OBJETIVO: Compreender a concepção de saúde e a origem do sofrimento psíquico por adeptos de uma comunidade tradicional de terreiro. PROCEDIMENTOS METODOLÓGICOS: Estudo de caso qualitativo realizado em uma comunidade tradicional de terreiro na cidade de Porto Alegre (RS), entre 2007 e 2008. Foram participantes o sacerdote/Babalorixá e seis adeptos do terreiro. Para a coleta dos dados e construção do corpus de análise, foram realizadas entrevistas abertas, gravadas e transcritas. A categorização dos depoimentos, a partir do enfoque sistêmico complexo, possibilitou a construção de dois eixos temáticos: 1) terreiro e concepção de saúde e 2) origem do sofrimento psíquico e identidade cultural. ANÁLISE DOS RESULTADOS: Na comunidade de terreiro, as terapêuticas tradicionais em saúde, como o uso de ervas, banhos, dietas e/ou ritos de iniciação foram associados a terapêuticas convencionais propostas pelo Sistema Único de Saúde. Consideram em sua concepção etiológica do sofrimento psíquico e em sua concepção de saúde os vínculos e a pertença a um território, as relações entre os sujeitos e a relação entre suas dimensões física, psíquica e espiritual. CONCLUSÕES: O modo de compreender e agir no mundo, vivido no terreiro, com seus mitos e ritos, crenças e valores, constitui um conjunto de saberes legítimos em seu contexto que, muitas vezes, se contrapõe e escapa aos saberes e verdades técnico-científicas dos profissionais. O terreiro é um espaço marcado pelo acolhimento, aconselhamento e tratamento de seus adeptos, integrando nessas práticas as dimensões física, psíquica e espiritual. Quanto à saúde da população negra, põem-se em evidência que o sofrimento psíquico é resultante do desenraizamento das culturas negro-africanas.

OBJETIVO: Analisar a associação de determinantes sociodemográficos com o desenvolvimento de problemas de comportamento e de competência social em crianças. MÉTODOS: Estudo transversal realizado com 479 escolares entre seis e 13 anos de idade, da primeira série do ensino fundamental de escolas públicas em São Gonçalo (RJ), em 2005. Foram investigadas variáveis socioeconômicas, estrutura familiar, escolaridade dos pais, cor da pele da criança, problemas de comportamento e competência social. Foram calculadas razões de prevalência com respectivo intervalo com 95% de confiança. Os dados apresentados foram expandidos para a população de alunos da rede de ensino investigada. RESULTADOS: Crianças abaixo da linha de pobreza, de cor da pele negra, com pais com baixa escolaridade, e vivendo em famílias monoparentais ou compostas por madrasta/padrasto apresentaram mais precária competência social e mais problemas de comportamento. À medida que se elevavam os fatores de risco, crescia a prevalência das crianças com baixa competência social e problemas de comportamento. CONCLUSÕES: A associação entre os determinantes sociodemográficos com a maior prevalência de problemas de comportamento e com mais precária competência social em crianças requer que ações preventivas e de assistência sejam tomadas como prioridade para as políticas públicas, minorando a existência de dificuldades sociais e emocionais infantis graves, que podem se manter até a vida adulta.

OBJETIVO: Analisar processos de estigmatização e modalidades de violência vividos por portadores de transtornos mentais. MÉTODOS: Estudo qualitativo baseado em entrevistas individuais com usuários e grupos focais com familiares e profissionais de cinco centros de atenção psicossocial, nos municípios de Itaberaba, Lauro de Freitas, Salvador e Vitória da Conquista (BA) e em Aracaju (SE), em 2006-2007. As categorias de análise foram construídas com base no conceito de estigma proposto por Goffman e foram sistematizadas quatro tipos de violência: interpessoal, institucional, simbólica e estrutural. RESULTADOS: Usuários e familiares relataram exemplos de desqualificações, repreensões, constrangimentos, humilhações, negligência e agressões físicas, com fins de dominação, exploração e opressão. Profissionais referiram que pessoas que sofrem transtornos mentais permanecem como alvo de preconceitos arraigados e naturalizados na cultura. A principal conseqüência foi a manutenção do isolamento, da vida social como forma de "tratamento" ou como atitude excludente manifestada por reações discriminatórias, que se apresentam como rejeição, indiferença e agressividade verbal ou física. CONCLUSÕES: As variadas formas de expressão do estigma denotam uma situação sociocultural de violências contra os portadores de transtornos mentais. Propõe-se a constituição de observatórios estaduais capazes de planejar e avaliar contra-ações às estigmatizações.

Independent component analysis (ICA) has recently been proposed as a tool to unmix hyperspectral data. ICA is founded on two assumptions: 1) the observed spectrum vector is a linear mixture of the constituent spectra (endmember spectra) weighted by the correspondent abundance fractions (sources); 2)sources are statistically independent. Independent factor analysis (IFA) extends ICA to linear mixtures of independent sources immersed in noise. Concerning hyperspectral data, the first assumption is valid whenever the multiple scattering among the distinct constituent substances (endmembers) is negligible, and the surface is partitioned according to the fractional abundances. The second assumption, however, is violated, since the sum of abundance fractions associated to each pixel is constant due to physical constraints in the data acquisition process. Thus, sources cannot be statistically independent, this compromising the performance of ICA/IFA algorithms in hyperspectral unmixing. This paper studies the impact of hyperspectral source statistical dependence on ICA and IFA performances. We conclude that the accuracy of these methods tends to improve with the increase of the signature variability, of the number of endmembers, and of the signal-to-noise ratio. In any case, there are always endmembers incorrectly unmixed. We arrive to this conclusion by minimizing the mutual information of simulated and real hyperspectral mixtures. The computation of mutual information is based on fitting mixtures of Gaussians to the observed data. A method to sort ICA and IFA estimates in terms of the likelihood of being correctly unmixed is proposed.

Background: Chronic musculoskeletal pain involves connective tissue remodeling triggered by inflammatory mediators, such as bradykinin. Fibroblast cells signaling involve changes in intracellular Ca2+ ([Ca2+]i). ATP has been related to connective tissue mechanotransduction, remodeling and chronic inflammatory pain, via P2 purinoceptors activation. Here, we investigated the involvement of ATP in bradykinin-induced Ca2+ signals in human subcutaneous fibroblasts. Results: Bradykinin, via B2 receptors, caused an abrupt rise in [Ca2+]i to a peak that declined to a plateau, which concentration remained constant until washout. The plateau phase was absent in Ca2+-free medium; [Ca2+]i signal was substantially reduced after depleting intracellular Ca2+ stores with thapsigargin. Extracellular ATP inactivation with apyrase decreased the [Ca2+]i plateau. Human subcutaneous fibroblasts respond to bradykinin by releasing ATP via connexin and pannexin hemichannels, since blockade of connexins, with 2- octanol or carbenoxolone, and pannexin-1, with 10Panx, attenuated bradykinin-induced [Ca2+]i plateau, whereas inhibitors of vesicular exocytosis, such as brefeldin A and bafilomycin A1, were inactive. The kinetics of extracellular ATP catabolism favors ADP accumulation in human fibroblast cultures. Inhibition of ectonucleotidase activity and, thus, ADP formation from released ATP with POM-1 or by Mg2+ removal from media reduced bradykinin-induced [Ca2+]i plateau. Selective blockade of the ADP-sensitive P2Y12 receptor with AR-C66096 attenuated bradykinin [Ca2+]i plateau, whereas the P2Y1 and P2Y13 receptor antagonists, respectively MRS 2179 and MRS 2211, were inactive. Human fibroblasts exhibited immunoreactivity against connexin-43, pannexin-1 and P2Y12 receptor. Conclusions: Bradykinin induces ATP release from human subcutaneous fibroblasts via connexin and pannexin-1-containing hemichannels leading to [Ca2+]i mobilization through the cooperation of B2 and P2Y12 receptors.

OBJETIVO: Estimar a prevalência do uso regular de serviços odontológicos por adultos e idosos em comunidade vulnerável e identificar fatores associados. MÉTODOS: Estudo transversal de base populacional com 3.391 adultos e idosos de áreas de vulnerabilidade social de Porto Alegre, RS, de julho a dezembro de 2009. Foi utilizada amostragem sistemática com probabilidade proporcional ao tamanho de cada um dos 121 setores censitários. O desfecho "utilização regular de serviços odontológicos" foi definido consultar com o dentista regularmente, tendo ou não problemas de saúde bucal. Foi aplicado questionário padronizado, que incluiu variáveis demográficas, socioeconômicas, tipo de local procurado, autopercepção de saúde bucal e necessidades autopercebidas. Utilizou-se teste qui-quadrado para heterogeneidade na análise bivariada e na análise ajustada regressão de Poisson, com variância robusta e teste de heterogeneidade de Wald. RESULTADOS: A prevalência do uso regular de serviços odontológicos foi de 25,7%. A prevalência foi maior entre os indivíduos com escolaridade > 12 anos (RP 2,48 [IC95% 1,96;3,15]), mais ricos (RP: 1,95 [IC95% 1,03;1,53]), que utilizaram serviços privados de saúde (RP1,43 [IC95% 1,20;1,71]), com ótima autopercepção de saúde bucal (RP 4,44 [IC95% 3,07;6,42]) e autopercepção de necessidade de consultas para fins de revisão (RP 2,13 [IC95% 1,54;2,96]). CONCLUSÕES: Observam-se desigualdades na utilização regular de serviços odontológicos. Ações que contribuam para aumentar o conhecimento sobre a saúde bucal e melhoria do autocuidado, além de acesso a serviços odontológicos que visem à integralidade da atenção, podem contribuir para o aumento do uso regular dos serviços odontológicos.

This paper reports on the analysis of tidal breathing patterns measured during noninvasive forced oscillation lung function tests in six individual groups. The three adult groups were healthy, with prediagnosed chronic obstructive pulmonary disease, and with prediagnosed kyphoscoliosis, respectively. The three children groups were healthy, with prediagnosed asthma, and with prediagnosed cystic fibrosis, respectively. The analysis is applied to the pressure–volume curves and the pseudophaseplane loop by means of the box-counting method, which gives a measure of the area within each loop. The objective was to verify if there exists a link between the area of the loops, power-law patterns, and alterations in the respiratory structure with disease. We obtained statistically significant variations between the data sets corresponding to the six groups of patients, showing also the existence of power-law patterns. Our findings support the idea that the respiratory system changes with disease in terms of airway geometry and tissue parameters, leading, in turn, to variations in the fractal dimension of the respiratory tree and its dynamics.

Scheduling of constrained deadline sporadic task systems on multiprocessor platforms is an area which has received much attention in the recent past. It is widely believed that finding an optimal scheduler is hard, and therefore most studies have focused on developing algorithms with good processor utilization bounds. These algorithms can be broadly classified into two categories: partitioned scheduling in which tasks are statically assigned to individual processors, and global scheduling in which each task is allowed to execute on any processor in the platform. In this paper we consider a third, more general, approach called cluster-based scheduling. In this approach each task is statically assigned to a processor cluster, tasks in each cluster are globally scheduled among themselves, and clusters in turn are scheduled on the multiprocessor platform. We develop techniques to support such cluster-based scheduling algorithms, and also consider properties that minimize total processor utilization of individual clusters. In the last part of this paper, we develop new virtual cluster-based scheduling algorithms. For implicit deadline sporadic task systems, we develop an optimal scheduling algorithm that is neither Pfair nor ERfair. We also show that the processor utilization bound of us-edf{m/(2m−1)} can be improved by using virtual clustering. Since neither partitioned nor global strategies dominate over the other, cluster-based scheduling is a natural direction for research towards achieving improved processor utilization bounds.

The enzyme hydrogenase isolated from the sulphate reducing anaerobic bacterium Desulfovibrio gigas was encapsulated in reverse micelles of AOT–water–isooctane. The enzyme ability to consume molecular hydrogen was studied as a function of the micelle size (given by Wo = [H2O]/[organic solvent]). A peak of catalytic activity was obtained for Wo = 18, a micelle size theoretically fitting the heterodimeric hydrogenase molecule. At this Wo value, the recorded catalytic activity was slightly higher than in a buffer system(Kcat = 169.43 s−1 against the buffer value of 151 s−1). The optimal buffer used to encapsulate the enzyme was found to be imidazole 50 mM, pH 9.0. The molecular hydrogen production activity was also tested in this reverse micelle medium.

OBJECTIVE To describe the trend for malignant skin neoplasms in subjects under 40 years of age in a region with high ultraviolet radiation indices.METHODS A descriptive epidemiological study on melanoma and nonmelanoma skin cancers that was conducted in Goiania, Midwest Brazil, with 1,688 people under 40 years of age, between 1988 and 2009. Cases were obtained fromRegistro de Câncer de Base Populacional de Goiânia(Goiania’s Population-Based Cancer File). Frequency, trends, and incidence of cases with single and multiple lesions were analyzed; transplants and genetic skin diseases were found in cases with multiple lesions.RESULTS Over the period, 1,995 skin cancer cases were observed to found, of which 1,524 (90.3%) cases had single lesions and 164 (9.7%) had multiple lesions. Regarding single lesions, incidence on men was observed to have risen from 2.4 to 3.1/100,000 inhabitants; it differed significantly for women, shifting from 2.3 to 5.3/100,000 (Annual percentage change – [APC] 3.0%, p = 0.006). Regarding multiple lesions, incidence on men was observed to have risen from 0.30 to 0.98/100,000 inhabitants; for women, it rose from 0.43 to 1.16/100,000 (APC 8.6%, p = 0.003). Genetic skin diseases or transplants were found to have been correlated with 10.0% of cases with multiple lesions – an average of 5.1 lesions per patient. The average was 2.5 in cases without that correlation.CONCLUSIONS Skin cancer on women under 40 years of age has been observed to be increasing for both cases with single and multiple lesions. It is not unusual to find multiple tumors in young people – in most cases, they are not associated with genetic skin diseases or transplants. It is necessary to avoid excessive exposure to ultraviolet radiation from childhood.

O presente trabalho foi elaborado no âmbito da Dissertação Final de Mestrado do curso de Engenharia Mecânica – Gestão Industrial do Instituto Superior de Engenharia do Porto. Este foi realizado numa empresa da indústria de pneus, a Continental Mabor S.A.. Nos dias atuais, a indústria esta cada vez mais competitiva, os custos e os prazos da entrega são cada vez mais reduzidos e a qualidade cada vez mais exigente, assim sendo, é imprescindível uma constante melhoria do sistema produtivo. Este fato fez com que o presente trabalho tivesse como principal objetivo determinar o estado atual e delinear um plano de melhoria para um equipamento (Extrusora de pisos nº6), recentemente instalado na fábrica mas oriundo de outra fábrica do grupo, recorrendo para tal ao Lean Manufacturing e a ferramentas que lhe estão associadas. Inicialmente realizou-se uma análise e diagnóstico ao processo de extrusão de pisos na Extrusora nº6, com o intuito de avaliar todas as suas ineficiências e ainda formular um plano de melhorias para a ineficiência de maior impacto no sistema produtivo. Esta análise foi realizada em diferentes turnos e diferentes equipas de trabalho de forma a se obter uma amostra mais representativa da realidade global. Após esta análise verificou-se que as principais ineficiências eram Setup, conformidade do material, dimensões e encravamentos, entre outros. Sendo que os Setup provocam um tempo de paragem de 101 minutos por turno, escolheu-se esta perturbação como o foco do plano de melhorias a realizar posteriormente. De forma a reduzir os tempos de mudança (Setup), o autor no presente trabalho utilizou ferramentas Lean Manufacturing, principalmente o SMED. Conjuntamente com o SMED ainda foram utilizadas outras ferramentas Lean Manufacturing tais como: 5S, Gestão Visual, Problem Solving e Normalização do Método de Trabalho. Após a implementação de todas estas ferramentas obteve-se uma redução de tempos de mudança de 43% com 1 operador e 71% com 2 operadores, ou seja, reduziu-se de 40,5 minutos gastos por turno para a mudança de fieira para 23,13 min e 11,79 min respetivamente, o que corresponde a um ganho monetário anual de 63.621€ ou 105.045€, respetivamente. Com este trabalho conclui-se que a utilização de ferramentas Lean Manufacturing contribuem para a redução dos desperdícios do processo produtivo. Por isso, espera-se que este estudo seja aplicado na Extrusora nº6 e nas restantes Extrusoras de pisos existentes na fábrica, e num futuro próximo que se realize estudos semelhantes em máquinas com diferentes funções.

The aim of this case series was to describe the clinical, laboratory and epidemiological characteristics and the presentation of bacillary angiomatosis cases (and/or parenchymal bacillary peliosis) that were identified in five public hospitals of Rio de Janeiro state between 1990 and 1997; these cases were compared with those previously described in the medical literature. Thirteen case-patients were enrolled in the study; the median age was 39 years and all patients were male. All patients were human immunodeficiency virus type 1 (HIV-1) infected and they had previous or concomitant HIV-associated opportunistic infections or malignancies diagnosed at the time bacillary angiomatosis was diagnosed. Median T4 helper lymphocyte counts of patients was 96 cells per mm³. Cutaneous involvement was the most common clinical manifestation of bacillary angiomatosis in this study. Clinical remission following appropriate treatment was more common in our case series than that reported in the medical literature, while the incidence of relapse was similar. The frequency of bacillary angiomatosis in HIV patients calculated from two of the hospitals included in our study was 1.42 cases per 1000 patients, similar to the frequencies reported in the medical literature. Bacillary angiomatosis is an unusual opportunistic pathogen in our setting.

Heparin, in some regions of Brazil has been used in the treatment of bothropic accidents, but the data found in the literature are inconclusive about its effectiveness. The venoms of Bothrops atrox and of B. erythromelas were characterized according to their biological activities. The capacity of heparin in neutralizing these activities was tested with doses of 3 and 6 IU in isolated form and associated to Antibothropic Serum (ABS). It was verified that heparin, in doses of 3 and 6 IU, was not effective in neutralizing the desfibrinating and edema-forming activities of B. atrox venom and the hemorrhagic and coagulant actions of both venoms. Heparin diminished the effectiveness of the ABS in the neutralization of the hemorrhagic and edema-forming activities of the B. atrox venom. However, heparin in the 6 IU dose was capable of neutralize the edema-forming of the B. erythromelas and increase the effectiveness of the ABS. Heparin also neutralized the phospholipasic A2 activity of B. atrox (14.3%) and B. erythromelas (28.0%) venoms. For B. erythromelas venom, the associated treatment, heparin and ABS, was more effective in the neutralization of its lethal activity.

The objective of this study was to evaluate the prevalence and risk factors associated with HCV infection in a group of HIV seropositive patients. We analyzed the medical records of 1,457 patients. All patients were tested for HCV infection by third generation ELISA. Whenever possible, a sample of the positive patients was also tested for HCV by PCR. HCV positive patients were analyzed according to their risk factors for both infections. The prevalence of anti-HCV positive patients was 17.7% (258 patients). Eighty-two (82) of these patients were also tested by PCR and 81 were positive for HCV virus (98%). One hundred fifty-one (58.5%) were intravenous drug users (IDU); 42 (16.3%) were sexual partners of HIV patients; 23 (8.9%) were homosexual males; 12 (4.7%) had received blood transfusion; 61 (17.5%) had promiscuous sexual habits; 14 (5.4%) denied any risk factor; 12 (4.7%) were sexual partners of IDU. Two hundred four patients mentioned only one risk factor. Among them, 28 (10.9%) were sexual partners of HIV-positive patients. Although intravenous drug use was the most important risk factor for co-infection, sexual transmission seemed to contribute to the high HCV seroprevalence in this group of patients.
