Thymic T cell lineage commitment is dependent on Notch1 (N1) receptor-mediated signaling. Although the physiological ligands that interact with N1 expressed on thymic precursors are currently unknown, in vitro culture systems point to Delta-like 1 (DL1) and DL4 as prime candidates. Using DL1- and DL4-lacZ reporter knock-in mice and novel monoclonal antibodies to DL1 and DL4, we show that DL4 is expressed on thymic epithelial cells (TECs), whereas DL1 is not detected. The function of DL4 was further explored in vivo by generating mice in which DL4 could be specifically inactivated in TECs or in hematopoietic progenitors. Although loss of DL4 in hematopoietic progenitors did not perturb thymus development, inactivation of DL4 in TECs led to a complete block in T cell development coupled with the ectopic appearance of immature B cells in the thymus. These immature B cells were phenotypically indistinguishable from those developing in the thymus of conditional N1 mutant mice. Collectively, our results demonstrate that DL4 is the essential and nonredundant N1 ligand responsible for T cell lineage commitment. Moreover, they strongly suggest that N1-expressing thymic progenitors interact with DL4-expressing TECs to suppress B lineage potential and to induce the first steps of intrathymic T cell development.

Résumé Le but de cette étude est d'évaluer la faisabilité et l'efficacité d'un traitement des carcinomes pharyngo-laryngés avancés par combinaison de chimiothérapie intensive associé à une radiothérapie accélérée. Vingt-trois patients ont été inclus (age médian 54 ans, entre 35 et 70 ans). Les localisations tumorales étaient l'hypopharynx (n=7), base de langue (n=10), nasopharynx (n=2) ou l'oesophage proximal (n.1), ou sans porte d'entrée (n=3). Le traitement comprend trois cycles de chimiothérapie (cisplatin 100mg/m2 à J1 ; 5-FU 1000mg/m2 par jour pendant 5 jours en perfusion continue, précédé par de l'amifostine 910mg/m2 ; répété toutes les trois semaines). La radiothérapie concomitante, accélérée (dose totale de 70Gy en 6 semaines) a été débuté au premier jour du deuxième cycle de chimiothérapie. Vingt et un patients ont pu achever la radiothérapie. Dix-huit patients étaient en rémission complète à la fin du traitement. Avec un suivi médian de 45 mois, le taux de survie globale atteint 56% (95% Cl, 32-79%). Le contrôle loco-régional était de 71% (95% CI, 52-91%). La toxicité associée au traitement consistait en une insuffisance rénale réversible (≥grade II) chez 9 patients (43%) et une agranulocytose fébrile chez 9 patients (43%). Tous les patients ont présenté une mucite modérée à sévère (grade II/III) et 19 patients ont montré une toxicité cutanée de grade III. En conclusion, le traitement combiné de radiothérapie accélérée avec une chimiothérapie concomitante à base de Cisplatin/5-FU full-dose avec amifostine est faisable. La toxicité est importante mais reste maîtrisable dans le cadre d'un centre multidisciplinaire. Le taux de survie globale à 4 ans est prometteur, la recherche en vue de traitements moins toxiques doit se poursuivre. Abstract The purpose of this study was to evaluate the feasibility and efficacy of a treatment concept combining three cycles of full-dose chemotherapy (CT) with concomitant accelerated uninterrupted radiotherapy (RI). Twenty- three patients (median age: 54 years, range: 35-70) with locally advanced squamous cell carcinoma of the head and neck (SCCHN) were included. The primary tumor involved the hypopharynx (n=7), base of the tongue (n=10), nasopharynx (n=2) or upper esophagus (n=1) or its location was unknown (n=3). Treatment consisted of three cycles of chemotherapy (cisplatin 100 mg/m2 on day 1; 5-FU 1,000 mg/m2 per day for 5 days as a continuous infusion, preceded by amifostine 910 mg/m2). repeated every 3 weeks. Uninterrupted concomitant boost-accelerated RI (total dose of 70 Gy in 6 weeks) started together on day 1 of the second cycle. All but two patients received the full course of RT. Eighteen patients achieved complete remission (78%). At a median follow-up of 45 months the overall survival was 56% (95% c.i. 32-79%) and the loco-regional control 71% (95% c.i. 52-91%). Toxicity involved reversible renal insufficiency of grade II in 9 patients (39%) and neutropenic fever in 9 patients (39%). All patients suffered from moderate to severe mucositis (grade HMI), and 19 patients presented cutaneous toxicity grade III. Concomitant boost-accelerated RI combined with concurrent full-dose cisplatin/5-FU chemotherapy and amifostine is feasible with manageable, although substantial, toxicity. The overall survival of 4 years is promising. Newer regimens causing less acute mucosal and skin toxicity are needed.

Little attention has been paid so far to the influence of the chemical nature of the substance when measuring δ 15N by elemental analysis (EA)-isotope ratio mass spectrometry (IRMS). Although the bulk nitrogen isotope analysis of organic material is not to be questioned, literature from different disciplines using IRMS provides hints that the quantitative conversion of nitrate into nitrogen presents difficulties. We observed abnormal series of δ 15N values of laboratory standards and nitrates. These unexpected results were shown to be related to the tailing of the nitrogen peak of nitrate-containing compounds. A series of experiments were set up to investigate the cause of this phenomenon, using ammonium nitrate (NH4NO3) and potassium nitrate (KNO3) samples, two organic laboratory standards as well as the international secondary reference materials IAEA-N1, IAEA-N2-two ammonium sulphates [(NH4)2SO4]-and IAEA-NO-3, a potassium nitrate. In experiment 1, we used graphite and vanadium pentoxide (V2O5) as additives to observe if they could enhance the decomposition (combustion) of nitrates. In experiment 2, we tested another elemental analyser configuration including an additional section of reduced copper in order to see whether or not the tailing could originate from an incomplete reduction process. Finally, we modified several parameters of the method and observed their influence on the peak shape, δ 15N value and nitrogen content in weight percent of nitrogen of the target substances. We found the best results using mere thermal decomposition in helium, under exclusion of any oxygen. We show that the analytical procedure used for organic samples should not be used for nitrates because of their different chemical nature. We present the best performance given one set of sample introduction parameters for the analysis of nitrates, as well as for the ammonium sulphate IAEA-N1 and IAEA-N2 reference materials. We discuss these results considering the thermochemistry of the substances and the analytical technique itself. The results emphasise the difference in chemical nature of inorganic and organic samples, which necessarily involves distinct thermochemistry when analysed by EA-IRMS. Therefore, they should not be processed using the same analytical procedure. This clearly impacts on the way international secondary reference materials should be used for the calibration of organic laboratory standards.

Contexte : la prévalence des épisodes dépressifs majeurs parmi la population âgée générale est de 1-4%. Plusieurs études proposent la dissociation entre la dépression à début tardive (late onset depression, LOD), plus souvent associée à des déficits neuropsychologiques, des lésions cérébrales et des facteurs de risque cardio-vasculaire, et la dépression à début précoce (early onset depression, EOD) associée, elle, aux facteurs génétiques et à certains profiles de personnalité. Toutefois, aucune étude transversale ou longitudinale n'a jusqu'à maintenant mesuré et comparé de façon concomitante les profiles cognitifs, la neuro-imagerie (IRM) et les profiles de personnalité des patients âgés LOD et EOD euthymiques. Méthodes : ce travail se base sur une étude menée par différents services des Hôpitaux Universitaires de Genève (HUG) et du Centre Hospitalier Universitaire Vaudois (CHUV) qui ont collaboré afin de recruter le collectif de patients dépressifs nécessaire. La partie expérimentale est divisée en deux parties. La première, transversale, compare 30 EOD, 11 LOD et 30 sujets contrôles, puis 38 EOD à 62 sujets contrôles. Une évaluation neuropsychologique, des évaluations des lésions et volumes cérébraux à l'IRM, ainsi que des traits de personnalité ont été effectuées. La deuxième partie, longitudinale, évalue sur 2 ans 28 patients EOD à 48 sujets contrôles avec les mêmes outils. Résultats : lors de la première partie, transversale, les performances cognitives et les volumes cérébraux sont préservés chez les patients EOD, alors que les patients LOD présentent une réduction significative de la mémoire épisodique et un taux plus élevé de lésions cérébrales périventriculaires (hyperintensités de la matière blanche) en comparaison avec les patients EOD et les sujets contrôles. Au niveau des traits de personnalité, les patients EOD sont associés à un niveau élevé de Névrosisme, en particulier les facettes Anxiété (N1) et Dépression (N3) mais diminué d'Extraversion, en particulier les facettes Chaleur (E1) et Emotions positives (E6). Dans la seconde partie, longitudinale, les performances cognitives et les volumes cérébraux des patients EOD sont restés, après les 2 ans de suivi (follow-up) comparables aux sujets contrôles. Les niveaux élevés du Névrosisme et sa facette Anxiété (N1) constatés au baseline diminuèrent pour atteindre un niveau normal. Les niveaux diminués des facettes Chaleur (E1) et Emotions positives (E6) au baseline ne persistèrent pas non plus. Seule la facette Dépression (N3) est restée chez les patients EOD significativement plus élevée que chez les sujets contrôles après les 2 ans de suivi. Conclusion : nos résultats supportent la dissociation entre EOD, associée à des facteurs génétiques et psychosociaux, et LOD associée aux facteurs de risque et comorbidités cardio-vasculaires. Après rémission d'un épisode dépressif aigu, les performances cognitives ainsi que les volumes cérébraux des patients EOD restent intactes au long terme, alors que le patient LOD garde des lésions cérébrales ainsi que des atteintes au niveau de la mémoire épisodique. Au niveau de la personnalité, la facette Dépression (N3) du domaine Névrosisme, connu pour être un facteur de risque de dépression, reste une caractéristique bien présente chez le patient EOD.

Binding studies have been performed between amitriptyline and i) native alpha 1-acid glycoprotein (AAG); ii) its desialylated form; iii) its two variants, S-AAG and F-AAG; and iv) a mixture of S-AAG and F-AAG. Scatchard analysis revealed the presence of two classes of binding sites on AAG. For native AAG, the first class (of high affinity) has an association constant (Ka1) of 1.5 x 10(6) L mol-1 and a number of binding sites per mole of protein (n1) of 0.25, while the second class (of low affinity) has a Ka2 of 3.2 x 10(4) L mol-1 and a n2 of 0.94. Similar data were found for desialylated AAG. S-AAG and F-AAG do not differ in their association constants measured with amitriptyline, but in their number of binding sites per mole of protein (n): S-AAG: n1 = 0.56, n2 = 0.52; F-AAG: n1 = 0.17, n2 = 0.71. These results confirm those of a previous study, in which a higher affinity of S-AAG towards various basic drugs in comparison with F-AAG has been found.

Sex differences in cognition have been largely investigated. The most consistent sex differences favoring females are observed in object location memory involving the left hemisphere whereas the most consistent sex differences favoring males are observed in tasks that require mental rotation involving the right hemisphere. Here we used a task involving these two abilities to see the impact of mental rotation on object location memory. To that end we used a combination of behavioral and event-related potential (ERP) electroencephalography (EEG) measures.A computer screen displayed a square frame of 4 pairs of images (a "teddy" bear, a shoe, an umbrella and a lamp) randomly arranged around a central fixation cross. After a 10-second interval for memorization, images disappeared and were replaced by a test frame with no image but a random pair of two locations marked in black. In addition, this test frame was randomly displayed either in the original orientation (0° rotation) or in the rotated one (90° clockwise - CW - or 90° counterclockwise - CCW). Preceding the test frame, an arrow indicating the presence or the absence of rotation of the frame was displayed on the screen. The task of the participants (15 females and 15 males) was to determine if two marked locations corresponded or not to a pair of identical images. Each response was followed by feedback.Findings showed no significant sex differences in the performance of the original orientation. In comparison with this position, the rotation of the frame produced an equal decrease of male and female performance. In addition, this decrease was significantly higher when the rotation of the frame was in a CCW direction. We further assessed the ERP when the arrow indicated the direction of rotation as stimulus-onset, during four time windows representing major components C1, P1, N1 and N2. Although no sex differences were observed in performance, brain activities differed according to sex. Enhanced amplitudes were found for the CCW compared to CW rotation over the right posterior areas for the P1, N1 and N2 components for men as well as for women. Major topographical differences related to sex were measured for the CW rotation condition as marked lateralized amplitude: left-hemisphere amplitude larger than right one was measured during P1 time range for men. These similar patterns prolonged from P1 to N1 for women. Early distinctions were found in interaction with sex between CCW and CW waveform amplitudes, expressing over anterior electrode sites during C1 time range (0-50 ms post-stimulus).In conclusion (i) women do not outperform men in object location memory in this study (absence of rotation condition); (ii) mental rotation, in particular the direction of rotation, influences performance on object location memory; (iii) CCW rotation is associated with activity in the right parietal hemisphere whereas the CW rotation involves the left parietal hemisphere; (iv) this last effect is less pronounced in males, which could explain why greater involvement of right parietal areas in men and of bilateral posterior areas in women is generally reported in mental rotation tasks; and (v) the early distinctions between both directions of rotation located over anterior sites could be related to sex differences in their respective involvement of control mechanisms.

The role of albumin in tubular secretion of the organic anions p-aminohippurate (PAH, 21% albumin-bound at 1 microM) and methotrexate (MTX, 55% bound at 1 microM), and of the organic cation N1-methylnicotinamide (NMN, not bound), was investigated in isolated rabbit S2 proximal tubules. PAH or MTX secretory rates were low in the absence of colloids or in the presence of 1 g/dl dextran 40, and were reversibly two- to sevenfold stimulated by either 1 g/dl bovine (BSA, either regular, defatted, and/or dialyzed) or rabbit serum albumin, or by dialyzed native rabbit plasma. NMN secretion was not stimulated by either dextran or albumin. Luminal BSA had no effect, but stimulation of PAH secretion was observed when albumin was present in both lumen and bath. This secretion was BSA concentration-dependent up to a 1 g/dl BSA. Saturation experiments suggested that 1 g/dl BSA may increase PAH apparent affinity for secretion, with no change in its maximum velocity. Albumin appears therefore to facilitate organic anion proximal secretion by an effect unrelated to oncotic pressure or to the extent of organic anion binding.

ABSTRACT This thesis is composed of two main parts. The first addressed the question of whether the auditory and somatosensory systems, like their visual counterpart, comprise parallel functional pathways for processing identity and spatial attributes (so-called `what' and `where' pathways, respectively). The second part examined the independence of control processes mediating task switching across 'what' and `where' pathways in the auditory and visual modalities. Concerning the first part, electrical neuroimaging of event-related potentials identified the spatio-temporal mechanisms subserving auditory (see Appendix, Study n°1) and vibrotactile (see Appendix, Study n°2) processing during two types of blocks of trials. `What' blocks varied stimuli in their frequency independently of their location.. `Where' blocks varied the same stimuli in their location independently of their frequency. Concerning the second part (see Appendix, Study n°3), a psychophysical task-switching paradigm was used to investigate the hypothesis that the efficacy of control processes depends on the extent of overlap between the neural circuitry mediating the different tasks at hand, such that more effective task preparation (and by extension smaller switch costs) is achieved when the anatomical/functional overlap of this circuitry is small. Performance costs associated with switching tasks and/or switching sensory modalities were measured. Tasks required the analysis of either the identity or spatial location of environmental objects (`what' and `where' tasks, respectively) that were presented either visually or acoustically on any given trial. Pretrial cues informed participants of the upcoming task, but not of the sensory modality. - In the audio-visual domain, the results showed that switch costs between tasks were significantly smaller when the sensory modality of the task switched versus when it repeated. In addition, switch costs between the senses were correlated only when the sensory modality of the task repeated across trials and not when it switched. The collective evidence not only supports the independence of control processes mediating task switching and modality switching, but also the hypothesis that switch costs reflect competitive interterence between neural circuits that in turn can be diminished when these neural circuits are distinct. - In the auditory and somatosensory domains, the findings show that a segregation of location vs. recognition information is observed across sensory systems and that these happen around 100ms for both sensory modalities. - Also, our results show that functionally specialized pathways for audition and somatosensation involve largely overlapping brain regions, i.e. posterior superior and middle temporal cortices and inferior parietal areas. Both these properties (synchrony of differential processing and overlapping brain regions) probably optimize the relationships across sensory modalities. - Therefore, these results may be indicative of a computationally advantageous organization for processing spatial anal identity information.

Esta é uma abordagem da relação entre a juventude e a violência analisada em perspectiva jurídico-formal, que tem a ver com a natureza do Estado instaurado após a independência, manifesta na criação de um quadro jurídico que estaria em conformidade com os ideais libertadores. Os anseios de construção de uma nova sociedade tinham que ter um enquadramento jurídico e as estruturas, nomeadamente, legislação, instituições e o próprio pensamento jurídico construído ao longo da época da colonização, entravam em contradição com eles. Os choques entre os objectivos e as práticas fizeram da juventude os principais actores e alvos. A percepção de que a revogação em bloco de toda a legislação portuguesa em vigor no território antes da independência podia traduzir-se num vazio jurídico, ainda com consequências mais prejudiciais para o funcionamento do novo Estado, levou à adopção de uma lei de enquadramento (Lei nº1/73, de 24 de Setembro), que mantinha a vigência de muitas leis portuguesas no território. A evolução da realidade social e o desmoronamento de algumas utopias do período pós-independência, criaram várias situações e as próprias leis existentes tornaram-se obsoletas e fonte de conflitos. No que toca ao tratamento dos jovens que se encontravam em situações conflituosas, um exemplo desta complicação é a própria alteração do âmbito da menoridade, e consequentemente da juventude em termos formais. Recorda’se que a maioridade que era considerada, na época colonial, a partir dos 21 anos, passou em 1976 a ser a partir dos 18. Estas complicações foram reforçadas pelas discrepancias entre as normas de imputaçáo da responsabilidade, constantes no Direito Penal e as de Direito Civil, em Geral. Portanto a harmonização do tratamento juridicos dos jovens, variam consoante o ramo do direito que trata a situação em que estão envolvidos. Nao varia apenas quando um jovem é agente de um acto de violação ou quando é vitima.

Previous electrophysiological studies revealed that human faces elicit an early visual event-related potential (ERP) within the occipito-temporal cortex, the N170 component. Although face perception has been proposed to rely on automatic processing, the impact of selective attention on N170 remains controversial both in young and elderly individuals. Using early visual ERP and alpha power analysis, we assessed the influence of aging on selective attention to faces during delayed-recognition tasks for face and letter stimuli, examining 36 elderly and 20 young adults with preserved cognition. Face recognition performance worsened with age. Aging induced a latency delay of the N1 component for faces and letters, as well as of the face N170 component. Contrasting with letters, ignored faces elicited larger N1 and N170 components than attended faces in both age groups. This counterintuitive attention effect on face processing persisted when scenes replaced letters. In contrast with young, elderly subjects failed to suppress irrelevant letters when attending faces. Whereas attended stimuli induced a parietal alpha band desynchronization within 300-1000 ms post-stimulus with bilateral-to-right distribution for faces and left lateralization for letters, ignored and passively viewed stimuli elicited a central alpha synchronization larger on the right hemisphere. Aging delayed the latency of this alpha synchronization for both face and letter stimuli, and reduced its amplitude for ignored letters. These results suggest that due to their social relevance, human faces may cause paradoxical attention effects on early visual ERP components, but they still undergo classical top-down control as a function of endogenous selective attention. Aging does not affect the face bottom-up alerting mechanism but reduces the top-down suppression of distracting letters, possibly impinging upon face recognition, and more generally delays the top-down suppression of task-irrelevant information.

BACKGROUND: Survival after pancreatic head adenocarcinoma surgery is determined by tumor characteristics, resection margins, and adjuvant chemotherapy. Few studies have analyzed the long-term impact of postoperative morbidity. The aim of the present study was to assess the impact of postoperative complications on long-term survival after pancreaticoduodenectomy for cancer. METHODS: Of 294 consecutive pancreatectomies performed between January 2000 and July 2011, a total of 101 pancreatic head resections for pancreatic ductal adenocarcinoma were retrospectively analyzed. Postoperative complications were classified on a five-grade validated scale and were correlated with long-term survival. Grade IIIb to IVb complications were defined as severe. RESULTS: Postoperative mortality and morbidity were 5 and 57 %, respectively. Severe postoperative complications occurred in 16 patients (16 %). Median overall survival was 1.4 years. Significant prognostic factors of survival were the N-stage of the tumor (median survival 3.4 years for N0 vs. 1.3 years for N1, p = 0.018) and R status of the resection (median survival 1.6 years for R0 vs. 1.2 years for R1, p = 0.038). Median survival after severe postoperative complications was decreased from 1.9 to 1.2 years (p = 0.06). Median survival for N0 or N1 tumor or after R0 resection was not influenced by the occurrence and severity of complications, but patients with a R1 resection and severe complications showed a worsened median survival of 0.6 vs. 2.0 years without severe complications (p = 0.0005). CONCLUSIONS: Postoperative severe morbidity per se had no impact on long-term survival except in patients with R1 tumor resection. These results suggest that severe complications after R1 resection predict poor outcome.

Objectifs: Evaluer l'intérêt d'une double infiltration sous guidage scanner épiduro-foraminale versus foraminale seule dans les conflits disco -radiculaires. Matériels et méthodes: Etude randomisée prospective monocentrique portant sur 110 patients évaluant à J1, J7 et J30 l'évolution clinique par EVA, ODI et prise antalgiques. L'ensembledes interventions est réalisé sous guidage scanner avec opacification de la zone traitée pour confirmer le bon positionnement des corticoïdes . Résultats: Une différence significative (p<0,04) est montrée pour les hernies postéro-latérales avec une meilleure efficacité de l'injection double épidurale et foraminale.Aucune complication majeure n'a été constatée sur les 400 patients eligibles. Conclusion: Les infiltrations sous scanner sont des interventions efficaces et sûres . Le double abord épidural et foraminal présente un intérêt dans les herniespostéro-latérales.

O presente trabalho aborda a questão ligada a supervisão e prática reflexiva dos professores na escola da SOS-Fazenda no Concelho da Praia , com o pólo nº1 de Achada Batalha no Concelho de São Miguel, sobretudo no que tange a problemática da supervisão e processo reflexivo dos professores nessas duas escolas e apresentar algumas sugestões para o melhoramento do seu funcionamento no futuro próximo. O trabalho teve como suporte teórico alguns autores como: Durkhein,Cipriano Carlos Luckesi, José Alberto Correia, Jorge Nunes, Isabel Alarcão , Júlia Oliveira Formosinho e outros autores que retratam a problemática da supervisão pedagógica aos professores como uma forma de melhorar a qualidade da reflexão das suas acções junto dos seus alunos. Os dados foram recolhidos junto dos professores das duas escolas e de acordo com as documentações existentes sobre a supervisão pedagógica. Do trabalho conclui-se que a melhoria da prática da supervisão nas duas escolas, depende muito do desenvolvimento das acções de formação e de um forte acompanhamento dos professores nos seus locais de trabalho, o que terá sempre reflexo na motivação dos docentes contribuindo de forma directa na melhoria da aprendizagem, com impacto na diminuição de repetências e abandono escolar.
