Limited open reduction and internal fixation of displaced intra-articular fractures of the calcaneum

The extended lateral L-shaped approach for the treatment of displaced intra-articular fractures of the calcaneum may be complicated by wound infection, haematoma, dehiscence and injury to the sural nerve. In an effort to reduce the risk of problems with wound healing a technique was developed that combined open reduction and fixation of the joint fragments and of the anterior process with percutaneous reduction and screw fixation of the tuberosity. A group of 24 patients with unilateral isolated closed Sanders type II and III fractures was treated using this technique and compared to a similar group of 26 patients managed by the extended approach and lateral plating. The operation was significantly shorter (p < 0.001) in the first group, but more minor secondary procedures and removal of heel screws were necessary. There were no wound complications in this group, whereas four minor complications occurred in the second group. The accuracy and maintenance of reduction, and ultimate function were equivalent.

INTRODUCTION: We studied intra-individual and inter-individual variability of two online sedation monitors, BIS and Entropy, in volunteers under sedation. METHODS: Ten healthy volunteers were sedated in a stepwise manner with doses of either midazolam and remifentanil or dexmedetomidine and remifentanil. One week later the procedure was repeated with the remaining drug combination. The doses were adjusted to achieve three different sedation levels (Ramsay Scores 2, 3 and 4) and controlled by a computer-driven drug-delivery system to maintain stable plasma concentrations of the drugs. At each level of sedation, BIS and Entropy (response entropy and state entropy) values were recorded for 20 minutes. Baseline recordings were obtained before the sedative medications were administered. RESULTS: Both inter-individual and intra-individual variability increased as the sedation level deepened. Entropy values showed greater variability than BIS(R) values, and the variability was greater during dexmedetomidine/remifentanil sedation than during midazolam/remifentanil sedation. CONCLUSIONS: The large intra-individual and inter-individual variability of BIS and Entropy values in sedated volunteers makes the determination of sedation levels by processed electroencephalogram (EEG) variables impossible. Reports in the literature which draw conclusions based on processed EEG variables obtained from sedated intensive care unit (ICU) patients may be inaccurate due to this variability. Chemistry data from 16, 50-115 m deep, sub-annually dated ice cores are used to investigate spatial and temporal concentration variability of sea-salt (ss) SO42- and excess (xs) SO42- over West Antarctica and the South Pole for the last 200 years. Low-elevation ice-core sites in western West Antarctica contain higher concentrations Of SO42- as a result of cyclogenesis over the Ross Ice Shelf and proximity to the Ross Sea Polynya. Linear correlation analysis of 15 West Antarctic ice-core SO42- time series demonstrates that at several sites concentrations Of ssSO(4)(2-) are higher when sea-ice (SIE) extent is greater, and the inverse for XSS04. Concentrations Of XSS04 from the South Pole site (East Antarctica) are associated with SIE from the Weddell region, and West Antarctic XSSO42- concentrations are associated with SIE from the Bellingshausen-Amundsen-Ross region. The only notable rise of the last 200 years in xsSO(4)(2-), around 1940, is not related to SIE fluctuations and is most likely a result of increased xsSO(4)(2-) production in the mid-low latitudes and/or an increase in transport efficiency from the mid-low latitudes to central West Antarctica. These high-resolution records show that the source types and source areas Of ssSO(4)(2-) and xsSO(4)(2-) delivered to eastern and western West Antarctica and the South Pole differ from site to site but can best be resolved using records from spatial ice-core arrays such as the International Trans-Antarctic Scientific Expedition (ITASE).

The field of molecule-based magnets is a relatively new branch of chemistry, which involves the design and study of molecular compounds that exhibit a spontaneous magnetic ordering below a critical temperature, Tc. One major goal involves the design of materials with tuneable Tc's for specific applications in memory storage devices. Molecule-based magnets with high magnetic ordering temperatures have recently been obtained from bimetallic and mixed-valence transition metal μ-cyanide complexes of the Prussian blue family. Since the μ-cyanide linkages permit an interaction between paramagnetic metal ions, cyanometalate building blocks have found useful applications in the field of molecule-based magnets. Our work involves the use of octacyanometalate building blocks for the self-assembly of two new classes of magnetic materials namely, high-spin molecular clusters which exhibit both ferromagnetic intra- and intercluster coupling, and specific extended network topologies which show long-range ferromagnetic ordering.
