O presente Relatório de Estágio apresenta os resultados de 800 horas de estágio realizado na Direcção de Serviços do Médio Oriente e Magrebe â Ministério dos Negócios Estrangeiros. Este focou-se maioritariamente na organização e actualização dos instrumentos bilaterais entre Portugal e os países MOM, sendo pontuado também por pesquisas ocasionais relativas à realidade política interna e/ou externa destes mesmos países. Primeiramente apresenta-se o enquadramento da DSMOM no Ministério dos Negócios Estrangeiros, bem como as suas atribuições e caracterização, descrevendo-se e analisando-se posteriormente as tarefas realizadas. Propõe-se ainda um breve enquadramento da relevância dos países MOM na política externa portuguesa

INTRODUCTION: Chlamydia infection is associated with debilitating human diseases including trachoma, pneumonia, coronary heart disease and urogenital diseases. Serotypes of C. trachomatis show a fair correlation with the group of diseases they cause, and their distribution follows a well-described geographic pattern. Serotype A, a trachoma-associated strain, is known for its limited dissemination in the Middle East and Northern Africa. However, knowledge on the spread of bacteria from the genus Chlamydia as well as the distribution of serotypes in Brazil is quite limited. METHODS: Blood samples of 1,710 individuals from ten human population groups in the Amazon region of Brazil were examined for antibodies to Chlamydia using indirect immunofluorescence and microimmunofluorescence assays. RESULTS: The prevalence of antibodies to Chlamydia ranged from 23.9% (Wayana-Apalai) to 90.7% (Awa-Guaja) with a mean prevalence of 50.2%. Seroreactivity was detected to C. pneumoniae and to all serotypes of C. trachomatis tested; furthermore, we report clear evidence of the as-yet-undescribed occurrence of serotype A of C. trachomatis. CONCLUSIONS: Specific seroreactivity not only accounts for the large extent of dissemination of C. trachomatis in the Amazon region of Brazil but also shows an expanded area of occurrence of serotype A outside the epidemiological settings previously described. Furthermore, these data suggest possible routes of Chlamydia introduction into the Amazon region from the massive human migration that occurred during the 1,700s.

An earlier study revealed the strong phylogeographical structure of the lesser white-toothed shrew (Crocidura suaveolens group) within the northern Palaearctic. Here, we aim to reconstruct the colonization history of Mediterranean islands and to clarify the biogeography and phylogeographical relationships of the poorly documented Middle East region with the northern Palaearctic. We performed analyses on 998-bp-long haplotypes of the mitochondrial cytochrome b gene of 143 samples collected around the Mediterranean basin, including islands and the Middle East. The analyses suggest that the Cypriot shrew belongs to the rare group of relict insular Pleistocene mammal taxa that have survived to the present day. In contrast, the Cretan, Corsican and Menorcan populations were independently introduced from the Middle East during the Holocene. The phylogeographical structure of this temperate Palaearctic species within the Middle East appears to be complex and rich in diversity, probably reflecting fragmentation of the area by numerous mountain chains. Four deeply divergent clades of the C. suaveolens group occur in the area, meaning that a hypothetical contact zone remains to be located in central western Iran.

Il y a environ 125 millions d'années, au Crétacé inférieur, la position des continents et le climat terrestre étaient bien différents de ce que l'on connait aujourd'hui. Le Sud-Est de la France, secteur de cette étude, était alors recouvert d'eau, sous un climat chaud et humide. Sur la bordure de cette étendue d'eau (appelée bassin Vocontien), qui correspond aujourd'hui aux régions de la Provence, du Vaucluse, du Gard, de l'Ardèche et du Vercors, des plateformes carbonatées, (telles que les Bahamas), se développaient. Le calcaire, formé à partir des sédiments accumulés sur ces plateformes, est appelé Urgonien. L'objectif de cette étude est de définir les facteurs qui ont influencé le développement de cette plateforme carbonatée dite « urgonienne » et dans quelle mesure. Plusieurs missions de terrain ont permis de récolter de nombreux échantillons de roche en 52 lieux répartis sur l'ensemble du Sud-Est de la France. Les observations réalisées sur le terrain ainsi que les données acquises en laboratoire (microfaune, microfacies et données géo-chimiques) ont permis, de subdiviser chacune des 52 séries urgoniennes en séquences stratigraphiques et cortèges sédimentaires. La comparaison des épaisseurs et des faciès de chaque cortège sédimentaire permet de concevoir la géométrie et l'évolution paléogéographique de la plateforme urgonienne. Les résultats de cette étude démontrent que son organisation est principalement dirigée par des failles qui ont jouées pendant le dépôt des sédiments. Sur la bordure nord du bassin Vocontien, trois failles subméridiennes contrôlent la géométrie et la répartition des environnements de dépôt. Sur sa bordure sud, ces failles synsédimentaires d'orientation N30° et N110° délimitent des blocs basculés. En tête de bloc, des séries d'épaisseurs réduites à faciès de lagon interne se sont déposées alors que les pieds de blocs sont caractérisés par des épaisseurs importantes et la présence de faciès plus externes. Ces concepts ont ensuite été testés en construisant un modèle numérique en trois dimensions de l'Urgonien du Sud-Est de la France. Sa cohérence avec les données acquises tout au long de cette étude d'une part, et sa cohérence géométrique d'autre part, valide les théories avancées. Des formations équivalentes à l'Urgonien sont réparties dans le monde entier et notamment au Moyen-Orient où elles constituent les réservoirs pétroliers les plus importants. Etre capable de caractériser les facteurs ayant influencé son architecture permet par la suite une meilleure exploitation de ses ressources énergétiques. -- Au Crétacé inférieur, l'intense activité magmatique due à la dislocation du super-continent Pangée influence fortement les conditions environnementales globales. Au Barrémien terminal et Aptien basal, période géologique dont fait l'objet cette étude, le bassin Vocontien, puis Bédoulien, recouvre le Sud-Est de la France, sous un climat chaud et humide. Sur les bordures de ces bassins, des plateformes carbonatées se mettent en place. Les sédiments qui se déposent sur ces plateformes sont à l'origine de la formation urgonienne. Afin d'étudier cette formation, une charte biostratigraphique, principalement basée sur les Orbitolinidés, et un modèle de faciès ont été développés. Les assemblages faunistiques, la succession des faciès, les observations de terrain ainsi que l'étude de signaux géochimiques ont permis le découpage séquentiel de la série urgonienne le long de 54 coupes et puis, répartis sur l'ensemble du Sud-Est de la France. Les corrélations induites par cette étude stratigraphique ont mis en évidence d'importantes variations d'épaisseur et d'environnements de dépôt au sein même de la plateforme urgonienne. Ces variations sont expliquées par le jeu de failles syn-sédimentaires qui ont compartimentées la plateforme urgonienne en blocs. Sur la bordure sud du bassin Vocontien, ces failles d'orientation N30° et N110° délimitent six blocs basculés. Au sommet du Barrémien terminal, la subsidence des blocs situés le plus au sud s'amplifie jusqu'à provoquer l'ouverture du bassin de la Bédoule au sud du secteur d'étude. Cette théorie d'évolution a ensuite été testée par l'élaboration d'un modèle numérique en trois dimensions de l'Urgonien du Sud-Est de la France. Sa cohérence avec les données acquises tout au long de cette étude d'une part, et sa cohérence géométrique d'autre part, valide les théories avancées. Des analogues de l'Urgonien sont répartis dans le monde entier et notamment au Moyen-Orient où ils représentent d'importants réservoirs pétroliers. Être capable de caractériser les facteurs ayant influencé l'architecture de l'Urgonien du Sud-Est de la France permet par la suite une meilleure exploitation de ses ressources énergétiques. -- During the Early Cretaceous epoch, intensive magmatic activity due to the dislocation of the super-continent Pangaea, highly influenced global environmental conditions, which were characterized by a warm and generally humic climate. In this context, carbonate platforms were important in tropical and subtropical shallow-water regions, and especially during the late Barremian and early Aptian, platform carbonates of so-called Urgonian affinity are widespread. In southeastern France, the Urgonian platform was part of the northern Tethyan margin and bordered the Vocontian and the Bedoulian basins. The goal of this thesis was the systematic study of the Urgonian Formation in this region, and in order to achieve this goal, a biostratigraphic chart and a facies model were developed. The faunistic assemblages, the facies succession, the field observations and the study of geochemical signals lead to a sequential subdivision of the Urgonian series along 54 sections and wells allocated in five different regions in southeastern France (Gard, Ardèche, Vercors, Vaucluse and Provence). Correlations from this stratigraphic study highlight important variations in thickness and depositional environments of the Urgonian series. These variations are explained by relative movements induced by syn-sedimentary faults, which divided the Urgonian platforms into blocks. On the southern border of the Vocontian basin, these faults, oriented N30° and N110°, delineate six tilted blocks. At the top of the upper Barremian carbonates, subsidence of the two southern blocks accelerated leading to the opening of the Bedoulian basin. The reconstruction of the sequence-stratigraphic and paleoenvironmental evolution of the Urgonian platforms was then tested by the construction of a 3D numerical model of the Urgonian formation of southeastern France. Firstly, its consistency with the data collected during this study, and secondly, its geometrical coherence validate the proposed theory. Urgonian analogs exist all over the world and particularly in Middle East where they constitute important oil reservoirs. The exact reconstruction of the major factors, which influenced the architecture of these formations, will allow for a better exploitation of these energy resources.

The location and timing of domestication of the olive tree, a key crop in Early Mediterranean societies, remain hotly debated. Here, we unravel the history of wild olives (oleasters), and then infer the primary origins of the domesticated olive. Phylogeography and Bayesian molecular dating analyses based on plastid genome profiling of 1263 oleasters and 534 cultivated genotypes reveal three main lineages of pre-Quaternary origin. Regional hotspots of plastid diversity, species distribution modelling and macrofossils support the existence of three long-term refugia; namely the Near East (including Cyprus), the Aegean area and the Strait of Gibraltar. These ancestral wild gene pools have provided the essential foundations for cultivated olive breeding. Comparison of the geographical pattern of plastid diversity between wild and cultivated olives indicates the cradle of first domestication in the northern Levant followed by dispersals across the Mediterranean basin in parallel with the expansion of civilizations and human exchanges in this part of the world.

My interest in higher education and citizenship in the Middle East at large and in Jordan in particular is fostered by some of the reflections Eickelman proposed (1992). Being a quite recent phenomenon, intimately linked with the more general topic of state formation it seemed to me more suitable to study it in a little country with a recent history (a field study left almost unexplored until now as far as Jordan is concerned, to the best of my knowledge, since Antoun 1994 focuses on the migration as a quest for higher education). The process of state formation in Jordan is quite studied. I thus intended to study the higher education policies as an attempt both to create a national citizenry and more recently as a way of controlling the more problematic part of the population (youth, which constitutes more than the double of the population. See UNDP and Ministry of Planning 2000). How do the young students enter the university system, and in which way does this system work? How is this system designed, in order to retain social control of the students (since they are usually perceived to be a factor of social and political instability, as in Iran or in Egypt)? Is there any significant difference between different faculties? And if so, why? My conclusions at this stage are that the university system is an integral part of the survival of the regime. The system works quite well, and Jordan has one of the best educational position in the region. Yet there are important distinctions to be made: the access to the better faculties is socially selective while the less valued faculties are left to the poorer and less wealthy youth. This results in a different treatment of the students and of the courses that I analysed. In the better faculties the teaching standards are quite high, and the relationship between professors and students is almost on a same-level base, while in the less privileged faculties the opposite is true. Thus we can observe a concrete politics of divide et impera intended to split the youth in two. For the more privileged there are some freedoms, both within and outside classes, designed I guess at forging them as autonomous individuals. On the opposite the less privileged are kept under tight control, even if also these students are a privileged category among youth at large.

The ancestors of present-day man (Homo sapiens sapiens) appeared in East Africa some three and a half million years ago (Australopithecs), and then migrated to Europe, Asia, and later to the Americas, thus beginning the differentiation process. The passage from nomadic to sedentary life took place in the Middle East in around 8000 BC. Wars, spontaneous migrations and forced migrations (slave trade) led to enormous mixtures of populations in Europe and Africa and favoured the spread of numerous parasitic diseases with specific strains according to geographic area. The three human plasmodia (Plasmodium falciparum, P. vivax, and P. malariae) were imported from Africa into the Mediterranean region with the first human migrations, but it was the Neolithic revolution (sedentarisation, irrigation, population increase) which brought about actual foci for malaria. The reservoir for Leishmania infantum and L. donovani - the dog - has been domesticated for thousands of years. Wild rodents as reservoirs of L. major have also long been in contact with man and probably were imported from tropical Africa across the Sahara. L. tropica, by contrast, followed the migrations of man, its only reservoir. L. infantum and L. donovani spread with man and his dogs from West Africa. Likewise, for thousands of years, the dog has played an important role in the spread and the endemic character of hydatidosis through sheep (in Europe and North Africa) and dromadary (in the Sahara and North Africa). Schistosoma haematobium and S. mansoni have existed since prehistoric times in populations living in or passing through the Sahara. These populations then transported them to countries of Northern Africa where the specific, intermediary hosts were already present. Madagascar was inhabited by populations of Indonesian origin who imported lymphatic filariosis across the Indian Ocean (possibly of African origin since the Indonesian sailors had spent time on the African coast before reaching Madagascar). Migrants coming from Africa and Arabia brought with them the two African forms of bilharziosis: S. haematobium and S. mansoni.

BACKGROUND: The incidence of and mortality from alcohol-related conditions, liver disease and hepatocellular cancer (HCC) are increasing in the UK. We compared mortality rates by country of birth to explore potential inequalities and inform clinical and preventive care. DESIGN: Analysis of mortality for people aged 20 years and over using the 2001 Census data and death data from 1999 and 2001-2003. SETTING: England and Wales. MAIN OUTCOME MEASURES: Standardized mortality ratios (SMRs) for alcohol-related deaths and HCC. RESULTS: Mortality from alcohol-related deaths (23 502 deaths) was particularly high for people born in Ireland (SMR for men [M]: 236, 95% confidence interval [CI]: 219-254; SMR for women [F]: 212, 95% CI: 191-235) and Scotland (SMR-M: 187, CI: 173-213; SMR-F 182, CI: 163-205) and men born in India (SMR-M: 161, CI: 144-181). Low alcohol-related mortality was found in women born in other countries and men born in Bangladesh, Middle East, West Africa, Pakistan, China and Hong Kong, and the West Indies. Similar mortality patterns were observed by country of birth for alcoholic liver disease and other liver diseases. Mortality from HCC (8266 deaths) was particularly high for people born in Bangladesh (SMR-M: 523, CI: 380-701; SMR-F: 319, CI: 146-605), China and Hong Kong (SMR-M: 492, CI: 168-667; SMR-F: 323, CI: 184-524), West Africa (SMR-M: 440, CI, 308-609; SMR-F: 319, CI: 165-557) and Pakistan (SMR-M: 216, CI: 113-287; SMR-F: 215, CI: 133-319). CONCLUSIONS: These findings show persistent differences in mortality by country of birth for both alcohol-related and HCC deaths and have important clinical and public health implications. BACKGROUND: Data for trends in glycaemia and diabetes prevalence are needed to understand the effects of diet and lifestyle within populations, assess the performance of interventions, and plan health services. No consistent and comparable global analysis of trends has been done. We estimated trends and their uncertainties in mean fasting plasma glucose (FPG) and diabetes prevalence for adults aged 25 years and older in 199 countries and territories. METHODS: We obtained data from health examination surveys and epidemiological studies (370 country-years and 2·7 million participants). We converted systematically between different glycaemic metrics. For each sex, we used a Bayesian hierarchical model to estimate mean FPG and its uncertainty by age, country, and year, accounting for whether a study was nationally, subnationally, or community representative. FINDINGS: In 2008, global age-standardised mean FPG was 5·50 mmol/L (95% uncertainty interval 5·37-5·63) for men and 5·42 mmol/L (5·29-5·54) for women, having risen by 0·07 mmol/L and 0·09 mmol/L per decade, respectively. Age-standardised adult diabetes prevalence was 9·8% (8·6-11·2) in men and 9·2% (8·0-10·5) in women in 2008, up from 8·3% (6·5-10·4) and 7·5% (5·8-9·6) in 1980. The number of people with diabetes increased from 153 (127-182) million in 1980, to 347 (314-382) million in 2008. We recorded almost no change in mean FPG in east and southeast Asia and central and eastern Europe. Oceania had the largest rise, and the highest mean FPG (6·09 mmol/L, 5·73-6·49 for men; 6·08 mmol/L, 5·72-6·46 for women) and diabetes prevalence (15·5%, 11·6-20·1 for men; and 15·9%, 12·1-20·5 for women) in 2008. Mean FPG and diabetes prevalence in 2008 were also high in south Asia, Latin America and the Caribbean, and central Asia, north Africa, and the Middle East. Mean FPG in 2008 was lowest in sub-Saharan Africa, east and southeast Asia, and high-income Asia-Pacific. In high-income subregions, western Europe had the smallest rise, 0·07 mmol/L per decade for men and 0·03 mmol/L per decade for women; North America had the largest rise, 0·18 mmol/L per decade for men and 0·14 mmol/L per decade for women. INTERPRETATION: Glycaemia and diabetes are rising globally, driven both by population growth and ageing and by increasing age-specific prevalences. Effective preventive interventions are needed, and health systems should prepare to detect and manage diabetes and its sequelae. FUNDING: Bill & Melinda Gates Foundation and WHO.

Urinary schistosomiasis remains a significant burden for Africa and the Middle East. The success of population-based control programs will depend on their impact, over many years, on Schistosoma haematobium reinfection and associated disease. In a multi-year (1984-1992) control program in Kenya, we examined risk for S. haematobium reinfection and late disease during and after annual school-based treatment. In this setting, long-term risk of new infection was independently associated with location, age, hematuria, and incomplete treatment, but not with sex or frequency of water contact. Thus, very local environmental features and age-related factors played an important role in S. haematobium transmission, such that population-based control programs should optimally tailor their efforts to local conditions on a village-by-village basis. In 2001-2002, the late benefits of earlier participation in school-based antischistosomal therapy were estimated in a cohort of formerly-treated adult residents compared to never-treated adults from the same villages. Among age-matched subjects, current infection prevalence was lower among those who had received remote therapy. In addition, prevalence of bladder abnormality was lower in the treated group, who were free of severe bladder disease. Treatment of affected adults resulted in rapid resolution of infection and any detectable bladder abnormalities. We conclude that continued treatment into adulthood, as well as efforts at long-term prevention of infection (transmission control) are necessary to achieve optimal morbidity control in affected communities.

There is little literature about the clinical presentation and time-course of postoperative venous thromboembolism (VTE) in different surgical procedures. RIETE is an ongoing, prospective registry of consecutive patients with objectively confirmed, symptomatic acute VTE. In this analysis, we analysed the baseline characteristics, thromboprophylaxis and therapeutic patterns, time-course, and three-month outcome of all patients with postoperative VTE. As of January 2006, there were 1,602 patients with postoperative VTE in RIETE: 393 (25%) after major orthopaedic surgery (145 elective hip arthroplasty, 126 knee arthroplasty, 122 hip fracture); 207 (13%) after cancer surgery; 1,002 (63%) after other procedures. The percentage of patients presenting with clinically overt pulmonary embolism (PE) (48%, 48%, and 50% respectively), the average time elapsed from surgery to VTE (22 +/- 16, 24 +/- 16, and 21 +/- 15 days, respectively), and the three-month incidence of fatal PE (1.3%, 1.4%, and 0.8%, respectively), fatal bleeding (0.8%, 1.0%, and 0.2%, respectively), or major bleeding (2.3%, 2.9%, and 2.8%, respectively) were similar in the three groups. However, the percentage of patients who had received thromboprophylaxis (96%, 76% and 52%, respectively), the duration of prophylaxis (17 +/- 9.6, 13 +/- 8.9, and 12 +/- 11 days, respectively) and the mean daily doses of low-molecular-weight heparin (4,252 +/- 1,016, 3,260 +/- 1,141, and 3,769 +/- 1,650 IU, respectively), were significantly lower in those undergoing cancer surgery or other procedures. In conclusion, the clinical presentation, time-course, and three-month outcome of VTE was similar among the different subgroups of patients, but the use of prophylaxis in patients undergoing cancer surgery or other procedures was suboptimal.
