The authors evaluated ten years of surgical reanimation in the University Centre of Lausanne (CHUV). Irreversible coagulopathy (IC) is the predominant cause of death for the polytraumatized patient. Acidosis, hypothermy, and coagulation troubles are crucial elements of this coagulopathy. The authors looked for a criterion allowing the identification of dying of IC. In a retrospective study, laboratory results of pH, TP, PTT, thrombocyte count and the need for blood transfusion units were checked for each major step of the primary evaluation and treatment of the polytraumatized patients. These results were considered as critical according to criteria of the literature (30). The authors conclude that the apparation of a third critical value may be useful to identify the polytraumatized patient at risk of dying of IC status. This criterion may also guide the trauma team in selecting a damage control surgical approach (DCS). This criterion was then introduced into an algorithm involving the Emergency Department, the operating room and the Intensive Care Unit. This criterion is a new tool to address the patient at the crucial moment to the appropriate hospital structure.

SummaryMulticellular organisms have evolved an immune system in order to cope with the constant threats they are facing. Foreign pathogens or endogenous danger signals released by injured or dying host cells can be readily detected through a set of germline- encoded pattern-recognition receptors. The NOD-like receptors are a cytoplasmic family of pattern-recognition receptors that have recently attracted considerable attention due to their ability to form inflammasomes, which are molecular complexes responsible for the activation of caspase-1 and the subsequent processing of the pro¬inflammatory cytokines IL-IB and 11-18 into their mature, bioactive form.In this study, we describe a novel pro-inflammatory signaling pathway, whereby the endoplasmic reticulum promotes inflammation through activation of the NLRP3 inflammasome. This was shown to be independent of the classical endoplasmic reticulum stress response pathway constituted by the effectors IREla, PERK and ATF6a. In keeping with other known NLRP3 activators, generation of reactive oxygen species and potassium efflux were required. We also provide evidence that calcium signaling is critical to this pathway, and possibly integrates signaling triggered by various NLRP3 inflammasome activators. Moreover, the mitochondrial channel VDAC1 was instrumental in mediating this response. We thus propose that the endoplasmic reticulum acts as an integrator of stress and is able to activate the mitochondria in a calcium-dependent manner in order to promote NLRP3 inflammasome activation in response to a wide range of activators.Given the role played by inflammation in the pathogenesis of atherosclerosis, we decided to investigate a possible role for the NLRP3 inflammasome in the progression of the disease. Using an ApoE mouse model, we find that deficiency in the NLRP3 inflammasome components NLRP3, ASC or Caspase-1 does not impair atherosclerosis progression, nor does it impact plaque stability. While previous studies have clearly shown a role for the interleukin-1 family of ligands in atherosclerosis, our results suggest that its contribution might be more complex than previously appreciated, and further research is thus warranted in this field.RésuméLes organismes multicellulaires ont développé un système immunitaire pour faire face aux menaces qui les entourent. Des pathogènes étrangers ou des signaux de danger relâchés par des cellules de l'hôte en détresse peuvent être rapidement détectés via un assemblage de récepteurs spécifiques qui sont présents dès la naissance. Certains membres de la famille de récepteurs NOD ont récemment attiré beaucoup d'attention au vu de leur capacité à former des inflammasomes, complexes moléculaires responsables de l'activation de la easpase-1 et de la maturation des cytokines pro-inflammatoires IL- 1β et IL-18 en leur forme bioactive.Dans cette étude, nous décrivons une nouvelle voie de signalisation pro-inflammatoire, par laquelle le réticulum endoplasmique induit l'inflammation via l'activation de l'inflammasome NLRP3. Cette voie est indépendante de la voie classique de réponse au stress du réticulum endoplasmique, qui comprend les effecteurs IRE1, PERK et ATF6. Comme pour d'autres activateurs de NLRP3, la génération de radicaux libres d'oxygène ainsi que Γ efflux de potassium sont requis. Nous montrons également que le calcium joue un rôle critique dans cette voie, et intègre possiblement la signalisation provoquée par divers activateurs de l'inflammasome NLRP3. De plus, le canal mitochondrial VDAC1 est essentiel dans cette réponse. Nous proposons donc que le réticulum endoplasmique agit comme un intégrateur de stress, activant la mitochondrie d'une façon calcium-dépendante pour promouvoir l'activation de l'inflammasome NLRP3 en réponse à divers activateurs.Au vu du rôle joué par l'inflammation dans la pathogenèse de l'athérosclérose, nous avons étudié un possible rôle pour l'inflammasome NLRP3 dans la progression de la maladie. Dans un modèle de souris ApoE, l'absence des composants de l'inflammasome NLRP3 que sont NLRP3, ASC et Caspase-1 n'influence pas la progression des plaques ni leur stabilité. Alors que d'autres études ont démontré un rôle pour les membres de la famille de l'interleukine-1 dans l'athérosclérose, nos résultats suggèrent que leur contribution pourrait être plus complexe que précédemment apprécié, et d'autres recherches dans ce domaine sont donc nécessaires.

OBJECTIVES: To investigate unenhanced postmortem 3-T MR imaging (pmMRI) for the detection of pulmonary thrombembolism (PTE) as cause of death. METHODS: In eight forensic cases dying from a possible cardiac cause but with homogeneous myocardium at cardiac pmMRI, additional T2w imaging of the pulmonary artery was performed before forensic autopsy. Imaging was carried out on a 3-T MR system in the axial and main pulmonary artery adapted oblique orientation in situ. In three cases axial T2w pmMRI of the lower legs was added. Validation of imaging findings was performed during forensic autopsy. RESULTS: All eight cases showed homogeneous material of intermediate signal intensity within the main pulmonary artery and/or pulmonary artery branches. Autopsy confirmed the MR findings as pulmonary artery thrombembolism. At lower leg imaging unilateral dilated veins and subcutaneous oedema with or without homogeneous material of intermediate signal intensity within the popliteal vein were found. CONCLUSIONS: Unenhanced pmMRI demonstrates pulmonary thrombembolism in situ. PmMR may serve as an alternative to clinical autopsy, especially when consent cannot be obtained. KEY POINTS: ? In the summer of 2008, the state of Iowa suffered from a series of severe storms that produced tornadoes and heavy rainfall, which resulted in widespread flooding. The Summer Storms1 lasted from late May through mid-August, with the most intense storms occurring over a month-long period from May 25 to June 25. The Summer Storms exacted a major human and economic toll on Iowa, resulting in 18 fatalities and 106 injuries, forcing the evacuation of approximately 38,000 Iowans, and impacting 21,000 housing units. Iowa’s public and private sectors suffered significant monetary damages. Eighty-six of the ninety-nine counties in the state were included in the Governor’s disaster declarations. Presidential disaster declarations made residents in 84 counties eligible for Public Assistance and 78 counties for Individual Assistance. The Rebuild Iowa Advisory Commission estimated $798.3 million in damages to publicly owned buildings and infrastructure, including damages of $53 million to public transportation and $342 million to public utilities.

En primer lloc, es presenta una breu història dels drets d'autor i del copyright (tots esl drets reservats) fins avui. Avui la digitalització ha donat pas a un nou model de llicències copyleft (alguns drets reservats). S'ofereixen respostes a les preguntes més freqüents sobre l'aplicació dels drets d'autor en e l'àmbit acadèmic i la investigació. Es presenten dos models de llicències copyleft: Creative Commons i les llicències d'Universitat (Harvard). A través de diferents declaracions (Budapest, Bethesda i Berlín) es defineixen els principis i acords internacionals en matèria d'accés obert a la comunitat científica i acadèmica en la darrera dècada. Finalment, es defineix el concepte d'"Educació OOberta" i es descriu el paper dels Repositoris Institucionals en l'enmagatzament i difusió del coneixement científic i acadèmic.

PURPOSE: The purpose of this study is to explore the periodical patterns of events and deaths related to cardiovascular disease (CVD), acute myocardial infarction (AMI) and stroke in Swiss adults (≥ 18 years). METHODS: Mortality data for period 1969-2007 (N=869,863 CVD events) and hospitalization data for period 1997-2008 (N=959,990 CVD events) were used. The annual, weekly and circadian distribution of CVD-related deaths and events were assessed. Multivariate analysis was conducted using multinomial logistic regression adjusting for age, gender and calendar year and considering deaths from respiratory diseases, accidents or other causes as competitive events. RESULTS: CVD deaths and hospitalizations occurred less frequently in the summer months. Similar patterns were found for AMI and stroke. No significant weekly variation for CVD deaths was found. Stratification by age and gender showed subjects aged <65 years to present a higher probability of dying on Mondays and Saturday, only for men. This finding was confirmed after multivariate adjustment. Finally, a circadian variation in CVD mortality was observed, with a first peak in the morning (8-12 am) and a smaller second peak in the late afternoon (2-6 pm). This pattern persisted after multivariate adjustment and was more pronounced for AMI than for stroke. CONCLUSION: There is a periodicity of hospitalizations and deaths related to CVD, AMI and stroke in Switzerland. This pattern changes slightly according to the age and sex of the subjects. Although the underlying mechanisms are not fully identified, preventive measures should take into account these aspects to develop better strategies of prevention and management of CVD.

Este trabajo trata sobre la edad a la que niños y niñas accedían al trabajo remunerado en la España del siglo XVIII. Utilizando como fuente los Memoriales del Catastro de Ensenada (1751-1753), se propone la edad de diez años como edad media de acceso, a partir de la cual se puede calcular la tasa de actividad. La Mancha era una región rica en industria rural, sobre todo de manufacturas textiles, que abastecían al mercado madrileño, el de otras ciudades e incluso al colonial, y que generaban una fuerte demanda de trabajo femenino. Esta estructura productiva explica por qué las niñas se incorporaban al trabajo remunerado antes que los niños, por qué eran escolarizadas desde muy pequeñas en las ‘escuelas de labor’ y no en las escuelas de primeras letras como los niños, y por qué, en definitiva, sus tasas de alfabetización un siglo después eran inferiores a la ya muy baja media femenina española. En la España del siglo XVIII mujeres y niñas estaban masivamente empleadas en las manufacturas, sobre todo textiles, como ocurría en gran parte de Europa. Entender y contabilizar el trabajo realizado por mujeres y niñas, mucho más difundido de lo que se cree habitualmente, y centrado en manufacturas y servicios, transforma nuestra visión de la estructura de la actividad en la España pre-industrial, normalmente identificada con el trabajo agrícola y ganadero.

The intracellular location of nucleic acid sensors prevents recognition of extracellular self-DNA released by dying cells. However, on forming a complex with the endogenous antimicrobial peptide LL37, extracellular DNA is transported into endosomal compartments of plasmacytoid dendritic cells, leading to activation of Toll-like receptor-9 and induction of type I IFNs. Whether LL37 also transports self-DNA into nonplasmacytoid dendritic cells, leading to type I IFN production via other intracellular DNA receptors is unknown. Here we found that LL37 very efficiently transports self-DNA into monocytes, leading the production of type I IFNs in a Toll-like receptor-independent manner. This type I IFN induction was mediated by double-stranded B form DNA, regardless of its sequence, CpG content, or methylation status, and required signaling through the adaptor protein STING and TBK1 kinase, indicating the involvement of cytosolic DNA sensors. Thus, our study identifies a novel link between the antimicrobial peptides and type I IFN responses involving DNA-dependent activation of cytosolic sensors in monocytes.

The therapeutic efficacy of anticancer chemotherapies may depend on dendritic cells (DCs), which present antigens from dying cancer cells to prime tumor-specific interferon-gamma (IFN-gamma)-producing T lymphocytes. Here we show that dying tumor cells release ATP, which then acts on P2X(7) purinergic receptors from DCs and triggers the NOD-like receptor family, pyrin domain containing-3 protein (NLRP3)-dependent caspase-1 activation complex ('inflammasome'), allowing for the secretion of interleukin-1beta (IL-1beta). The priming of IFN-gamma-producing CD8+ T cells by dying tumor cells fails in the absence of a functional IL-1 receptor 1 and in Nlpr3-deficient (Nlrp3(-/-)) or caspase-1-deficient (Casp-1(-/-)) mice unless exogenous IL-1beta is provided. Accordingly, anticancer chemotherapy turned out to be inefficient against tumors established in purinergic receptor P2rx7(-/-) or Nlrp3(-/-) or Casp1(-/-) hosts. Anthracycline-treated individuals with breast cancer carrying a loss-of-function allele of P2RX7 developed metastatic disease more rapidly than individuals bearing the normal allele. These results indicate that the NLRP3 inflammasome links the innate and adaptive immune responses against dying tumor cells.

En les cures pal·liatives pediàtriques es contemplen les dimensions essencials de la persona: la física, la psicològica, la social i l’espiritual. Tot i que s’han fet estudis sobre la dimensió espiritual, en la revisió bibliogràfica es mostra que és un àmbit poc desenvolupat. És important conèixer l’aspecte de creences i valors per tal que la infermera oncològica, juntament amb altres professionals, puguin proporcionar una atenció holística. L’objectiu d’aquest estudi és identificar les necessitats espirituals en adolescents (13 a 18 anys) malalts de càncer en situació avançada-terminal. S’utilitza la metodologia qualitativa. La mostra és de vuit adolescents, que són seleccionats d’acord amb els criteris d’inclusió. Aquests seran entrevistats per la infermera, la investigadora, la qual realitzarà entrevistes semiestructurades i un fotomuntatge depenent de la predisposició de l’adolescent. L’àmbit de realització d’aquest estudi és en un hospital de tercer nivell on es proporcioni atenció especialitzada amb pediatria oncològica i, concretament, en una unitat de cures pal·liatives pediàtriques. Per finalitzar, és important tenir en compte que les entrevistes i el fotomuntatge poden tenir les seves limitacions i que no sempre s’obtingui el resultat esperat.

Rationale: Although associated with adverse outcomes in other cardiopulmonary conditions, the prognostic value of hyponatremia, a marker of neurohormonal activation, in patients with acute pulmonary embolism (PE) is unknown. Objectives: To examine the associations between hyponatremia and mortality and hospital readmission rates for patients hospitalized with PE. METHODS: We evaluated 13,728 patient discharges with a primary diagnosis of PE from 185 hospitals in Pennsylvania (January 2000 to November 2002). We used random-intercept logistic regression to assess the independent association between serum sodium levels at the time of presentation and mortality and hospital readmission within 30 days, adjusting for patient (race, insurance, severity of illness, use of thrombolytic therapy) and hospital factors (region, size, teaching status). Measurements and Main Results: Hyponatremia (sodium ?135 mmol/L) was present in 2,907 patients (21.1%). Patients with a sodium level greater than 135, 130-135, and less than 130 mmol/L had a cumulative 30-day mortality of 8.0, 13.6, and 28.5% (P < 0.001), and a readmission rate of 11.8, 15.6, and 19.3% (P < 0.001), respectively. Compared with patients with a sodium greater than 135 mmol/L, the adjusted odds of dying were significantly greater for patients with a sodium 130-135 mmol/L (odds ratio [OR], 1.53; 95% confidence interval [CI], 1.33-1.76) and a sodium less than 130 mmol/L (OR, 3.26; 95% CI, 2.48-4.29). The adjusted odds of readmission were also increased for patients with a sodium of 130-135 mmol/L (OR, 1.28; 95% CI, 1.12-1.46) and a sodium less than 130 mmol/L (OR, 1.44; 95% CI, 1.02-2.02). Conclusions: Hyponatremia is common in patients presenting with PE, and is an independent predictor of short-term mortality and hospital readmission.

Recent findings have implicated Fas/Fas ligand (FasL) in mediating the death of keratinocytes in spongiotic lesions. We asked whether dying keratinocytes could potentially initiate a protective response of the skin to limit the destruction of the epidermis in the spongiotic areas. In addition to apoptosis, treatment of keratinocyte cultures in vitro with FasL triggers a profound phoshorylation of the epidermal growth factor receptor (EGFR) and of its downstream effectors ERK and protein kinase B (PKB/Akt). Using a variety of inhibitors and blocking antibodies, we demonstrated that: (i) apoptosis is required for the generation of the signal(s) leading to the activation of EGFR, ERK, and Akt; (ii) the activation of EGFR, ERK, and Akt by FasL is indeed mediated by its bona fide receptor Fas; (iii) the activation of EGFR is essential for the subsequent activation of ERK and Akt; and (iv) apoptotic keratinocytes secrete soluble EGFR ligands (including amphiregulin) that are processed from membrane-bound proligand forms by metalloproteinase(s). Our findings demonstrate a potential mechanism for the restriction and repair of spongiotic damage in eczemas.
