Introduction In the Family Health Strategy (FHS), the treatment of Diabetes Mellitus (DM) includes education and lifestyle change strategies. Physiotherapists have a key role in this health setting. Objectives To implement actions of evaluation and guidelines for patients with type 2 DM who attend a Family Health Strategy (FHS), regarding diabetic foot and the practice of regular physical exercise in the control and prevention of the complications of Diabetes Mellitus. Methods 17 individuals from an FHS were evaluated, with the following procedures: clinical and anthropometric parameters, inspection, a questionnaire on diabetic neuropathy, tests of vibratory and tactile sensitivity, muscle function, range of motion, functional analysis, questions about exercise practice and guidance regarding controlling blood glucose and foot care. Results Deformities, dry skin, calluses, dehydration, ulceration, cracking and brittle nails were found. Peripheral neuropathy was not observed; tactile sensitivity was altered in the heel region and the vibratory sense was absent in 5% of individuals. A decrease in functionality of ankle movements was verified. Of the participants, 76% were sedentary, 24% knew about the benefits of practicing regular exercise, 25% had undergone a medical evaluation prior to performing physical exercise and, of these, 25% were supervised by a qualified professional. Discussion The implementation of physiotherapy actions in diabetics from an FHS was important for highlighting the presence of risk factors for diabetic complications. Conclusions Individuals attending the FHS need more information and programs for the prevention of diabetic complications.

Objective This study investigated environmental endotoxin exposure during early life, sensitization to aeroallergens, the production of cytokines by LPS-stimulated leukocytes, and the development of a wheezing phenotype in a prospective cohort of infants with high risk of developing allergic diseases. Materials and Methods Eighty-four infants were followed from birth until 30 months of age. We assessed endotoxin concentration in house dust of their homes during the first 6 months of life. At age 30 months they were clinically evaluated to determine the development of wheezing and other clinical events, were skin prick tested, and had blood samples collected for the evaluation of cytokine release by LPS-stimulated peripheral blood mononuclear cells (PBMC). Results The level of endotoxin exposure during early life was not associated with development of a wheezing phenotype. On the other hand a higher incidence of respiratory infections occurred among recurrent wheezing (RW) infants. PBMC from RW children exposed to higher levels of environmental endotoxin (above 50?EU/mg) released less Interleukin (IL)-12p70 and IFN-? compared to the non-RW group. TNF-a, IL-10, IL-4, IL-5, and IL17 production by LPS-stimulated PBMC from RW and non-RW children was equivalent in both groups of environmental endotoxin exposure. Conclusion In this prospective cohort of infants with high risk of developing allergic diseases we observed that RW and non-RW children were exposed to similar levels of endotoxin early in life. LPS-stimulated PBMC from RW infants exposed to higher levels of endotoxin released significantly less IL-12 and IFN-? compared to non-RW infants. Pediatr Pulmonol. 2012. 47:10541060. The relationship between maternal factors and the response of preterm infants to pain and stress experienced during heel puncture while in maternal kangaroo care was investigated. This descriptive study included 42 mothers and their preterm infants cared for in a neonatal unit. Data were collected in the baseline, procedure, and recovery phases. We measured the neonates' facial actions, sleep and wake states, crying, salivary cortisol levels, and heart rate, in addition to the mothers' behavior, salivary cortisol levels, and mental condition. The influence of the maternal explanatory variables on the neonatal response variables were verified through bivariate analysis, ANOVA, and multiple regression. The mothers' behavior and depression and/or anxiety did not affect the neonates' responses to pain and stress, though the mothers' levels of salivary cortisol before the procedure explained the variance in the neonates' levels of salivary cortisol after the procedure (p=0.036). Additionally, the mothers' baseline levels of salivary cortisol along with the neonates' age explained the variance in the neonates' heart rate (p=0.001). The ability of mothers to regulate their own stress contributed to the infants' responses to pain and stress.

Background In the last 20 years, there has been an increase in the incidence of allergic respiratory diseases worldwide and exposure to air pollution has been discussed as one of the factors associated with this increase. The objective of this study was to investigate the effects of air pollution on peak expiratory flow (PEF) and FEV1 in children with and without allergic sensitization. Methods Ninety-six children were followed from April to July, 2004 with spirometry measurements. They were tested for allergic sensitization (IgE, skin prick test, eosinophilia) and asked about allergic symptoms. Air pollution, temperature, and relative humidity data were available. Results Decrements in PEF were observed with previous 24-hr average exposure to air pollution, as well as with 310-day average exposure and were associated mainly with PM10, NO2, and O3 in all three categories of allergic sensitization. Even though allergic sensitized children tended to present larger decrements in the PEF measurements they were not statistically different from the non-allergic sensitized. Decrements in FEV1 were observed mainly with previous 24-hr average exposure and 3-day moving average. Conclusions Decrements in PEF associated with air pollution were observed in children independent from their allergic sensitization status. Their daily exposure to air pollution can be responsible for a chronic inflammatory process that might impair their lung growth and later their lung function in adulthood. Am. J. Ind. Unstable shoes have been designed to promote "natural instability" and during walking they should simulate barefoot gait, enhancing muscle activity and, thus, attributing an advantage over regular tennis shoes. Recent studies showed that, after special training on the appropriate walking pattern, the use of the Masai Barefoot Technology (MBT) shoe increases muscle activation during walking. Our study presents a comparison of muscle activity as well as horizontal and vertical forces during gait with the MBT, a standard tennis shoe and barefoot walking of healthy individuals without previous training. These variables were compared in 25 female subjects and gait conditions were compared using ANOVA repeated measures (effect size:0.25). Walking with the MBT shoe in this non-instructed condition produced higher vertical forces (first vertical peak and weight acceptance rate) than walking with a standard shoe or walking barefoot, which suggests an increase in the loads received by the musculoskeletal system, especially at heel strike. Walking with the MBT shoe did not increase muscle activity when compared to walking with the standard shoe. The barefoot condition was more effective than the MBT shoe at enhancing muscle activation. Therefore, in healthy individuals, no advantage was found in using the MBT over a standard tennis shoe without a special training period. Further studies using the MBT without any instruction over a longer period are needed to evaluate if the higher loads observed in the present study would return to their baseline values after a period of adaptation, and if the muscle activity would increase over time. Objective Growth hormone (GH)/insulin-like growth factor (IGF) axis and insulin are key determinants of bone remodelling. Homozygous mutations in the GH-releasing hormone receptor (GHRHR) gene (GHRHR) are a frequent cause of genetic isolated GH deficiency (IGHD). Heterozygosity for GHRHR mutation causes changes in body composition and possibly an increase in insulin sensitivity, but its effects on bone quality are still unknown. The objective of this study was to assess the bone quality and metabolism and its correlation with insulin sensitivity in subjects heterozygous for a null mutation in the GHRHR. Patients and methods A cross-sectional study was performed on 76 normal subjects (68.4% females) (N/N) and 64 individuals (64.1% females) heterozygous for a mutation in the GHRHR (MUT/N). Anthropometric features, quantitative ultrasound (QUS) of the heel, bone markers [osteocalcin (OC) and CrossLaps], IGF-I, glucose and insulin were measured, and homeostasis model assessment of insulin resistance (HOMAIR) was calculated. Results There were no differences in age or height between the two groups, but weight (P = 0.007) and BMI (P = 0.001) were lower in MUT/N. There were no differences in serum levels of IGF-I, glucose, T-score or absolute values of stiffness and OC, but insulin (P = 0.01), HOMAIR (P = 0.01) and CrossLaps (P = 0.01) were lower in MUT/N. There was no correlation between OC and glucose, OC and HOMAIR in the 140 individuals as a whole or in the separate MUT/N or N/N groups. Conclusions This study suggests that one allele mutation in the GHRHR gene has a greater impact on energy metabolism than on bone quality.

Objective: Chronic rhinitis and adenoid hypertrophy are the main causes of nasal obstruction in children and proper treatment of these factors seem essential for controlling nasal obstructive symptoms. This study aims to evaluate the effects of topical mometasone treatment on symptoms and size of adenoid tissue in children with complaints of nasal obstruction and to compare this approach to continuous nasal saline douching plus environmental control alone. Methods: Fifty-one children with nasal obstructive complaints were submitted to a semi-structured clinical questionnaire on nasal symptoms, prick test and nasoendoscopy. Nasoendoscopic images were digitalized, and both adenoid and nasopharyngeal areas were measured in pixels. The relation adenoid/nasopharyngeal area was calculated. Patients were subsequently re-evaluated in two different periods: following 40 days of treatment with nasal douching and environmental prophylaxis alone; and after an subsequent 40 day-period, when topical mometasone furoate (total dose: 100 mu g/day) was superposed. Results: Nasal symptoms and snoring significantly improved after nasal douching, and an additional gain was observed when mometasone furoate was included to treatment. Saline douching did not influence the adenoid area, whereas a significant reduction on adenoid tonsil was observed after 40 days of mometasone treatment (P < 0.0001). Conclusion: Nasal saline douching significantly improved nasal symptoms without interfering in adenoid dimension. In contrast, mometasone furoate significantly reduced adenoid tissue, and led to a supplementary improvement of nasal symptoms. (C) 2012 Elsevier Ireland Ltd. All rights reserved.

Background: Walking with high-heeled shoes is a common cause of venous complaints such as pain, fatigue, and heavy-feeling legs. The aim of the study was to clarify the influence of high-heeled shoes on the venous return and test the hypothesis that women wearing different styles of high-heeled shoes present an impaired venous return when compared with their values when they are barefoot. Methods: Thirty asymptomatic women (mean age, 26.4 years) wearing appropriately sized shoes were evaluated by air plethysmography (APG), a test that measures changes in air volume on a cuff placed on the calf, while they performed orthostatic flexion and extension foot movements and altered standing up and lying down. The test was repeated in four situations: barefoot (0 cm), medium heels (3.5 cm), stiletto high heels (7 cm), and platform high heels (7 cm). The APG values of venous filling index (VFI), ejection fraction (EF), and residual volume fraction (RVF) were divided into four groups according to heel height and compared by repeated-measures analysis of variance. Results: RVF was increased in the groups wearing high heels (stiletto and platform) compared with the barefoot group (P < .05). RVF was increased in the medium-heel group (3.5 cm) compared with the barefoot group (P < .05), and despite the lack of statistical significance, the medium-heel group showed lower values of RVF compared with the two high-heel groups. The EF parameter followed the opposite tendency, showing higher values for the barefoot group compared with the other three groups (P < .05). Values for VFI were similar in the three situations evaluated. Conclusions: High heels reduce muscle pump function, as demonstrated by reduced EF and increased RVF values. The continuous use of high heels tends to provoke venous hypertension in the lower limbs and may represent a causal factor of venous disease symptoms. (J Vasc Surg 2012;56:1039-44.)

In der vorliegenden Arbeit sollte die Fähigkeit untersucht werden, Schmerzreize auf der Haut zu lokalisieren und deren Intensität zu differenzieren. Während dieser Diskriminationsaufgaben wurde die elektrische Aktivität des Gehirns gemessen.Traditionell werden dem nozizeptiven System nur geringe Diskriminationsleistungen zugeschrieben. In einer ersten Versuchsreihe sollten daher die räumlichen Diskriminationsleistungen für nozizeptive und taktile Reize verglichen werden. Auf dem Handrücken konnten schmerzhaft Laserhitzereize genauso gut lokalisiert werden wie taktile Reize (von-Frey-Haar). Nur ein mechanischer Nadelreiz, der taktiles und nozizeptives System koaktivierte, konnte noch besser lokalisiert werden. In der zweiten Versuchsreihe wurden während verschiedener Diskriminationsaufgaben (räumliche Diskrimination, Intensitätsdiskrimination) und einer Ablenkungsaufgabe (mentale Arithmetik) Laser-evozierte Potenziale von der Kopfhaut abgeleitet. Eine Dipolquellenanalyse zeigte als erstes eine Aktivierung des frontalen Operculums, entsprechend einem zur Zeit noch umstrittenen Projektionsgebiet eines nozizeptiven Thalamuskerns (VMpo), gefolgt vom primären somatosensorische Kortex (SI) und dem Gyrus cinguli. Im Gegensatz zum taktilen System wurde SI signifikant später aktiviert als SII (bzw. das Operculum). Die Diskriminationsaufgaben erhöhten die Aktivität aller Quellen im Vergleich zu der Ablenkungsbedingung. Dies konnte sogar für die früheste Quelle im Operculum gezeigt werden.Die frühe sensorisch-diskriminative Komponente der Schmerzverarbeitung im Operculum zeigte eine Hemisphärenasymmetrie, mit stärkerer Aktivierung der linken Hemisphäre unabhängig von der Stimulationsseite.

The full blood cell (FBC) count is the most common indicator of diseases. At present hematology analyzers are used for the blood cell characterization, but, recently, there has been interest in using techniques that take advantage of microscale devices and intrinsic properties of cells for increased automation and decreased cost. Microfluidic technologies offer solutions to handling and processing small volumes of blood (2-50 uL taken by finger prick) for point-of-care(PoC) applications. Several PoC blood analyzers are in use and may have applications in the fields of telemedicine, out patient monitoring and medical care in resource limited settings. They have the advantage to be easy to move and much cheaper than traditional analyzers, which require bulky instruments and consume large amount of reagents. The development of miniaturized point-of-care diagnostic tests may be enabled by chip-based technologies for cell separation and sorting. Many current diagnostic tests depend on fractionated blood components: plasma, red blood cells (RBCs), white blood cells (WBCs), and platelets. Specifically, white blood cell differentiation and counting provide valuable information for diagnostic purposes. For example, a low number of WBCs, called leukopenia, may be an indicator of bone marrow deficiency or failure, collagen- vascular diseases, disease of the liver or spleen. The leukocytosis, a high number of WBCs, may be due to anemia, infectious diseases, leukemia or tissue damage. In the laboratory of hybrid biodevices, at the University of Southampton,it was developed a functioning micro impedance cytometer technology for WBC differentiation and counting. It is capable to classify cells and particles on the base of their dielectric properties, in addition to their size, without the need of labeling, in a flow format similar to that of a traditional flow cytometer. It was demonstrated that the micro impedance cytometer system can detect and differentiate monocytes, neutrophils and lymphocytes, which are the three major human leukocyte populations. The simplicity and portability of the microfluidic impedance chip offer a range of potential applications in cell analysis including point-of-care diagnostic systems. The microfluidic device has been integrated into a sample preparation cartridge that semi-automatically performs erythrocyte lysis before leukocyte analysis. Generally erythrocytes are manually lysed according to a specific chemical lysis protocol, but this process has been automated in the cartridge. In this research work the chemical lysis protocol, defined in the patent US 5155044 A, was optimized in order to improve white blood cell differentiation and count performed by the integrated cartridge.

Die Dissertationsschrift beschäftigt sich mit der Entwicklung und Anwendung einer alternativen Probenzuführungstechnik für flüssige Proben in der Massenspektrometrie. Obwohl bereits einige Anstrengungen zur Verbesserung unternommen wurden, weisen konventionelle pneumatische Zerstäuber- und Sprühkammersysteme, die in der Elementspurenanalytik mittels induktiv gekoppeltem Plasma (ICP) standardmäßig verwendet werden, eine geringe Gesamteffizienz auf. Pneumatisch erzeugtes Aerosol ist durch eine breite Tropfengrößenverteilung gekennzeichnet, was den Einsatz einer Sprühkammer bedingt, um die Aerosolcharakteristik an die Betriebsbedingungen des ICPs anzupassen.. Die Erzeugung von Tropfen mit einer sehr engen Tropfengrößenverteilung oder sogar monodispersen Tropfen könnte die Effizienz des Probeneintrags verbessern. Ein Ziel dieser Arbeit ist daher, Tropfen, die mittels des thermischen Tintenstrahldruckverfahrens erzeugt werden, zum Probeneintrag in der Elementmassenspektrometrie einzusetzen. Das thermische Tintenstrahldruckverfahren konnte in der analytischen Chemie im Bereich der Oberflächenanalytik mittels TXRF oder Laserablation bisher zur gezielten, reproduzierbaren Deposition von Tropfen auf Oberflächen eingesetzt werden. Um eine kontinuierliche Tropfenerzeugung zu ermöglichen, wurde ein elektronischer Mikrokontroller entwickelt, der eine Dosiereinheit unabhängig von der Hard- und Software des Druckers steuern kann. Dabei sind alle zur Tropfenerzeugung relevanten Parameter (Frequenz, Heizpulsenergie) unabhängig voneinander einstellbar. Die Dosiereinheit, der "drop-on-demand" Aerosolgenerator (DOD), wurde auf eine Aerosoltransportkammer montiert, welche die erzeugten Tropfen in die Ionisationsquelle befördert. Im Bereich der anorganischen Spurenanalytik konnten durch die Kombination des DOD mit einem automatischen Probengeber 53 Elemente untersucht und die erzielbare Empfindlichkeiten sowie exemplarisch für 15 Elemente die Nachweisgrenzen und die Untergrundäquivalentkonzentrationen ermittelt werden. Damit die Vorteile komfortabel genutzt werden können, wurde eine Kopplung des DOD-Systems mit der miniaturisierten Fließinjektionsanalyse (FIA) sowie miniaturisierten Trenntechniken wie der µHPLC entwickelt. Die Fließinjektionsmethode wurde mit einem zertifizierten Referenzmaterial validiert, wobei für Vanadium und Cadmium die zertifizierten Werte gut reproduziert werden konnten. Transiente Signale konnten bei der Kopplung des Dosiersystems in Verbindung mit der ICP-MS an eine µHPLC abgebildet werden. Die Modifikation der Dosiereinheit zum Ankoppeln an einen kontinuierlichen Probenfluss bedarf noch einer weiteren Reduzierung des verbleibenden Totvolumens. Dazu ist die Unabhängigkeit von den bisher verwendeten, kommerziell erhältlichen Druckerpatronen anzustreben, indem die Dosiereinheit selbst gefertigt wird. Die Vielseitigkeit des Dosiersystems wurde mit der Kopplung an eine kürzlich neu entwickelte Atmosphärendruck-Ionisationsmethode, die "flowing atmospheric-pressure afterglow" Desorptions/Ionisations Ionenquelle (FAPA), aufgezeigt. Ein direkter Eintrag von flüssigen Proben in diese Quelle war bislang nicht möglich, es konnte lediglich eine Desorption von eingetrockneten Rückständen oder direkt von der Flüssigkeitsoberfläche erfolgen. Die Präzision der Analyse ist dabei durch die variable Probenposition eingeschränkt. Mit dem Einsatz des DOD-Systems können flüssige Proben nun direkt in die FAPA eingetragen, was ebenfalls das Kalibrieren bei quantitativen Analysen organischer Verbindungen ermöglicht. Neben illegalen Drogen und deren Metaboliten konnten auch frei verkäufliche Medikamente und ein Sprengstoffanalogon in entsprechend präpariertem reinem Lösungsmittel nachgewiesen werden. Ebenso gelang dies in Urinproben, die mit Drogen und Drogenmetaboliten versetzt wurden. Dabei ist hervorzuheben, dass keinerlei Probenvorbereitung notwendig war und zur Ermittlung der NWG der einzelnen Spezies keine interne oder isotopenmarkierte Standards verwendet wurden. Dennoch sind die ermittelten NWG deutlich niedriger, als die mit der bisherigen Prozedur zur Analyse flüssiger Proben erreichbaren. Um im Vergleich zu der bisher verwendeten "pin-to-plate" Geometrie der FAPA die Lösungsmittelverdampfung zu beschleunigen, wurde eine alternative Elektrodenanordnung entwickelt, bei der die Probe länger in Kontakt mit der "afterglow"-Zone steht. Diese Glimmentladungsquelle ist ringförmig und erlaubt einen Probeneintrag mittels eines zentralen Gasflusses. Wegen der ringförmigen Entladung wird der Name "halo-FAPA" (h-FAPA) für diese Entladungsgeometrie verwendet. Eine grundlegende physikalische und spektroskopische Charakterisierung zeigte, dass es sich tatsächlich um eine FAPA Desorptions/Ionisationsquelle handelt.

Proximal humerus fractures (pHF) are common. In this retrospective study intra-operative and postoperative data and complications of patients stabilized with conventional semirigid techniques (pins, n=30; helix wire, n=19) or a novel semirigid technique, the intramedullary claw (IMC, n=82) were compared. The type and frequency of postoperative complications differed between the groups (p<0.001). The IMC is a novel semirigid technique to stabilize pHF and seems to result in fewer complications than pins or helix wire. The frequency and relevance of a loss of repositioning in patients after IMC implantation need to be elucidated in long-term studies.

Properdin, a serum glycoprotein, is an important component of innate immunity, the only known positive regulator of complement, acting as an initiation point for alternative pathway activation. As an X-linked protein, we hypothesized that properdin may play a modulatory role in the pathogenesis of viral wheeze in children, which tends to be more common and more severe in boys. We aimed to determine properdin levels in a community-based paediatric sample, and to assess whether levels of properdin were associated with childhood wheeze phenotypes and atopy. We studied 137 school-children aged 8-12 yrs, a nested sample from a cohort study. Properdin was measured by a commercial enzyme-linked immunoabsorbant assay. We assessed wheeze by questionnaire, validated it by a nurse-led interview and performed skin prick tests and a methacholine challenge in all children. Forty children (29%) reported current wheeze. Serum properdin levels ranged between 18 and 40 microg/ml. Properdin was not associated with age, gender, atopy, bronchial responsiveness, current wheeze (neither the viral wheeze nor multiple-trigger wheeze phenotype) or severity of wheeze, but was slightly lower in south Asian (median 21.8 microg/ml) compared with white children (23.3 microg/ml; p = 0.006). Our data make it unlikely that properdin deficiency is common in healthy children or that levels of properdin are a major risk factor for wheeze or atopy.

The skin irritant polyyne falcarinol (panaxynol, carotatoxin) is found in carrots, parsley, celery, and in the medicinal plant Panax ginseng. In our ongoing search for new cannabinoid (CB) receptor ligands we have isolated falcarinol from the endemic Sardinian plant Seseli praecox. We show that falcarinol exhibits binding affinity to both human CB receptors but selectively alkylates the anandamide binding site in the CB(1) receptor (K(i)=594nM), acting as covalent inverse agonist in CB(1) receptor-transfected CHO cells. Given the inherent instability of purified falcarinol we repeatedly isolated this compound for biological characterization and one new polyyne was characterized. In human HaCaT keratinocytes falcarinol increased the expression of the pro-allergic chemokines IL-8 and CCL2/MCP-1 in a CB(1) receptor-dependent manner. Moreover, falcarinol inhibited the effects of anandamide on TNF-alpha stimulated keratinocytes. In vivo, falcarinol strongly aggravated histamine-induced oedema reactions in skin prick tests. Both effects were also obtained with the CB(1) receptor inverse agonist rimonabant, thus indicating the potential role of the CB(1) receptor in skin immunopharmacology. Our data suggest anti-allergic effects of anandamide and that falcarinol-associated dermatitis is due to antagonism of the CB(1) receptor in keratinocytes, leading to increased chemokine expression and aggravation of histamine action.
