OBJETIVO: Determinação da freqüência das alterações cardíacas e sua evolução nas crianças expostas ao HIV-1 por via perinatal. MÉTODOS: Realizada avaliação seqüencial clínico-cardiológica, eletrocardiográfica e ecocardiográfica Doppler em 84 crianças expostas ao HIV-1. RESULTADOS: Grupo I (sororreversão) 43 crianças (51,2%). Ausência de alterações clínicas. ECG: distúrbio de condução de ramo direito 5 casos. ECO: CIA (1 caso) e CIV (1 caso). Grupo II 41 infectados (48,8%) com 51,2% de alterações cardiológicas. Crianças assintomáticas ou com sintomas leves, sem imunossupressão: alterações clínico-ecocardiográficas ausentes; ECG: distúrbio de condução de ramo direito (2 casos). Crianças com comprometimento clínico-imunológico moderado e severo: Alterações encontradas: 1) Clínicas (31,7%): taquicardia isolada (1 caso), ICC (12 casos). 2) Eletrocardiográficas (43,9%): taquicardia sinusal associada a outras alterações (10 casos), distúrbio de condução de ramo direito (5 casos), BDAS (1 caso), HBAD (1 caso), alterações da repolarização ventricular (11 casos), SVD (2 casos), SVE (1 caso), desvio do AQRS para direita (1 caso), arritmias (3 casos). 3) Ecocardiográficas (26,8%): miocardiopatia dilatada (5 casos), derrame pericárdico com tamponamento (2 casos), hipertensão pulmonar (2 casos) e prolapso da valva mitral (1 caso). CONCLUSÃO: O envolvimento cardíaco foi uma característica do grupo infectado. Houve maior prevalência de alterações nas crianças pertencentes à categoria clínico-imunológica mais avançada. Os achados clínicos, eletrocardiográficos e ecocardiográficos mais freqüentes foram, respectivamente, ICC, alterações da repolarização ventricular e miocardiopatia dilatada. Esta foi reversível em um caso. As alterações eletrocardiográficas foram significantemente mais freqüentes que as clínicas e ecocardiográficas.

Numerosas evidencias, clínicas y experimentales, han demostrado que la hiponutrición temprana, en coincidencia con el período de ontogénesis del SNC (Sistema Nervioso Central), produce una serie de alteraciones morfológicas, neurofisiológicas, neuroquímicas y comportamentales que perduran en el sujeto adulto, aún luego de largos períodos de recuperación nutricional. (...) En los últimos 20 años, el concepto de cómo la malnutrición afecta las funciónes cerebrales superiores ha evolucionado considerablemente. Los hallazgos más recientes, basados en cambios en la reactividad a fármacos y a alteraciones en el funcionalismo de receptores neuronales, sugieren que los comportamientos y procesos cognitivos afectados por la hiponutrición temprana podrían ser consecuencia de la incapacidad de sujetos malnutridos en producir respuestas emocionales adecuadas y cambios adaptativos en los sistemas neuronales ante situaciones estresantes, más que por déficit cognitivo per-se. La hiponutrición infantil representa un enorme impacto económico-social para la humanidad y por lo tanto la mejor comprensión de los mecanismos neuronales involucrados en la regulación de las funciones cerebrales superiores afectadas por la malnutrición temprana puede representar un significativo aporte al conocimiento de la fisiopatología y/o tratamiento de alteraciones que afectan a millones de niños. En el presente proyecto, en ratas adultas sometidas a un esquema de hiponutrición perinatal y sus respectivos controles, se estudiará: 1) El desarrollo de tolerancia y síndrome de abstinencia a la acción ansiolítica de drogas que median sus efectos a través del complejo-receptor GABA-A. Paralelamente a la evaluación conductual luego de tratamientos crónicos, se intentará correlacionar las alteraciones observadas con parámetros neuroquímicos del funcionalismo neuronal. 2) El desarrollo de tolerancia al efecto analgésico de drogas analgésicas narcóticas. Se intentará correlacionar los resultados con la densidad de receptores opiáceos en distintas estructuras cerebrales

FUNDAMENTO: A oxigenação por membrana extracorpórea (ECMO) em recém-nascidos e crianças tem resultados excelentes. A experiência em adultos é mais modesta e os resultados imediatos são inferiores. Entretanto, a sobrevida em cinco anos de pacientes que sobrevivem a esse método é bastante promissora. Não há relato na literatura brasileira de experiências com esse sistema nesse contexto. OBJETIVO: Relatar a experiência com o uso da oxigenação por membrana extracorpórea no adulto que se apresenta em choque cardiogênico agudo e refratário. MÉTODOS: Análise retrospectiva de prontuários médicos de pacientes submetidos a implante do sistema de oxigenação por membrana extracorpórea para a assistência circulatória no choque cardiogênico agudo e refratário. RESULTADOS: Onze pacientes (63,5 anos; 45,5% homens) foram considerados para análise no período de 2005 a 2007. O tempo médio de suporte circulatório foi de 77 horas (10-240h) e cinco pacientes estavam vivos em 30 dias (45,5%). Dois pacientes foram subseqüentemente submetidos a implante de assistência circulatória prolongada após um período de ressuscitação em ECMO, um dos quais foi submetido ao transplante cardíaco. As causas de morte durante a assistência com ECMO (seis pacientes) incluíram falência multiorgânica (66,6%) ou sangramento refratário (33,4%). CONCLUSÃO: O sistema ECMO é uma opção de assistência circulatória temporária para pacientes adultos com falência cardíaca aguda refratária, podendo ser utilizado como ponte para a recuperação ou com o intuito de selecionar candidatos à assistência circulatória prolongada (ponte para a ponte).

La hipoxia perinatal se considera hoy una de las causas de mayor morbimortalidad en el recién nacido estando altamente asociada con déficits de orden neuro-psico-sensorial. Un adecuado tratamiento y rehabilitación implica la detección temprana de los déficits apuntando a la prevención. La neonatología actual se está orientando hacia técnicas no invasivas que valoran la respuesta hacia estímulos de diferentes cualidades sensoriales y permiten indagar capacidades tempranas de aprendizaje y memoria. La precoz madurez del sistema olfativo facilita el trabajo con estas claves brindando asímismo un panorama de la integración cortical. Hipótesis: Las capacidades de habituación-deshabituación olfativa reflejan el estado del SNC neonatal y se convierten en un indicador temprano de la funcionalidad cerebral en niños con alto riesgo neurológico. Objetivos: Evaluar la integridad funcional del cerebro de recién nacidos con riesgo neurológico mediante técnicas no invasivas de habituación-deshabituación. Estas téncicas se tomarán como indicadores del estado del SNC neonatal comparando sus resultados con los arrojados por el test de Bayley durante el primer año de vida. Asímismo, se relacionarán los resultados con las valoraciones clínicas y de imágenes (ecografía y RMN) obtendias. El objetivo último implica determinar la sensibilidad, especificidad y valor predictivo del test de Habituación-Deshabituación para el daño cerebral . Materiales y Métodos: Se evaluarán pacientes con antecedentes perinatales de hipoxia pertenecientes al Servicio del Neonatología del HUMN y al Hospital Materno Neonatal (Pcia de Córdoba) caracterizados por apgar < 3 a los 5 minutos ó < 5 a los 5 minutos, Ph de cordón < 7 ó entre 7 y 7,10 ó >7,10 con alguno de los criterios anteriores presentes y niños con manifestaciones sistémicas de asfixia o déficit de base en gas arterial superior a -15 Meq/L. De manera posterior a la firma del consentimiento informado se aplicará un esquema olfativo de habituación-deshabituación antes y después de las 6 horas de vida consistente en 10 presentaciones consecutivas de olor a limón o vainilla seguidos de 5 presentaciones consecutivas de olor a vainilla o limón que actúan como estímulos deshabituatorios. Se filmará la conducta motriz que será considerada como variable dependiente. Asímismo se evaluarán parámetros fisiológicos como presión areterial, saturación de oxígeno, frecuencia cardíaca y ritmo respiratorio. La evaluación de la habituación se complementará con estudios clínicos y ecografía cerebral antes de las 48 hs y 1 vez por semana. Todos estos bebés serán seguidos por el equipo de Seguimiento del Recién Nacido de Alto Riesgo. A los 6 meses y al año de vida se evaluará el desarrollo integral con el Test de Bayley . Resultados Esperados: Se espera encontrar diferencias en los patrones habituacionales en función del estado cerebral del bebé. Aquellos bebés con riesgo leve tenderán a mostrar patrones habituatorios que se acercan a los normales mientras que los que presenten riesgo moderado o grave exhibiran patrones habituacionales defectuosos. Estos resultados se correlacionarán con los estudios de seguimiento realizados y con los indices de desarrollo mental y motor arrojados por el test de Bayley durante el año de vida. Importancia del Proyecto: Definir precozmente al niño con riesgo neurológico permite intervenir a tiempo con la estimulación y rehabilitación adecuada. Teniendo en cuenta la valoración del examen clínico, los antecedentes perinatales y los estudios de imágenes que se realizan en el bebé junto a los resultados obtenidos en las pruebas habituacionales, el proyecto brinda la posibilidad de la construcción de un score de riesgo neurológico sencillo y de fácil utilización.

FUNDAMENTO: Sedentarismo é fator predisponente ao aparecimento/piora de outros fatores de risco cardiovascular, particularmente obesidade. OBJETIVO: Estabelecer nível de atividade física (NAF) e número diário de horas de TV (HTV) e a associação e/ou correlação destas variáveis com faixa etária, sexo, classe econômica, escola pública/privada, excesso de peso e obesidade, em crianças/adolescentes. MÉTODOS: Estudo transversal, base populacional escolar, ensino público e privado, fundamental e médio. Cálculo da amostra baseado na menor prevalência esperada de inúmeras variáveis, incluindo sedentarismo. Amostragem por conglomerados. Protocolo: Questionário estruturado, incluindo "Physical Activity Questionaire for Older Children" (PAQ-C); medidas de peso, altura, índice de massa corporal (IMC) e prega cutânea do tríceps (PCT). Análise estatística: Qui-quadrado; correlação linear. RESULTADOS: Nos 1.253 estudantes, com média de idade de 12,4 ± 2,9 anos, sendo 547 do sexo masculino, observou-se uma prevalência de sedentarismo em 93,5%, mais frequente em adolescentes do sexo feminino; não houve associação entre NAF e excesso de peso ou gordura corporal; futebol e dança foram as atividades mais frequentes em meninos e meninas, respectivamente; 60% dos estudantes não têm aulas de Educação Física. Média e mediana de HTV foram, respectivamente, 3,6 e 3 horas; houve associação significante entre maior HTV e obesidade e correlação significante entre NAF e idade (negativa) e entre IMC e PCT (positiva). CONCLUSÃO: O sedentarismo está presente em 93,5% das crianças e adolescentes de Maceió, sendo mais prevalente nos adolescentes e no sexo feminino, não havendo associação ou correlação desta variável com excesso de peso ou gordura corporal; obesidade associou-se a > 3 HTV.

Em pacientes com insuficiência coronariana aguda e choque cardiogênico, a mortalidade é alta. O dispositivo mais utilizado para suporte hemodinâmico é o balão intra-aórtico que, no entanto, pode ser insuficiente em pacientes com choque cardiogênico refratário. Relato de caso com dois dias de dor precordial opressiva e intensa, irradiada para membro superior esquerdo. ECG com supradesnivelamento anterior. Realizado angioplastia e implante de stent na artéria descendente anterior. Evolução com choque cardiogênico refratário ao uso de drogas vasoativas e balão intra-aórtico. Foram realizadas medidas hemodinâmicas e decidiu-se pela colocação do Impella® 2,5 por via percutânea para assistência circulatória.

Background: Combination therapy can play a significant role in the amelioration of several toxic effects of lead (Pb) and recovery from associated cardiovascular changes. Objective: To investigate the effects of combination therapy on the cardiovascular effects of perinatal lead exposure in young and adult rats Methods: Female Wistar rats received drinking water with or without 500 ppm of Pb during pregnancy and lactation. Twenty-two- and 70-day-old rat offspring who were or were not exposed to Pb in the perinatal period received meso-dimercaptosuccinic acid (DMSA), L-arginine, or enalapril and a combination of these compounds for 30 additional days. Noradrenaline response curves were plotted for intact and denuded aortas from 23-, 52-, 70-, and 100-day-old rats stratified by perinatal Pb exposure (exposed/unexposed) and treatment received (treated/untreated). Results: Systolic blood pressure was evaluated and shown to be higher in the 23-, 52-, 70-, and 100-day age groups with Pb exposure than in the corresponding control age groups: 117.8 ± 3.9*, 135.2 ± 1.3*, 139.6 ± 1.6*, and 131.7 ± 2.8*, respectively and 107.1 ± 1.8, 118.8 ± 2.1, 126.1 ± 1.1, and 120.5 ± 2.2, respectively (p < 0.05). Increased reactivity to noradrenaline was observed in intact, but not denuded, aortas from 52-, 70-, and 100-day-old exposed rats, and the maximum responses (g of tension) in the respective Pb-exposed and control age groups were as follows: 3.43 ± 0.16*, 4.32 ± 0.18*, and 4.21 ± 0.23*, respectively and 2.38 ± 0.33, 3.37 ± 0.13, and 3.22 ± 0.21, respectively (p < 0.05). Conclusions: All treatments reversed the changes in vascular reactivity to noradrenaline in rats perinatally exposed to Pb. The combination therapy resulted in an earlier restoration of blood pressure in Pb-exposed rats compared with the monotherapies, except for enalapril therapy in young rats. These findings represent a new approach to the development of therapeutic protocols for the treatment of Pb-induced hypertension.

Abstract Background: Preeclampsia has been associated with several risk factors and events. However, it still deserves further investigation, considering the multitude of related factors that affect different populations. Objective: To evaluate the maternal factors and adverse perinatal outcomes in a cohort of pregnant women with preeclampsia receiving care in the public health network of the city of Maceió. Methods: Prospective cohort study carried out in 2014 in the public health network of the city with a sample of pregnant women calculated based on a prevalence of preeclampsia of 17%, confidence level of 90%, power of 80%, and ratio of 1:1. We applied a questionnaire to collect socioeconomic, personal, and anthropometric data, and retrieved perinatal variables from medical records and certificates of live birth. The analysis was performed with Poisson regression and chi-square test considering p values < 0.05 as significant. Results: We evaluated 90 pregnant women with preeclampsia (PWP) and 90 pregnant women without preeclampsia (PWoP). A previous history of preeclampsia (prevalence ratio [PR] = 1.57, 95% confidence interval [95% CI] 1.47 - 1.67, p = 0.000) and black skin color (PR = 1.15, 95% CI 1.00 - 1.33, p = 0.040) were associated with the occurrence of preeclampsia. Among the newborns of PWP and PWoP, respectively, 12.5% and 13.1% (p = 0.907) were small for gestational age and 25.0% and 23.2% (p = 0.994) were large for gestational age. There was a predominance of cesarean delivery. Conclusion: Personal history of preeclampsia and black skin color were associated with the occurrence of preeclampsia. There was a high frequency of birth weight deviations and cesarean deliveries.

Choline supplementation improving memory functions in rodents is assumed to increase the synthesis and release of acetylcholine in the brain. We have found that a combined pre- and postnatal supplementation results in long-lasting facilitation of spatial memory in juvenile rats when training was conducted in presence of a local salient cue. The present work was aimed at analysing the effects of peri- and postnatal choline supplementation on spatial abilities of naive adult rats. Rats given a perinatal choline supplementation were trained in various cued procedures of the Morris navigation task when aged 5 months. The treatment had a specific effect of reducing the escape latency of the rats when the platform was at a fixed position in space and surrounded by a suspended cue. This effect was associated with an increased spatial bias when the cue and platform were removed. In this condition, the control rats showed impaired spatial discrimination following the removal of the target cue, most likely due to an overshadowing of the distant environmental cues. This impairment was not observed in the treated rats. Further training with the suspended cue at unpredictable places in the pool revealed longer escape latencies in the control than in the treated rats suggesting that this procedure induced a selective perturbation of the normal but not of the treated rats. A special probe trial with the cue at an irrelevant position and no escape platform revealed a significant bias of the control rats toward the cue and of the treated rats toward the uncued spatial escape position. This behavioural dissociation suggests that a salient cue associated with the target induces an alternative "non spatial" guidance strategy in normal rats, with the risk of overshadowing of the more distant spatial cues. In this condition, the choline supplementation facilities a spatial reliance on the cue, that is an overall facilitation of learning a set of spatial relations between several visual cues. As a consequence, the improved escape in presence of the cue is associated with a stronger memory of the spatial position following disappearance of the cue. This and previous observations suggest that a specific spatial attention process relies on the buffering of highly salient visual cues.to facilitate integration of their relative position in the environment.

Perinatal care of pregnant women at high risk for preterm delivery and of preterm infants born at the limit of viability (22-26 completed weeks of gestation) requires a multidisciplinary approach by an experienced perinatal team. Limited precision in the determination of both gestational age and foetal weight, as well as biological variability may significantly affect the course of action chosen in individual cases. The decisions that must be taken with the pregnant women and on behalf of the preterm infant in this context are complex and have far-reaching consequences. When counselling pregnant women and their partners, neonatologists and obstetricians should provide them with comprehensive information in a sensitive and supportive way to build a basis of trust. The decisions are developed in a continuing dialogue between all parties involved (physicians, midwives, nursing staff and parents) with the principal aim to find solutions that are in the infant's and pregnant woman's best interest. Knowledge of current gestational age-specific mortality and morbidity rates and how they are modified by prenatally known prognostic factors (estimated foetal weight, sex, exposure or nonexposure to antenatal corticosteroids, single or multiple births) as well as the application of accepted ethical principles form the basis for responsible decision-making. Communication between all parties involved plays a central role. The members of the interdisciplinary working group suggest that the care of preterm infants with a gestational age between 22 0/7 and 23 6/7 weeks should generally be limited to palliative care. Obstetric interventions for foetal indications such as Caesarean section delivery are usually not indicated. In selected cases, for example, after 23 weeks of pregnancy have been completed and several of the above mentioned prenatally known prognostic factors are favourable or well informed parents insist on the initiation of life-sustaining therapies, active obstetric interventions for foetal indications and provisional intensive care of the neonate may be reasonable. In preterm infants with a gestational age between 24 0/7 and 24 6/7 weeks, it can be difficult to determine whether the burden of obstetric interventions and neonatal intensive care is justified given the limited chances of success of such a therapy. In such cases, the individual constellation of prenatally known factors which impact on prognosis can be helpful in the decision making process with the parents. In preterm infants with a gestational age between 25 0/7 and 25 6/7 weeks, foetal surveillance, obstetric interventions for foetal indications and neonatal intensive care measures are generally indicated. However, if several prenatally known prognostic factors are unfavourable and the parents agree, primary non-intervention and neonatal palliative care can be considered. All pregnant women with threatening preterm delivery or premature rupture of membranes at the limit of viability must be transferred to a perinatal centre with a level III neonatal intensive care unit no later than 23 0/7 weeks of gestation, unless emergency delivery is indicated. An experienced neonatology team should be involved in all deliveries that take place after 23 0/7 weeks of gestation to help to decide together with the parents if the initiation of intensive care measures appears to be appropriate or if preference should be given to palliative care (i.e., primary non-intervention). In doubtful situations, it can be reasonable to initiate intensive care and to admit the preterm infant to a neonatal intensive care unit (i.e., provisional intensive care). The infant's clinical evolution and additional discussions with the parents will help to clarify whether the life-sustaining therapies should be continued or withdrawn. Life support is continued as long as there is reasonable hope for survival and the infant's burden of intensive care is acceptable. If, on the other hand, the health care team and the parents have to recognise that in the light of a very poor prognosis the burden of the currently used therapies has become disproportionate, intensive care measures are no longer justified and other aspects of care (e.g., relief of pain and suffering) are the new priorities (i.e., redirection of care). If a decision is made to withhold or withdraw life-sustaining therapies, the health care team should focus on comfort care for the dying infant and support for the parents.

As part of a collaborative project on the epidemiology of craniofacial anomalies, funded by the National Institutes for Dental and Craniofacial Research and channeled through the Human Genetics Programme of the World Health Organization, the International Perinatal Database of Typical Orofacial Clefts (IPDTOC) was established in 2003. IPDTOC is collecting case-by-case information on cleft lip with or without cleft palate and on cleft palate alone from birth defects registries contributing to at least one of three collaborative organizations: European Surveillance Systems of Congenital Anomalies (EUROCAT) in Europe, National Birth Defects Prevention Network (NBDPN) in the United States, and International Clearinghouse for Birth Defects Surveillance and Research (ICBDSR) worldwide. Analysis of the collected information is performed centrally at the ICBDSR Centre in Rome, Italy, to maximize the comparability of results. The present paper, the first of a series, reports data on the prevalence of cleft lip with or without cleft palate from 54 registries in 30 countries over at least 1 complete year during the period 2000 to 2005. Thus, the denominator comprises more than 7.5 million births. A total of 7704 cases of cleft lip with or without cleft palate (7141 livebirths, 237 stillbirths, 301 terminations of pregnancy, and 25 with pregnancy outcome unknown) were available. The overall prevalence of cleft lip with or without cleft palate was 9.92 per 10,000. The prevalence of cleft lip was 3.28 per 10,000, and that of cleft lip and palate was 6.64 per 10,000. There were 5918 cases (76.8%) that were isolated, 1224 (15.9%) had malformations in other systems, and 562 (7.3%) occurred as part of recognized syndromes. Cases with greater dysmorphological severity of cleft lip with or without cleft palate were more likely to include malformations of other systems.

Adverse events in utero are associated with the occurrence of chronic diseases in adulthood. We previously demonstrated in mice that perinatal hypoxia resulted in altered pulmonary circulation in adulthood, with a decreased endothelium-dependent relaxation of pulmonary arteries, associated with long-term alterations in the nitric oxide (NO)/cyclic GMP pathway. The present study investigated whether inhaled NO (iNO) administered simultaneously to perinatal hypoxia could have potential beneficial effects on the adult pulmonary circulation. Indeed, iNO is the therapy of choice in humans presenting neonatal pulmonary hypertension. Long-term effects of neonatal iNO therapy on adult pulmonary circulation have not yet been investigated. Pregnant mice were placed in hypoxia (13% O2) with simultaneous administration of iNO 5 days before delivery until 5 days after birth. Pups were then raised in normoxia until adulthood. Perinatal iNO administration completely restored acetylcholine-induced relaxation, as well as endothelial nitric oxide synthase protein content, in isolated pulmonary arteries of adult mice born in hypoxia. Right ventricular hypertrophy observed in old mice born in hypoxia compared to controls was also prevented by perinatal iNO treatment. Therefore, simultaneous administration of iNO during perinatal hypoxic exposure seems able to prevent adverse effects of perinatal hypoxia on the adult pulmonary circulation.
