Shiftwork is a major source of stress for many worker's. This study highlights the role that organizational and psychosocial variables play in alleviating the negative health effects of 10 and 14-h shifts. It examines the direct and mediated effects of coping strategies, social support and control of shifts on work/non-work conflict and subjective health. Participants are 60 ambulance workers, aged 22 to SS years. A structural equation model with good fit demonstrates complex effects of social support from various sources (supervisors, co-workers and family), coping and control on work/non-work conflict and subjective health., Conceptually, the research contributes to the development of a theoretical framework that can assist in explaining how key psychosocial and organizational variables influence the psychological and physical symptoms experienced by shiftworkers. RESUMO: A mamografia é o método de diagnóstico mais eficaz para deteção precoce de carcinoma da mama. A realização periódica deste exame tem vindo a ser associada a melhores prognósticos como deteção da doença em fases mais precoces e redução de mortalidade. Contudo, a mamografia não é encarada como mais um exame complementar de diagnóstico e constitui, para a maioria das mulheres, uma situação de ameaça. Na verdade, muitas mulheres referem ter experienciado ansiedade, dor ou desconforto durante a realização deste procedimento. Por outro lado, uma percentagem elevada de mulheres falha na realização periódica de mamografias. Este artigo pretende abordar as reações psicológicas associadas ao exame de mamografia e enfatizar a forma como os técnicos de radiologia podem melhorar o atendimento das mulheres que realizam este exame. O papel do técnico de radiologia no fornecimento de informação antes do exame, na criação de uma atmosfera agradável e empática e na utilização de estratégias de confronto para redução de dor e desconforto e na promoção do controlo e autonomia da paciente são alguns dos aspetos abordados.

Os profissionais de saúde mental são o principal instrumento de intervenção nesta área considerada como prioridade de Saúde Pública e estão sujeitos a desgaste emocional e stress, que pode afetar negativamente a sua qualidade de vida. Com este estudo pretende-se avaliar a perceção do estado de saúde e da qualidade de vida relacionada com a saúde nos profissionais de saúde mental. Para responder ao objetivo traçado optou-se por realizar um estudo observacional analítico transversal, com abordagem quantitativa. Foi utilizado o SF36v2 como instrumento genérico de avaliação da Qualidade de Vida, que já se encontra validado para a população portuguesa, complementado por um questionário socioprofissional. A recolha de dados decorreu online, de 28 de Janeiro a 30 de Abril de 2013. A amostra foi constituída por 201 profissionais de saúde mental de Portugal Continental. Os profissionais de saúde mental que integraram a amostra revelaram pior perceção do estado de saúde e qualidade de vida, quando comparados com os valores de referência para a população portuguesa com nível de instrução alto. Em termos de domínios do instrumento, piores resultados foram encontrados para todos eles com exceção do domínio Saúde Mental. A avaliação da qualidade de vida dos profissionais de saúde mental possibilita a implementação alterações no funcionamento dos serviços de saúde mental, nomeadamente de programas de gestão de stress, de estratégias de coping e de competências de interação e comunicação entre os profissionais, podendo contribuir para uma melhoria na prestação dos cuidados de saúde aos utentes.

Introduction: University students are frequently exposed to events that can cause stress and anxiety, producing elevated cardiovascular responses. Repeated exposure to academic stress has implications to students’ success and well-being and may contribute to the development of long-term health problems. Objective: To identify stress levels and coping strategies in university students and assess the impact of stress experience in heart rate variability (HRV). Methods: 17 university students, 19-23 years, completed the University Students Stress Inventory, the Depression Anxiety Stress Scales and the Ways of Coping Questionnaire. Two 24h-Holter recordings were performed, on academic activity days, including one of them an exam situation. Results: Students tend to present moderate stress levels, and prefer problem-focused coping strategies in order to manage stress. Exam situations are perceived as significant stressors. Although we found no significant differences in HRV (SDNN), between days with and without an exam, we registered a lower SDNN score and a variation in heart rate (HR) related to exam situation (maximum HR peak at 10 minutes before the exam, and total HR recovery 20 minutes after the exam), reflecting sympathetic activation due to stress. Conclusions: These results suggest that academic events, especially those related to exam situations, are the cause of stress in university students, with implications at cardiovascular level, underlying the importance of interventions that help these students improve their coping skills and optimize stress management, in order to improve academic achievement and promote well-being and quality of life.

Adjustment to emerging economies is benefited if Western expatriates recognise they are experiencing a liminal situation, which can lead to the instrumental utilisation of coping strategies as equivalent to rites of passage between distinct ethical frameworks. Given the characteristics ascribed to rites, the ethical dilemma resulting from the simultaneous demand to abide by local rules and to respect Western ethical principles is more easily solved. Consequently, effective and sustainable adjustment is favoured. Implications for organisations and individuals are discussed.

The aim of this study is to examine the relationship among psychological, clinical and sociodemographic variables, and quality of life in women with urinary incontinence. The sample consisted of 80 women diagnosed with urinary incontinence (UI) followed in a Northern Central Hospital in Portugal. Participants answered the Incontinence Quality of Life (I-QOL); Satisfaction with Sexual Relationship Questionnaire (SSRQ); Hospital Anxiety and Depression Scales (HADS) and the Brief Cope. The results revealed that women with higher quality of life considered their symptoms of urine loss as mild or moderated compared to those with severe urine loss. The less severe urine loss was associated with greater sexual satisfaction and less use of religion and self-blame as coping strategies. In terms of coping, women who considered the loss of urine as severe expressed more feelings regarding UI. Stress urinary incontinence, high sexual satisfaction, and less use of denial, distraction, and religion as coping strategies, predicted higher quality of life. According to the results, UI has an impact on women’s sexual satisfaction and quality of life. Therefore, intervention programs should target these women, including their partners, helping them to adjust to their condition and teaching effective coping strategies in order to improve their sexual satisfaction and quality of life.

Este estudo avaliou as relações entre a qualidade de vida, otimismo, enfrentamento, morbidade psicológica e estresse familiar, os preditores da qualidade de vida e da morbidade psicológica e analisou as diferenças nestas variáveis em função do sexo, do tipo e da duração do tratamento, idade e nível de escolaridade em pacientes com câncer colorretal. Uma amostra de 100 pacientes em tratamento de quimioterapia participou no estudo. Os resultados revelaram que as mulheres, os pacientes diagnosticados nos últimos 12 meses e os que realizaram cirurgia/quimioterapia apresentam maior risco de desenvolver morbidade psicológica. A depressão mostrou ser um preditor da qualidade de vida. A idade e a escolaridade mostraram ser relevantes particularmente ao nível das estratégias de enfrentamento utilizadas e na qualidade de vida. Os resultados enfatizam a necessidade das intervenções focarem a morbidade, otimismo e ter em consideração o gênero, a duração do diagnóstico e o tipo de tratamento, em particular.

Este estudio explora las diferencias en los hábitos de consumo de sustancias psicoactivas, entre jóvenes de Barcelona y Bogotá. Evalúa la influencia de la percepción de riesgo sobre hábitos de consumo y estrategias de afrontamiento. Adicionalmente examina la influencia de la gravedad percibida de una situación estresante sobre estas últimas. Participaron 865 jóvenes de ambas ciudades, entre los 15 y los 18 años. Se utilizaron las variables de riesgo estudiadas por Benthin, Slovic y Severson (1993) para evaluar la percepción de riesgo. Los hábitos de consumo se evaluaron mediante la frecuencia, la intención de consumo, así como la edad de inicio. Se utilizó el CRI:Youth de Moos (1992) para determinar las estrategias de afrontamiento y la valoración del problema estresante. Se encontró que existen diferencias en la edad en que se inicia el consumo de alcohol y en la que se embriagan por primera vez según el género, la ciudad donde residen y la edad del adolescente. Los jóvenes de Barcelona tienen una propensión y un consumo real de marihuana y tabaco mayor que los jóvenes de Bogotá. Percibir placer o beneficios predice un incremento en la intención y la frecuencia de consumo de la mayoría de las sustancias. La facilidad para acceder a éstas sólo presenta una asociación con el uso frecuente del tabaco. Los datos sugieren que la gravedad percibida de estresores relativos a las drogas y la ciudad de residencia tienen un efecto sobre la utilización de las estrategias de evitación y aproximación cognitiva. Adicionalmente no se detectaron diferencias en función de las estrategias de afrontamiento empleadas según las variables de percepción de riesgo a excepción de la presión percibida, la cual aumenta el uso de la reevaluación del problema y la búsqueda de recompensas.

Excessive drinking contributes significantly to social problems, physical and psychological illness, injury and death. Hidden effects include increased levels of violence, accidents and suicide. Most alcohol-related harm is caused by excessive drinkers whose consumption exceeds recommended drinking levels, not the drinkers with severe alcohol dependency problems. One way to reduce consumption levels in a community may be to provide a brief intervention in primary care over one to four sessions. This is provided by healthcare workers such as general physicians, nurses or psychologists. In general practice, patients are routinely asked about alcohol consumption during registration, general health checks and as part of health screening (using a questionnaire). They tend not to be seeking help for alcohol problems when presenting. The intervention they are offered includes feedback on alcohol use and harms, identification of high risk situations for drinking and coping strategies, increased motivation and the development of a personal plan to reduce drinking. It takes place within the time-frame of a standard consultation, 5 to 15 minutes for a general physician, longer for a nurse.A total of 29 controlled trials from various countries were identified, in general practice (24 trials) or an emergency setting (five trials). Participants drank an average of 306 grams of alcohol (over 30 standard drinks) per week on entry to the trial. Over 7000 participants with a mean age of 43 years were randomised to receive a brief intervention or a control intervention, including assessment only. After one year or more, people who received the brief intervention drank less alcohol than people in the control group (average difference 38 grams/week, range 23 to 54 grams). For men (some 70% of participants), the benefit of brief intervention was a difference of 57 grams/week, range 25 to 89 grams (six trials). The benefit was not clear for women. The benefits of brief intervention were similar in the normal clinical setting and in research settings with greater resources. Introduction Music performance anxiety (MPA, often referred to as "stage fright") is one of the leading severe medical problems among musicians. For about 15-25% of musicians MPA is a serious problem. Particularly high levels of MPA are observed among music students. Musical performance can induce negative emotions, including anxiety, which in some individuals can approach extreme levels of terror and take the form of panic attack, impair the quality of the performance, lead to avoidance of performance situations, and consequently have debilitating effects on the career. Coping efforts used by musicians in their attempts to manage MPA, such as sedatives, alcohol, and β-blockers can have deleterious health side-effects. Music ranks high in the cultural and economic life of Switzerland. In ten university music schools, students from all around the world are educated to become professional musicians. Despite the importance of musical education in Switzerland, data concerning the phenomenon of MPA are largely lacking. Goal and Methods The main goal of this research was to survey the occurrence, experience, and management of MPA among full-time music students in French Swiss conservatories. A questionnaire was developed based on the literature and interviews with music students and teachers and distributed to all the students of the conservatories of Fribourg, Geneva, Lausanne, and Neuchâtel in the spring 2007. 194 students (61% women) returned the questionnaire. Results The size of the problem: MPA is a major problem for 1/3 of the students (ranks 3 and 4). The consequences of MPA: 22% and 35% of the students think that they have failed exams and auditions, respectively, because of MPA. Further, 25% of the students have already avoided performing and 11% have interrupted public performances because of MPA. Coping with MPA: 90% of the students have never used alcohol prior to performing, whereas 97% and 81%, respectively, have never used recreation drugs and medication. The majority of students use relaxation exercises, respiratory exercises, and meditation techniques to prepare themselves. About ¾ of the students think that the use of alcohol and recreational drugs to manage MPA is never justified. 53% of the students think that the use of medication is justified on some occasions. Need for information and support: 66% of the students would like to receive more support and help to cope with music performance situations. This support should mainly come from their teachers and specialists. 53% of the students know nothing or little about possible means for the management of MPA. About 50% consider themselves not at all or little informed about the possible risks associated with the consumption of alcohol, recreational drugs, and medication for the management of performance situations. 89% would like to know more about MPA and 94% think that this topic should be discussed much more in their musical education at the conservatory. Conclusions The results of this survey indicate that MPA is a major problem for 1/3 of the students with serious consequences on their career. There is a huge need for more information and support on how to manage the stress due to performance situations. The use of alcohol, recreational drugs, and medication is modest but the students are poorly informed about possible side-effects of these coping strategies. It seems clear that more should be done in the French Swiss conservatories about music performance anxiety to inform, educate, and prepare the students for their future professional career.

This study had mainly two goals: First of all, adult attachement was investigated in 28 patients with chronic psychosis and 18 controls, using the"Adult Attachment Interiew". This, as no previous research had ever done so with a scientific validated instrument. Diagnostic evaluations were performed with the SCID and symptomtic evaluation with the BPRS. Results showed that patients were more likely to have experienced traumatic experiences with their primary caregivers during their childhood then have controls. We were able to identify as well a high prevalence of insecure avoidant attachement in patients, which highlights the functional dimension of psychosis in the maintenance of the relation to primary attachement figures. This study shows that insecure avoidant attachement is related to an early outset as well as to an unfavourable prognosis. Second, quite a few researches on spirituality in psychosis have shown that patients were more likely to have spiritual believes or practices then indiviuals of a nonclinical population. Coping strategies related to spirituality and religion were identified as efficient for the majority. They can help dealing with all types of difficulties, specific symptoms and may enhance life quality (Mohr et al., 2006). Therefore we have investigated some of the underlying psychological processes in this type of coping strategies, using the conceptual framework of Bowlby's attachement theory. For most participants, both patients and controls, « spiritual figures » are functionning as attachement figures. Most of them are transposing the internal working model, achieved in relation to their primary caregivers, towards their "spiritual figure" even in case of an insecure attachement. - Cette recherche poursuit essentiellement deux objectifs. Premièrement, elle examine les modèles d'attachement à l'aide du « Adult Attachment Interview » chez 28 personnes souffrant de psychose chronique et chez 18 personnes ne souffrant d'aucune affection psychiatrique. En effet, aucune étude avant celle-ci n'avait investigué l'attachement chez des patients psychotiques chroniques à l'aide d'instruments scientifiquement validés. Les évaluations diagnostiques ont été réalisées à l'aide du SCID et les évaluations symptomatologiques à l'aide du BPRS. Dans ce contexte, nous avons pu montrer qu'une majorité de patients avaient été victimes d'expériences relationnelles traumatisantes en lien avec leurs premières figures d'attachement durant l'enfance. Nos résultats indiquent également l'importante prévalence d'un attachement insécure-détaché, qui reflète la dimension fonctionnelle des symptômes psychotiques visant la préservation du lien aux principales figures d'attachement. Ceux-ci montrent encore qu'un attachement insécure-détaché est associé à un début précoce de la maladie ainsi qu'à un pronostic défavorable. Deuxièmement, de nombreuses études ont montré que les patients psychotiques avaient davantage recours à des croyances et pratiques spirituelles/religieuses pour tenter de faire face à leurs difficultés que la population générale. Ce recours s'avère être efficace pour un grand nombre d'entre eux. Il leur permet souvent une meilleure gestion des difficultés, des symptômes et globalement améliore leur qualité de vie (Mohr et al., 2006). Nous avons donc examiné certaines des dimensions psychiques sousjacentes à ce type de stratégie de coping à l'aide de l'arrière-plan conceptuel de la théorie de l'attachement de Bowlby. Nous avons effectivement constaté qu'un grand nombre de participants, patients et contrôles, investissent des figures spirituelles sur le mode de l'attachement. Même en cas d'un attachement insécure, la majorité d'entre eux transposent leurs modèles internes opérants, acquis dans leur relation aux premières figures d'attachement, sur leur croyance en des figures spirituelles.

This article highlights some of the links between pre-morbid personality and clinical features of dementia. Indeed, personality characteristics forge an individual's coping strategies and thus influence the expression of behavioural and psychiatric syndromes of dementia (BPSD) or its precursor stages. Some personality traits such as neuroticism may impact on cognitive decline. BPSD being among the most important determinants of a patient's and their proxies' burden, a better understanding of the links between pre-morbid personality characteristics and BPSD will help define care strategies.
