Groundwater samples were analysed for Rn-222, Ra-226, and Ra-228 in Guarani aquifer spreading around I million kin 2 within four countries in South America, and it was found that their activity concentrations are lognormally distributed. Population-weighted average activity concentration for these radionuclides allowed to estimate a value: either slightly higher (0.13 mSv/year) than 0.1 mSv for the total effective dose or two times higher (0.21 mSv/year) than this limit, depending on the choice of the dose conversion factor. Such calculation adds useful information for the appropriate management of this transboundary aquifer that is socially and economically very important to about 15 million inhabitants living in Brazil, Argentina, Uruguay and Paraguay. This investigation was carried out within the Parana sedimentary basin, Brazil, involved the sampling of groundwater and air, and was realized with the purpose of evaluating the radioactivity, due to radon gas, in a thermal spa utilizing the waters from Guarani (Botucatu-Piramboia) aquifer. The results reported here provide additional information relative to that of previous studies focusing on the presence of radionuclides in the aquifer, which have mainly characterized those belonging to uranium and thorium series decay, such as the uranium isotopes (U-238 and U-234), radium isotopes (Ra-226 and Ra-228), radon daughters (Bi-214 and Pb-214) and radon (Rn-222) itself the results obtained were compared with the maximum permissible concentration limits in air and drinking water defined by international standards, such as the guidelines for drinking water quality established by the World Health Organization. The possible processes responsible for the presence of radon in the aquifer were also considered in order to evaluate the data obtained.

Environmental biomonitoring has demonstrated that organisms such as crustaceans, fish and mushrooms are useful to evaluate and monitor both ecosystem contamination and quality. Particularly, some mushroom species have a high capacity to retain radionuclides and some toxic elements from the soil and the air. The potential of mushrooms to accumulate radionuclides in their fruit-bodies has been well documented. However, there are no studies that determine natural and artificial radionuclide composition in edible mushrooms, in Brazil. Artificial (Cs-137) and natural radioactivity (K-40. Ra-226. Ra-228) were determined in 17 mushroom samples from 3 commercialized edible mushroom species. The edible mushrooms collected were Agaricus sp., Pleurotus sp. and Lentinula sp. species. The activity measurements were carried out by gamma spectrometry. The levels of Cs-137 varied from 1.45 +/- 0.04 to 10.6 +/- 0.3 Bq kg(-1), K-40 levels varied from 461 +/- 2 to 1535 +/- 10 Bq kg(-1), Ra-226 levels varied from 14 +/- 3 to 66 +/- 12 Bq kg(-1) and Ra-228 levels varied from 6.2 +/- 0.2 to 54.2 +/- 1.7 Bq kg(-1). Cs-137 levels in Brazilian mushrooms are in accordance with the radioactive fallout in the Southern Hemisphere. The artificial and natural activities determined in this study were found to be below the maximum permissible levels as established by national legislation. Thus, these mushroom species can be normally consumed by the population without any apparent risks to human health.

BACKGROUND: This study investigated the role of a negative FAST in the diagnostic and therapeutic algorithm of multiply injured patients with liver or splenic lesions. METHODS: A retrospective analysis of 226 multiply injured patients with liver or splenic lesions treated at Bern University Hospital, Switzerland. RESULTS: FAST failed to detect free fluid or organ lesions in 45 of 226 patients with spleen or liver injuries (sensitivity 80.1%). Overall specificity was 99.5%. The positive and negative predictive values were 99.4% and 83.3%. The overall likelihood ratios for a positive and negative FAST were 160.2 and 0.2. Grade III-V organ lesions were detected more frequently than grade I and II lesions. Without the additional diagnostic accuracy of a CT scan, the mean ISS of the FAST-false-negative patients would be significantly underestimated and 7 previously unsuspected intra-abdominal injuries would have been missed. CONCLUSION: FAST is an expedient tool for the primary assessment of polytraumatized patients to rule out high grade intra-abdominal injuries. However, the low overall diagnostic sensitivity of FAST may lead to underestimated injury patterns and delayed complications may occur. Hence, in hemodynamically stable patients with abdominal trauma, an early CT scan should be considered and one must be aware of the potential shortcomings of a "negative FAST".

AIMS Follicular thyroid carcinoma (FTC) has been a diagnostic challenge for decades. The PAX8-PPARγ rearrangement has been detected in FTC and classic papillary thyroid carcinomas (PTCs). The aims of this study were to assess the presence of PAX8-PPARγ by using tissue microarrays in a large cohort of different thyroid neoplasms, and to assess its diagnostic and prognostic implications. METHODS AND RESULTS Fluorescence in-situ hybridization (FISH) analysis for PAX8-PPARγ was performed on 226 thyroid tumours, comprising FTCs (n = 59), PTCs (n = 126), poorly differentiated thyroid carcinomas (PDs; n = 34), follicular thyroid adenomas (FTAs; n = 5), and follicular tumours of unknown malignant potential (FTUMPs; n = 2). PAX8-PPARγ was detected in 12% of FTCs, 1% of PTCs, 7% of PDs, and in both cases of FTUMP. There was no correlation between the extent of capsular or vascular invasion and PAX8-PPARγ, or between lymph node or haematogenous metastasis and PAX8-PPARγ. Overall survival (OS), tumour-specific survival (TSS) and relapse-free-survival (RFS) were not influenced by PAX8-PPARγ. CONCLUSIONS In this study, we demonstrate for the first time the presence of PAX8-PPARγ in PDs and FTUMPs, whereas in FTCs and PTCs the prevalence of PAX8-PPARγ is lower than previously reported. PAX8-PPARγ did not correlate with invasiveness or affect prognosis in any tumour type.
