Introdução e objetivos: A síncope recorrente tem um impacto significativo na qualidade de vida. O desenvolvimento de escalas de medida de fácil aplicabilidade clínica para avaliar este impacto é fundamental. O objetivo do presente estudo é a validação preliminar da escala Impact of Syncope on Quality of Life, para a população portuguesa. Métodos: O instrumento foi submetido a um processo de tradução, validação, adequação cultural e cognitive debriefing. Participaram 39 doentes com história de síncopes recorrentes (>1 ano de evolução), submetidos a teste de inclinação em mesa basculante (teste de tilt), que constitui uma amostra de conveniência, com idade de 52,1±16,4 anos (21-83; 43,5% do sexo masculino), a maioria com uma situação profissional ativa (n=18) ou reformados (n=13). A versão portuguesa resultou numa versão semelhante unidimensional à original com 12 itens agregados num único somatório, tendo passado por validação estatística, com avaliação da fidelidade, validade e estabilidade no tempo. Resultados: Em relação à fidelidade, a consistência interna da escala é de 0,9. Avaliámos a validade convergente, tendo obtido resultados estatisticamente significativos (p<0,01). Avaliámos a validade divergente tendo obtido resultados estatisticamente significativos. Relativamente à estabilidade no tempo foi efetuado um teste-reteste do instrumento aos seis meses após o teste de inclinação com 22 doentes desta amostra não submetidos a intervenção clínica, que não mostrou alterações estatisticamente significativas da qualidade de vida. Conclusões: Os resultados obtidos indicam a pertinência da utilização deste instrumento em contexto português na avaliação da qualidade de vida de doentes com síncope recorrente.

INTRODUCTION AND OBJECTIVES: Recurrent syncope has a significant impact on quality of life. The development of measurement scales to assess this impact that are easy to use in clinical settings is crucial. The objective of the present study is a preliminary validation of the Impact of Syncope on Quality of Life questionnaire for the Portuguese population. METHODS: The instrument underwent a process of translation, validation, analysis of cultural appropriateness and cognitive debriefing. A population of 39 patients with a history of recurrent syncope (>1 year) who underwent tilt testing, aged 52.1 ± 16.4 years (21-83), 43.5% male, most in active employment (n=18) or retired (n=13), constituted a convenience sample. The resulting Portuguese version is similar to the original, with 12 items in a single aggregate score, and underwent statistical validation, with assessment of reliability, validity and stability over time. RESULTS: With regard to reliability, the internal consistency of the scale is 0.9. Assessment of convergent and discriminant validity showed statistically significant results (p<0.01). Regarding stability over time, a test-retest of this instrument at six months after tilt testing with 22 patients of the sample who had not undergone any clinical intervention found no statistically significant changes in quality of life. CONCLUSIONS: The results indicate that this instrument is of value for assessing quality of life in patients with recurrent syncope in Portugal.

RESUMO: Introdução: A relação cinemática entre as articulações do CAO apresenta grande importância na função do MS, e é por isso cada vez mais investigada e descrita. O posicionamento da omoplata ganha um importante papel para compreender as DCAO. É indiscutível o importante papel da omoplata na dinâmica do MS, bem como o posicionamento escapular como parâmetro clínico de disfunção do CAO. Todos estes factores criam a necessidade de desenvolver instrumentos de avaliação da posição da articulação ET. A grande maioria dos testes de avaliação, restringem a sua avaliação às disfunções da articulação GU, não integrando uma avaliação mais dinâmica e interactiva que respeite os pressupostos teóricos inerentes ao REU. È importante também que os métodos de avaliação sejam de fácil aplicabilidade clínica e que avaliem fidedignamente e com validade os outcomes. Objectivos: Contribuir para o desenvolvimento de uma metodologia de avaliação da omoplata em diferentes amplitudes do MS, através do estudo da validade concorrente, da fidedignidade intra e inter-observador. Metodologia: A amostra foi constituída por 20 elementos seleccionados por conveniência, entre o corpo de discentes da ESS-IPS, sem história de disfunção do CAO. Foi realizada uma análise cinemática ao MS de cada sujeito, usando um aparelho de análise por varrimento electromagnético, o FOB. Em cada sujeito foram ainda medidas, as distâncias escapulares em estudo, usando a fita métrica. Cada método de medição foi constituído por dois momento (teste e reteste), em cada momento, as medidas eram recolhidas por dois investigadores distintos. Resultados e Discussão: Foram considerados como positivos os resultados que se apresentassem acima do limiar de 0,5, que classifica uma correlação como moderada a excelente. Os resultados da validade mostram que para o investigador 1 nas medidas M1e M2 apenas houve correlação com valores excelentes a moderados até aos 30º de elevação do MS. Já para M3 apenas aos 30º no plano da omoplata essa mesma correlação não tem valores próximos do cutpoint. Em M4 nenhum valor tem correlação significativa com os valores do FOB, chegando mesmo a haver correlação negativa para os 120º no plano da omoplata. Em M5 apenas os 0º mostram valores correlacionais excelentes a moderados. Para o investigador 2, em M1 e M2 á semelhança do investigador 1, só existem valores de correlação significativos até aos 30º de elevação do MS. Já para M3 todos os valores mostram excelente a moderada correlação á excepção dos 120º no plano frontal. Em M4 este investigador apresenta maus resultados. Já em M5 os valores de correlação são moderados aos 0º e aos 90º. No que diz respeitos aos resultados dos CCI intra-observador,podemos afirmar que foi em M5 que estes valores mais se aproximaram do cut point. M1 e M2 são as medidas onde se encontram resultados menos satisfatórios. É aos 60º que existem valores mais satisfatórios, seguidos pelos 0º e 30º, quando nos aproximamos de graus mais elevados, como 90º e 120º, estes valores tendem a baixar. Quanto á fidedignidade inter-observador para M1 apenas aos 90º e aos 120º houve valores de correlação abaixo do cutpoint. Em M2, só os 60º do plano sagital não teve valores acima do cutpoint, em M3 apenas os 30º plano sagital não obtiveram valores acima do cut-point, o mesmo acontece para M3 aos 30º plano da omoplata e em M5 aos 30º e 120º no plano frontal. Conclusão: Os resultados deste estudo indicam que a metodologia em causa apresenta elevado grau de fidedignidade inter-observador, já no que toca é fidedignidade intra-observador o grau de semelhança não é tão elevado. Também o erro associado á medida não ultrapassou 1,5cm, sendo considerado baixo. Na validade concorrente concluímos que é aos 0º que a metodologia se torna uma opção válida na aferição das distâncias medidas com uma boa a excelente concordância com o FOB. As medidas consideradas como opções válidas, nas diferentes amplitudes, podem funcionar como parâmetros clínicos de caracterização do posicionamento da omoplata, podendo vir a contribuir ainda para a caracterização da orientação da mesma.-------------------- ABSTRACT:Introduction: The kinematic relationship between the joints of the CAO has great importance in the function of MS, which is why more and more investigated and described. The positioning of the blade gets an important role in understanding the DCAO. There is no doubt the important role of the scapula in the dynamics of MS as well as the positioning of scapular dysfunction as a clinical parameter of the CAO. All these factors create the need to develop tools for evaluating the place of articulation ET. Most assessment tests, restrict its assessment to GU joint disorders, not incorporating a more dynamic and interactive way that respects the theoretical assumptions inherent in the REU. It is also important that the methods are easy to apply clinical and reliably to assess the validity and outcomes. Objectives: To contribute to the development of a methodology for evaluating the scapula in different ranges of MS, through the study of concurrent validity, reliability of intra-and inter-observer. Methodology: The sample consisted of 20 selected elements for convenience, between the body of students of IPS-ESS with no history of dysfunction of the CAO. We performed a kinematic analysis of each subject to MS, using a scanning device for electromagnetic analysis, the FOB. In each subject were also measured, the scapular distances under study, using tape measure. Each method of measuring the time consisted of two (test and retest), in each moment, the measures were collected by two different investigators. Results and Discussion: We considered as positive results that were above the threshold of 0.5, which ranks as a moderate to excellent correlation. The results show that the validity for the researcher in an action M1e M2 was only correlated with moderate to excellent values up to 30º of elevation of the MS. As for M3 only to 30º in the plane of the scapula has the same correlation values near the cutpoint. M4 has no value in correlation with the values of the FOB, and even negative correlation to 120 ° in the plane of the scapula. In M5 show only the values 0 ° correlational excellent to moderate. For investigator 2 in M1 and M2 will be like an investigator, there are only significant correlation values up to 30º of elevation of the MS. As for M3 all the values show excellent correlation to moderate with the exception of 120 ° in the frontal plane. In this researcher M4 has bad results. M5 already in the correlation values are moderate to 0º and 90 º. Regarding the results of ICC intra-observer, we can say that M5 was that these values come closest to the cut point. M1 and M2 are measures which are less than satisfactory results. At 60 ° there are more satisfactory values, followed by 0º and 30º, when we approached the highest levels, such as 90 ° and 120 °, these values tend to decrease. The inter-observer reliability for M1 only to 90 º and 120 ºcorrelation values were below the cutpoint. In M2, only 60 of the sagital plane did not have values above the cutpoint in M3 only 30º sagital plane did not obtain values above the cut-point, the same goes for M3 at 30 ° plane of the scapula and M5 at 30 º and 120 º in frontal plane. Conclusion: The results of this study indicate that the methodology in question has a high degree of inter-observer reliability, as far as intra-observer reliability is the degree of similarity is not as high. Also the error of measure did not exceed 1.5 cm, and is considered low. In concurrent validity conclude that it is at 0 ° the methodology becomes a valid option for the measurement of distances measured with a good to excellent agreement with the FOB. The measures considered as valid options in different amplitudes, can function as clinical parameters to characterize the positioning of the shoulder blade and could further contribute to the characterization of the orientation of the same.

Introduction: Informal caregivers provide a significant part of the total care needed by dependent older people poststroke. Although informal care is often the preferred option of those who provide and those who receive informal care, informal caregivers often report lack of preparation to take care of older dependent people. This article outlines the development and psychometric testing of informal caregivers’ skills when providing care to older people after a stroke – ECPICID-AVC. Design: Prospective psychometric instrument validation study. Methods: Eleven experts participated in a focus group in order to delineate, develop and validate the instrument. Data were gathered among adult informal caregivers (n = 186) living in the community in Northern Portugal from August 2013 to January 2014. Results: The 32-item scale describes several aspects of informal caregiver’s skills. The scale has eight factors: skill to feed/hydrate by nasogastric feeding, skill to assist the person in personal hygiene, skill to assist the person for transferring, skill to assist the person for positioning, skill to provide technical aids, skill to assist the person to use the toilet, skill to feed/hydrate and skill to provide technical aids for dressing/undressing. Analysis demonstrated adequate internal consistency (Cronbach’s alpha = 0.83) and good temporal stability 0.988 (0.984–0.991). Conclusion: The psychometric properties of the measurement tool showed acceptable results allowing its implementation in clinical practice by the nursing community staff for evaluating practical skills in informal caregivers when providing care to older stroke survivors living at home.

Cultured primary fetal cells from one organ donation could possibly meet the exigent and stringent technical aspects for development of therapeutic products. These cell types have fewer technological limitations for cellular proliferation capacity (simple culture conditions) and maintenance of differentiated phenotype, and they also have low probability for transmission of communicable diseases. Master and Working Cell Banks (MCB, WCB) can be obtained from one fetal organ donation, permitting multiple tissues (skin, bone, cartilage, muscle and intervertebral disc) to be processed in short periods of time with identical methods to assure a stringent tracing of the processes for the production of standardized therapeutic agents. Clinical use of biologics from embryo and fetal tissues is relatively new and current legislation and ethics have some differences between countries to date. In addition, specific cell delivery systems for each tissue type can be adapted to the clinical application. Since it is the intention that banked primary fetal cells enhance the prospective treatment of hundreds of thousands of patients with only one organ donation, it is imperative to show consistency, traceability and safety of the processes including donor tissue selection, cell banking, cell testing and growth of cells in out-scaling for the preparation of bio-engineered products for clinical application.

BACKGROUND: Comparative effectiveness research in spine surgery is still a rarity. In this study, pain alleviation and quality of life (QoL) improvement after lumbar total disc arthroplasty (TDA) and anterior lumbar interbody fusion (ALIF) were anonymously compared by surgeon and implant. METHODS: A total of 534 monosegmental TDAs from the SWISSspine registry were analyzed. Mean age was 42 years (19-65 years), 59% were females. Fifty cases with ALIF were documented in the international Spine Tango registry and used as concurrent comparator group for the pain analysis. Mean age was 46 years (21-69 years), 78% were females. The average follow-up time in both samples was 1 year. Comparison of back/leg pain alleviation and QoL improvement was performed. Unadjusted and adjusted probabilities for achievement of minimum clinically relevant improvements of 18 VAS points or 0.25 EQ-5D points were calculated for each surgeon. RESULTS: Mean preoperative back pain decreased from 69 to 30 points at 1 year (ØΔ 39pts) after TDA, and from 66 to 27 points after ALIF (ØΔ 39pts). Mean preoperative QoL improved from 0.34 to 0.74 points at 1 year (ØΔ 0.40pts). There were surgeons with better patient selection, indicated by lower adjusted probabilities reflecting worsening of outcomes if they had treated an average patient sample. ALIF had similar pain alleviation than TDA. CONCLUSIONS: Pain alleviation after TDA and ALIF was similar. Differences in surgeon's patient selection based on pain and QoL were revealed. Some surgeons seem to miss the full therapeutic potential of TDA by selecting patients with lower symptom severity.

BACKGROUND Several questionnaires have been used to measure health related quality of life (HRQoL) in patients with psoriasis, few have been adapted for use in Spain; none of them was developed specifically for the Spanish population. The purpose of the study was to validate and assess the sensitivity to change of a new questionnaire to measure HRQOL in patients with psoriasis (PSO-LIFE). METHODS Observational, prospective, multicenter study performed in centers around Spain. Patients with active or inactive psoriasis completed the PSO-LIFE together with other Dermatology Quality of Life Index (DLQI) and Psoriasis Disability Index (PDI). A control group of patients with urticaria or atopic dermatitis was also included. Internal consistency and test-retest reliability of the PSO-LIFE were assessed by calculating Cronbach's alpha and Intraclass Correlation Coefficient (ICC). Validity was assessed by examining factorial structure, the capacity to discriminate between groups, and correlations with other measures. Sensitivity to change was measured using effect sizes. RESULTS The final sample included for analysis consisted of 304 patients and 56 controls. Mean (SD) age of psoriasis patients was 45.3 (14.5) years compared to 38.8 (14) years for controls (p < 0.01). Cronbach's alpha for the PSO-LIFE was 0.95 and test-retest reliability using the ICC was 0.98. Factor analysis showed the questionnaire to be unidimensional. Mean (SD) PSO-LIFE scores differed between patients with psoriasis and controls (64.9 [22.5] vs 69.4 [17.3]; p < 0.05), between those with active and inactive disease (57.4 [20.4] vs 76.4 [20.6]; p < 0.01), and between those with visible and non-visible lesions (63.0 [21.9] vs. 74.8 [23.9]; p < 0.01). The correlation between PSO-LIFE and PASI scores was moderate (r = -0.43) while correlations with DLQI and PDI dimensions ranged from moderate to high (between 0.4 and 0.8). Effect size on the PSO-LIFE in patients reporting 'much improved' health status at study completion was 1.01 (large effect size). CONCLUSIONS The present results provide substantial support for the reliability, validity, and responsiveness of the PSO-LIFE questionnaire in the population for which it was designed.

BACKGROUND The purpose of the present study is to translate and validate the "Hip and Knee Outcomes Questionnaire", developed in English, into Spanish. The 'Hip and Knee Outcomes Questionnaire is a questionnaire planned to evaluate the impact in quality of life of any problem related to the human musculoskeletal system. 10 scientific associations developed it. METHODS The questionnaire underwent a validated translation/retro-translation process. Patients undergoing primary knee arthroplasty, before and six months postoperative, tested the final version in Spanish. Psychometric properties of feasibility, reliability, validity and sensitivity to change were assessed. Convergent validity with SF-36 and WOMAC questionnaires was evaluated. RESULTS 316 patients were included. Feasibility: a high number of missing items in questions 3, 4 and 5 were observed. The number of patients with a missing item was 171 (51.35%) in the preoperative visit and 139 (44.0%) at the postoperative. Internal validity: revision of coefficients in the item-rest correlation recommended removing question 6 during the preoperative visit (coefficient <0.20). Convergent validity: coefficients of correlation with WOMAC and SF-36 scales confirm the questionnaire's validity. Sensitivity to change: statistically significant differences were found between the mean scores of the first visit compared to the postoperative. CONCLUSION The proposed translation to Spanish of the 'Hip and Knee Questionnaire' is found to be reliable, valid and sensible to changes produced at the clinical practice of patients undergoing primary knee arthroplasty. However, some changes at the completion instructions are recommended. LEVEL OF EVIDENCE Level I. Prognostic study.

OBJECTIVE: Depth of emotional processing has shown to be related to outcome across approaches to psychotherapy. Moreover, a specific emotional sequence has been postulated and tested in several studies on experiential psychotherapy (Pascual-Leone & Greenberg, 2007). This process-outcome study aims at reproducing the sequential model of emotional processing in psychodynamic psychotherapy for adjustment disorder and linking these variables with ultimate therapeutic outcome. METHOD: In this study, 32 patients underwent short-term dynamic psychotherapy. On the basis of reliable clinical change statistics, a subgroup (n = 16) presented with good outcome and another subgroup (n = 16) had a poor outcome in the end of treatment. The strongest alliance session of each case was rated using the observer-rated system Classification of Affective Meaning States. Reliability coefficients for the measure were excellent (κ = .82). RESULTS: Using 1 min as the fine-grained unit of analysis, results showed that the experience of fundamentally adaptive grief was more common in the in-session process of patients with good outcome, compared with those with poor outcomes (χ2 = 6.56, p = .01, d = 1.23). This variable alone predicted 19% of the change in depressive symptoms as measured by the Beck Depression Inventory at the end of treatment. Moreover, sequences of the original model were supported and related to outcome. CONCLUSIONS: These results are discussed within the framework of the sequential model of emotional processing and its possible relevance for psychodynamic psychotherapy. Tissue-specific stem cells found in adult tissues can participate in the repair process following injury. However, adult tissues, such as articular cartilage and intervertebral disc, have low regeneration capacity, whereas fetal tissues, such as articular cartilage, show high regeneration ability. The presence of fetal stem cells in fetal cartilaginous tissues and their involvement in the regeneration of fetal cartilage is unknown. The aim of the study was to assess the chondrogenic differentiation and the plasticity of fetal cartilaginous cells. We compared the TGF-β3-induced chondrogenic differentiation of human fetal cells isolated from spine and cartilage tissues to that of human bone marrow stromal cells (BMSC). Stem cell surface markers and adipogenic and osteogenic plasticity of the two fetal cell types were also assessed. TGF-β3 stimulation of fetal cells cultured in high cell density led to the production of aggrecan, type I and II collagens, and variable levels of type X collagen. Although fetal cells showed the same pattern of surface stem cell markers as BMSCs, both type of fetal cells had lower adipogenic and osteogenic differentiation capacity than BMSCs. Fetal cells from femoral head showed higher adipogenic differentiation than fetal cells from spine. These results show that fetal cells are already differentiated cells and may be a good compromise between stem cells and adult tissue cells for a cell-based therapy.

OBJECTIVE: To compare image quality of a standard-dose (SD) and a low-dose (LD) cervical spine CT protocol using filtered back-projection (FBP) and iterative reconstruction (IR). MATERIALS AND METHODS: Forty patients investigated by cervical spine CT were prospectively randomised into two groups: SD (120 kVp, 275 mAs) and LD (120 kVp, 150 mAs), both applying automatic tube current modulation. Data were reconstructed using both FBP and sinogram-affirmed IR. Image noise, signal-to-noise (SNR) and contrast-to-noise (CNR) ratios were measured. Two radiologists independently and blindly assessed the following anatomical structures at C3-C4 and C6-C7 levels, using a four-point scale: intervertebral disc, content of neural foramina and dural sac, ligaments, soft tissues and vertebrae. They subsequently rated overall image quality using a ten-point scale. RESULTS: For both protocols and at each disc level, IR significantly decreased image noise and increased SNR and CNR, compared with FBP. SNR and CNR were statistically equivalent in LD-IR and SD-FBP protocols. Regardless of the dose and disc level, the qualitative scores with IR compared with FBP, and with LD-IR compared with SD-FBP, were significantly higher or not statistically different for intervertebral discs, neural foramina and ligaments, while significantly lower or not statistically different for soft tissues and vertebrae. The overall image quality scores were significantly higher with IR compared with FBP, and with LD-IR compared with SD-FBP. CONCLUSION: LD-IR cervical spine CT provides better image quality for intervertebral discs, neural foramina and ligaments, and worse image quality for soft tissues and vertebrae, compared with SD-FBP, while reducing radiation dose by approximately 40 %.

El treball de recerca del Màster en Arxivística i Gestió de Documents de l’ESAGED (Escola Superior d’Arxivística i Gestió de Documents) ha consistit en l’estudi i l’avaluació del registre general d’entrades i sortides de documents de l’Ajuntament de Santa Perpètua de Mogoda, en fase de producció. A més d’estudiar-ne el context extern i intern s’han auditat les funcionalitats de l’eina tecnològica que gestiona el Registre General. S’explica el desenvolupament de la recerca i els resultats obtinguts. Finalment, es presenten les propostes de millora en fase de producció a fi d’augmentar l’autenticitat i la integritats de les dades, i també unes propostes de conservació del Registre General actual. RÉSUMÉ L’étiologie de la schizophrénie est complexe et le modèle de vulnérabilité-stress (Nuechterlein & Dawson, 1984) propose que des facteurs de vulnérabilité d’ordre génétique combinés à une histoire environnementale de stress particulier pousseraient l’individu vers un état clinique de psychose. L’objectif principal de cette thèse est de mieux comprendre la réaction physiologique des personnes schizophrènes face à un stress psychologique, tout en conceptualisant les symptômes psychotiques comme faisant partie d’un continuum, plutôt que de les restreindre sur un plan catégoriel. Afin de faire la différence entre les patients schizophrènes et les individus de la population générale, au-delà de la sévérité de leurs symptômes psychotiques, leur réaction au stress est comparée et le phénomène de seuil critique dans la réaction de cortisol est exploré en tant que point décisif pouvant distinguer entre les deux groupes. La première étude de cette thèse (Brenner et al., 2007) examine la fiabilité, la validité et la structure factorielle du Community Assessment of Psychic Experiences (CAPE) (Stefanis et al., 2002), avec un échantillon francophone et anglophone de la population nord américaine, un questionnaire auto-administré de 42 items qui évalue les expériences quasi-psychotiques présentes dans la population générale : des symptômes positifs (ou psychotiques), négatifs (ou végétatifs) et dépressifs. Ce questionnaire a été complété par un échantillon de 2 275 personnes de la population montréalaise. Les résultats appuient la consistance interne des 3 sous-échelles originales. De plus, l’analyse factorielle exploratoire suggère des solutions de 3-5 facteurs, où les solutions à 4 et 5 facteurs proposent de séparer les symptômes positifs en sous-catégories plus spécifiques. Finalement, cette étude suggère une version plus courte du CAPE, avec seulement 23 items, tout en préservant les mêmes trois échelles originales. La toile de fond de cet article confirme l’existence du phénomène du continuum de la psychose, où une variation de symptômes psychotiques peut se retrouver aussi bien dans la population générale que dans la population clinique. Dans une deuxième étude (Brenner et al., 2009), cette thèse examine à quel point la réponse de l’hormone de stress, le cortisol, à un test de stress psychosocial nommé le Trier Social Stress Test (TSST) (Kirschbaum, Pirke, & Hellhammer, 1993), peut établir une différence entre les sujets témoins et les patients schizophrènes, tout en contrôlant des variables importantes. Un groupe de 30 personnes schizophrènes et un groupe de 30 sujets de la population générale, recrutés lors de la première étude de cette thèse, ont participé à cette recherche qui est construite selon un plan expérimental. Le groupe témoin inclut des personnes légèrement symptomatiques et un chevauchement des scores psychotiques existe entre les deux groupes. Suite au stresseur, les deux groupes démontrent une augmentation significative de leur rythme cardiaque et de leur pression artérielle. Cependant, leur réponse de cortisol a tendance à différer : les patients schizophrènes présentent une réponse de cortisol plus petite que celle des témoins, mais en atteignant un seuil statistique significatif seulement à la mesure qui suit immédiatement le stresseur. Ces résultats significatifs sont obtenus en contrôlant pour la sévérité des symptômes positifs, un facteur discriminant significatif entre les deux groupes. Ainsi, le niveau de cortisol mesuré immédiatement après le stresseur se révèle être un marqueur de seuil critique pouvant établir une distinction entre les deux groupes. Aussi, leur réponse de cortisol maximale a tendance à apparaître plus tard que chez les sujets témoins. De façon générale, la réaction au stress des deux groupes étudiés est un autre moyen d’observer la continuité d’un comportement présent chez les individus, jusqu’à ce qu’un seuil critique soit atteint. Ainsi, il est possible de trancher, à un moment donné, entre psychose clinique ou absence de diagnostic.

La recherche sur l’utilisation des connaissances issues de la recherche (CIR) est en pleine expansion. Plusieurs études ont montré que l’utilisation des CIR contribue à améliorer la réussite scolaire des élèves. Toutefois, il semble que les enseignants les utilisent très peu. Pour améliorer cette utilisation, il est nécessaire de développer un modèle explicatif de l’utilisation des connaissances issues de la recherche. Ce modèle permet de rendre compte des mécanismes menant à l’utilisation des connaissances issues de la recherche en milieu de pratique. Il permet également de mettre en évidence les facteurs les plus influents et l’agencement de ces différents facteurs dans le but de favoriser une meilleure utilisation des connaissances issues de la recherche. Ce dernier aspect constitue l’objectif principal de cette thèse pour le domaine spécifique de l’éducation. Pour ce faire, un questionnaire sur l’utilisation des connaissances issues de la recherche (QUC) est validé et un modèle est proposé. Le premier article de la thèse examine la fiabilité, la validité et la structure factorielle du QUC avec un échantillon francophone et anglophone d’un milieu défavorisé. Le QUC, composé de 43 items, mesure six dimensions, soit : l’utilisation des connaissances issues de la recherche, le contexte organisationnel, la stratégie de soutien, l’opinion des utilisateurs et leur expertise ainsi que la fréquence d’accès aux sources des connaissances issues de la recherche. Au total, 2270 enseignants ont complété le questionnaire. Les résultats des différentes analyses montrent que le QUC doit être réduit à 20 items regroupés en cinq facteurs : l’utilisation des connaissances issues de la recherche, l’opinion des utilisateurs, la stratégie de soutien, l’expertise des utilisateurs et les facteurs organisationnels. Le second article de cette thèse examine quel modèle permet de mieux expliquer l’utilisation des CIR. Pour ce faire, deux modèles sont testés avec le même échantillon que dans le premier article, soit un modèle de liens directs et un modèle avec médiation. Pour le premier modèle, un lien direct entre l’utilisation des connaissances issues de la recherche et les facteurs qui favorisent cette utilisation est postulé : l’expertise, l’opinion, la stratégie de soutien et le facteur organisationnel auront des liens directs avec l’utilisation des connaissances issues de la recherche. Le second modèle (modèle avec médiation) se base quant à lui sur les théories de l’action raisonnée et du comportement planifié et postule que le lien entre les facteurs et l’utilisation n’est pas direct; certains facteurs ont des effets directs et d’autres des effets indirects. Concrètement, ce second modèle postule que l’expertise et l’opinion agissent comme médiateurs entre, d’une part, les deux variables indépendantes (le facteur organisationnel et la stratégie de soutien), et, d’autre part, la variable dépendante (l’utilisation des connaissances issues de la recherche). Les résultats des analyses montrent que le modèle avec médiation permet de mieux comprendre l’utilisation des connaissances issues de la recherche. Cette étude a permis de valider un questionnaire sur l’utilisation des connaissances issues de la recherche et aboutit à la proposition d’un modèle qui permet d’expliquer l’utilisation des connaissances issues de la recherche spécialement conçu pour le domaine de l’éducation. Elle fournit des outils (un questionnaire validé et un modèle) qui permettent de mieux cerner le paradigme de l’utilisation des connaissances issues de la recherche, ce qui pourrait favoriser une plus grande utilisation des CIR par les intervenants scolaires. En effet, les résultats de cette recherche peuvent guider les décideurs dans l’implantation des programmes menant à l’utilisation des connaissances issues de la recherche qui s’adressent aux enseignants. Ces résultats indiquent sur quels facteurs agir en premier lieu afin d’améliorer l’utilisation des CIR chez les enseignants du secondaire dans les milieux défavorisés. Le présent modèle pourrait être utilisé dans d’autres milieux scolaires (par exemple les milieux non défavorisés ou les milieux scolaires primaires) après validation.
