Small-scale dairy systems play an important role in the Mexican dairy sector and farm planning activities related to resource allocation have a significant impact on the profitability of such enterprises. Linear programming is a technique widely used for planning and ration formulation, and partial budgeting is a technique for assessing the impact of changes on the profitability of an enterprise. This study used both methods to optimise land use for forage production and nutrient availability, and to evaluate the economic impact of such changes in small-scale Mexican dairy systems. The model showed satisfactory performance when optimal solutions were compared with the traditional strategy. The strategy using fresh ryegrass, maize silage and oat hay, and the strategy using a combination of alfalfa hay, maize silage, fresh ryegrass and oat hay appeared attractive options for providing a better nutrient supply and maintaining a higher stocking rate throughout the year than the traditional strategy.

Background: The role of osteocytes in bone structure and function remains partially unresolved. Their participation in mechanotransduction, i.e., the conversion of a physical stimulus into a cellular response, has been hypothesized. The present study was an evaluation of the osteocyte density in the peri-implant bone of immediately loaded and submerged dental implants. Methods: Fourteen male patients were included in the study; all of them were partially edentulous and needed a posterior mandibular restoration. Implants were inserted in these areas; half of the sample was loaded immediately (included in a fixed provisional prosthesis on the same day as implant surgery), whereas the other half was left to heal submerged. Fourteen implants (seven immediately loaded and seven unloaded) were retrieved with a trephine after a healing period of 8 weeks. The specimens were treated to obtain thin ground sections, and histomorphometry was used to evaluate the osteocyte index in the peri-implant bone. Results: A higher and statistically significant number of osteocytes was found in the peri-implant bone around immediately loaded implants (P=0.0081). A correlation between the percentage of bone-implant contact and osteocyte density was found for immediately loaded implants (P=0.0480) but not for submerged implants (P=0.2667). Conclusion: The higher number of osteocytes in the peri-implant bone around immediately loaded implants could be related to the functional adaptation required by the loading stimulus, which also explains the hypothesized involvement of the osteocytes in the maintenance of the bone matrix. Objectives: To evaluate the use of the center of the incisive papilla as a guide for the selection of the proper width of maxillary dentures in 4 racial groups. Method and Materials: One hundred sixty stone casts were obtained from impressions of the maxillary arch of white, black, mixed, and Asian subjects. The occlusal surfaces of the casts were photocopied and the images placed on a digitizer. The most anterior and posterior points of the papilla and cusp tips of the canines were digitized. Dentofacial Planner Plus software was used to calculate the distance from a line passing through the cusp tips of the canines to the center of the papilla, defined as the midpoint of the anterior and posterior points of the papilla. The selection error (in millimeters) due to the clinical application of the method of the incisive papilla was calculated and analyzed. Results: In all studied racial groups, there was no coincidence between the center of the incisive papilla and the canine line. The utilization of the center of the papilla would lead to the selection of wider artificial teeth. In 24.9% of the white, 19.3% of the mixed, 32.9% of the black, and 15.5% of the Asian populations, errors greater than 4 mm would be present with the utilization of the papilla. Conclusion: The method of the center of the incisive papilla is not accurate, but may aid in initial artificial teeth selection for the racial groups studied. (Quintessence Int 2008;39:841-845)

OBJECTIVE: Early identification of craniofacial morphological characteristics allows orthopedic segmented interventions to attenuate dentoskeletal discrepancies, which may be partially disguised by natural dental compensation. To investigate the morphological characteristics of Brazilian children with Class III malocclusion, in stages I and II of cervical vertebrae maturation and compare them with the characteristics of Class I control patients. METHODS: Pre-orthodontic treatment records of 20 patients with Class III malocclusion and 20 control Class I patients, matched by the same skeletal maturity index and sex, were selected. The craniofacial structures and their relationships were divided into different categories for analysis. Angular and linear measures were adopted from the analyses previously described by Downs, Jarabak, Jacobson and McNamara. The differences found between the groups of Class III patients and Class I control group, both subdivided according to the stage of cervical vertebrae maturation (I or II), were assessed by analysis of variance (ANOVA), complemented by Bonferroni's multiple mean comparisons test. RESULTS: The analysis of variance showed statistically significant differences in the different studied groups, between the mean values found for some angular (SNA, SNB, ANB) and linear variables (Co - Gn, N - Perp Pog, Go - Me, Wits, S - Go, Ar - Go). CONCLUSION: Assessed children displaying Class III malocclusion show normal anterior base of skull and maxilla, and anterior positioning of the mandible partially related to increased posterior facial height with consequent mandibular counterclockwise rotation.

Being the surface roughness a fundamental property in the fit of the castings, it was determined the roughness of 5 high-copper casting alloys, according to the heating over their melting temperature. That way, the specimens made with smooth plastic in the shape of a rectangular plate, 2 mm x 7 mm x 11 mm were invested into a cristobalite investment. After heating and elimination on the plastic, the molds were filled by 3 g of molten alloy, casted in a electrical casting machine at the casting temperature at 25 degrees C and 50 degrees C overheating. The surface roughness was measured in a roughness analyser, Talysurf. As a result, it was verified that there is a variation of surface roughness among the alloys tested, and the heating of the alloys until 50 degrees C over the melting temperature does not interfere in the roughness.

It was evaluated the castability of four copper-aluminium alloy according the melting casting method used. The specimens were made using polyester mesh screen, with 11 x 11 filaments of 0.26 mm thick, fixed along of two adjacent edges in wax bar, with the sprue attached at their junction. The alloys were in an electrical casting machine and a centrifugal casting machine with an air/gas torch. The castability values were obtained by the percentage of completed segments of the resulting cast alloy screen. It was verified that the use of the electrical casting machine produced higher castability values to the copper-aluminium alloys than those produced by a centrifugal casting machine with an air/gas torch.

It was evaluated the Vickers hardness of five high-copper casting alloys, in their original package form and after casting, according to the casting method used. That way, ten ingots, supplied by the manufacturers of each alloy, were included in self-curing acrylic resin, polished, numerated and submitted to Vickers hardness test at load of 200 g during 30 seconds. Afterwards the numerated ingots were removed from the acrylic resin and five of those were cast in an electrical casting machine and the other five in a centrifugal casting machine with an air/gas torch. The specimens obtained were included in self-curing acrylic resin, polished and submitted to Vickers hardness test. As a result it was verified that there is a variation of hardness among the alloys tested, and the use of the electrical casting machine produced lower hardness values than those produced when used the centrifugal casting machine with an air/gas torch. Also, there is a decrease of hardness of the cast alloys when they are tested in their original form and after casting.
