While researchers have extensively evaluated the beneficial effects of coffee consumption in reducing the frequency of certain diseases, studies examining the differences between organic and conventional coffee intake are still needed. Therefore, this paper aims to investigate the functional effects of organic and conventional coffee by examining both its chemical composition and its mutagenic/antimutagenic properties. Infusions of 10% or 20% (w/v) of organic and conventional coffee were administered by gavage (10 mL/kg b.w., once or twice a day) to male Swiss mice against doxorubicin (DXR) and 1,2-dimethylhydrazine dihydrochloride (DMH)-induced mutagenicity. The levels of chlorogenic acids, caffeine and trigonelline from the coffee infusions and oxidative stress analysis from the liver were measured by HPLC. Gut and bone marrow micronucleus assays were used as mutagenic/antimutagenic endpoints, as well as the crypt measurements and gut apoptosis index. The in vivo tests revealed that only organic coffee exerted protective effects, despite oxidative stress analysis and crypt measurements not showing differences among treatments. Intriguingly, the low dose (10% w/v mL/kg) displayed a robust protective effect that showed a significant reduction in bone marrow micronuclei (26.8%), gut micronuclei (11.5%) and apoptosis (27.8%), whereas the higher coffee dose (2 × 20% w/v) only showed a protective effect against bone marrow micronucleus (43.7%). These results highlight that organic coffee could be considered to have beneficial functional effects, although it is still a challenge to define conclusions from analytical data and all the possible interactions from this complex food matrix. © 2013 Elsevier Ltd. Background: The present study aims to evaluate the effects of orthodontic movement (OM) on the periodontal tissues of rats with ligature-induced periodontal disease. Methods: Eighty-eight rats were divided into four groups: 1) negative control (sham operated); 2) periodontal disease; 3) OM; and 4) periodontal disease followed by OM (OMP). Rats were sacrificed 3 hours or 1, 3, or 7 days after OM commencement. Bone volume fraction (BVF) and bone mineral density (BMD) were assessed in hemimaxillae by microcomputed tomography analysis. Expression of the proinflammatory cytokines interleukin (IL)-1β and tumor necrosis factor (TNF)-α were evaluated in gingival samples by quantitative polymerase chain reaction and enzyme-linked immunosorbent assay, and in the furcation region by immunohistochemistry analysis (IHC). Results: The OMP group had lower BVF and BMD levels compared to the other groups at day 7 (P <0.05). Maximum messenger ribonucleic acid expression of both cytokines was observed in the OMP group at day 1 (P <0.05). In the same period, all proteins were expressed in high levels for all test groups compared to the control group. The number of cells positive for IL-1β and TNF-α by IHC was highest in the OMP group at day 1, with progressive reduction thereafter. Conclusion: The results suggest that OM acts synergistically with periodontal disease in periodontal breakdown through upregulation of proinflammatory cytokines.

Chronic inflammatory processes close to bone often lead to loss of bone in diseases such as rheumatoid arthritis, periodontitis, loosened joint prosthesis and tooth implants. This is mainly due to local formation of bone resorbing osteoclasts which degrade bone without any subsequent coupling to new bone formation. Crucial for osteoclastogenesis is stimulation of mononuclear osteoclast progenitors by macrophage colony-stimulating factor (M-CSF) and receptor activator of nuclear factor-κB ligand (RANKL) which induces their differentiation along the osteoclastic lineage and the fusion to mature, multinucleated osteoclasts. M-CSF and RANKL are produced by osteoblasts/ osteocytes and by synovial and periodontal fibroblasts and the expression is regulated by pro- and anti-inflammatory cytokines. These cytokines also regulate osteoclastic differentiation by direct effects on the progenitor cells. In the present overview, we introduce the basic concepts of osteoclast progenitor cell differentiation and summarize the current knowledge on cytokines stimulating and inhibiting osteoclastogenesis by direct and indirect mechanisms. © Informa Healthcare USA, Inc.

Doenças infecciosas crônicas, como a tuberculose (Tb), são causas de distúrbios metabólicos que interferem na utilização e mobilização de nutrientes corporais, afetando o estado nutricional. A função imunológica de doentes desnutridos é alterada, o que, provavelmente, implica maior incidência de infecções. Neste estudo objetivou-se diagnosticar o estado nutricional de pacientes recém-internados por tuberculose no Hospital Universitário João de Barros Barreto, em Belém, aplicando uma técnica de avaliação subjetiva global (ASG) e verificando sua concordância com um método objetivo, o índice de massa corporal (IMC). Foi descritivo e parte dele descritivo-analítico, transversal, tendo como população de referência pacientes de tuberculose encaminhados para hospitalização, excluindo aqueles com meningite tuberculosa. As variáveis controladas foram peso, altura, sexo, formas de Tb, alterações de peso, de gordura subcutânea, de massa muscular, da ingesta alimentar e da capacidade funcional, a presença de sintomas gastrointestinais, sinais de edema e ascite. O perfil nutricional observado foi de 77,8% de desnutridos, sendo, destes, 83,78% moderados e 16,22% graves, com um IMC médio de 18,36 ± 3,86 Kg/m². A maioria relatou perda ponderal nos últimos seis meses antes da internação, em níveis de 5% a 10% do seu peso habitual; 73,82% relataram modificação para menos na ingesta alimentar, e presença de sintomas gastrointestinais em 64,58% deles, sendo a anorexia o mais freqüente, seguida de náuseas, vômitos e diarréia. A perda da capacidade funcional foi referida em 100% (nutridos e desnutridos). Ao exame físico foi possível perceber que 79,16% apresentavam perda de gordura subcutânea e massa muscular, conjuntamente, e nenhum caso apresentou edema ou ascite. A Tb pulmonar foi a mais incidente, sendo encontrados três casos de Tb ganglionar e um de Tb óssea, associada à SIDA. O IMC, em relação à ASG, apresentou um coeficiente de Kappa igual a 0,47, validade sensível de 67% e especificidade de 100%. A ASG se mostrou uma técnica de boa aplicabilidade, demonstrando na prática dados inerentes não só ao estado nutricional, mas também ao modo de vida dos indivíduos, oferecendo subsídios relevantes para a terapêutica no nível hospitalar. O diagnóstico nutricional mostrou uma prevalência elevada de desnutridos, tanto pelo método subjetivo, como pelo objetivo e que a preocupação com o estado nutricional precoce propicia uma adequada intervenção nutricional. Os resultados recomendam a continuidade deste estudo, avaliando a situação nutricional quando da internação e acompanhando, periodicamente, cada caso, de modo a subsidiar melhor assistência por parte de toda a equipe de saúde a este grupo de doentes.

Mucopolysaccharidoses (MPS) are rare genetic diseases caused by the deficiency of one of the lysosomal enzymes involved in the glycosaminoglycan (GAG) breakdown pathway. This metabolic block leads to the accumulation of GAG in various organs and tissues of the affected patients, resulting in a multisystemic clinical picture, sometimes including cognitive impairment. Until the beginning of the XXI century, treatment was mainly supportive. Bone marrow transplantation improved the natural course of the disease in some types of MPS, but the morbidity and mortality restricted its use to selected cases. The identification of the genes involved, the new molecular biology tools and the availability of animal models made it possible to develop specific enzyme replacement therapies (ERT) for these diseases. At present, a great number of Brazilian medical centers from all regions of the country have experience with ERT for MPS I, II, and VI, acquired not only through patient treatment but also in clinical trials. Taking the three types of MPS together, over 200 patients have been treated with ERT in our country. This document summarizes the experience of the professionals involved, along with the data available in the international literature, bringing together and harmonizing the information available on the management of these severe and progressive diseases, thus disclosing new prospects for Brazilian patients affected by these conditions.

The head is the most important and specialized region in the body because it contains a range of specialized organs and, because it has interconnections between specialized organs, there is a great overlap of images. Thus, computed tomography (CT) helps in diagnosing diseases in this region, such as oral conditions, as they provide millimetric slices or cuts and demonstrate the relationship between the various anatomical structures involved, in volume and depth. Within dentistry, CT helps in the identification of pathological processes such as infection, tumors, visualization of embedded teeth and bone bed. This study aimed to assess the density of the mandibular alveolar bone at a determined point to later predict how periodontal disease is involved in bone resorption. For this, we performed a blind retrospective study (n = 124) of the CT scan files of dog skulls at FMVZ-UNESP in order to determine the density of the jaw bone using a Hounsfield scale, in the region of the dental apex of the cranial root of the first molar tooth in dogs. The results obtained were evaluated using mean and standard deviation (27.28 +/- 9.53 HU) in order to predict the normal density of the mandibular alveolar bone in the studied region. Thus, this data analysis allows a more concise evaluation of bone resorption of mandibular alveolar bone and, therefore, provides an adequate surgical planning in cases of osteosynthesis given mainly by the presence of installed periodontal disease.
