Uma possível disfunção nos processos intracelulares de transdução de sinais pode estar implicada na fisiopatologia do transtorno bipolar (Soares and Mallinger 2000). Particularmente, a via intracelular do fosfoinositol (PI) pode ser um sitio de disfunção (Soares et al 1997b). Plaquetas, células periféricas, e amostras de cérebro pós-mortem têm sido utilizados como modelos em estudos prévios com o objetivo de investigar esta hipótese. Os achados que emergem destes estudos são consistentes com a hipótese de uma hiperatividade na via do PI na fase maníaca, que pode estar relacionada com a fase do transtorno. Nesta tese de doutoramento está descrita uma série de estudos preliminares que utilizaram um modelo plaquetário para estudar alterações da via do PI em pacientes com transtorno afetivo bipolar, e também em relação ao mecanismo de ação do carbonato de lítio. Utilizando um método para quantificação de fosfoinositois em membrana plaquetária desenvolvido pelo Professor Alan Mallinger e colaboradores na Universidade de Pittsburgh, o passo inicial envolveu o estudo da reprodutibilidade teste/reteste do mesmo, que foi documentada como estando dentro de limites aceitáveis (Soares et al 1999a). Subseqüentemente, conduzimos um estudo preliminar em pacientes bipolares já tratados com monoterapia com o lítio (Soares et al 1999b). Nestes pacientes, alem de estudarmos os níveis de fosfolipídios de membrana comparados com voluntários normais, também estudamos, em colaboração com o Professor Husseini Manji e colaboradores do National Institute of Mental Health (NIMH), EUA, os níveis de subespécies especificas de PKC em plaquetas de pacientes bipolares (Soares et al 2000a). Nossos achados principais destes estudos colaborativos sugerem uma redução dos níveis plaquetários de PIP2, concomitantemente com redução seletiva de subespécies especificas de PKC. Como um passo adicional, também estudamos os níveis de fosfoinositois de membrana em indivíduos bipolares na fase depressiva, quando não medicados, que estavam significativamente elevados comparados com voluntários normais (Soares et al 2001). Por último, conduzimos um ensaio clinico aberto com carbonato de lítio em doses terapêuticas em um grupo pequeno de pacientes bipolares, que sugeriu uma diminuição significativa dos níveis de PIP2 após o tratamento (Soares et al 2000b). Estes estudos documentam uma disfunção na via do PI em plaquetas de pacientes bipolares, possivelmente modulada pelo tratamento com o lítio, sugerindo o envolvimento desta via na fisiopatologia do transtorno e no mecanismo de ação do lítio. não se sabe se tais alterações plaquetárias refletem o que se passa nesta via em neurônios humanos in vivo, o que constitui uma limitação importante, em potencial, desta abordagem. No entanto, na falta de metodologia de neuroimagem que permita o estudo destas vias no cérebro de pacientes vivos, o modelo periférico plaquetário é de valia para se começar a fazer inferências sobre o funcionamento desta via em pacientes bipolares. Estudos futuros com amostras maiores de pacientes que possam segui-los em fases diferentes do transtorno e tentar determinar correlações entre alterações nesta via produzidas pelo lítio e os efeitos terapêuticos desta medicação serão de grande valia.

Although the bipolar disorder (BD) occurs almost with the same frequency in both genders, the phenomenology and the outcome of the illness differ between them. Nevertheless, there is evidence that women with BD show, more than men, delayed beginning, especially in their fifth decade, more rapid cycling outcome, more depressive episodes, more dysphoric mania, more mixed states and more BD type II. Even so, the findings are not always consistent. Although the risk of comorbidities in BD includes, for both the sorts, excessive alcoholic consumption and drugs, bipolar men would have greater probability of being alcohol dependent, of not seeking treatment and of committing suicide. Suggested hypotheses to explain such differences vary from those centered in cultural or psychological aspects to those that focus on the steroids hormones, and other hormones such as cortisol, thyroid hormones and even on the cerebral anatomy. The reproductive cycle (menstrual cycle, pregnancy and menopause) influences on the BD phenomenology and its relevance to the therapeutical options in the treatment of the BD in women are presented in the last part of this review. Further investigations must to be done in order to clarify this controversy. However, up to now the data indicate that estrogen therapy is not to be primarily indicated to prevent depression, Alzheimer disease or cognition impairment.

To quantify the effects of methylmercury (MeHg) on amacrine and on ON-bipolar cells in the retina, experiments were performed in MeHg-exposed groups of adult trahiras (Hoplias malabaricus) at two dose levels (2 and 6 µg/g, ip). The retinas of test and control groups were processed by mouse anti-parvalbumin and rabbit anti-aprotein kinase C (aPKC) immunocytochemistry. Morphology and soma location in the inner nuclear layer were used to identify immunoreactive parvalbumin (PV-IR) and aPKC (aPKC-IR) in wholemount preparations. Cell density, topography and isodensity maps were estimated using confocal images. PV-IR was detected in amacrine cells in the inner nuclear layer and in displaced amacrine cells from the ganglion cell layer, and aPKC-IR was detected in ON-bipolar cells. The MeHg-treated group (6 µg/g) showed significant reduction of the ON-bipolar aPKC-IR cell density (mean density = 1306 ± 393 cells/mm2) compared to control (1886 ± 892 cells/mm2; P < 0.001). The mean densities found for amacrine PV-IR cells in MeHg-treated retinas were 1040 ± 56 cells/mm2 (2 µg/g) and 845 ± 82 cells/mm2 (6 µg/g), also lower than control (1312 ± 31 cells/mm2; P < 0.05), differently from the data observed in displaced PV-IR amacrine cells. These results show that MeHg changed the PV-IR amacrine cell density in a dose-dependent way, and reduced the density of aKC-IR bipolar cells at the dose of 6 µg/g. Further studies are needed to identify the physiological impact of these findings on visual function.

Eacy system consists of (2+1)-dimensional evolution equations for two complex functions and of quite strong differential constraints. It may be interpreted formally as an integro-differential equation in (1+1) dimensions. © 1988.

Repetitive transcranial magnetic stimulation (rTMS) has been widely tested and shown to be effective for unipolar depression. Although it has also been investigated for bipolar depression (BD), there are only few rTMS studies with BD. Here, we investigated 56 patients with BD who received rTMS treatment until remission (defined as Hamilton Depression Rating Scores <= 7). We used simple and multiple logistic regressions to identify clinical and demographic predictors associated with duration of treatment (defined as <15 vs. >15 rTMS sessions). Age, refractoriness, number of prior depressive episodes, and severe depression at baseline were associated with a longer rTMS treatment. In the multivariate analysis, refractoriness (likelihood ratio (LR) = 4.33; p < 0.01) and baseline severity (LR = 0.18, p < 0.01) remained significant predictors. Our preliminary study showed that, in remitted patients, refractoriness and severity of index episode are associated with the need of a longer rTMS treatment; providing preliminary evidence of important factors associated with rTMS parameters adjustment.

V393 Scorpii is a double periodic variable characterized by a relatively stable non-orbital photometric cycle of 253 d. Mennickent et al. argue for the presence of a massive optically thick disc around the more massive B-type component and describe the evolutionary stage of the system. In this paper, we analyse the behaviour of the main spectroscopic optical lines during the long non-orbital photometric cycle. We study the radial velocity of the donor determining its orbital elements and find a small but significant orbital eccentricity (e = 0.04). The donor spectral features are modelled and removed from the spectrum at every observing epoch using the light-curve model given by Mennickent et al. We find that the line emission is larger during eclipses and mostly comes from a bipolar wind. We also find that the long cycle is explained in terms of a modulation of the wind strength; the wind has a larger line and continuum emissivity at the high state. We report the discovery of highly variable chromospheric emission in the donor, as revealed by the Doppler maps of the emission lines Mg II 4481 and C I 6588. We discuss notable and some novel spectroscopic features like discrete absorption components, especially visible at blue depressed O I 7773 absorption wings during the second half-cycle, Balmer double emission with V/R curves showing 'Z-type' and 'S-type' excursions around secondary and main eclipses, respectively, and H beta emission wings extending up to +/- 2000 km s(-1). We also discuss possible causes for these phenomena and for their modulations with the long cycle.
